Resistance training and HIIT improve cardiometabolic health in high risk older adults

Progressive resistance training (PRT) and high-intensity interval training (HIIT) improve cardiometabolic health in older adults. Whether combination PRT+HIIT (COMB) provides similar or additional benefit is less clear. This systematic review with meta-analysis of controlled trials examined effects of PRT, HIIT and COMB compared to non-exercise control in older adults with high cardiometabolic risk. Databases were searched until January 2021, with study quality assessed using the PEDro scale. Risk factor data was extracted and analysed using RevMan V.5.3. We analysed 422 participants from nine studies (7 PRT, n=149, 1 HIIT, n=10, 1 COMB, n=60; control n=203; mean age 68.1±1.4 years). Compared to control, exercise improved body mass index (mean difference (MD) -0.33 [-0.47, -0.20], p≤0.0001), body fat % (standardised mean difference (SMD) -0.71 [-1.34, -0.08], p=0.03), aerobic capacity (SMD 0.41 [0.05, 0.78], p=0.03), low-density lipoprotein (SMD -0.27 [-0.52, -0.01], p=0.04), and blood glucose (SMD -0.31 [-0.58, -0.05], p=0.02). Therefore, PRT, HIIT and COMB can improve cardiometabolic health in older adults with cardiometabolic risk. Further research is warranted, particularly in HIIT and COMB, to identify the optimal exercise prescription, if any, for improving older adults cardiometabolic health.

ABSTRACT Background: High intensity interval training (HIIT) is a protocol of short work intervals of vigorous to high intensity interspersed with active or passive (cessation of movement) recovery periods. HIIT has been employed since the mid-20th century to improve athletic exercise performance. Regular exercise reduces elevated low-density lipoprotein (LDL), atherosclerosis formation, and risk factors of cardiovascular disease (CVD). This study aimed to examine the effect of high-intensity interval training on low density lipoprotein level in patients with type 2 diabetes mellitus (DM). Subjects and Method: This was meta-analysis and systematic review. The study was conducted by search published article from year 2010 to 2020 in PubMed, Science Direct, Research Gate, and Google Scholar databases. The inclusion criteria were full text, using randomized controlled trial study design, high-intensity interval training intervention, and reporting mean and standard deviation. Study subjects were type 2 DM patients aged 25-65 years. The study outcome was LDL reduction. The articles were analyzed by PRISMA flow chart and Revman 5.3. Results: 7 studies from America, Europe, Australia, and Asia showed that high intensity interval training reduced LDL level in type 2 DM patients (Mean Difference= -0.06; 95% CI= 1.32 to -0.47; p<0.001) with I2= 92% (p <0.93). Objectives: To use a quantitative approach to examine the effects of high-intensity interval training (HIIT) interventions on sleep for adults. Methods: PubMed, Ebsco, Web of Science, Cochrane Central Register of Controlled Trials, the China National Knowledge Infrastructure, and Wanfang Data were searched from their inception to December 2020. Intervention studies with a control group that examined the effects of HIIT interventions on sleep were included in this meta-analysis. The risk of bias was assessed using the tool provided by the Cochrane Handbook for Systematic Reviews of Interventions. Effect sizes (ESs), calculated as weighted mean difference (WMD) and standardized mean difference (SMD), were used to examine the effects of objective outcomes and subjective outcomes separately. Results: A large increase in sleep quality (SQ) reflected by the Pittsburgh Sleep Quality Index global scores [WMD = −0.90, 95%CI (−1.72, −0.07), p = 0.03, n = 8] and a small-to-medium favorable effect on sleep efficiency (SE) [SMD = 0.43, 95%CI (0.20, 0.65), p = 0.0002, n = 10] were found after HIIT intervention. In addition, sub-analyses results suggest that ESs were moderated by the type, duration and frequency, as well as the length of the HIIT intervention. Conclusions: HIIT may be a promising way to improve overall subjective SQ and objective SE. PROSPERO, protocol registration number: CRD42021241734.

ObjectiveTo determine the effect of prolonged exposure to a submarine environment on biomarkers of cardiometabolic risk in Royal Navy (RN) submariners.MethodsSerum lipids (cholesterol (C), triglyceride (TG), low-density lipoprotein cholesterol, high-density lipoprotein cholesterol (HDL-C), non-HDL-C), glucose, insulin and anthropometrics were compared within three RN submarine crews before and after submerged patrols of 12 or 6 weeks, and with a crew that remained ashore (SUB-HOME). Dietary intake and activity patterns were self-reported during each patrol. Differences were assessed in crew characteristics using one-way analysis of variance and in serum lipids using paired t-tests.ResultsPostpatrol, the mean body weight of submerged crews decreased (−1.4±4.2 kg, p=0.0001), but increased in SUB-HOME (1.9±1.8 kg, p=0.0001). Modest improvements in serum lipids (mean individual change (mmol/L); C=−0.3±0.7, p=0.0001; TG=−0.3±0.7, p=0.0001; HDL-C=−0.1±0.3, p=0.0001; non-HDL-C=−0.2±0.6, p=0.012), glucose (−0.2±0.5, p=0.0001) and insulin (−1.5±4.6 mU/L, p=0.001) were observed in submerged crews. Changes in serum lipids were positively associated with changes in body weight within crews combined. Energy intake was maintained during submerged patrols but was lower compared with non-submerged (11 139±2792 vs. 9617±2466 kJ, p=0.001; 11 062±2775 vs. 9632±2682 kJ, p=0.003).ConclusionsThe environment of a submerged submarine produced no adverse effects on serum biomarkers of cardiometabolic risk in crew. Conversely, modest improvements in these biomarkers were associated with a decrease in body weight.

Muscle strength after detraining is still higher than the level before training, which is an important issue for middle-aged and older adults. The purpose of the present study was to examine the effects of detraining duration (DD), resistance training duration (RTD), and intensity on the maintenance of resistance training (RT) benefits after detraining in middle-aged and older adults. A systematic search yielded 15 randomized control trails (n = 383) eligible for inclusion. The results showed that RTD ≥ 24 weeks and DD ≥ RTD, the RT benefits were still significantly maintained even with medium and low intensity (standardized mean difference = 1.16, 95% confidence interval, CI [0.38, 1.94], p = .004). When RTD < 24 weeks and DD ≤ RTD, only the high-intensity groups maintained the RT benefits (DD, 4–6 weeks: standardized mean difference = 0.71, 95% CI [0.34, 1.08], p = .0002; DD 8–16 weeks: standardized mean difference = 1.35, 95% CI [0.66, 2.04], p = .0001). However, when DD > RTD, the RT benefits were not maintained even with high intensity. In summary, when RTD was less than 24 weeks, RTD > DD was an important factor in maintaining muscle strength.

Interval training (HIIT / SIT) combined with resistance training (RT) has been highlighted as a strategy for the improvement of health-related physical fitness markers (HRPF) in adults. Thus, the aim of this meta-analysis was to compare the efficacy of combined training (HIIT / SIT + RT) with other exercise protocols on HRPF markers in adults. A systematic search was performed in MEDLINE via PebMed, Cochrane-CENTRAL, SPORTDiscus, LILACS, SCIELO and Scopus databases between January and March 2017, using the following keywords in English and Portuguese: physical fitness, high-intensity interval training, sprint interval training, resistance training and adults. The quality of studies was evaluated using the PEDro scale. After applying both inclusion and exclusion criteria, nine articles were selected (n = 231). The extraction of means and standard deviations from studies was performed independently by two authors and the RevMan software was used to perform the meta-analysis. Combined training interventions lasted from 6 to 12 weeks and generated greater increase in maximal oxygen uptake than other forms of exercise. The combination of interval training and strength training may be considered more effective to improve aerobic capacity levels in adults.
