One advantage of the more consistent application of diagnostic criteria for identifying individuals with a depressive disorder is that it allows national and international comparisons to be undertaken. Large-scale studies make it possible to estimate the overall prevalence and distribution of depression cases by country, culture, economic and social status, and other demographic features such as age, gender, and marital status. ‘Who is at risk of depression?’ explores the epidemiology of depression (the distribution and determinants of depression-related states), gives examples of the presentation of depression across the lifespan, and discusses the association between physical disorders and depression. Lastly, it highlights some of the current thinking on prevention of suicide.

Background. There have been few large-scale epidemiological studies which have examined the prevalence of bipolar disorder. The authors report 12-month prevalence data for DSM-IV bipolar disorder from the Australian National Survey of Mental Health and Well-Being.Method. The broad methodology of the Australian National Survey has been described previously. Ten thousand, six hundred and forty-one people participated. The 12-month prevalence of euphoric bipolar disorder (I and II) – similar to the euphoric-grandiose syndrome of Kessler and co-workers – was determined. Those so identified were compared with subjects with major depressive disorder and the rest of the sample, on rates of co-morbidity with anxiety and substance use disorders as well as demographic features and measures of disability and service utilization. Polychotomous logistic regression was used to study the relationship between the three samples and these dependent variables.Results. There was a 12-month prevalence of 0·5% for bipolar disorder. Compared with subjects with major depressive disorder, those with bipolar disorder were distinguished by a more equal gender ratio; a greater likelihood of being widowed, separated or divorced; higher rates of drug abuse or dependence; greater disability as measured by days out of role; increased rates of treatment with medicines; and higher lifetime rates of suicide attempts.Conclusions. This large national survey highlights the marked functional impairment caused by bipolar disorder, even when compared with major depressive disorder.

AbstractBackgroundAttention impairment is an under-investigated feature and diagnostic criterion of Major Depressive Disorder (MDD) that is associated with poorer outcomes. Despite increasing knowledge regarding mechanisms of attention in healthy adults, we lack a detailed characterization of attention impairments and their neural signatures in MDD.MethodsHere, we focus on selective attention and advance a deep multi-modal characterization of these impairments in MDD, using data acquired from n = 1008 patients and n = 336 age- and sex-matched healthy controls. Selective attention impairments were operationalized and anchored in a behavioral performance measure, assessed within a battery of cognitive tests. We sought to establish the accompanying neural signature using independent measures of functional magnetic resonance imaging (15% of the sample) and electroencephalographic recordings of oscillatory neural activity.ResultsGreater impairment on the behavioral measure of selective attention was associated with intrinsic hypo-connectivity of the fronto-parietal attention network. Not only was this relationship specific to the fronto-parietal network unlike other large-scale networks; this hypo-connectivity was also specific to selective attention performance unlike other measures of cognition. Selective attention impairment was also associated with lower posterior alpha (8–13 Hz) power at rest and was related to more severe negative bias (frequent misidentifications of neutral faces as sad and lingering attention on sad faces), relevant to clinical features of negative attributions and brooding. Selective attention impairments were independent of overall depression severity and of worrying or sleep problems.ConclusionsThese results provide a foundation for the clinical translational development of objective markers and targeted therapeutics for attention impairment in MDD.

In the last decade unprecedented technological changes have taken place, resulting in the emergence of a fundamentally new economic model. Based on the widespread spread of smartphones, the world has become more “connected”. The digitalization of demand and supply contributed to the creation of entirely new digital markets managed by platform enterprises based on an open business model, that enabled external consumers and producers to connect and interact with each other. A more interconnected world generates vast amounts of data, allowing platform companies to invest in machine learning and artificial intelligence and ultimately improve their efficiency. Finally, a steady digitalization of business processes, markets and global value chains is observed. In these circumstances, approaches to value addition are fundamentally changing in the context of new dimensions of the digital economy, the analysis of which was the purpose of our study. It has been identified, that in the absence of a standardized international methodology for measuring the digital economy, the latter is so far possible on disparate development-left and national statistics. Initiatives taken at the international level to overcome national differential approaches are still insufficient, as there is a lack of statistics and variables related to digital data. It has been revealed, that the lack of quality statistics on key indicators of the digital economy makes it difficult to assess the value added in the world economy scale and international comparisons. Much of the challenges of measuring value added in the digital economy, as shown in the article, are related to the principle of “scale without mass,” the intangible nature of capital, the intense growth of large-scale cross-border data flows, and the emergence of new sources of value creation.

Macroscopic structural abnormalities in the thalamus and thalamic circuits have been shown to contribute to the neuropathology of major depressive disorder (MDD). However, cytoarchitectonic properties underlying these macroscopic abnormalities remain unknown. The purpose of this study was to identify systematic deficits of brain architecture in depression, from structural brain network organization to microstructural properties. A multi-modal neuroimaging approach including diffusion, anatomical and quantitative magnetic resonance imaging (MRI) was used to examine structural-related alternations in 56 MDD patients compared with 35 age- and sex-matched controls. Structural networks were constructed and analyzed using seed-based probabilistic tractography. Morphometric measurements, including cortical thickness and voxel-based morphometry (VBM), were evaluated across the whole brain. A conjunction analysis was then conducted to identify key regions showing common structural alternations across modalities. The microstructural properties, macromolecular tissue volume (MTV) and T1 relaxation times of identified key regions were then calculated. Results showed multiple alterations of structural connectivity within a set of subcortical areas and their connections to cortical regions in MDD patients. These subcortical regions included the putamen, thalamus and caudate, which are predominately involved in the limbic-cortical-striatal-pallidal-thalamic network (LCSPT). Structural connectivity was disrupted within and between large-scale networks, mainly including subcortical networks, default mode networks and salience/ventral attention networks. Consistently, these regions also exhibited widespread volume reductions in MDD patients, specifically the bilateral thalamus, left putamen and right caudate. Importantly, the microstructural properties, T1 relaxation time of left thalamus were increased and negatively correlated with its gray matter volume in MDD patients. The present work to date sheds light on the neuropathological disruptions of LCSPT circuit in MDD, providing the first multi-modal neuroimaging evidence for the macro-micro structural abnormalities of the thalamus in patients with MDD. These findings have implications in understanding the abnormal changes of brain structures across development of MDD.

ObjectiveTo investigate whether baseline concentrations of plasma total tau (t-tau) and neurofilament light (NfL) chain proteins are associated with annual percent change (APC) of the basal forebrain cholinergic system (BFCS) in cognitively intact older adults at risk for Alzheimer disease (AD).MethodsThis was a large-scale study of 276 cognitively intact older adults from the monocentric INSIGHT-preAD (Investigation of Alzheimer's Predictors in Subjective Memory Complainers) cohort. Participants underwent baseline assessment of plasma t-tau and NfL concentrations as well as baseline and 24-month follow-up MRI scans. Linear models with and without influential observations (calculated using the Cook distance) were carried out to investigate the effect of plasma NfL and t-tau concentrations, and their interaction effect with β-amyloid status and APOE genotype, on the APC of the whole BFCS and its anterior (Ch1/2) and posterior (Ch4) subdivisions separately.ResultsHigher plasma t-tau concentrations at baseline were associated with higher BFCS rate of atrophy (model without influencers: n = 251, F value = 4.6815; p value = 0.031). Subregional analyses showed similar results for both the APC of the Ch1/2 (model without influencers: n = 256, F value = 3.9535, p corrected = 0.047) and Ch4 BFCS sectors (model without influencers: n = 253, F value = 4.9090, p corrected = 0.047). Baseline NfL, β-amyloid load, and APOE ε4 carrier status did not affect APC of the BFCS.ConclusionIncreased concentrations of baseline plasma t-tau may predict in vivo structural BFCS atrophy progression in older adults at risk for AD, independently of β-amyloid status and APOE genotype.

There is a general belief that reduced stock numbers on hill areas will be beneficial to the environment. A decrease in stock numbers may lead to farmers opting for lower input systems of hill sheep production in order to reduce costs, to meet the requirements of specific schemes or simply to maintain profitability. A large scale systems experiment has been set up at SAC Kirkton and Auchtertyre Farms to examine the effects of extensification on the ecology, animal production and welfare and the socio-economics of the local community. Within this extensive system, a 4-year trial was undertaken to examine the effects of a limited increase in inputs to twin-bearing ewes which are particularly at risk within such a system.

Studies of preadolescents strongly suggest that the prevalence of depression and suicidal activity among them has been seriously underestimated. The authors review current thinking about preadolescent suicide; discuss its implications for clinicians, supervisors, and agency administrators; and recommend a research agenda to guide and support effective practice with preadolescents at risk for suicide.

The abundance–occupancy relationship is one of the most well-examined relationships in ecology. At the species level, a positive association has been widely documented. However, until recently, research on the nature of this relationship at broad taxonomic and spatial scales has been limited. Here, we perform a comparative analysis of 12 taxonomic groups across a large spatial scale (Canada), using data on Canadian species at risk: amphibians, arthropods, birds, freshwater fishes, lichens, marine fishes, marine mammals, molluscs, mosses, reptiles, terrestrial mammals, and vascular plants. We find a significantly positive relationship in all taxonomic groups with the exception of freshwater fishes (negative association) and lichens (no association). In general, our work underscores the strength and breadth of this apparently fundamental relationship and provides insight into novel applications for large-scale population dynamics. Further development of species-independent abundance–occupancy relationships, or those of a similar nature, might well prove instrumental in serving as starting points for developing species-independent reference points and recovery strategies.
