ABSTRACT: Throughout pregnancy, maternal hemodynamic adaptation is needed to ensure proper uterine perfusion and fetal development. When the uteroplacental vascular system is formed, starting with reduced resistance to uterine arterial flow, this results in decreased total vascular resistance, an activation of neuroendocrine vasoactive peptides, an increase in circulating blood and changes in the cardiovascular system morphophysiology to respond to the increasing demands of uterine perfusion. There has been considerable study of hemodynamic adaptation in pregnant women and this assessment has become a diagnostic tool for fatal obstetric disorders. However, in bitches the available information in this regard is limited; therefore a parallel was drawn between other species of animals and women, in order to subsidize the paucity of information about this process and facilitate the understanding of maternal-fetal hemodynamic adaptation in pregnant bitches. This review and literature analysis aimed\ to discuss morphophysiological cardiovascular adaptations during pregnancy and the possible disorders that can affect this process in pregnant female dogs.

Smoking during pregnancy can endanger pregnancy and the fetus, especially the health of pregnant women and fetal development in the womb. One of the complications of pregnancy that causes fetal death is due to oxygenation disorders. In Indonesia, more than half of households have at least one smoker, and almost all smokers’ smokes at home. The cause of neonatal death is fetal death in utero, asphyxia or respiratory problems due to smoking and premature. This study aims to determine the knowledge of pregnant women about the dangers of smoking to pregnancy and the fetus in Gasaribu Village, Laguboti, Toba Samosir Regency. This research was conducted in September-October 2019 using adescriptive research design. The number of samples was in this study were 40 pregnant women. Sampling is done by using saturated sampling. The results showed that good knowledge of 16 people (40%) was enough 22 people (55%) and less 2 people (5%).  The results of this study are expected to be used as additional material in adding knowledge and information to increase real health education (real) to the public about the dangers of smoking to pregnancy and the fetus.

Objective. To increase the effectiveness of treatment of placental insufficiency in pregnant women with the inclusion of L-carnitine (Inestom) in complex therapy. Materials and methods. A total of 76 pregnant women aged 18 to 39 years were examined at 2240 weeks of gestation. All the examined patients were divided into two groups. Patients of the first group (n = 37) received only basic treatment, which included the use of a complex of vasodilators, tocolytics, drugs that improve microcirculation and rheological properties of blood. Patients of the second group (n = 39) received L-carnitine along with basic therapy (Inestom, Help SA Pharmaceuticals, Greece). Results. The use of the drug Inestom as a part of the complex treatment of placental insufficiency led to the normalization of utero-placental-fetal blood flow and intrauterine fetal development in 94.7 % of pregnant women. After the course of drug therapy, there was an increase in the total score of cardiotography, which in the first group of patients was 7.08 0.06 before treatment and 7.13 0.11 after treatment (p 0.05), and in the second group 7.12 0.11 before treatment and 7.95 0.05 after (p 0.05). The results of antenatal dopplerometry showed that in pregnant women who received Inestom as a part of the complex treatment, there was a decrease in the mean values of all vascular resistance indices. Conclusions. The use of Inestom in the complex treatment of placental insufficiency has a positive effect on the state of hemodynamics in the "mother-placenta-fetus" system, and affects the intrauterine state of the fetus.

AbstractObjectivesTo characterise the diets of pregnant women in the Republic of Seychelles and to determine the contribution of fish to intakes of nutrients important for fetal and neonatal development.DesignObservational, prospective study.SettingSeychelles Child Development Centre, Mahé, Republic of Seychelles.Subjects and methodsPregnant women (n 300) were recruited at their first visit to an antenatal clinic. At 28 weeks’ gestation subjects completed a 4 d diet diary (n 273) and intakes were analysed using dietary analysis software.ResultsMean (sd) energy intake was 9·0 (2·5) MJ/d and fat intakes were higher than UK recommendations for almost two-thirds of the cohort. Fish consumption was lower than in previous surveys, suggesting a move towards a more Westernised diet. Low intakes of a number of nutrients important during pregnancy for fetal development (Fe, Zn, Se and iodine) were observed. However, women who met the current recommendations for these nutrients consumed significantly more fish than those who did not (97 v. 73 g/d).ConclusionsThe present study highlights the importance of fish in the diet of pregnant Seychellois women for ensuring adequate intakes of micronutrients important in fetal development. Dietary patterns in Seychelles, however, are in a state of transition, with a move towards a Western-style diet as evidenced by higher fat and lower fish intakes. If these dietary trends continue and fish consumption declines further, micronutrient status may be compromised. These findings suggest caution in establishing public health policies that promote limitation of fish intake during pregnancy.

BACKGROUND Hospital-based health promotion resources to assist pregnant women in adopting a healthy lifestyle and optimizing gestational weight gain are important, but with limited effects. Increasingly, women are using mobile apps to access health information during the antenatal period. OBJECTIVE The aims of the study were to investigate app-usage by Chinese women during pregnancy and to gain a better understanding of their views and attitudes toward apps containing health information. METHODS A mixed methods study design was applied. Study participants were recruited from 2 maternity hospitals in Shanghai, China, between March and July 2018. A self-administered Web-based survey was conducted with 535 pregnant Chinese women on their sources of health information and reasons for using apps during pregnancy. A total of 4 semistructured focus groups were also conducted with the pregnant women (n=28). RESULTS The use of pregnancy-related apps and the internet was common among the respondents. Almost half of the women had used pregnancy-related apps. Specifically, the use of apps for health information declined as pregnancy progressed from 70% (35/50) in the first trimester to 41.3% (143/346) in the third trimester. The main reason for using an app was to monitor fetal development (436/535, 81.5%), followed by learning about nutrition and recording diet in pregnancy (140/535, 26.2%). The women found that the apps were useful and convenient and can support lifestyle modifications during pregnancy. However, some apps also contained misinformation or incorrect information that could cause anxiety as reported by the participants. Many women expressed the need for developing an app containing evidence-based, well-informed, and tailored health information to support them during pregnancy. CONCLUSIONS The study suggests that apps were widely used by many Chinese women during pregnancy to monitor fetal development, to obtain diet and physical activity information, and to track their body changes. The women highly appreciated the evidence-based information, expert opinions, and tailored advice available on apps. Smartphone apps have the potential to deliver health information for pregnant women.

The article presents three clinical observations of successful conservative treatment of pregnant women with monochorionic, diamniotic twins (MDT), coming naturally. In all cases, there were manifestations of twin-to-twin transfusion syndrome (TTTS) with progressive blood flow disorders in the utero-placental space with a pronounced delay in the development of one of the fetuses. In the early stages of pregnancy was supposed to terminate the pregnancy, but the women refused. Pregnant women were admitted to the perinatal center at 23, 23 and 24-25 weeks, respectively. During the examination, the diagnosis was confirmed, at the same time in all cases, the patients found manifestations of endotoxicosis and inflammatory response. Pregnant conducted methods of detoxicational (efferent) therapy in the form of photomodification of blood with sessions of laser and ultraviolet radiation in combination: in the first case — plasmaexchange with 5% albumin solution No 6 and the cascading plasmafiltration No 5; in the second observation - with medium volume plasmapheresis No 7, hemosorption No 1; in the 3rd observation – with plasmapheresis No 7, hemosorption No 3. Complications at efferent therapy was not, indices of homeostasis of pregnant women in the dynamics normalized. Blood flow disorders in the placenta persisted, but there was a steady increase in body weight of the fetus. In general, women’s pregnancies were prolonged for 11, 13 and 9 weeks, respectively (in the first two cases, women were even temporarily discharged home). Childbirths in all cases were performed by operational pathway, there were no complications in childbirth and after childbirth in mothers. All six children were born alive with mild and moderate prematurity and did not need long-term intensive care. All were on breast-feeding, in dynamics surveillance caught up with contemporaries in development.

Pregnancy is a physiological condition. During pregnancy there is discomfort which is a result of the physiological adaptation process of the mother's body system to her pregnancy. Discomfort that is still within normal limits can turn out to be abnormal due to the mother's unpreparedness for her pregnancy. Healthy pregnant women will have an impact on optimal fetal development. Labor preparation  also starts from pregnancy. Prental yoga is a promotive and preventive effort to improve the health of the mother so that she can adapt to physical and psychological changes during pregnancy and prepare for labor. Prenatal yoga join by pregnant women starting from the second trimester, done every Tuesday and Friday for 90 minutes. Each session starts with pranayama (breathing) practice, asana practice and kegel exercise. At the end, a relaxation and positive affirmation exercise was carried out. The results achieved in this activity are (1) increased physical fitness of pregnant women. (2) increasing the psychological readiness of mothers.
