Background: Carcinoma esophagus is the 6th most common cancer in India with an incidence of 5.04% It most commonly affects older age individuals, with incidence peaking in seventh and eighth decades of life. Adenocarcinoma (AC) and Squamous cell carcinoma (SCC) are the two main histological variants. Our aim was to study the risk factors, clinical and epidemiological profile of patients diagnosed with Esophageal Cancer from two tertiary care centres located in North India.Methods:This was a retrospective study carried in departments of gastroenterology of two tertiary care hospitals North India. Data was collected from respective institutes for the previous 1 year i.e. from 1st October 2020 to 30th September 2021. All patients presenting to Gastroenterology OPD or emergency with dysphagia who were diagnosed as having Carcinoma esophagus were included in the study.Results:A total of 106 patients were included in this study. There were 44 females & 66 males. The most common presenting complaint was Dysphagia (48%) followed by Loss of weight (26.4%), Vomiting (in 23.6%) and Odynophagia (22.6%). Involvement of the lower one third of the oesophagus was the most common (37.7%). SCC (Squamous Cell Carcinoma) was the most common type of oesophageal carcinoma in this study, seen approximately in 80% of cases.Conclusion:Our study from North India suggested poor oral hygiene as a strong risk for carcinoma esophagus Males were more commonly affected and SCC was the predominant variant.

Background: Initial results of the chemo-radiotherapy for oesophageal cancer followed by surgery study (CROSS) comparing neoadjuvant chemoradiotherapy (NACRT) plus surgery versus surgery alone in patients with squamous cell carcinoma and adenocarcinoma of the oesophagus or oesophagogastric junction showed a significant increase in 5-year overall survival in favour of the NACRT plus surgery group after a median of 45 months' follow-up. In this study we will interpret the short-term results of NACRT on resectable, locally advanced oesophageal carcinoma.Methods: Patients with clinically resectable, locally advanced cancer of the oesophagus or oesophagogastric junction (clinical stage T1N1M0 or T2-3N0-1M0, according to the AJCC, 8th  edition) were assigned to receive weekly administration of four cycles of NACRT (intravenous carboplatin [AUC 2 mg/mL per min] and intravenous paclitaxel [50 mg/m 2 of body-surface area] for 23 days) with concurrent radiotherapy (41·4 Gy, given in 23 fractions of 1·8 Gy on 5 days per week) followed by McKeown’s oesophagectomy from 01 January, 2020 to 31 May, 2021.Results: It was observed in our study that 38.46% patients had achieved a CPR after the administration of NACRT as per the CROSS-trial protocol which is comparable to PCR achieved in CROSS trial (29%). All the patients underwent an R0 resection during surgery (100%) which is comparable to CROSS trial (92%).12Conclusions: In our study which had collected data over a period of 17 months we learnt that the administration of NACRT in locally advanced oesophageal cancer was effective in reducing the tumor burden and achieving a satisfactory CPR of 38.46%.

Background: Oesophageal cancer (OC) is a cancer that develops in the gastrointestinal tract and is caused by a variety of circumstances. It has quite an uneven geographic distribution with male predominance and squamous cell carcinoma being most frequent type of oesophageal carcinoma in developing countries. As Pakistan lacks any proper cancer registry this study was needed to have an idea of the OC statistics in South Punjab region. Objective was to determine the prevalence of OC in the general population and in young people of South Punjab, PakistanMethods: Retrospective cross-sectional study. Two hundred and twenty-three patients who presented to Multitest lab from 2014 to 2018 were included in the study.Results: The 186 patients were positive for OC. Sixty patients were below 40 years of age. Male to female ratio was 1.24:1. Most frequent symptom was dysphagia and squamous cell carcinoma was histological subtype.Conclusions: In our study based on South Punjab region, oesophageal carcinoma has become a common condition and is more prevalent in males. It commonly involves upper oesophagus and squamous cell carcinoma is the most frequent type. It can also be concluded that it is in increasing in young population of South Punjab.

ObjectivesLobaplatin (LBP), a third-generation cisplatin derivative has shown promising activity and few side effects in oesophageal squamous cell carcinoma (ESCC) in previous reports. We compared LBP plus docetaxel with cisplatin plus docetaxel as adjuvant chemotherapy in ESCC patients to determine the effects on overall survival (OS) and toxicity.MethodsA multicentre retrospective study was performed using propensity score matching (PSM) with the Medicine-LinkDoc database. Patients diagnosed with stage II-III ESCC treated with adjuvant chemotherapy (cisplatin plus docetaxel or LBP plus docetaxel) between January 2013 and December 2016 were selected from 6 centres in China.ResultsThere were 733 eligible ESCC patients. After PSM (1:1 ratio), 458 patients remained. The 5-year OS rates of the cisplatin and LBP groups were 25.9% and 23.6%, respectively (P=0.457). Leukopenia (grade III-IV/I-II/0: 2.62%/34.5%/59.39% versus 5.24%/43.23%/45.85%; P=0.0176), neutropenia (grade III-IV/I-II/0: 6.55%/37.56%/51.09% versus 4.37%/53.28%/36.34%; P=0.0015), nephrotoxicity (grade I-II/0: 13.97%/76.86% versus 26.64%/65.94%; P<0.001) and gastrointestinal symptoms (grade III-IV/I-II/0: 2.18%/54.59%/32.31% versus 6.55%/65.07%/20.88%; P=0.0011) were more frequent in the cisplatin group.ConclusionsCompared with cisplatin plus docetaxel, LBP plus docetaxel provided the same survival benefits but lower side effects of myelosuppression and gastrointestinal symptoms. Abstract Introduction Oesophagectomy is the standard of care for resectable oesophageal carcinoma and remains pivotal for curative treatment. Pull-up with gastric conduit remains the most common reconstruction. Delayed gastric emptying (DGE) is a recognised complication and may lead to delayed postoperative recovery and prolonged hospital stay. Botulinum Toxin BTA has been used to relax the hypertonic pylorus intraoperatively and improve DGE. The aim of this review was to assess the efficacy of intraoperative BTA on gastric emptying and the need for postoperative endoscopic pyloric intervention (EPI). Method Systematic review of literature was conducted using electronic database searched up to October 2020 including Cochrane Library, Medline, Embase and Goggle Scholars to identify relevant studies. Data was extracted and critically appraised by two independent authors. Results Eleven studies were included for systematic review with 1800 patients assigned to either BTA, pyloroplasty, pyloromyotomy, or no-intervention. Ten were observational studies and one was a randomised experimental study. Only one study reported a statistically significant benefit of BTA on early DGE (Day 4, 59%, P < 0.001). Three studies reported higher rates of postoperative EPI when chemical pyloroplasty was used. Length of Hospital stay was higher for patients who had BTA compared to other techniques in three studies. Conclusions This systematic review did not demonstrate any significant benefit of intraoperative BTA on gastric emptying on the long term. The rate of endoscopic pyloric intervention was higher in these patients as reported by some studies. Further studies are required to investigate the outcome of chemical pyloroplasty during esophagectomy.

Adenocarcinoma of the bladder is a rare form of malignancy accounting for fewer than 2% of bladder tumours. It is most commonly a result of direct invasion from prostatic, rectal or gynaecological primaries and less commonly presents from distant haematological or lymphatic metastasis. We report a rare case of oesophageal carcinoma metastasising to the bladder. It involves a 71-year-old man with progressive dysphagia and diagnostic computerised tomography findings of thickening in the oesophagus, bladder and common bile duct. Subsequent endoscopic biopsies of the oesophageal and bladder abnormalities showed immunohistochemical features consistent with upper gastrointestinal malignancy. This report aims to add to current clinical evidence of this route of metastasis and also highlight some of the key markers used by pathologists in interpretation of specimens. It also emphasises the essential role of a multidisciplinary approach for the diagnosis of such rare conditions.
