Context: The molecular pathogenesis of sporadic insulinomas is unknown. There is a lack of biomarker to distinguish benign and malignant form of insulinoma. Objective: Our objective was to confirm the occurrence of microsatellite instability (MSI) in insulinomas, to identify alterations of mismatch repair (MMR) genes in the tumors, and to evaluate the possibility to distinguish benign and malignant insulinoma or to predict the clinical outcome of patients with these alterations. Design and Patients: We detected MSI and inactivation of MLH1 gene in 55 sporadic insulinomas by PCR, immunohistochemical staining, allelic typing, analysis of promoter methylation, and exon mutations. Their correlations with clinicopathological characteristics were analyzed with univariate and multivariate statistic analysis. Results: A high rate of MSI (MSI-H) was found in 33% of sporadic insulinomas. Reduced expression of mutL homolog 1 (MLH1) protein was observed in 36% of insulinomas and correlated with MSI-H (P = 0.008). Promoter methylation and loss of heterozygosity of MLH1 gene was found in 31 and 49% of insulinomas, respectively. Reduced expression of MLH1 and MSI-H were significantly associated with both tumor malignancy (P = 0.033 and P = 4.8 × 10−6, respectively) and incurable disease (P = 0.006 and P = 0.001, respectively). Conclusion: High frequency of MSI occurred in sporadic insulinomas. The silencing of MLH1 gene may partially contribute to the MSI-H in the tumors. Assessing MSI-H and expressions of MLH1 could be used to distinguish benign and malignant insulinomas and to predict the outcome of patients. Detecting of a high rate of microsatellite instability can be used to distinguish malignancy from benign, and predict clinical outcome of the sporadic insulinomas.

The complex hydrogeomorphological processes within the active channel of rivers strongly influence riparian vegetation development and organization, particularly in mountain streams where such processes can be remarkably impacted by engineering control works. In four mountain reaches of Calabrian fiumaras we analyze, through previously arranged methods (integrated by a multivariate statistic analysis), the relationships among hydrogeomorphological river characteristics and structure and the development of riparian vegetation within the active channel in transects located in proximity of check dams and in less disturbed sites. The results of this study demonstrate clear and relevant contrasts, due to the presence of check dams, in the physical and vegetation properties of upstream, downstream and intermediate sites around check dams. The multivariate statistical approach through the Principal Component Analysis (PCA) highlighted evident relationships in all transects between groups of physical and vegetation properties. The regression analysis performed between the vegetation properties and the width:depth ratio or the specific discharge showed very different relationships between groups of transects, due to evident changes in channel morphology and in flow regime locally induced by check dams. Overall we have shown that check dams have far reaching effects in the extent and development of riparian vegetation of mountain torrent reaches, which extend far beyond physical adjustments to changed morphological, hydraulic and sedimentary conditions.

This research aims to facilitate and assist student learning in finding, gathering and analyzing what is needed to develop spiritual potential, self-control, personality, intelligence, noble character, by using seamless mobile media to get the required help in his life. Integration with guided inquiry learning the values of Islam and science in the phenomenon of natural at millennial students utilize information technology of learning resources available environment. The concurrent triangulation strategy is done by collecting quantitative and qualitative data concurrently, then comparing the two databases related to the presence or absence of convergence, differences, or combination. Based on this multivariate statistic analysis, there are differences in learning outcomes between millennial students who use seamless mobile media and non-seamless mobile media. Efforts to integrate Islamic values and science are realized learning outcomes that are; 1) wisdom of greatness, 2) usefulness in life, 3) artificial projects, 4) written reports, 5) responsibilities, 6) accuracy, 7) curiosity. The results of learning patterns that integrate Islamic values and science using seamless mobile media increase students’ skills, abilities, and knowledge. The millennial students can complete artificial projects and report very well their meaningful learning activities to have gratitude to all who made God’s creation on Earth. Efforts to integrate Islamic values and science essential to develop knowledge and ethical character. In Islam, every human being born in a sacred environment, and education becomes a deciding factor in the quality of a child’s religion by providing learning of meaning to improve the quality of millennial students.

Objective: To determine the relationship between hopelessness, loneliness and the level of family health in school-age adolescents with and without suicidal ideation to deepen in the knowledge of this phenomenon from a nursing point of view and to pose promotion and prevention options in Bogotá, Colombia. Method: This was a cross-sectional, comparative, correlational, descriptive and quantitative study. The sample was selected in the period between February and April 2009, through mixed sampling and 482 school-attending 14-17 year-old adolescents were included. Widely recognized and highly qualified psychometric tools were used to measure the variables concerned. Univariate, bivariate and multivariate statistic analysis were used to describe the behavior of each variable, to explore the relationships and contrasts between these variables and to estimate the effect of the independent variables on the presence of suicidal thoughts. This study has followed the ethical considerations necessary for research involving human beings. Results: 20% of the sample used reported the presence of suicidal thoughts or desires. All variables studied influence the presence of suicidal thoughts except age, socioeconomic strata, borough and type of school. However, with the use of logistic regression methods, the variables that best express the presence of suicidal thoughts are: previous suicide attempt, low self-esteem, depression and belonging to an unhealthy family. Conclusions: Data on suicidal ideation and attempts made in this study support the idea of implementing greater preventive efforts within families and schools. Identifying adolescents at risk of suicide or mental health disorders, and build effective prevention and treatment programs is a priority.
