<p>Droughts and floods due to extreme climate events has caused yield loss in various regions of Indonesia, including the Provinces of Aceh and North Sumatra. An early detection model needs to be developed to anticipate the negative impacts of extreme climate event. The model may describe the association of surplus and rainfall deficits with paddy damage due to drought and flood. We used Standardized Precipitation Index (SPI) to explore drought and flood characteristics in period 1989-2016. The study aimed: (i) to analyze the relationship between SPI and paddy damage due to drought and flood events, (ii) to analyze the critical value of the duration and intensity of SPI which causes paddy damage, and (iii) to determine which districts were prone to drought and flood in the Provinces of Aceh and North Sumatra. The results concluded that SPI-3 and -6 months can better describe the frequency of drought and rice flooding. In addition, drought on paddy occured mostly if the SPI was smaller than -1 which took place within 4-5 months, whereas flood occured if the SPI was greater than 1. Short duration drought (2-3 months) were observed in five districts in Aceh (2) and North Sumatra (3). On other hand, more flood districts were identified (9 districts).</p>

Stroke is a sudden and severe injury to the brain vessels of the blood vessels. injuries can be caused by clogging and narrowing, resulting in insufficient blood supply, causing disability in the form of paralysis of limbs, speech disorders, thought processes, memory and can cause death.In Stroke Hospital Bukittinggi, the incidence of stroke increases by the end of 2016 is the case of inpatient care in Stroke Hospital Bukittinggi as 4,100 people and outpatient in Stroke Hodpital Bukittinggi as many as 11,113 people. The purpose of this study was to determine the knowledge and attitude of the family on the ability of early detection of ischemic stroke. This research method using descriptive method of analysis with crossectional approach by using questioner and then processed data using chi squere test. The sample in this research is 75 people. The result of the statistical test in obtaining the result between the knowledge of the family on the ability of early detection (sign and symptom) in stroke patient P Value = 0,011 (P> α), it can be concluded there is correlation of knowledge to the ability of early detection with value (P = 0.011 OR = 3,927). Based on statistical test result between family attitudes toward the ability of early detection (sign and symptom) got result P value = 0,186 (P <α) this show that there is no relation between family attitude with ability of early detection (sign and symptom) in ischemic stroke patient at Emergency Unit Stroke Hospital Bukittinggi2018. Suggestions in this study can be used as input for the hospital in order to improve health service and able to increase promotive effort by giving education of ischemicstroke attack to the family. Keywords: Early Detection,Knowledge, Family, Attitudes, Ischemic Stroke Acut   Stroke adalah suatu cedera mendadak dan berat pada pembuluh pembuluh darah otak. cedera dapat disebabkan oleh sumbatan dan penyempitan, menyebabkan kurangnya pasokan darah yang memadai, menyebabkan cacat berupa kelumpuhan anggota gerak, gangguan bicara, proses pikir, daya ingat dan dapat menyebabkan kematian.Di RSSN Bukitinggi , angka kejadian stroke meningkat akhir tahun 2016 ditemui kasus rawat inap di RSSN Bukittinggi sebnyak 4.100 orang dan rawat jalan di RSSN Bukittinggi sebanyak 11.113 orang. Tujuan penelitian ini adalah untuk mengetahui hubungan pengetahuan dan sikap keluarga terhadap kemampuan deteksi dini (tanda dan gejala)  serangan stroke iskemik di ruang IGD RSSN Bukittinggi tahun 2018. Metode penelitian ini menggunakan metode deskriptif analisis dengan pendekatan crossectional dengan menggunakan alat ukur kuesioner kemudian data diolah menggunakan uji chi squere. Sampel dalam penelitian ini ini sebanyak 75 orang. Hasil uji statistik di peroleh hasil antara pengetahuan keluarga terhadap kemampuan deteksi dini (tanda dan gejala) pada pasien stroke P Value = 0,011 (P>α) maka dapat disimpulkan ada hubungan pengetahuan terhadapkemampuan deteksi dini dengan nilai (P=0,011 OR= 3,927). Berdasarkan hasil uji statistikantara sikap keluarga terhadap kemampuan deteksi dini (tanda dan gejala) didapatkan hasil P value= 0,186 (P<α) hal ini menunjukan bahwa tidak ada hubungan antara sikap kelurga dengan kemampuan deteksi dini ( tanda dan gejala) pada pasien stroke iskemik di IGD RSSN Bukittinggi2018. Education, Information and Communication (EIC) based on Karo Culture on Knowledge and Attitude of Women of Childbearing Age for the Early Detection of Cervical Cancer at the Village of Cinta Rakyat, Merdeka District, Karo Region. Quasi experiment with non-equivalent pre-test and post-test, using a control group with the intervention of EIC based on Karo culture through counseling in Cinta Rakyat village, Merdeka district and Karo Regency. In order to know the influence of EIC based on Karo culture to knowledge and attitude, this research conducted an independent t-test with normality-test as starting point. This research also employed pair t-test to know two different variables: knowledge and attitude. Purposive sampling was conducted with univariate and bivariate data analysis.There is an influence of EIC based on Karo culture through composed songs and traditional dance called Landek on knowledge and attitude before and after the intervention (p = 0,000). This research found that there is an influence of counseling on knowledge and attitude before and after the intervention (p = 0,000). There is no difference between EIC based on culture through composed songs and dance, and counseling to increase knowledge (p = 0,498). However, EIC based on culture is more effective way than counseling to develop attitude toward cervical cancer prevention (p = 0,027). This research, therefore, argues that health officers/Promkes at the Health Office and at the center of community health/PUSKESMAS are expected to conduct counseling on cervical cancer using composed songs and dance (Landek) for prevention strategy, especially in the area of the center of community health in Karo District.

Background: The service use of visual inspection of cervix with acetic acid (VIA) has been low. This technique designed to detect cervical cancer at its earliest stage to prevent the unnecessary burden of its later stages. This study was aimed at showing the influence of knowledge, attitude and culture on the use of VIA service.Methods: This research was a paired case-control study, conducted in 2019 in in the working area of Kalijudan and Mulyorejo Community Health Centers, Surabaya. Ninety eight female respondents of childbearing age were grouped into control and case groups with 49 respondents respectively. Samples were chosen consecutively. Cases were obtained from the health centres’ records, while controls were chosen from cases’ close neighbours. Interviews were conducted in the respondents’ house using closed questionnaires.Results: The multivariate analysis showed that knowledge (p<0.001), attitudes (p=0.012) and culture (p=0.045) affected the use of VIA early detection services.Conclusion: This study pointed out that knowledge, attitude and cultural factors were influential factors in the use of VIA early detection services. Health workers may improve women’s knowledge and attitude through health promotion, and take into account supportive local cultural factors in the program to enhance the use of VIA.

PurposeBreast cancer is one of the most prevalent malignancies worldwide. This study aimed to compare the level of knowledge and attitude of females regarding Breast cancer and to determine the role of knowledge, attitude and barriers in performing regular self-examination.Design/methodology/approachNon-physician females aged 18 years old or above were enrolled in this cross-sectional study in two groups of usual clients and healthcare staff from January 2018 to January 2019 from a healthcare center in Sari, a major city in the northern district of Iran. A questionnaire was used to score the participants’ knowledge and attitude levels using questions about the participants' knowledge and attitude towards Breast cancer along with their status on Breast self-examination and barriers. Mean scores were used for statistical analysis using SPSS V25. p < 0.05 was considered significant.FindingsA final dataset of 279 females were collected. A significant difference in the knowledge and attitude regarding breast cancer was found between the two study groups (p < 0.001). The practice of BSE was significantly lower in the usual clients compared to non-physician healthcare staff (p < 0.001). The most frequent barriers for not performing a regular BSE were fear of finding a mass in usual clients (17.8%) and lack of confidence in healthcare staff (3.8%).Research limitations/implicationsLimitations include single-centered sample selection.Practical implicationsGiven the importance of early detection in breast cancer prevention and the general taboo regarding breast cancer screening methods in certain parts of the world, leading to poor results in early detection and prevention, the authors believe that it is of superior importance to address and promote positive attitudes in general population towards breast self-examination.Originality/valueGiven the importance of early detection in breast cancer prevention, and the general taboo regarding breast cancer screening methods in certain parts of the world, leading to poor results in early detection and prevention, the authors believe that it is of superior importance to address and promote positive attitudes in general population toward breast self-examination. The decreasing number of MMR and IMR can be achieved if the number of high-risk pregnant women decreases. To anticipate this, an approach should be made through individuals who are closest to the community to provide information about high-risk pregnancies such as health cadres. However, cadres&#39; knowledge and attitudes regarding their roles and duties as assistants for high-risk pregnant women and early detection of high risk are still very low. Therefore, it is necessary to increase the knowledge and attitude of health cadres by using appropriate and effective learning media sources in accordance with their knowledge and needs. The general purpose of this study is to explore and identify the perspectives and experiences of health cadres in providing assistance to high-risk pregnant women. The study uses the qualitative research method . It was conducted in the working area of Rasimah Ahmad Bukittinggi Health Center in July - October 2018. The subjects of this study consisted of health cadres, KIA program designers, and policy makers. The data were collected by using in Depth Interview and Focus Group Discussion. They were analyzed by using interactive analysis method.The result of the study shows that there is still a lack of knowledge of cadres regarding their roles and duties as assistants for high-risk pregnant women and early detection of high risk of pregnancy. This is due to the absence of handbook for cadres in providing information / and counseling to regnant women other than the KIA books they have been using and the experiences they have gained so far. The conclusion of this study is there is lack of learning media sources for cadres in providing services to pregnant women. Hence, the learning media resources are urgently needed as a reference in giving quality assistance.
