ABSTRACT: Throughout pregnancy, maternal hemodynamic adaptation is needed to ensure proper uterine perfusion and fetal development. When the uteroplacental vascular system is formed, starting with reduced resistance to uterine arterial flow, this results in decreased total vascular resistance, an activation of neuroendocrine vasoactive peptides, an increase in circulating blood and changes in the cardiovascular system morphophysiology to respond to the increasing demands of uterine perfusion. There has been considerable study of hemodynamic adaptation in pregnant women and this assessment has become a diagnostic tool for fatal obstetric disorders. However, in bitches the available information in this regard is limited; therefore a parallel was drawn between other species of animals and women, in order to subsidize the paucity of information about this process and facilitate the understanding of maternal-fetal hemodynamic adaptation in pregnant bitches. This review and literature analysis aimed\ to discuss morphophysiological cardiovascular adaptations during pregnancy and the possible disorders that can affect this process in pregnant female dogs.

We used Gaussian modeling to depict the changes in finger photoplethysmographic (PPG) pulse during pregnancy in healthy women. We enrolled 70 healthy pregnant women and recorded their PPG pulses in 11–13 gestational weeks, 20–22 gestational weeks, and 37–39 gestational weeks. Three independent positive Gaussian functions were utilized to decompose the pulses, and each Gaussian function extracted three key parameters: the peak amplitude ([Formula: see text]), the peak position ([Formula: see text]), and the half-width ([Formula: see text]). The method of ANOVA and post-hoc multiple comparisons of mathematical statistics were utilized to study the differences of these parameters between the three trimesters. We found that in the first trimester [Formula: see text] increased significantly ([Formula: see text]: [Formula: see text] versus [Formula: see text], [Formula: see text]). [Formula: see text] and [Formula: see text] increased in the first trimester ([Formula: see text]: [Formula: see text] versus [Formula: see text], [Formula: see text]; [Formula: see text]: [Formula: see text] versus [Formula: see text], [Formula: see text]), then decreased significantly ([Formula: see text]: [Formula: see text] versus [Formula: see text], [Formula: see text]: [Formula: see text] versus [Formula: see text], [Formula: see text]). [Formula: see text] is associated with cardiac output, and [Formula: see text] and [Formula: see text] are associated with peripheral vascular resistance. The results of this study were consistent with the conclusion that healthy pregnant women exhibited high flow state of the cardiovascular system and their peripheral vascular resistance decreased first and then gradually recovered during pregnancy. This study indicated that PPG pulse could also reflect the changes in the maternal cardiovascular system during pregnancy.

The main data on general issues of epidemiology, etiology, pathogenesis and classification of chronic glomerulonephritis (CGN) in Ukraine were described in the article. The main information on the peculiarities of this clinic disease in pregnant women, on the course and complications of pregnancy, features of fetal development in pregnant women suffering from chronic pyelonephritis were highlighted. Separately, the questions devoted to the optimal tactics of diagnosis and treatment of CGN in pregnant women with characteristics of medicines and their groups applicable in this category of patients are disclosed. The rules for management of pregnancy, delivery and postpartum period are described. Key words: сhronic glomerulonephritis, pregnancy, treatment.

Autoregulatory dynamics of the cardiovascular system play an important role in maintaining oxygenated blood transportation throughout the human body. In this work, a feedback dynamics model of the cardiovascular system with respect to heartrate and peripheral vascular resistance effects on longer-term blood pressure changes in the systemic circulation is presented. The model is identified from data taken from a swine test subject, instrumented in part with a wearable, non-invasive sensor for estimating peripheral arterial radius. Comparative simulations for the open and close loop model highlight significantly changed hemodynamics after hemorrhage.

Smoking during pregnancy can endanger pregnancy and the fetus, especially the health of pregnant women and fetal development in the womb. One of the complications of pregnancy that causes fetal death is due to oxygenation disorders. In Indonesia, more than half of households have at least one smoker, and almost all smokers’ smokes at home. The cause of neonatal death is fetal death in utero, asphyxia or respiratory problems due to smoking and premature. This study aims to determine the knowledge of pregnant women about the dangers of smoking to pregnancy and the fetus in Gasaribu Village, Laguboti, Toba Samosir Regency. This research was conducted in September-October 2019 using adescriptive research design. The number of samples was in this study were 40 pregnant women. Sampling is done by using saturated sampling. The results showed that good knowledge of 16 people (40%) was enough 22 people (55%) and less 2 people (5%).  The results of this study are expected to be used as additional material in adding knowledge and information to increase real health education (real) to the public about the dangers of smoking to pregnancy and the fetus.

СОСТОЯНИЕ ГЕМОДИНАМИКИ У БЕРЕМЕННЫХ С ОЖИРЕНИЕМ - В данной статье представлены изменения состояния гемодинамики у беременных с ожирением при неосложнённой беременности. В исследование включены результаты комплексного обследования 84 беременных в возрасте от 23 до 28 лет с ожирением различной степени выраженности при сроках гестации 36-38 недель. В контрольную группу вошла 21 беременная аналогичного возраста и срока гестации с нормальной массой тела. У всех наблюдаемых пациентов беременность согласно консультациям акушер-гинекологов была признана неосложнённой. Степень ожирения оценивали по индексу массы тела. Проведённое исследование показало, что избыточный вес тела по мере её прогрессирования оказывает крайне неблагоприятное влияние на гемодинамику и функциональное состояние сердечно-сосудистой системы в целом, снижает коронарные резервы. Наиболее выраженные нарушения функционального состояния сердечно-сосудистой системы имеют место при ожирении II и III степеней.<br />СТАН ГЕМОДИНАМІКИ У ВАГІТНИХ З ОЖИРІННЯМ - Уданій статті представлено зміни стану гемодинаміки у вагітних з ожирінням при неускладненій вагітності. У дослідження включені результати комплексного обстеження 84 вагітних у віці від 23 до 28 років з ожирінням різного ступеня вираження при термінах гестації 36-38 тижнів. У контрольну групу ввійшла 21 вагітна аналогічного віку та терміну гестації з нормальною масою тіла. У всіх спостережуваних пацієнтів вагітність згідно з консультаціями акушер-гінекологів була визнана неусклад- неною. Ступінь ожиріння оцінювали за індексом маси тіла. Проведене дослідження показало, що надлишкова маса тіла в міру її прогресування украй несприятливо впливає на гемо- динаміку і функціональний стан серцево-судинної системи в цілому, знижує коронарні резерви. Найбільш виражені порушення функціонального стану серцево-судинної системи мають місце при ожирінні II і III ступенів.<br />HEMODYNAMIC STATUS OF PREGNANT WOMEN WITH OBESITY - In this article are represented the changes in hemodynamic status of pregnant women with obesity in uncomplicated pregnancy. In the study was included the full survey results of 84 pregnant women aged between 23 and 28 years with obesity of varying severity at 36-38 weeks gestation. The control group included 21 pregnant women of similar age and gestational age with normal body weight. All observed patients, pregnancy according obstetrical consultation was recognized uncomplicated. The degree of obesity was assessed by body mass index. Researchers conducted showed that overweight as its progression has a very adverse effect on hemodynamics and functional state of the cardiovascular system as a whole, reduces coronary reserve. The most pronounced violation of the functional state of the cardiovascular system occurs in obesity 2nd and 3rd degree.<br />Ключевые слова: беременность, ожирение, гемодинамика.<br />Ключові слова: вагітність, ожиріння, гемодинаміка

Nuclear factor of activated T cells (NFAT) is a transcription factor with a multidirectional regulatory function, that is widely expressed in immune cells, including cells in the cardiovascular system, and non-immune cells. A large number of studies have confirmed that calcineurin/NFAT signal transduction is very important in the development of vascular system and cardiovascular system during embryonic development, and plays some role in the occurrence of vascular diseases such as atherosclerosis, vascular calcification, and hypertension. Recent in vitro and in vivo studies have shown that NFAT proteins and their activation in the nucleus and binding to DNA-related sites can easily ɨnduce the expression of downstream target genes that participate in the proliferation, migration, angiogenesis, and vascular inflammation of vascular wall related cells in various pathophysiological states. NFAT expression is regulated by various signaling pathways, including CD137-CD137L, and OX40-OX40L pathways. As a functionally diverse transcription factor, NFAT interacts with a large number of signaling molecules to modulate intracellular and extracellular signaling pathways. These NFAT-centered signaling pathways play important regulatory roles in the progression of atherosclerosis, such as in vascular smooth muscle cell phenotypic transition and migration, endothelial cell injury, macrophage-derived foam cell formation, and plaque calcification. NFAT and related signaling pathways provide new therapeutic targets for vascular diseases such as atherosclerosis. Hence, further studies of the mechanism of NFAT in the occurrence and evolution of atherosclerosis remain crucial.

Objective. To increase the effectiveness of treatment of placental insufficiency in pregnant women with the inclusion of L-carnitine (Inestom) in complex therapy. Materials and methods. A total of 76 pregnant women aged 18 to 39 years were examined at 2240 weeks of gestation. All the examined patients were divided into two groups. Patients of the first group (n = 37) received only basic treatment, which included the use of a complex of vasodilators, tocolytics, drugs that improve microcirculation and rheological properties of blood. Patients of the second group (n = 39) received L-carnitine along with basic therapy (Inestom, Help SA Pharmaceuticals, Greece). Results. The use of the drug Inestom as a part of the complex treatment of placental insufficiency led to the normalization of utero-placental-fetal blood flow and intrauterine fetal development in 94.7 % of pregnant women. After the course of drug therapy, there was an increase in the total score of cardiotography, which in the first group of patients was 7.08 0.06 before treatment and 7.13 0.11 after treatment (p 0.05), and in the second group 7.12 0.11 before treatment and 7.95 0.05 after (p 0.05). The results of antenatal dopplerometry showed that in pregnant women who received Inestom as a part of the complex treatment, there was a decrease in the mean values of all vascular resistance indices. Conclusions. The use of Inestom in the complex treatment of placental insufficiency has a positive effect on the state of hemodynamics in the "mother-placenta-fetus" system, and affects the intrauterine state of the fetus.

In France, almost 23,000 cases of measles and 10 deaths have been reported between January 2008 and August 2012. French health authorities recommend delivery of human polyvalent immunoglobulins in the event of exposure to a measles case for some categories of unvaccinated persons (children under the age of 12 months, immunocompromised persons and pregnant women), within six days after exposure and following laboratory confirmation of the contact case. We carried out a postal survey among 368 French hospital pharmacies to evaluate the number of persons affected by this measure between 1 January 2010 and 31 August 2011, to describe the characteristics of these patients and to evaluate the application of the recommendations in terms of delay between exposure and immunoglobulin delivery, and confirmation of the contact case. The response rate to the survey was 73%. In total, 400 immunoglobulin deliveries were listed, most of them for children under the age of one year, and 84% of the 250 administrations with available information occurred within six days after exposure, as recommended. However, only 48% of the 209 treated contacts with available information were laboratory-confirmed when the immunoglobulins were delivered. This survey is the first evaluation of this recommendation since its introduction in 2005 and suggests that the recommendations may need to be updated.

The existence of a homeostatic state of stresses and strains has been axiomatic in the cardiovascular system. The objective of this study was to determine the distribution of circumferential stress and strain along the aorta and throughout the coronary arterial tree to test this hypothesis. Silicone elastomer was perfused through the porcine aorta and coronary arterial tree to cast the arteries at physiological pressure. The loaded and zero-stress dimensions of the vessels were measured. The aorta (1.8 cm) and its secondary branches were considered down to 1.5 mm diameter. The left anterior descending artery (4.5 mm) and its branches down to 10 μm were also measured. The Cauchy mean circumferential stress and midwall stretch ratio were calculated. Our results show that the stretch ratio and Cauchy stress were lower in the thoracic than in the abdominal aorta and its secondary branches. The opening angle (θ) and midwall stretch ratio (λ) showed a linear variation with order number ( n) as follows: θ = 10.2 n + 63.4 ( R2 = 0.989) and λ = 4.47 × 10−2 n + 1.1 ( R2 = 0.995). Finally, the stretch ratio and stress varied between 1.2 and 1.6 and between 10 and 150 kPa, respectively, along the aorta and left anterior descending arterial tree. The relative uniformity of strain (50% variation) from the proximal aorta to a 10-μm arteriole implies that the vascular system closely regulates the degree of deformation. This suggests a homeostasis of strain in the cardiovascular system, which has important implications for mechanotransduction and for vascular growth and remodeling.
