Background: Betel quid dependence (BQD) is associated with abnormalities in the widespread inter-regional functional connectivity of the brain. However, no studies focused on the abnormalities in the topological organization of brain functional networks in chewers in Mainland China.Methods: In the current study, resting-state functional magnetic resonance images were acquired from 53 BQD individuals and 37 gender- and age-matched healthy controls (HCs). A functional network was constructed by calculating the Pearson correlation coefficients among 90 subregions in the human Brainnetome Atlas. The topological parameters were compared between BQD individuals and HCs.Results: The results showed that BQD individuals presented a small-world topology, but the normalized characteristic path length (λ) increased compared with HCs (0.563 ± 0.030 vs. 0.550 ± 0.027). Compared to HCs, BQ chewers showed increased betweenness centrality (Be) in the right supplementary motor area, right medial superior frontal gyrus, right paracentral lobule, right insula, left posterior cingulate gyrus, right hippocampus, right post-central gyrus, right superior parietal gyrus, and right supramarginal gyrus, while decreased Be was found in the orbitofrontal area and temporal area, which is associated with reward network, cognitive system, and default mode network. The area under the curve (AUC) value of λ displayed a positive correlation with the duration of BQ chewing (r = 0.410, p = 0.002).Conclusions: The present study revealed the disruption of functional connectome in brain areas of BQD individuals. The findings may improve our understanding of the neural mechanism of BQD from a brain functional network topological organization perspective.

Background: Melatonin, produced by the pineal gland, is known for its antioxidant, oncostatic, and anti-inflammatory properties. However, studies on serum melatonin levels in different cancer types have yielded conflicting results, and little is known about the clinical significance of serum melatonin in oral squamous cell carcinoma (OSCC) in the Southern Asian population. Therefore, we explored its role in OSCC in this study. Methods: A total of 67 male OSCC patients and 78 healthy controls were enrolled in this case–control study. The serum levels of melatonin were determined by enzyme-linked immunosorbent assay (ELISA) and compared between the two groups. Results: The serum melatonin levels were significantly lower in OSCC patients compared with healthy controls (mean ± standard deviation, 15.0 ± 4.6 vs. 18.5 ± 11.8 pg/mL, p = 0.02). In the subgroup of age less than 55 years (mean age of OSCC), OSCC patients had a significantly decreased melatonin level than healthy controls (mean melatonin, 15.7 ± 12.6 vs. 20.8 ± 3.9 pg/mL, p = 0.02). Decreased serum melatonin (odds ratio (OR): 0.95, 95%CI: 0.91–0.99), alcohol consumption (OR: 29.02, 95%CI: 11.68–72.16), betel quid chewing (OR:136.44, 95%CI: 39.17–475.27), and cigarette smoking (OR:29.48, 95%CI: 11.06–78.60) all increased the risk of OSCC under univariate analyses of logistic regression. Betel quid chewing (OR: 45.98, 95%CI: 10.34–204.49) and cigarette smoking (OR:6.94, 95%CI: 1.60–30.16) were the independent risk factors for OSCC in Taiwan. In addition, a negative correlation between age and melatonin level was observed in healthy controls (Pearson r = −0.24, p = 0.03). However, the negative correlation was lost in patients with OSCC. Melatonin concentration had no association with the severity of OSCC. Conclusion: Overall, our study provides evidence that serum melatonin levels decreased in OSCC patients in Taiwan and the decreased level is much significant in young populations and suggests that the decreased melatonin was associated with OSCC, especially in young populations. Further studies are warranted to investigate whether melatonin can be a useful non-invasive screening tool for OSCC.

Introduction: Oral cancer and Oral Potentially Malignant Disorders (OPMDs) arising due to risk habits like betel quid (BQ) chewing, smoking and alcohol use are preventable despite the high prevalence and poor survival rates. However, dependence to risk habits has become a barrier to prevent OPMDs and oral cancer. Objectives: This study was conducted to determine the dependence to risk habits among patients with such oral lesions, and the quitting among dependent users after diagnosis. Materials and method: A case-control study was conducted among 100 participants, 50 diagnosed with OPMDs and oral cancer from two Oral and Maxillo Facial clinics in Sri Lanka and a similar control group without any oral lesions. All participants practiced one or more risk habits such as BQ chewing, smoking and/or alcohol use. Dependence was assessed using Sinhala language translations of standardized scales such as Betel Quid Dependence Scale for BQ chewers, Fagerstrom Test for Nicotine Dependence for tobacco smokers and CAGE (“Cut down”, “Annoyed”, “Guilty” and “Eye opener”) scale for alcohol users. Results: Dependence on habits was more among the case group (BQ-93%; smoking-14%; alcohol-66%). Quitting rates of BQ and alcohol among those who were diagnosed with oral cancer [BQ (89%), alcohol (89%)] were higher (p<0.05) than those with OPMDs [BQ (50%), alcohol (22%)]. Lack of awareness due to low socio-economic status (97%) was a significant association for BQ dependence (p=0.01).Conclusion: In conclusion, development of OPMDs and oral cancer increases with dependence to BQ chewing, smoking and alcohol use. Increased awareness about OPMDs and its risk of malignancy is needed among public. Prevention of dependence to these risk habits also becomes essential.

Betel quid (BQ) is one of the most popular addictive substances in the world. However, the neurophysiological mechanism underlying BQ addiction remains unclear. This study aimed to investigate whether and how BQ chewing would affect brain function in the framework of a dynamic brain network model. Resting-state functional magnetic resonance imaging scans were collected from 24 male BQ-dependent individuals and 26 male non-addictive healthy individuals before and promptly after chewing BQ. Switching rate, a measure of temporal stability of functional brain networks, was calculated at both global and local levels for each scan. The results showed that BQ-dependent and healthy groups did not significantly differ on switching rate before BQ chewing (F = 0.784, p = 0.381, analysis of covariance controlling for age, education, and head motion). After chewing BQ, both BQ-dependent (t = 2.674, p = 0.014, paired t-test) and healthy (t = 2.313, p = 0.029, paired t-test) individuals showed a significantly increased global switching rate compared to those before chewing BQ. Significant corresponding local-level effects were observed within the occipital areas for both groups, and within the cingulo-opercular, fronto-parietal, and cerebellum regions for BQ-dependent individuals. Moreover, in BQ-dependent individuals, switching rate was significantly correlated with the severity of BQ addiction assessed by the Betel Quid Dependence Scale scores (Spearman's rho = 0.471, p = 0.020) before BQ chewing. Our study provides preliminary evidence for the acute effects of BQ chewing on brain functional dynamism. These findings may provide insights into the neural mechanisms of substance addictions.

This study’s objective was to describe the relationship between the main risk factors for oral cancer, including tobacco (in the form of cigarettes, smokeless tobacco (SLT), secondhand smoking (SS)), alcohol, and betel quid (BQ), and the occurrence of oral potentially malignant disorders (OPMDs). A community-based case-control study was conducted with a population of 1448 adults aged 40 years or above in northeastern Thailand. Patients aged 60 years or above (OR 1.79, p < 0.001) and female patients (OR 2.17, p < 0.001) had a significant chance of having OPMDs. Our multivariate analysis showed that the most potent risk factor for OPMDs occurrence was betel quid (BQ) (adjusted OR 4.65, p < 0.001), followed by alcohol (OR 3.40, p < 0.001). Even former users were at risk of developing OPMDs. The synergistic effect between these main risk factors was significantly shown in the group exposed to SLT, SS, BQ, and alcohol. The most potent synergistic effect was found in the group exposed to SLT, BQ and alcohol with the OR = 20.96.
