ABSTRACT Background The impact that the coronavirus disease 2019 (COVID-19)–related early lockdown has had on dietary habits of the population and on food insecurity is unknown. Objective The aim of this study was to document the change in diet quality and in food insecurity observed during the COVID-19–related early lockdown. We hypothesized that the lockdown was associated with a deterioration in overall diet quality and an increase in food insecurity. Methods Data are from a COVID-19 subsample of NutriQuébec, a web-based cohort destined to study temporal changes in dietary habits among adults in Quebec, Canada. Participants completed questionnaires before (between June 2019 and February 2020) and during (April to May 2020) early lockdown, including a validated web-based 24-h recall (n = 853) and a questionnaire on food security (n = 922). Primary study outcomes were temporal changes in diet quality measured by the Healthy Eating Index (HEI)–2015 and in the prevalence of food insecurity. Results There was a small increase in the HEI-2015 during the COVID-19 early lockdown compared with baseline (+1.1 points; 95% CI: 0.6, 1.5), mostly due to small improvements in the intakes of whole grains, greens and beans, refined grains, total vegetables, total dairy, seafood and plant proteins, added sugar, and total protein subscores of the HEI-2015. Exploratory analyses suggested that individuals aged 18–29 y (+3.6 points; 95% CI: 2.4, 4.7), participants with lower education (+1.9 points; 95% CI: 1.3, 2.6), or with obesity (+3.8 points; 95% CI: 2.7, 4.8) showed particularly important increases in the HEI-2015. The prevalence of food insecurity was reduced from 3.8% at baseline to 1.0% during the early lockdown (prevalence ratio = 0.27; 95% CI: 0.08, 0.94). Conclusions Contrary to our hypotheses, diet quality has slightly improved and prevalence of food insecurity was reduced in this sample of adults from Quebec during the COVID-19–related early lockdown. These results may be generalizable only to relatively healthy populations.

AbstractObjectiveTo discern the impact of food donations provided by a food pantry and soup kitchen on food security.DesignIn this cross-sectional study, participants completed a demographic questionnaire, core food security module, FFQ and list of food donations. The FFQ was utilized to assess diet quality as estimated via the 2010 Healthy Eating Index.SettingClients were selected randomly from a food pantry and soup kitchen in Central Texas, USA.ParticipantsA total of 222 adults.ResultsApproximately 73 % of participants lacked food security. Compared with the food secure, the food insecure consisted of 61 % men, 42 % Caucasians, 56 % single and 67 % homeless. Also, of the food insecure, 60 % were soup kitchen clients and 64 % had an annual income <$US 1000 (P<0·01). The probability of food insecurity was reduced by ≥1·17-fold when the total dietary intake included the food donations, as these were rich in fruits, total vegetables and grains, dairy and protein foods (P<0·05).ConclusionsFood insecurity was quite prevalent in this sample of individuals who visited food pantries and soup kitchens. The addition of food donations improved the quality of the participants’ total diet and had a positive influence on food security. Thus, community organizations should financially support these food assistance agencies and strive to offer a variety of healthy and tasty foods in adequate quantities to provide optimum diet quality.

We explore the association between quality of diet and food security status among older persons. Cross-sectional survey of large national samples, from the National Health & Nutrition Examination Surveys (1988-1994 & 2001-2002), in the United States. A total of 5,039 and 2,040 men and women aged 60 and older, respectively. The dependent variable was the healthy eating index (HEI; 2000 and 2005) as a measure of diet quality. The independent variable of interest was food security status and was categorized as food secure, food insecurity receiving food stamps and food insecurity not receiving food stamps. In multivariate analyses, food insecure individuals not receiving food stamps had lower total HEI-2000 scores compared to food secure individuals (b=-3.91, SE=1.81, P=0.0310). Factors independently associated with high HEI (2000 or 2005) scores were female gender, being married, highly educated or wealthy, with very good/ excellent perceived health, or without functional limitations. Food secure individuals had better quality of diet than food insecure individuals. Among food insecure individuals, receiving food stamps was not associated with diet quality. Food insecure families with older individuals may need new methods of dissemination of food and nutrition programs – methods that yield positive and promising changes in the health status of this special population group.

Abstract Objectives To determine how food insecurity is associated with overall diet quality in college students following the implementation of a new food scholarship program. Changes of Healthy Eating Index (HEI) and food security status were compared over a 10-week period. Methods A new food scholarship program at Texas Woman's University in Houston provided students fresh fruits and vegetables, dairy and meat products, as well as non-perishable foods twice monthly. The Houston Food Bank donated all foods. Baseline and 10-week data were collected. Food security was measured with the USDA 6-item short form of the Food Security Survey Module. Total HEI-2015 and component scores were calculated from analysis of 3-day food records and compared by paired t-tests. ANCOVA estimated the impact of food security (high, low, very low) on HEI post scores, controlling for HEI baseline scores, age, gender, and race. Results Participants (n = 49) were primarily female (n = 38, 78%) with an average age of 28 ± 8 years. Ethnic/race distribution was 49% White, 27% Asian, 20% Black, and 4% other. Participants were primarily single (76%). Food security was low (31.9%) or very low (19.1%) at baseline; at post-test, participants reporting very low food secure dropped to 15.6%. Total HEI-2015 scores at baseline (55.9 ± 11.54) did not increase significantly after 10 weeks (57.7 ± 10.63) (P = 0.298). However, after 10 weeks, total vegetable scores significantly increased (2.64 ± 1.39 to 3.19 ± 1.33) (P = 0.023). There was no significant food security group effect (P = 0.49); however, there was a positive association between age and HEI post scores independent of the other model effects (β = 0.59 + 0.17; P = 0.001). Conclusions HEI-2015 total vegetable component scores increased with implementation of a food scholarship program. Nevertheless, many college students remained food insecure and overall diet quality needs improvement. Funding Sources Food donated by the Houston Food Bank.

Abstract Objectives Food insecurity has been associated with poor diet-related health in women, but few prior studies focused on the postpartum period—a particularly important time for the health of women and children. We examined associations between food security and maternal diet quality in a racially diverse cohort of postpartum women. We further assessed whether participation in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) or the Supplemental Nutrition Assistance Program (SNAP) modified this potential relation. Methods We examined 363 women in central North Carolina from the Nurture study (2013–2017). We computed maternal food security status using the 10-item USDA food security survey module at 3 months postpartum. We calculated Alternate Healthy Eating Index-2010 (AHEI) scores (range of 0 to 100) and component scores from food frequency questionnaires completed at 6 months postpartum. We conducted multiple linear regression examining associations between food security status (high, marginal, low, and very low) and AHEI, adjusting for age, race, education, marital status, number of children, breastfeeding, and total calories. We assessed WIC and SNAP as potential effect modifiers. Results Among women, 64.4% were Black and had a mean (standard deviation, SD) age of 28.2 (5.9) years. Nearly half (45.7%) had a high school diploma or less. Of women, 21.3% had low or very low food security. Mean (SD) AHEI score was 41.5 (11.3). We found no evidence of an interaction between food security and WIC or SNAP so we adjusted for both. In adjusted models, food security status at 3 months postpartum was not associated with AHEI (data not shown) at 6 months postpartum. However, low (ß: −0.64; 95% CI: −1.15, −0.13; P = 0.01) and very low (ß: −0.57; 95% CI: −1.02, −0.13; P = 0.01) food security were associated with a less healthy score for trans fat intake. Conclusions Food security status was not associated with overall diet quality in postpartum women. However, food insecure women showed higher consumption of trans fats. Future research should evaluate interventions to alleviate food insecurity in postpartum women, and assess whether these interventions lead to improved diet quality. Abstract Objectives While food and cash-assistance have been shown to improve food security in Syrian refugees living in Lebanon, no studies have investigated dosage variability of cash transfers on food security in this setting. We assessed the impact of short- (≤12 months) and long-term (&gt;12 months) multi-purpose cash assistance (MPC) provided by the World Food Programme (WFP) and the United Nations High Commissioner for Refugees (UNHCR), over and above food assistance, and the impact of discontinuation from MPC, on food security of Syrian refugees. Methods We used a quasi-experimental fuzzy Regression Discontinuity Design (RDD). A multi-dimensional household survey collected data in three-waves, at 6-month intervals. Households were sampled from a listing of registered Syrian refugees with WFP/UNHCR Vulnerability scores +/−10 points around the eligibility cut-off for MPC. Of 17,740 households approached, 11,457 completed the survey (65%). We assessed household food insecurity using the Food Insecurity Experience Scale and Livelihood Coping Strategies Index and diet quality using Food Consumption Score (FCS), and Household Dietary Diversity Score (HDDS). We estimated the treatment effect of MPC using two-stage least squares regression controlling for demographic characteristics (age, sex, head of household education) and receipt of other cash assistance. Results Food insecurity experience in those receiving long-term MPC decreased (P &lt; 0.05), with a concomitant 7.7 percentage point decrease in reliance on emergency coping strategies (P &lt; 0.01). Long-term MPC recipients had a 6 point increase in FCS (P &lt; 0.01) along with an increase in diet diversity (P &lt; 0.05), with higher consumption of vegetables, fish, chicken, and eggs found in both short-term and long-term MPC recipients. Conclusions There was a significant improvement in food security and diet quality among households receiving long-term MPC but not among those receiving short-term MPC (except for a small increase in diet diversity) or who were discontinued. Positive impact was observed above and beyond food assistance. Stability of assistance is therefore key in achieving impact from MPC n food security in this population. The main objective of this study was to compare self-rated diet quality with a more comprehensive score of diet quality and to assess the ability of self-rated diet quality to predict adherence to healthy eating guidelines. This study also aimed to evaluate the influence of individual characteristics on the association between self-rated diet quality and the overall diet quality score. As part of the PRédicteurs Individuels, Sociaux et Environnementaux (PREDISE) study, 1045 participants (51% women) from the Province of Québec, Canada, self-rated their diet quality (“In general, would you say that your dietary habits are excellent, very good, good, fair, or poor?”). Three Web-based 24-h food recalls were completed, generating data for the calculation of the Canadian Healthy Eating Index (C-HEI) score, an overall diet quality indicator. Participants rated their diet quality as excellent (2.4%), very good (22.7%), good (49.5%), fair (20.3%), or poor (5.1%). C-HEI scores differed significantly between diet ratings, in the expected direction (p < 0.0001). Self-rated diet quality predicted adherence to healthy eating guidelines (i.e., C-HEI > 68) with a sensitivity of 44.5% and a specificity of 81.5% (C-statistic = 0.63). Sex significantly modified the association between self-rated diet quality and C-HEI score (p interaction = 0.0131); women had higher C-HEI scores than did men in the “good” and “fair” ratings. Self-rated diet quality can be useful in obtaining an overview of the diet quality of a population, but the results of this study suggest that such data should be used with caution given their poor ability to predict adherence to healthy eating guidelines. Individual characteristics may influence one’s ability to appropriately self-evaluate diet quality.

Abstract Objective: To determine whether food security, diet diversity and diet quality are associated with anthropometric measurements and body composition among women of reproductive age. The association between food security and anaemia prevalence was also tested. Design: Secondary analysis of cross-sectional data from the Healthy Life Trajectories Initiative (HeLTI) study. Food security and dietary data were collected by an interviewer-administered questionnaire. Hb levels were measured using a HemoCue, and anaemia was classified as an altitude-adjusted haemoglobin level < 12·5 g/dl. Body size and composition were assessed using anthropometry and dual-energy x-ray absorptiometry. Setting: The urban township of Soweto, Johannesburg, South Africa. Participants: Non-pregnant women aged 18–25 years (n 1534). Results: Almost half of the women were overweight or obese (44 %), and 9 % were underweight. Almost a third of women were anaemic (30 %). The prevalence rates of anaemia and food insecurity were similar across BMI categories. Food insecure women had the least diverse diets, and food security was negatively associated with diet quality (food security category v. diet quality score: B = –0·35, 95 % CI –0·70, –0·01, P = 0·049). Significant univariate associations were observed between food security and total lean mass. However, there were no associations between food security and body size or composition variables in multivariate models. Conclusions: Our data indicate that food security is an important determinant of diet quality in this urban-poor, highly transitioned setting. Interventions to improve maternal and child nutrition should recognise both food security and the food environment as critical elements within their developmental phases.

Abstract Aim This study aimed to assess the impact of the Supporting Wellness at Pantries (SWAP) system on client food selections at a food pantry. Subject and methods In a pre–post comparison study design, a client-choice food pantry implemented SWAP by reorganizing its inventory to promote healthy options. Each product was ranked as “choose often” (green), “sometimes” (yellow), or “rarely” (red) based on saturated fat, sodium, and sugar. Signage was added to indicate each item’s SWAP rank and healthier foods were placed at eye level. Client food baskets were assessed at time 1 (n = 121) and time 2 (n = 101). The proportions of green and red foods selected were compared using regression analyses. Results The regression analyses showed that the proportion of green foods selected by clients increased by 11% (p < 0.001) and the proportion of red foods selected decreased by 7% (p < 0.001) after SWAP was implemented (n = 222). Conclusions SWAP has the potential to positively shift client choices among the items available in a food pantry setting. SWAP is one component of a suite of changes to the charitable food system that have the potential to alleviate food insecurity, improve diet quality, and assist clients in managing diet-related diseases.

Abstract Background Food insecurity impacts nearly one-in-four Latinx households in the United States and has been exacerbated by the novel coronavirus or COVID-19 pandemic. Methods We examined the impact of COVID-19 on household and child food security in three preexisting, longitudinal, Latinx urban cohorts in the San Francisco Bay Area (N = 375 households, 1875 individuals). Households were initially recruited during pregnancy and postpartum at Zuckerberg San Francisco General Hospital (ZSFG) and UCSF Benioff prior to the COVID-19 pandemic. For this COVID-19 sub-study, participants responded to a 15-min telephonic interview. Participants answered 18 questions from the US Food Security Food Module (US HFSSM) and questions on types of food consumption, housing and employment status, and history of COVID-19 infection as per community or hospital-based testing. Food security and insecurity levels were compared with prior year metrics. Results We found low levels of household food security in Latinx families (by cohort: 29.2%; 34.2%; 60.0%) and child food security (56.9%, 54.1%, 78.0%) with differences between cohorts explained by self-reported levels of education and employment status. Food security levels were much lower than those reported previously in two cohorts where data had been recorded from prior years. Reported history of COVID-19 infection in households was 4.8% (95% Confidence Interval (CI); 1.5–14.3%); 7.2% (95%CI, 3.6–13.9%) and 3.5% (95%CI, 1.7–7.2%) by cohort and was associated with food insecurity in the two larger cohorts (p = 0.03; p = 0.01 respectively). Conclusions Latinx families in the Bay Area with children are experiencing a sharp rise in food insecurity levels during the COVID-19 epidemic. Food insecurity, similar to other indices of poverty, is associated with increased risk for COVID-19 infection. Comprehensive interventions are needed to address food insecurity in Latinx populations and further studies are needed to better assess independent associations between household food insecurity, poor nutritional health and risk of COVID-19 infection.

Abstract Objectives The emergence of food insecurity as a primary nutrition-related health issue among older adults suggests a need to examine how nutritional assistance programs are related to food insecurity and dietary quality in aging populations. This project examines food insecurity and dietary quality in US adults age 65 and older and the impact of nutrition assistance programs. Methods The sample was drawn from the 2012 Health and Retirement Study and 2013 Health Care and Nutrition Study and included 3779 respondents representing a population of 37,217,566 adults aged 65 and older. Food insecurity was a binary measure based on the USDA six-item US Adult Food Security Survey Module. Two forms of nutritional assistance included receipt of supplemental food from sources such as food banks and Meals-on-Wheels (1 = yes, 0 = no) and reported receipt of SNAP benefits (1 = yes, 0 = no). Dietary quality was measured using the Alternative Healthy Eating Index-2010 based on a food frequency questionnaire. General linear modeling adjusting for covariates and complex sampling design was used to test if nutritional assistance moderated the association between food insecurity and AHEI-2010. Results Around 10% of the sample was food insecure, 14% reported receipt of supplemental food, and 6.4% were SNAP benefit recipients. In covariate-adjusted models, food insecurity and receipt of SNAP benefits were not associated with AHEI-2010, but receipt of supplemental food was (b = −1.39, SE = 0.67, P = 0.038). Receipt of supplemental food moderated the association between AHEI-2010 and food insecurity (P = 0.001). Simple effect estimates suggested that among those not receiving supplemental food, the food insecure had lower AHEI-2010 scores than the food secure (b = −2.15, SE = 0.88, P = 0.014). Among those receiving supplemental food, the food insecure had greater AHEI-2010 scores than the food secure (b = 2.62, SE = 1.25, P = 0.035) and similar AHEI-2010 scores as the food secure not receiving supplemental food. Conclusions Preliminary analysis suggests that receipt of supplemental food appears to be associated with better dietary quality among food-insecure older adults and confirms the importance of food assistance programs in combating the negative effect of food insecurity on dietary quality.
