Correlates of physical activity counseling by health providers to patients with diabetes and hypertension attended by the Family Health Strategy in the state of Pernambuco, Brazil

OBJECTIVE: To evaluate the access to drugs for hypertension and diabetes and the direct cost of buying them among users of the Family Health Strategy (FHS) in the state of Pernambuco, Brazil. METHODS: Population-based, cross-sectional study of a systematic random sample of 785 patients with hypertension and 823 patients with diabetes mellitus who were registered in 208 randomly selected FHS teams in 35 municipalities of the state of Pernambuco. The selected municipalities were classified into three levels with probability proportional to municipality size (LS, large-sized; MS, medium-sized; SS, small-sized). To verify differences between the cities, we used the χ2 test. RESULTS: Pharmacological treatment was used by 91.2% patients with hypertension whereas 85.6% patients with diabetes mellitus used oral antidiabetic drugs (OADs), and 15.4% used insulin. The FHS team itself provided antihypertensive medications to 69.0% patients with hypertension, OADs to 75.0% patients with diabetes mellitus, and insulin treatment to 65.4%. The 36.9% patients with hypertension and 29.8% with diabetes mellitus that had to buy all or part of their medications reported median monthly cost of R$ 18.30, R$ 14.00, and R$ 27.61 for antihypertensive drugs, OADs, and insulin, respectively. CONCLUSION: It is necessary to increase efforts to ensure access to these drugs in the primary health care network.

Abstract This study aimed to determine the prevalence and associated factors with physical activity counseling among Brazilian Family Health Strategy workers. This is a cross-sectional study conducted with a random sample of 591 health workers who work in the Family Health Teams of João Pessoa-PB, Brazil. Counseling for physical activity was defined as any advising targeted for increasing patients’ physical activity levels conducted for at least six months. The following factors were considered: time working in health care units, amount of daily attendance, continuing education, perceived barriers, self-efficacy, attitude, self-rated health, physical activity level and nutritional status. Prevalence of counseling was 46.3%, being higher among physicians (74.5%; 95%CI: 59.6-85.2) and nurses (60.3%; 95%CI: 48.0-71.4) compared to community health workers (42.9%; 95%CI: 38.2-47.7) and nurses assistants (31.5%; 95%CI: 20.2-45.4). The results showed health professionals with positive self-rated health, without perception of barriers, having a positive attitude and high self-efficacy were more likely to perform physical activity counseling. Knowledge and actions on factors associated with physical activity counseling can help broaden the involvement of primary health care providers in health education.

AbstractObjective: to verify the correlation between the rates of hospitalization for primary care-sensitive cardiovascular diseases and the coverage by the Family Health Strategy of residents of the State of Paraná, by regional health divisions, from 2000 to 2011.Method: ecological study developed from data of the Hospital Information System of the Brazilian Unified Health System (SUS) and the Department of Primary Care of the Ministry of Health. The rates of hospitalization for cardiovascular diseases were correlated with the annual coverage by the Family Health Strategy using Pearson's and Spearman's correlation coefficients.Result: there was a strong and negative correlation in the State of Paraná (r=-0.91; p <0.001) and in most regional health divisions, with the highest correlations observed in the Metropolitan and Toledo (r =-0.93; p<0.001) and Paranaguá (r=-0.92, p<0.001) regional health divisions.Conclusion: the results suggest that the increase in the coverage by the Family Health Strategy was an important factor for decrease in the hospitalizations for cardiovascular conditions among residents of the State of Paraná and in most regional health divisions. Other studies should be performed to analyze the factors and causes in regional health divisions where there was no correlation with increase in the Family Health Strategy.

This cross-sectional study aimed to describe the most common correct and incorrect self-administration techniques for insulin using disposable syringes by patients cared for by the Family Health Strategy (FHS), relate the findings to sociodemographic variables and also identify the professional responsible for teaching this technique. A total of 169 patients were selected by simple random sampling in 37 FHS units in a city in the state of Minas Gerais, Brazil from August to October 2006. The results identified errors in all the steps recommended by the American Diabetes Association and Brazilian Diabetes Association for the safe administration of insulin, from hand washing to compression on the injection site. The FHS favors the development of interventions focused on the needs of the clientele registered at the unit, stimulating self-care. Results from this study can contribute to the planning of these interventions.

Objective: From the perspective of professionals acting in the Family Health Strategy (FHS) in the state of Piauí, Brazil, the aim of this study was to asses the articulated actions of technical-pedagogical and clinical-care support offered by the Family Health Support Centres (FHSC) to the FHS’s professionals. Methodology: This is an analytical census retrospective study, with a cross sectional design developed in a quantitative approach with a descriptive and exploratory nature. The research data was collected through the Program of Improvement in Quality of Access in Primary Care (PIAQ-PC) in Brazil, on its second cycle in 2013, and were analysed by using descriptive statistics. Results: The actions of clinical-care support has been further developed by FHSC, all with frequency greater than 85%. In Piauí, the fields where FHSC has showed to be the nutritional care, rehabilitation and maternal and child care and also non-communicative diseases NCD that showed frequency higher than 85%. Conclusion: The FHSC initiative contributes significantly with their services to the FHS to achieve its goals. However, to make the work of these teams more effective there must be ownership of Primary Care Services by its user and appreciation of it by the managers. The developed actions are being supported and agreed on among the matrix support teams.  Keywords: Primary Health Care; Health  Promotion; Family Health.

A Estratégia Saúde da Família (ESF) visa mudar o padrão vigente de uma assistência marcada por ações curativas para outro com maior ênfase na promoção e prevenção. Entretanto, a atividade física, parte integrante dessas ações, nem sempre é de conhecimento dos profissionais da equipe mínima da ESF. Neste sentido, com a criação do Núcleo de Apoio a Saúde da Família, o profissional de Educação Física (PEF) passou a ter maior destaque nas práticas de saúde em geral, compondo a equipe de apoio da Unidade de Saúde da Família (USF), garantindo suporte para realização de práticas corporais / atividades físicas (PCAF) inclusive dos demais trabalhadores, uma vez que o ambiente laboral poderá afetar negativamente seus desempenhos profissionais. Até o presente momento, a literatura não traz referências a respeito das repercussões de uma capacitação com esse tema em profissionais de diferentes áreas de uma USF. Assim, este estudo de campo exploratório de caráter qualitativo objetivou analisar as repercussões de uma capacitação realizada por um PEF sobre assuntos referentes às PCAF na saúde e afetividade de profissionais em uma USF. Participaram nove profissionais atuantes em uma USF, selecionados por amostragem deliberada por variedades de tipo. Para a coleta de dados, foi realizada entrevista semidirigida. Para a análise e interpretação dos dados, foi utilizada a análise de conteúdo apoiada no referencial teórico do cuidando do cuidador mediante trajetória ideográfica-nomotética. A capacitação realizada por um PEF sobre assuntos referentes às PCAF em uma USF implicou em repercussões positivas na saúde e afetividade dos profissionais participantes.Palavras-chave: Educação Física e Treinamento. Saúde do Trabalhador. ABSTRACT: The Family Health Strategy (ESF) aims to change the assistance standard, which was marked by curative actions, to a new way to offer care with emphasis in health promotion and prevention. However, physical activity is an essential component of these actions, and it is not always a common knowledge among the minimum ESF team. Therefore, with the implementation of the Support Nucleus to the Family Health Strategy, the Physical Education professional (PEF) started playing a relevant role in general health practices, who is indicated to be present in the support team to the Family Health Unit (USF), ensuring support to physical activity practices (PCAF), including for the other team members, since workplace may affect negatively their performance. Nevertheless, until the present moment, literature regarding the long-term repercussions of an educational training to USF professionals were not found. The aim of this exploratory qualitative study was to analyse the health and affectivity repercussions of a PCAF educational training provided by a PEF and offered to professionals in an USF. Nine USF professionals participated, who were selected through multiple-types deliberated sampling. Semi-structured interviews were conducted. Content analysis was used to interpret the data, supported by caring the carer theoretical referential, using an ideographic-nomothetic trajectory. An educational training conducted by a PEF regarding PCAF in a USF led to positive health and affectivity repercussions on participants.Keywords: Physical Education and Training. Introduction The guided interventions in drug therapies contribute to the effectiveness of health promotion in the Family Health Strategy (FHS). Objective To evaluate the effectiveness of an action grounded in physical activity in heath state of users in the Unified Health System [Sistema Único de Saúde – SUS] of the Family Health Strategy of the Municipality of Iguaí (BA). Methodology Intervention study with an epidemiological approach, studies in the community, sample of 30 registered users on FHU aged ≥ 60 years old. Data collection was performed using an instrument that contained sociodemographic information, and evaluation of blood pressure and glucose levels, weight and height for later calculation of Body Mass Index (BMI). The physical activity program was conducted during the period of 10 months, with a frequency of 5 days/weeks. The activities were carried out in a city of FHU of Iguaí (BA). The data from this study were tabulated and analyzed using SPSS 15.0. Results After the intervention, there was a statistically significant reduction in levels in the blood pressure and glucose levels, and reduction in body mass index, indicating the importance of this intervention as a form of non-pharmacological treatment for hypertension and diabetes. Conclusion We observed the effectiveness of the intervention was possible to reduce risk factors, but it is necessary that the projects have sustainability, to continue with its development.

OBJECTIVE: To evaluate the trend of hospitalizations due to primary care sensitive conditions and its relationship with the Family Health Strategy coverage. METHODS: Ecological study of time series using the records from the Hospital Information System, from 2005 to 2015, with data for the state of Goiás, Brazil. Trend analyses were performed by the generalized linear regression method of Prais-Winsten with robust variance, which allowed to verify if the trend of hospitalizations due to primary care sensitive conditions was stationary (p > 0.05), declining (p < 0.05 and negative regression coefficient), or ascending (p < 0.05 and positive regression coefficient) in each region of Goiás and for each diagnosis group, stratified by sex. Pearson correlation was used to verify the degree of association between the Family Health Strategy coverage and the rate of hospitalizations due to primary care sensitive conditions. RESULTS: Hospitalizations due to primary care sensitive conditions accounted for 1,092,070 (30.0%) of hospitalizations in Goiás. The average hospitalizations rate due to primary care sensitive conditions was statically less than the rate for other conditions in the analyzed period (167.6% against 386.2%; t = -13.18; p < 0.001). There has been a downward trend in hospitalizations trend due to primary care sensitive conditions in Goiás and in most health regions. The trends varied between sexes in the groups of causes. We observed a negative correlation between the Family Health Strategy coverage and the hospitalizations trend due to primary care sensitive conditions in the state and also in most health regions. CONCLUSIONS: Hospitalizations due to primary care sensitive conditions had a significant reduction trend over the analyzed period. Despite this progressive decrease, this rate remains high and the reduction trend was not linear for all causes. These results allow for directing public policies, while drawing a general overview of hospitalizations due to primary care sensitive conditions by sex and region in the state.
