196 A Rare Case of Chemical Burn: Dithranol With Salicylic Acid

Abstract A 16-year-old female with psoriasis was admitted to our Plastic Surgery department with a significant chemical burn to the neck, upper torso, and left cheek (TBSA 6%). She applied a concoction of cream prescribed by her dermatologist in her native country, Poland when she returned to the United Kingdom. A few hours after application she developed a burn with pH of 5. A review of the cream revealed a mixture of 19% Dithranol and 5% Salicylic acid. This combination is recognized for managing psoriasis, however the strength of dithranol in the combination given is of a high concentration (normally <3%). This alone can cause a burn to the skin if left for a prolonged period of time. Salicylic acid is an enhancer which augments the stability of dithranol and increases its penetration and efficacy. The concentration of 5% is also on the higher end. Our patient was admitted for pain relief and further irrigation till normalization of the pH which was achieved after 3 days. A worrying aspect in our patients’ case is that she was given the cream to commence at home. High concentration preparation is normally commenced in a controlled setting under medical supervision.

(1) Background: Exopolysaccharide (EPS) derivatives, produced by Alteromonas infernus bacterium, showed anti-metastatic properties. They may represent a new class of ligands to be combined with theranostic radionuclides, such as 47Sc/44Sc. The goal of this work was to investigate the feasibility of such coupling. (2) Methods: EPSs, as well as heparin used as a drug reference, were characterized in terms of molar mass and dispersity using Asymmetrical Flow Field-Flow Fractionation coupled to Multi-Angle Light Scattering (AF4-MALS). The intrinsic viscosity of EPSs at different ionic strengths were measured in order to establish the conformation. To determine the stability constants of Sc with EPS and heparin, a Free-ion selective radiotracer extraction (FISRE) method has been used. (3) Results: AF4-MALS showed that radical depolymerization produces monodisperse EPSs, suitable for therapeutic use. EPS conformation exhibited a lower hydrodynamic volume for the highest ionic strengths. The resulting random-coiled conformation could affect the complexation with metal for high concentration. The LogK of Sc-EPS complexes have been determined and showing that they are comparable to the Sc-Hep. (4) Conclusions: EPSs are very promising to be coupled with the theranostic pair of scandium for Nuclear Medicine.

The structure of the thin liquid films determines the stability of foams and emulsions. In this work the bubbles stretched length with different hollow SiO2 particles concentration is measured when the foam has been stilled for different time. The results show that the bubbles stretched length is longer than that of bubbles when the foam is free of hollow SiO2 particles even when the foam has been stilled for 500mins. The bubbles stretched length increases with increasing the concentration of hollow SiO2 particles. A strong hydration effect leaves a large volume of hydration layers on the solid particles surfaces in aqueous solutions. The water in hydration layers can help the film keep a certain thickness. The existence of hydration forces leads that two particles cannot be too close each other. The high concentration surfactant limited in the fixed area helps the film keep good elasticity. Therefore the film has a long life time with compatible thickness and elasticity and the three-phrase foam is upper stable.

Disappearing ink was prepared using different concentrations of thymolphthalein, phenolphthalein and their mixture, applying to different types of handwriting surfaces such as cotton, polyester and polyamide. The effects of thymolphthalein, phenolphthalein and alkali concentrations (i.e. pH) on the fading time were studied. The handwriting stability increased when the concentration of thymolphthalein or phenolphthalein was increased. At a high concentration of alkali with phenolphthalein and its mixture with thymolphthalein, the handwriting stability decreased with polyester or polyamide handwriting surfaces and the stability increased in the case of cotton. When the faded handwriting surfaces were subject to the thermal effect in a thermostatically controlled oven at 100°C for 10 minutes by hanging them with suitable hooks, there was no change in the faded handwriting. When they were at 150°C for 20 minutes, the faded phenolphthalein ink, which contained 0.5ml of 1N NaOH, was visible and shown red on the polyester and polyamide handwriting surfaces, but there was no change on the faded cotton handwriting surface.

Cilia and flagella are assembled by intraflagellar transport (IFT) of protein complexes that bring tubulin and other precursors to the incorporation site at their distal tip. Anterograde transport is driven by kinesin, whereas retrograde transport is ensured by a specific dynein. In the protist Trypanosoma brucei, two distinct genes encode fairly different dynein heavy chains (DHCs; ∼40% identity) termed DHC2.1 and DHC2.2, which form a heterodimer and are both essential for retrograde IFT. The stability of each heavy chain relies on the presence of a dynein light intermediate chain (DLI1; also known as XBX-1/D1bLIC). The presence of both heavy chains and of DLI1 at the base of the flagellum depends on the intermediate dynein chain DIC5 (FAP133/WDR34). In the IFT140RNAi mutant, an IFT-A protein essential for retrograde transport, the IFT dynein components are found at high concentration at the flagellar base but fail to penetrate the flagellar compartment. We propose a model by which the IFT dynein particle is assembled in the cytoplasm, reaches the base of the flagellum, and associates with the IFT machinery in a manner dependent on the IFT-A complex.

Abstract The complexities of the United Kingdom’s decision to withdraw from the European Union while simultaneously honoring its prior commitments to its decentralized, autonomous, and constituent regions have put constitutional questions back on the map. The dominant approach analyzes these questions premised on the “preservative” view of the constitution. This view prioritizes the stability and continuity of the institutions in Westminster (Parliament) and Whitehall (central executive). However, the preservative view of the constitution is theoretically and practically deficient as it cannot give an account of the multipolar and decentralized developments of the past twenty years. Another interpretation regards the legal and political changes to the constitution as “transformative.” This view accentuates the fragility of the U.K. constitution due to a plurality of constitutional rules and the ongoing processes of devolution of powers within multilevel systems of government. This Article discusses that evolution of the U.K. constitution through the prism of comparative constitutional law and its appropriate methodology. The preservative model of the constitution favors a universalist method, whereas the transformative model requires a contextualist method. I argue that the experience of supranational (European Union) and infranational (devolution) power sharing has fundamentally altered the United Kingdom’s central constitutional concepts. To stabilize its fragmentary forces, the United Kingdom needs to adopt concepts that reflect the state as divided, the constitution as transitional, sovereignty as an attribute of the state rather than Parliament, and democracy as conflicted. Nothing less than the future of the United Kingdom as a state is at stake.

Abstract Background: The accuracy and precision of methods for the measurement of the anticonvulsants phenytoin, phenobarbital, primidone, carbamazepine, ethosuximide, and valproate in human serum were assessed in 297 laboratories that were participants in the United Kingdom National External Quality Assessment Scheme (UKNEQAS). Methods: We distributed lyophilized, serum-based materials containing low, medium, and high weighed-in concentrations of the drugs. The 297 participating laboratories received the materials on two occasions, 7 months apart. Expected concentrations were determined by gas chromatography or HPLC methods in five laboratories using serum-based NIST reference materials as calibrators. Results: In general, bias was consistent across concentrations for a method but often differed in magnitude for different drugs. Bias ranged from −1.9% to 8.6% for phenytoin, −2.7% to 3.1% for phenobarbital, −2.7% to 0.5% for primidone, −8.6% to 0.3% for carbamazepine, −5.6% to 2.0% for ethosuximide, and −7.2% to 0.1% for valproate. Intralaboratory sources of imprecision significantly exceeded interlaboratory sources for many drug/method combinations. The mean CVs for intra- and interlaboratory errors for the different drugs were 6.3–7.8% and 3.3–4.2%, respectively. Conclusions: For these long-established and relatively high-concentration analytes, the closed analytical platforms generally performed no better than open systems or chromatography, where use of calibrators prepared in house predominated. To improve the accuracy of measurements, work is required principally by the manufacturers of immunoassays to ensure minimal calibration error and to eliminate batch-to-batch variability of reagents. Individual laboratories should concentrate on minimizing dispensing errors.

The Coronavirus pandemic has caused major change across the world and in the National Health Service. In order to cope and help limit contagion, numerous institutions recognised the need to adjust clinical practice quickly yet safely. In this paper, we aim to describe the changes implemented in a general surgery department at a district general hospital in the United Kingdom. Across the surgical specialties, frameworks, protocols and guidelines have been established locally and nationally. The aerosol generating procedures involved in general surgery required us to alter our daily activities. Modifications to patient management were necessary to try and reduce viral spread. Staff wellbeing was heavily promoted in order to help maintain the frontline workforce. A holistic approach was required.

The purpose of this paper is to obtain a start-up of short-cut nitrification systems to treat wastewater with high concentration of nitrogen and phosphorus under laboratory conditions. By starting the nitrification system at room temperature we can achieve the effective accumulation of nitrifying bacteria. Then by increasing experimental temperature and pH, and increasing the concentration of nitrogen and phosphorus in influent, to inhibit the growth of NOB in the systems and promote the accumulation of AOB. So we can make the effluent have a higher concentration of NO2--N. In the stable operation stage we can constantly improve ammonia concentration in the influent, to observe the stability of the each indicators of effluent. This experiment showed that the wastewater with high concentration of nitrogen and phosphorus can better promote the accumulation of AOB, to achieve the effect of short-range nitrification.
