AbstractNo single event hitherto has stumped the world more significantly than the present Corona pandemic. In a short span of a few months, the world economy, healthcare, livelihood, and human survival have been direly threatened by the rapid increase in the number of COVID-19 cases and fatalities. While dispensations across the world are finding ways to tackle the pestilence, the medical fraternity faces a unique conundrum. On the one side, the health care team is at the forefront fighting the disease and saving patients from the grip of death. On the other side, the healthcare industry is facing a crisis in terms of safety of healthcare personnel, difficulties in online tele-healthcare, economic sustainability, challenges in healthcare education and other problems in safe health care delivery. The authors provide a broad perspective at the current pandemic and its implications on orthopaedic practice in the near future.

Increasingly, health is recognized as a major force for economic development; and because economic development is central to political and social stability, health is being looked at as the great hope for the future of the world, as population sizes and disparities among them increase. This perspective has been growing ever since the 1993 World Development Report was released by the World Bank, and it has fueled an intensive scrutiny of health care around the world, focusing on systems and health care delivery on the one hand, and equitable access to the products of research on the other hand. In the middle of all of is this is a concern about how health care (which must include both the training of personnel from the basic low level health care worker to the physician), and research and development (which must include the financing of research in academia and the development of products primarily in the private sector) are organized, and how they do or do not address inequities between and within populations and nations.

We will highlight key events that have shaped the American care delivery system and discuss current actions that lay the foundation for the future of the health care service environment. As professional members of the health care team, our goal is to ensure that we continue to provide high-quality services to those with communication, cognitive-communication, motor speech, voice, and swallowing impairments.

BACKGROUND: An integrated clinical pathway (ICP) is a key method for structuring or planning processes of care, enabling the modernization of health-care delivery and coordination of multiple roles, forming a complete, patient-centered multidisciplinary health-care team and establishing the sequence of activities, promoting individual and team communication, collaboration, networking, and transparency, and reducing the cost of care. AIM: As there is a research gap in the area of communication among members of a multidisciplinary team for the treatment of patients through an ICP, the aim of this study was to determine the impact of communication of a member of a multidisciplinary team on the active participation of an individual in this multidisciplinary team. METHODS: A cross-sectional study of three ICPs, forchronic kidney disease, stroke, and total hip arthroplasty was conducted in a typical Slovenian general hospital. RESULTS: The results show that in the analyzed hospital, two of the three clinical pathways are not yet fully integrated. CONCLUSION: There is a weak influence of staff communication within a multidisciplinary team on an individual’s participation in this multidisciplinary team, indicating the need for various activities to actually implement clinical pathway “integration,” and promote better communication within teams to strengthen participation in multidisciplinary patient care pathways.

AbstractThe importance of interprofessional education and practice has been well documented for all health care disciplines. Our health care delivery system is challenged by the need to prepare health professions graduates with skills that get them ready to function as collaborative members of the health care team. Educators have long struggled to create interprofessional learning environments that would inculcate the needed values and competencies. The purpose of this article is to share one institution's path in developing an integrated context for learning across several disciplines to assure that graduates can fulfill their full professional roles in clinical care, education, advocacy, leadership, and quality improvement. Through a program called IMPACT Practice, the MGH Institute of Health Professions has developed an array of opportunities for students from different programs to interact with each other, emphasizing the collaborative skills that will benefit patients and clients as well as contribute to positive change within the health care system. These opportunities are based on core competencies developed by the Interprofessional Education Collaborative (IPEC) as well as institutional core competencies that go beyond IPEC to address the full professional role. In the second part of this article, the IMPACT experience will be described through the journey of one student in the Communication Sciences and Disorders program.

Abstract Background Integrated primary care teams are ideally positioned to support the mental health care needs arising during the COVID-19 pandemic. Understanding how COVID-19 has affected mental health care delivery within primary care settings will be critical to inform future policy and practice decisions during the later phases of the pandemic and beyond. The objective of our study was to describe the impact of the COVID-19 pandemic on primary care teams’ delivery of mental health care. Methods A qualitative study using focus groups conducted with primary care teams in Ontario, Canada. Focus group data was analysed using thematic analysis. Results We conducted 11 focus groups with 10 primary care teams and a total of 48 participants. With respect to the impact of the COVID-19 pandemic on mental health care in primary care teams, we identified three key themes: i) the high demand for mental health care, ii) the rapid transformation to virtual care, and iii) the impact on providers. Conclusions From the outset of the COVID-19 pandemic, primary care quickly responded to the rising mental health care demands of their patients. Despite the numerous challenges they faced with the rapid transition to virtual care, primary care teams have persevered. It is essential that policy and decision-makers take note of the toll that these demands have placed on providers. There is an immediate need to enhance primary care’s capacity for mental health care for the duration of the pandemic and beyond.

The present study aims to perform a segmentation of patients based on their loyalty behaviour. The analysis focuses on Valencia, a region in Spain that features a capitated financing and free-elective framework; such a framework is particularly suitable for this type of study because patient loyalty directly affects the system’s budget and economic viability. Using secondary data from the regional health council, the study focuses on relationships of influence and latent segmentation in answering seven research questions. The two-pronged statistical analysis is designed to analyse relationships of influence, on the one hand, and latent segmentation, on the other. Significant differences were found among the various scales analysed in the three patient loyalty behavioural models (capture, retention and desertion) for each variable within the scope, that is, subjective (gender, age and nationality) and circumstantial (size of the assigned and receiving hospital, location of the province of the assigned hospital). This finding indicates that it may be possible to develop patient profiles based on such variables to analyse different loyalty behaviours in patients and the impact of hospital communication strategies on these behaviours. Patient loyalty is essential to the viability of a capitated health care financing and management system. Likewise, identifying patient profiles would contribute to a better Valencian public health management. Accordingly, it might be applied to evaluate other health care financing systems.

The federal pandemic influenza plan predicts that 30% of the population could be infected. The impact of this pandemic would quickly overwhelm the public health and health-care delivery systems in the U.S. and throughout the world. Surge capacity for staffing, availability of drugs and supplies, and alternate means to provide care must be included in detailed plans that are tested and drilled ahead of time. Accurate information on the disease must be made available to health-care staff and the public to reduce fear. Spokespersons must provide clear, consistent messages about the disease, including actions to be taken to contain its spread and treat the afflicted. Home care will be especially important, as hospitals will be quickly overwhelmed. Staff must be prepared ahead of time to assure their ability and willingness to report to work, and public health must plan ahead to adequately confront ethical issues that will arise concerning the availability of treatment resources. The entire community must work together to meet the challenges posed by an epidemic. Identification and resolution of these challenges and issues are essential to achieve adequate public health preparedness.
