ABSTRACT Objective: Observe the workflow of nurses in hospitalization units identifying indirect care activities/interventions; measure the frequency and average time spent in performing them; and to verify the associations between average time of the activities interventions grouped into categories and per hospitalization unit. Method: Observational exploratory study using the timed technique. It was conducted in medical, surgical and specialized clinic units of a teaching hospital in the northwest of São Paulo Brazil, with 16 attending nurses as participants. Results: 90 hours of observation were performed, of which 58% (52 hours and 10 minutes) were related to indirect care activities of the patients. The most frequent activities/interventions were: "Communication" - 1,852 (44.1%), mean 34.6 (SD = 54); "Walking" - 1,023 (24.3%), mean 22 (SD = 49.2); and "Documentation" - 663 (15.8%), mean 82.7 (SD = 144.4). Conclusion: These findings favor a redesign of the work process and foster the need to update and refine the current workload measurement instruments.

This article concludes a series that described work completed by the American Nurses Association (ANA) Barriers to RN Scope of Practice Professional Issues Panel (Panel). The Panel has summarized the roles of the registered nurse (RN) as professional, advocate, innovator, and collaborative leader. As professionals, RNs are accountable for their own educational development, growth of their own practice, and execution of their own professional role. As advocates, RNs provide direct and indirect care focused on the achievement of optimal health. As innovators, RNs act as agents of change in driving processes and policy and leveraging technology. As collaborative leaders, RNs lead within the professional practice setting with state-specific nurse practice acts, a defined scope of practice, and nurse-led initiatives. In this article, Panel members describe their methodology, offer role definitions and recommendations, and provide individual recommendations for RNs to remove barriers and practice to the full extent of their scope of practice. We conclude with some future considerations.

This study examines how multidimensional gender and fathering beliefs of fathers may explain their relative involvement in childcare after considering paid leave uptake. We draw on cross-sectional survey data from one German state, which allow us to distinguish three belief dimensions: (1) gender traditionalism and essentialism, (2) fathering attitudes, and (3) fathering self-concepts and self-efficacy. By means of multiple linear regression models we investigate how the different dimensions of gender and fatherhood beliefs relate to fathers’ relative involvement in basic and indirect childcare tasks. Our results show that gender (essentialist) ideologies and fatherhood attitudes were strongly associated with fathers’ relative involvement in both childcare domains. The higher fathers perceived self-efficacy in fathering, the more involved they were in basic but not indirect care. All belief dimensions mediated the positive association of fathers’ uptake of paid leave with their involvement in basic childcare.

Abstract Background Latin America Telemedicine Infarct Network (LATIN) employed telemedicine to construct a population-based AMI program in Brazil, Colombia, Mexico, and Argentina. It increased access, accuracy and guidelines-based care and addressed fiscal issues. Previously, we demonstrated a cost and benefit analysis (CBA) of LATIN based upon avoiding unnecessary transfers and hospitalization. We have performed a scrupulous follow up of this initial observation with a long-term follow up from all expanded LATIN sites. Purpose To demonstrate that telemedicine avoids unnecessary transfer of patients. Methods 784,947 patients at LATIN spokes (small clinics in remote areas) were screened and CBA measured at hubs, spokes and telemedicine centers. Technology, transfer, inpatient, and procedure-related costs were included. A sensitivity analysis was performed for worst and best scenarios of costs, revenues, and savings. A comparison with Avera e-Emergency (Sioux Falls, SD) Telemedicine program, involving 85 rural hospitals in 7 states, is provided (13% transfer avoidance). Results Of 784,947 screened patients, 8,448 had STEMI (1.08%); 3,911 (46.3%) were urgently reperfused, 3,049 (78%) with PPCI. Time to Telemedicine Diagnosis was 3 min. With efficient triage, costs for non-AMI patients was controlled. LATIN expenses, including for IT and experts, were $272, and for transfer and indirect care, $1,068. Net savings/patient were $796. Savings, till date, range between $187.4 million and $62.4 million (Best scenario −30% transfer avoidance; Worse scenario −10% transfer avoidance). Conclusions Longitudinal analysis firms the trend of enormous cost savings with LATIN. Telemedicine avoids unnecessary transfers and hospitalization and it is a cost-effective strategy for population-based AMI programs.

The aim of this study was to identify forms of care of nurses in the operating room, and describe how context influences the implementation of this care. This is qualitative study, for which data were obtained by means of five interviews with nurses working in the operating room of a public hospital in the municipality of Duque de Caxias, in the Brazilian state of Rio de Janeiro, conducted between August and September of 2010, using a semistructured script. The methodology followed the Grounded Theory method. The results show that the nurses perform care motivated by professional commitment and satisfaction, with patient health being their primary focus. Professional context influences ways of working, since the relationship of care exists in the interdependence between the beings involved and working conditions. In conclusion, operating room nurses integrate several factors, acting sometimes as agents of indirect care, despite the difficulties of a complex and specific context.

Determining the best use of case managers’ time is difficult both because of the great variability in the factors and the lack of outcome studies. In place of the outcome studies, the responses to focus group questions of 89 experienced case managers from across Canada were used to indicate a value for case management functions in the areas of direct care, indirect care management, and program management. The case managers identified greater value for direct care with clients and families who are new to home care or who have complex problems in comparison to clients in more stable or manageable situations. Case managers were frustrated by the lack of agency and community policies and resources to allow case managers to properly support people in their homes. The case managers valued addressing problems on a program or community basis rather than the usual case-by-case. Although preliminary, the approach could be useful in assessing the efficient and effective use of a scarce resource, case managers’ time.

Penelitian ini bertujuan untuk melihat gambaran keterlibatan ayah terhadap anak remajanya  yang berusia 16-21 tahun.. Keterlibatan itu dilihat dari sudut pandang ayah. Ayah yang berperan aktif dalam keluarga memiliki dampak bagi dirinya sendiri dan juga bagi anak, baik anak perempuan maupun laki-laki. Keterlibatan ayah sendiri merupakan bentuk interaksi ayah terhadap anak yang tercakup dalam lima komponen, yaitu (a) <em>activity engagement</em>; (b) <em>warmth-responsiveness</em>; (c) <em>control</em>; (d) <em>indirect care</em>; dan (e) <em>process responsibility</em>. Pada remaja perempuan, peran ayah dapat membangun harga diri positif dan menumbuhkan keinginan berprestasi. Pada remaja laki-laki, peran ayah mampu mengembangkan motivasi untuk sukses dan melanjutkan pendidikan yang lebih tinggi. Penelitian ini melibatkan 201 partisipan yang merupakan seorang ayah berdomisili di DKI Jakarta dan memiliki anak berusia remaja (16-21 tahun). Pengumpulan data dilakukan dengan menggunakan kuesioner keterlibatan ayah dari sudut pandang ayah yang mengacu pada konsep <em>father involvement</em> (Pleck, dalam Lamb, 2010) yang diadaptasi oleh Kristianto, Gurmichele, Utama, Adriel, dan Sidarta (2016). Hasil penelitian terhadap 201 partisipan mengungkapkan bahwa sebagian besar ayah memiliki keterlibatan dalam pengasuhan dan pengelolaan rumah tangga yang tergolong sedang. Hasil tambahan penelitian ini menunjukkan bahwa tidak ada hubungan yang signifikan antara durasi kerja ayah dan keterlibatannya di dalam keluarga. Selain itu, tidak ada perbedaan keterlibatan ayah antara ayah yang memiliki istri bekerja dan tidak bekerja.
