BACKGROUND: Therapy for type 1 diabetes mellitus (T1DM) is still largely an unsolved clinical problem. Despite the introduction into clinical practice of modern insulin preparations, devices for its administration, as well as continuous monitoring of glucose levels, the goals of therapy are often not achieved. At the same time, the International Diabetes Federation (IDF) notes an increase in the prevalence and incidence of T1DM in children and adolescents in the world. The Federal Register of Diabetes Mellitus (FRDM) is a dynamically updated database of patients with diabetes, which allows assessing prevalence and incidence rates, achievement of glycemic control goals and the incidence of diabetes complications.AIM: analyze the epidemiological data of T1DM (prevalence, morbidity) in children and adolescents (patients from birth to 18 years of age) in Moscow according to the FRSD data and to assess their dynamics, as well as the dynamics of achieving the goals of glycemic control and the incidence of T1DM complications in 2015-2020.MATERIALS AND METHODS: The object of the study is a sample from the database of the Moscow segment of the FRDM of a cohort of patients with type 1 diabetes under 18 years of age who were registered for the period 01.01.2015-01.01.2021. Epidemiological prevalence and incidence rates are calculated per 100,000 of the relevant population.RESULTS: the number of children and adolescents with type 1 diabetes in Moscow as of 01.01.2021 was 4024 people (2962 children and 1062 adolescents). Over the period from 2015 to 2020, there was an increase in the prevalence of T1DM (possibly due to an increase in the quality of data registration in the FRSD) and a decrease in the incidence of both children and adolescents. There was also a decrease in the level of HbA1c and the proportion of patients with HbAc1> 8.0% among children with T1DM. Both children and adolescents with T1DM showed a decrease in the incidence of diabetic coma and ketoacidosis with a simultaneous increase in the incidence of severe hypoglycemia, as well as a decrease in the incidence of retinopathy and nephropathy. However, the incidence of neuropathy decreased among children and increased among adolescents.CONCLUSION: The data obtained on the dynamic management of adolescents with T1DM are the basis for considering the development of a profile program for their dynamic observation, taking into account the need for psychological and social support for patients and their families.

In this paper, the authors highlight the legal basis for the use of biometric personal data, and also pay attention to the main initiative of the state in this area: the creation of a unified biometric system. The study is particularly relevant in connection with the release of the new Federal law No. 168-FZ on June 8, 2020: On the unified Federal information register containing information about the population of the Russian Federation. In particular, an acute problem is the ability to integrate information from the unified biometric system and the Federal register that is being created.

Federal Research Centre for Projects Evaluation and Consulting Service (FRCEC) is one of the leading Russian institutes providing expertise, research, methodological, technological, organizational and information support of sci-tech and innovative activities in the country. In years, the Institute, now subordinate to the Ministry of Science and Higher Education of the Russian Federation, has developed the mechanisms of expertise and acquiring data on innovative achievements, their control and implementation. The Federal Register of Experts (FRE) has been continuously updated to include accredited authoritative scientists and professionals in priority sci-tech spheres. The FRE structure and functionality are described in detail. Beside FRE, several FRCEC information resources are also discussed in brief: information support of RF Presidential grants; system for controlling and monitoring minor innovative enterprises in science and education, information catalog of current innovative infrastructure of sci-tech activities in RF constituents, etc.

The objective of the study: to analyze the impact of response activities to the COVID-19 pandemic on the system of tuberculosis care provided to the population of the Russian Federation based on the data of the Federal Register of TB Cases (FRTBC). Subjects and methods. Using regression analysis of data obtained from the forms of federal and sectoral statistical observation, regression equations were drawn up, and estimated indicators were determined for each month of 2020. The estimated indicators were compared with the data obtained from monthly reports downloaded from FRTBC from January to June 2020. Results. The shortfall in the registration of all cases of tuberculosis treatment relative to estimated indicators in May-June 2020 amounted to 24.4-24.7%, including 24.8% for new cases and relapses. That is very close to the value estimated by the World Health Organisation (25%). The number of tuberculosis cases detected post mortem increased by 37.1%. The number of detected pediatric tuberculosis cases at the age of 0-14 and 15-17 years old decreased by 31.4 and 28.2%, respectively. In June 2020, there was an increase in almost all recorded indicators that was due to the intensified tuberculosis detection activities. Conclusions. FRTBC allows performing prompt monitoring of the impact of response activities to the COVID-19 pandemic on the system of tuberculosis care provided to the population. After a period of a significant decrease in the number of registered tuberculosis patients in April and May 2020, in June, as anti-epidemic activities related to COVID-19 were slowed down, the number of registered patients tended to reach the previous level and approach their estimated values.

Проведен анализ распространенности редких (орфанных) заболеваний («Перечень 24») и динамики выявляемости пациентов детского возраста в 85 регионах РФ в период 2013-2019 гг. Показан ежегодный прирост пациентов в Федеральном регистре (2013 г. - 4962 больных, 2014 г. - 6761, 2015 г. - 7038, 2016 г. - 7625, 2017 г. - 8245, 2018 г. - 8639, 2019 г. - 9088). Динамика доли детей с заболеваниями из «Перечня 24» среди всех больных колебалась в небольших пределах (в среднем 50%). Большинством заболеваний (16 из 24) болеют в основном дети, но они встречаются также у взрослых. Распространенность заболеваний значительно варьировала между субъектами РФ. Улучшение информированности, а также ведение Федерального регистра способствуют эффективной выявляемости и своевременному назначению терапии в детском возрасте. Однако низкие значения распространенности заболеваний, проявляющихся до 1-го года жизни, говорят о сложностях их диагностики и недостаточной выявляемости. The analysis of the prevalence of rare (orphan) diseases («List of 24») and the dynamics of detection of children’s patients in 85 subjects of the Russian Federation in the period 2013-2019 showed an annual increase in patients in the Federal register (2013 - 4962 patients, 2014 - 6761, 2015 - 7038, 2016 - 7625, 2017 - 8245, 2018 - 8639, 2019 - 9088). The dynamics of the share of children with diseases from the “List of 24” among all patients fluctuated within small limits (on average 50%). Most of the diseases (16 out of 24) are mostly between children, but they also occur in adults. The prevalence of diseases varied significantly between the subjects of the Russian Federation. Improving awareness, as well as maintaining a Federal register promote effective detection and timely treatment in children. However, the low prevalence of diseases that occur before the 1st year of life indicates the complexity of their diagnosis and lack of detection.

The paper examines the experience of the United States of America in recording and classifying convicted sexual offenders. The Federal register is a comprehensive criminal and administrative law tool for crime prevention. It has its origins in similar state registers. The author examines the principles of maintaining the register, the grounds for inclusion in the register and exclusion from it, the volume of data to be published, the frequency of data updates, and conducts a criminological characterization of the Institute. The paper analyzes the case law of the Supreme courts of the United States regarding the constitutionality of the rules and principles that form the institution of the registry, their retroactive application, and compliance with procedural and material guarantees of a fair trial. The author concludes that it is possible to introduce a similar Federal register in Russia, but taking into account the shortcomings identified in the study. Currently, the efforts of the legislator in this part are obvious (increasing criminal responsibility for violent sexual crimes with the establishment of restrictions on freedom, the emergence of new tools of "deterrence" in the hands of law enforcement agencies, such as administrative supervision). However, the measures are not comprehensive, often overlap, and do not achieve the stated goal (execution of restrictions under administrative supervision after serving the restriction of freedom). The author believes that it can be an independent institution, implemented from the stage of execution of the sentence, accessible to law enforcement agencies and victims, and in cases provided for by law — for social and educational institutions, guardianship authorities, family and child protection.
