The Transtheoretical Approach

The transtheoretical model outlines important dimensions of intentional behavior change from an integrative perspective. Stages describe the motivational and temporal dimension and the critical multidimensional tasks involved in creating sustained change. Processes are an eclectic set of cognitive/experiential and behavioral coping activities that act as mechanisms or engines of change that drive completion of stage tasks. The chapter summarizes the origins of the approach, its applicability and structure, therapeutic relationship, and diversity considerations. A case example illustrates its assessment and treatment foundations. The transtheoretical approach has demonstrated utility in tailoring treatment and predicting outcomes across a variety of health, mental health, and addictive behaviors.

El Modelo Transteórico es un modelo ampliamente utilizado para la explicación del cambio intencional, sobre todo cuando el cambio se refiere a conductas adictivas. A pesar de ello, también ha sido un modelo muy criticado, entre otros motivos por falta de validez. En este trabajo nos propusimos valorar la validez del propio modelo evaluando diferentesvariables psicológicas distintas a los propios constructos del modelo: amplificación somatosensorial, hábitos de salud, actitudes y opiniones.La muestra estuvo compuesta por 347 estudiantes de la Universidad de Huelva. La participación fue voluntaria. Los sujetos fumadores se distribuyeron según las etapas del Modelo Transteórico de la siguiente manera: 40.0% precontempladores, 18.3% contempladores, 9,7% preparadores, 9.0% acción y 23.0% mantenedores. Los resultados obtenidos apoyan la validez del modelo. AbstractThe Transtheoretical Model is widely used for explaining intentional behavior change, especially in addictive behaviors. Nevertheless, it is a much criticized model, due to its lack of validity. The aim of this study is to evaluate the model’s legitimacy assessing several psychological variables which differ from the its constructs: somatosensorial amplification, health habits, concern for health, attitudes and opinions. For it we use a students’ sample within the Huelva University. Participation was voluntary. The smokers were distributed according to the stages of the model Transtheoretical Change Model of the following way: 40.0% precontemplators, 18.3% contemplators, 9.7% preparers, 9.0% active, and 23.0% in maintenance. The results support the validity of the Transtheoretical Model.

In a longitudinal study, it is examined whether the transtheoretical processes of change do predict stage transitions in fruit intake. A random sample of an existing Internet research panel resulted in a cohort of 735 adults, who were examined three times with electronic questionnaires assessing stages of change, processes of change, and fruit intake. Cross-sectional differences were found for the processes of change between precontemplation and all further stages. Experiential as well as behavioral processes increased from precontemplation to action with similar patterns. Both experiential and behavioral processes predicted forward transition out of precontemplation and forward transition into action, whereas only behavioral processes predicted forward transition out of contemplation. The results indicate that the transtheoretical processes of change predict stage transitions for fruit intake, but that the pattern of relevant processes for fruit intake is not as straightforward as outlined by the transtheoretical model of behavior change.

Background Recent studies suggest social media may be an attractive strategy to promote mental health and wellness. There remains a need to examine the utility for individually tailored wellness messages posted to social media sites such as Facebook to facilitate positive psychological outcomes. Objective Our aim was to extend the growing body of evidence supporting the potential for social media to enhance mental health. We evaluated the influence of an 8-week social media intervention on anxiety in college students and examined the impact of dynamic (active) versus static (passive) Facebook content on physical activity behaviors. Methods Participants in the static group (n=21) accessed a Facebook page featuring 96 statuses. Statuses were intended to engage cognitive processes followed by behavioral processes of change per the transtheoretical model of behavior change. Content posted on the static Facebook page was identical to the dynamic page; however, the static group viewed all 96 statuses on the first day of the study, while the dynamic group received only 1 to 2 of these status updates per day throughout the intervention. Anxiety was measured using the Overall Anxiety Severity and Impairment Scale (OASIS). Time spent engaging in physical activity was assessed using the International Physical Activity Questionnaire (IPAQ). Results The OASIS change score for the dynamic Facebook group was statistically significant (P=.003), whereas the change score for the static group was not (P=.48). A statistically significant group-by-time interaction was observed (P=.03). The total IPAQ group-by-time interaction was not statistically significant (P=.06). Conclusions We observed a decrease in anxiety and increase in total physical activity for the dynamic group only. Dynamic social networking sites, featuring regularly updated content, may be more advantageous than websites that retain static content over time. Objective: There are presently no instruments available in German language to assess the therapeutic relationship in psychiatric care. This study validates the German version of the Scale to Assess the Therapeutic Relationship in Community Mental Health Care (D-STAR). Method: 460 persons with severe mental illness and 154 clinicians who had participated in a multicenter RCT testing a discharge planning intervention completed the D-STAR. Psychometric properties were established via item analysis, analyses of missing values, internal consistency, and confirmatory factor analysis. Furthermore, convergent validity was scrutinized via calculating correlations of the D-STAR scales with two measures of treatment satisfaction. Results: As in the original English version, fit indices of a 3-factor model of the therapeutic relationship were only moderate. However, the feasibility and internal consistency of the D-STAR was good, and correlations with other measures suggested reasonable convergent validity. Conclusions: The psychometric properties of the D-STAR are acceptable. Its use can be recommended in German-speaking countries to assess the therapeutic relationship in both routine care and research.

Severe behaviour problems (SBPs) in early childhood include oppositional and aggressive behaviours and predict negative mental health outcomes later in life. Although effective treatments for this group are available and numerous clinical practice guidelines have been developed to facilitate the incorporation of evidence-based treatments in clinical decision-making (NICE, 2013), many children with SBPs remain unresponsive to treatment (Lahey & Waldman, 2012). At present, it is unknown how many countries in Europe possess official clinical guidelines for SBPs diagnosis and treatment and what is their perceived utility. The aim was to create an inventory of clinical guidelines (and associated critical needs) for the diagnostics and treatment of SBPs in youth mental health across Europe according to academic experts and mental health clinicians’ opinions. To investigate the aim, two separate online semi-structured questionnaires were used, one directed at academics (N=28 academic experts; 23 countries), and the other at clinicians (N=124 clinicians; 24 countries). Three key results were highlighted. First, guidelines for SBPs are perceived as beneficial by both experts and clinicians. However, their implementation needs to be reinforced and content better adapted to daily practice. Improvements may include taking a multifactorial approach to assessment and treatment, involving the systems around the child, and multidisciplinary collaboration. Second, academic experts and clinicians support the need for further developing national / European guidelines. Finally, future guidelines should address current challenges identified by clinicians to be more applicable to daily practice.

Objetivo: Refletir teoricamente sobre a importância da Teoria das Relações Interpessoais de Peplau, contextualizada com o processo de formação da identidade do enfermeiro como terapeuta em saúde mental e psiquiatria. Metodologia: A partir do estudo da obra sobre Relações Interpessoais em Enfermagem de Peplau e demais textos, a reflexão organizou-se em três eixos: a teoria das relações interpessoais, a relação terapêutica na teoria de Peplau e o subpapel de assessor/counseling. Resultados: Os três eixos foram detalhados, aliando as contribuições da literatura, com novas possibilidades de aplicação do arcabouço teórico. The disclosure of absences from professional sporting activities to the media is a routine and generally unproblematic part of a sporting career. However, when the reason for the absence relates to mental health concerns, players can encounter difficulties in trying to define, describe and conceptualise their own issues while attempting to maintain privacy as they undergo assessment and treatment. Drawing on ethnomethodology and conversation analysis principles and methods, this paper explores first/initial public mental health disclosure narratives produced by players and sporting organizations across several professional sports via media interviews, press statements, and social media posts. The analysis focuses on (in)voluntary accounts produced by teams or players themselves during their careers and examines the different communication strategies they employ to categorise and explain their predicament. The analysis reveals how some players provide partial or proxy public disclosure announcements (due to a desire to mask issues or delayed help-seeking and assessment), whereas others prefer fuller disclosure of the problems experienced, including diagnoses and on-going treatment and therapy regimes. The paper outlines the consequences of these disclosure strategies and considers the implications they can have for a player’s wellbeing in these stressful circumstances.

Abstract Background Poor sleep and poor mental health go hand in hand and, together, can have an adverse impact on physical health. Given the already disproportionate physical health inequities experienced by people with a mental health condition worldwide, the need to consider and optimise sleep has been highlighted as a means of improving both physical and mental health status. Sleep recommendations recently developed by the United States’ National Sleep Foundation incorporate a range of sleep parameters and enable the identification of ‘suboptimal’ sleep. Among community-dwelling persons with and without a 12-month mental health condition in Australia, this study reports: [1] the prevalence of ‘suboptimal’ sleep and [2] rates of sleep assessment by a health care clinician/service and receipt of and desire for sleep treatment. Methods A descriptive study (N = 1265) was undertaken using self-report data derived from a cross-sectional telephone survey of Australian adults, undertaken in 2017. Results Fifteen per cent (n = 184) of participants identified as having a mental health condition in the past 12 months. Across most (7 of 8) sleep parameters, the prevalence of suboptimal sleep was higher among people with a mental health condition, compared to those without (all p < 0.05). The highest prevalence of suboptimal sleep for both groups was seen on measures of sleep duration (36–39% and 17–20% for people with and without a mental health condition, respectively). In terms of sleep assessment and treatment, people with a mental health condition were significantly more likely to: desire treatment (37% versus 16%), have been assessed (38% versus 12%) and have received treatment (30% versus 7%). Conclusions The prevalence of suboptimal sleep among persons with a mental health condition in Australia is significantly higher than those without such a condition, and rates of assessment and treatment are low for both groups, but higher for people with a mental health condition. Population health interventions, including those delivered as part of routine health care, addressing suboptimal sleep are needed.

Abstract Tobacco use is a leading preventable cause of early mortality and is prevalent among adults with mental health diagnoses, especially in the southern USA. Increasing cessation resources in outpatient mental health care and targeting individuals most receptive to changing their behavior may improve cessation. Drawing on the transtheoretical model, our goals were to develop an educational video about the Louisiana Tobacco Quitline and evaluate its acceptability. We designed the video with knowledge derived from Louisiana-specific data (2016 Louisiana Adult Tobacco Survey, N = 6,469) and stakeholder feedback. Bivariate associations between demographic/tobacco-use characteristics and participants’ stage of quitting (preparation phase vs. nonpreparation phase) were conducted, which informed design elements of the video. Four stakeholder advisory board meetings involving current smokers, mental health clinicians, and public health advocates convened to provide iterative feedback on the intervention. Our stakeholder advisory board (n = 10) and external stakeholders (n = 20) evaluated intervention acceptability. We found that 17.9% of Louisiana adults were current smokers, with 46.9% of them in the preparation phase of quitting. Using insights from data and stakeholders, we succeeded in producing a 2-min video about the Louisiana Tobacco Quitline which incorporated three themes identified as important by stakeholders: positivity, relatability, and approachability. Supporting acceptability, 96.7% of stakeholders rated the video as helpful and engaging. This study demonstrates the acceptability of combining theory, existing data, and iterative stakeholder feedback to develop a quitline educational video. Future research should examine whether the video can be used to reduce tobacco use.
