Construction of a full mouth speculum facilitating oral examinations, bronchoscopy and gastric tubing in elephants

Abstract Objective Here we tested the application of a full mouth speculum to sedated elephants in human care to gain access to the oral cavity, the trachea (bronchi) and esophagus (stomach) and therefore improve diagnostic and therapeutic options in elephant medicine. The construction of this oral speculum for elephants and the procedure are described. Material and methods The oral speculum is a steel construction consisting of 2 bite plates of 0.8 × 60.0 × 8.0 cm attached between 2 threaded guiding poles (40 cm). Through crank handles, the metal plates are dispersed once placed between the elephant‘s jaws in front of the molars. The oral speculum was applied in 26 elephants (6,16 Asian elephants, and 1,3 African elephants) during standing sedation. Results All sedated elephants tolerated the positioning of the mouth opener and subsequent manipulations well. The mouth opener was applied for the following procedures: inspection of the oral cavity (n = 2), placing a stomach tube (n = 16), and/or performing endoscopic examinations such as bronchoscopy (n = 20) and/or gastroscopy (n = 8). Conclusion This method provides a new possibility to open the jaws to gain access to the molars, larynx and pharynx in captive elephants without full immobilization. Valuable samples for diagnostics may be obtained or animals medicated via stomach tube with this application. Clinical relevance The mouth opener provides veterinarains with a new option to perform necessary diagnostic and therapeutic procedures around the oral cavity, airways and stomach in captive elephants during standing sedation with no need for a full anaesthesia.

Treatment of necrotic immature teeth has always been a real challenge for the clinician due to the open apex and weak root structure, which does not allow a conventional endodontic treatment. Several therapeutic options are possible to treat those teeth. Calcium hydroxide apexification is the oldest and most studied therapeutic option, but it has some disadvantages, including the long term of treatment, the possibility of reinfection, and the weakness of the wall. To solve these problems, several authors recommend the revascularization technique. This therapy allows the continuation of root edification with apical closure and thickening of the walls. The aim of our work is to compare the two therapeutic procedures, apexification with calcium hydroxide and revascularization, through a clinical case treated and followed up for 24 months at the pediatric dentistry department in Casablanca, Morocco.

That the mouth and the body are not disconnected should come as no surprise. In this article, the links between periodontal and systemic health will be reviewed by investigating these in both directions, starting with the more familiar effects of systemic conditions on periodontal health and progressing to the emerging effects of periodontal health on systemic health. Where suitable, tips will be provided for practitioners caring for patients with periodontitis and systemic diseases. CPD/Clinical Relevance: In recognition of the important role that the oral cavity plays in the body and the important role that oral healthcare providers play in the management of the general wellbeing of patients, this article highlights the link between periodontitis and systemic heath. This allows practitioners to care more holistically for their patients.

AbstractLaryngeal cysts from 72 patients were examined and reclassified according to a modified working classification. In this series, 47 patients (66 per cent) had epithelial cysts, 11 patients (15 per cent) oncocytic cysts and 14 patients (19 per cent) tonsillar cysts. Epithelial cysts were commonest in the region of the epiglottis (20/47) and laryngeal ventricle (24/47). Oncocytic cysts tended to lie in the region of the ventricle whereas tonsillar cysts occurred almost exclusively in the valleculae, epiglottis and pyriform region (13/14).The authors conclude that the modified working classification of laryngeal cysts is easy to apply, of clinical relevance, and allows classification of cysts where operative trauma to the specimen obscures the relationship of the cyst to the surface epithelium. The origin and significance of tonsillar cysts are discussed and a relationship to the lympho-epithelial cyst of the oral cavity is suggested.

Despite free health care services in Saudi Arabia, the prevalence of caries in children is substantially greater in comparison to other high-income countries. Dental fear in children may be an important issue that needs attention. Therefore, the aim was to investigate the role of dental fear in predicting untreated dental caries in schoolchildren. This analytical cross-sectional study included children aged 8–10 years residing in Saudi Arabia. Dental status via oral examinations was surveyed with the WHO standardized chart and the Children Fear Survey Schedule—Dental Subscale was used to score dental fear. Descriptive, binary, and multivariable logistic regression analyses were performed to report the findings at 5% statistical significance. Overall, there were 798 schoolchildren with an average fear score of 36. Nearly 70.4% reported fear of someone examining their mouth. About 76.9% had at least one carious tooth in their oral cavity. Children with dental fear were 1.8 times (OR = 1.80; 95%CI = 1.26, 2.56) more likely to have at least one untreated carious tooth in their oral cavity than those who did not express fear during oral examinations and dental procedures. Thus, the current study concludes that fear of dentists and dental treatment procedures successfully predicts untreated carious teeth in schoolchildren.

Mukha (oral cavity) is considered as a gateway for the alimentary canal, health of the oral cavity reflects the body health as healthy oral cavity aids in proper digestion of the food consumed. The oral cavity also takes part in bodi- ly functions like respiration and speech. It also contributes to the beauty and confidence of an individual. Mukha is comprised of Osta (lips), Dantamula (Gums), Danta (Tooth), Jinhva (Tongue), Taalu (Palate), Gala (Throat). Since the Oral cavity is constantly exposed to the external environment, there are more chances of trauma and infections. Hence to avoid the chances of oral cavity diseases it is very important to maintain good oral hygiene. Kriyakalpa is the therapeutic procedure applied locally and are considered under bahirparimarjana chikitsa. Ka- vala, Gandusha and Pratisarana are kriyakalpas explained for good oral hygiene. These can be used as a daily regimen to maintain good oral hygiene and in mukharoga as therapeutic procedures. These mukha kriyakalpa- due to their cleaning action and by increasing defence mechanism, promotes oral health and prevents many oral disorders. Various formulations for mukha kriyakalpa are available for a healthy person based on prakriti and for treatment based on vitiated dosha avastha.
