Abstract The John A Hartford Foundation and the Institute for Healthcare Improvement (IHI)’s 4Ms of mentation, mobility, medications and (what) matters most provide a much-needed framework for helping system leaders and frontline teams consistently deliver high-quality, age-friendly care. Geriatric Fast Facts (GFFs) is a virtual resource providing teachers/learners with peer-reviewed, evidence-based summaries on topics essential to older adult care via a searchable website [www.geriatricfastfacts.com]. To determine if GFFs can be classified by the 4Ms we initially did a free text search of all GFFs. That revealed GFFs whose foci were unrelated to the 4Ms (e.g., mobility emerged in a fluoroscopy GFF as a minor element related to patient positioning). Therefore, all GFFs were independently reviewed by a geriatrician and the website manager and classified according to the 4M rubric (a single GFF can be classified in multiple M’s such as #93 on Age Friendly Health Systems). Any differences were adjudicated by the GFF editor. 64% (60/ 93) of GFFs strongly linked to one of the 4Ms. The number of GFFs dedicated to the 4Ms are as follows: 20 what matters most, 18 medications, 13 mentation, and 9 mobility. Those that were not coded within 4Ms were often very disease/specialty oriented. A total of 36 were not classified. For example, GGF #39 focuses on the etiologies of anemia among older adults. The 4M framework can be easily applied to educational materials to support consistent and clear conceptual model across learning conditions and materials.

Rias-type bays are one of the most common coasts in Japan where aquacultures have been active due to sheltered geological shape with a deep bottom. The huge tsunami hit Sanriku Coast consisting of open rias-type bays near the epicenter facing Pacific Ocean on 11 March 2011. For recovering Sanriku Coast, it is important to include sustainability in its program. Satoumi is defined as the human use and management of coastal seas for high productivity while maintaining high biodiversity. Therefore, we proposed Satoumi approach to an open rias-type bay, Shizugawa Bay, in southern Sanriku Coast. We conducted scientific researches on mapping of coastal habitats and aquaculture facilities, hydrography, and material flows of nutrients, a minor element (Fe) and organic matters in the bay including those from the rivers and from the offshore waters. At the same time, Committee for Shizugawa Bay Management of Fishermen’s Cooperative of Miyagi Prefecture decided to decrease in aquaculture facilities for sustainable development of aquaculture. Based on these data, a physical-biological coupling model was used for calculating the number of aquaculture facilities that are suitable not only for yields but also for environments. These researches were established on strong collaborations among a fishermen’s’ cooperative, local governments and scientists. Results of this practice may help to realize sustainable coastal use of a rias-type bay.

Abstract Chlorine is a minor element present in obsidians in quantities greater than in average igneous rocks. The chlorine concentration in obsidians is generally low, of the order of tenths of wt %, but it exhibits an appreciable differentiation among geological sources. Despite these characteristics, chlorine has rarely been taken into consideration as a possible indicator of obsidian provenance and it does not appear in the chemical analytical tables accompanying the geochemical characterisation of obsidian samples. In this work, after an overview of chlorine geochemistry and cycle, we present thirty-one new electron microprobe (EPMA) analyses, including Cl, of geologic obsidians sampled from the four sources of the Central Mediterranean, exploited in prehistoric times (Monte Arci, Palmarola, Lipari and Pantelleria). The results are compared with 175 new EPMA analyses, including Cl, of archaeological obsidians already characterised in previous work and of known provenance. As such it was possible to ascertain that each source has a characteristic chlorine concentration, showing the utility of its use in the studies of obsidian provenance. Furthermore, given that the solubility of chlorine in silicate melts is correlated to its alkali content, in particular sodium, we assessed the efficacy of simple binary graphs Cl vs Na2O to better constrain the provenance of the obsidian samples.

Abstract Background Ileal pouch-anal anastomosis (IPAA) is a technically demanding procedure. Intraoperatively, great care must be taken to assure a straight superior mesenteric axis. Rarely, twisted pouches are inadvertently constructed, resulting in deviations of expected pouch function, i.e. patients readily able to open their bowels on average 7x/24 hours without pain. Twisted pouches may result in symptoms classified as pouch dysfunction. Herein we describe our quaternary pouch referral center experience with twisted pouch syndrome (TPS). Methods We performed a retrospective review of our prospectively maintained pouch registry from 1995 – 2020. Patients were identified using free-text search of redo IPAA operative reports for variations of the term “twist”. We defined twisted pouch syndrome as intraoperative findings of twisting of the pouch as the primary pathology. Data are presented as frequency (proportion) or median (interquartile range). Results Over 25-years, we identified 29 patients with confirmed TPS who underwent a redo pouch procedure by 10 surgeons. Overall, 65% were female, median BMI 21.2 (19.5 – 26) kg/m2. The duration from the index IPAA to the redo procedure was 4 (2 – 8) years; all (100%) were referral cases constructed elsewhere. Original diagnoses included: ulcerative colitis (90%), FAP (10%), lack of interstitial cells of Cajal in 1 patient (10%). All patients presented with symptoms of pouch dysfunction including erratic bowel habits (96%) with urgency/frequency, abdominal/pelvic/rectal pain (92%), and obstructive symptoms (88%). Most had (75%) been treated for chronic pouchitis with antibiotics or biologics, and 46% had undergone 1 or more additional surgery. Prior to redo IPAA procedure patients underwent a thorough workup: 100% pouchoscopy, 96% GGE, 93% underwent EUA, 88% MRI, 73% manometry, and 42% defecography. TPS was diagnosed in 15% by pouchoscopy, in 10% by imaging, and in 75% was diagnosed intra-operatively at re-diversion (20%) or revision/redo IPAA (55%). In terms of surgical intervention, 85% were initially re-diverted. A total of 18 (62%) underwent pouch revision, and 10 (38%) required redo-IPAA. Short-term outcomes: LOS 7.5 (5 – 9) days, any complication 48%, readmission 11%, reoperation 3.4%, zero mortalities. After a median follow-up 50 (28 – 60) months, 2 never had loop ileostomy closure, 1 had pouch excision, and 1 a Kock pouch, yielding an overall pouch survival rate of 86%. Conclusions Twisted pouch syndrome presents with pouch dysfunction manifest by erratic bowel habits, unexplained pain, and obstructive (defecation) symptoms. This syndrome may also mimic chronic pouchitis. Despite a thorough workup which may suggest a mechanical problem, many patient may not be diagnosed until time of redo pouch surgery. Redo surgery for twisted pouch syndrome results in long-term pouch survival for the majority.

<p>Since 2017, Open Data has been a part of the DWD data distribution strategy. Starting with a small selection of meteorological products, the number of available datasets has grown continuously over the last years. Since the start, users can access datasets anonymously via the website https://opendata.dwd.de to download file-based meteorological products. Free access and the variety of products has been welcomed by the general public as well as private met service providers. The more datasets are provided in a directory structure, however, the more tedious it is to find and select among all available data. Also, metadata and documentation were available, but on separate public websites. This turned out to be an issue, especially for new users of DWD's open data.</p><p>To help users explore the available datasets as well as to quickly decide on their suitability for a certain use case, the Open Data team at DWD is developing a geoportal. It enables free-text search along with combined access to data, metadata, and description along with interactive previews via OGC WMS.</p><p>Cloud technology is a suitable way forward for hosting the geoportal along with the data in its operational state. Benefits are expected for the easy integration of rich APIs with the geoportal, and the flexible and fast deployment and scaling of optional or prototypical services such as WMS-based previews. Flexibility is also mandatory to respond to fluctuating user demands, depending on time of day and critical weather situations, which is supported by containerization. The growing overall volume of meteorological data at DWD may mandate to allow customers to bring their code to the data – for on-demand processing including slicing and interpolation –  instead of transferring files to every customer. Shared cloud instances are the ideal interface for this purpose.</p><p>The contribution will outline a protoype version of the new geoportal and discuss further steps for launching it to the public.</p>

The article substantiates the need to carry out a comparative legal analysis of Ukrainian and foreign legislation on fixing the motive of the crime and its criminal- legal significance. Comparative analysis of the laws of foreign countries is made taking into account the membership of countries in the legal systems. The research of the legal status of the motive of crime in the legislation of foreign countries was carried out on the basis of consideration of basic acts of criminal legislation approved at the state level by the higher legislative bodies of a state. The analysis of various approaches to determining the motive of the crime based on the works of foreign and domestic scientists. On the basis of consideration of normative legal acts of criminal nature of foreign countries, the main terms, which indicate the motive of the crime are defined. The basic approaches of fixing the motive of crime in the legislation of foreign countries are defined and the ways of improvement of the Ukrainian legislation in this matter are proposed. It has been established that the legislation of some countries clearly traces the role of motive as a circumstance aggravating punishment and as a sign that must be taken into account directly in sentencing. In spite of this, most countries still hold a position of indifference to the motive of the crime, recognizing it as a minor element of the subjective side of the crime. It is established that the position of the Ukrainian legislator on the optionality of the sign of motive among other constituents of the subjective side of the crime and the obligatory sign among the elements of evidence in the criminal process is quite logical and justified. But many aspects related to the motive for the crime must be refined. Ukrainian legislation, including criminal law, become more progressive every year and meets European standards.

Species of the superfamily Acastacea constitute a minor element of North and South American Silurian trilobite faunas. Phacopidina? obsoleta (Ulrich and Delo) (Llandovery, Tennessee) provides the sole Silurian record of the “kloucekiine” grade (Acastacea s.l.); a lectotype is designated for this species. Acaste birminghamensis Norford (mid Llandovery, Alabama) lacks synapomorphies of post-Llandovery Acastidae s.s. (=Acastinae + “Acastavinae” + Asteropyginae), and is the basis for Llandovacaste n. gen. (Acastidae s.l.). A well-preserved sample of Andinacaste cf. A. ledgrandi Eldredge and Braniša from the Catavi Formation (Ludlow–Přídolí), Bolivia, displays apomorphic characters of the genal spines and hypostome shared with Devonian calmoniids. Coaptative structures, including vincular furrow/pits and “bifurcated” thoracic pleural tips, are documented for Andinacaste; similar enrollment morphologies arose in other acastomorph calmoniids. Poorly known Llandovery acastaceans from Paraguay and Venezuela may represent Andinacaste. Acaste zerinae n. sp. from the Pembroke Formation (Přídolí) of Maine is closely comparable to British late Wenlock A. downingiae (Murchison). The Australian Gedinnian acastine placed in Phacopinae indet. longisulcata (Shergold) is designated Acaste lokii n. sp.

Density functional theory calculations have been used to calculate the ground state structure and oxygen and hydrogen adsorption properties of the pure and doped-iron nanoclusters. Small atomic clusters containing two to six atoms have been considered and a single Fe atom has replaced by a minor element i.e. Zr, Ti, and Sc. Doping of a minor element increases the cluster stability and octahedron Fe5Zr is the most stable structure within this study. Zr- and Sc-doped clusters have the highest oxygen and hydrogen adsorption energy. The electronic structure shows a strong hybridization between the metal 3d and oxygen 2p orbitals with a small contribution from metal 4s and 3p orbitals. Additionally, H s and metal 4s states form a new peak below the Fermi energy and a small modification is observed for 3d orbitals near the Fermi level. A small amount of Zr- and Sc-doping into the Fe-based alloys might improve the oxide film adherence.

BackgroundMusculoskeletal (MSK) pain is the primary contributor to disability worldwide. There is a growing consensus that MSK pain is a recurrent multifactorial condition underpinned by health and lifestyle factors. Studies suggest that education on work-related pain and individualised advice could be essential and effective for managing persistent MSK pain.ObjectiveThe objective of this scoping review was to map the existing educational resources for work-related MSK (WRMSK) pain, and the effects of implementing educational strategies in the workplace on managing WRMSK pain.MethodsThis scoping review assessed original studies that implemented and assessed education as a strategy to manage WMSK pain. Literature search strategies were developed using thesaurus headings (ie, MeSH and CINAHL headings) and free-text search including words related to MSK in an occupational setting. The search was carried out in PubMed, CINAHL, Cochrane Library and Web of Science in the period 12–14 February 2019.ResultsA total of 19 peer-reviewed articles were included and the study design, aim and outcomes were summarised. Of the 19 peer-reviewed articles, 10 randomised controlled trial (RCT) studies assessed the influence of education on work-related MSK pain. Many studies provided a limited description of the education material and assessed/used different methods of delivery. A majority of studies concluded education positively influences work-related MSK pain. Further, some studies reported additive effects of physical activity or ergonomic adjustments.ConclusionsThere is a gap in knowledge regarding the best content and delivery of education of material in the workplace. Although beneficial outcomes were reported, more RCT studies are required to determine the effects of education material as compared with other interventions, such as exercise or behavioural therapy.
