Abstract BACKGROUND Validation of the 2016 LANO MRI scorecard for leptomeningeal metastasis failed for multiple reasons. The objective of this joint EORTC Brain Tumor Group and RANO effort was to validate the feasibility of the revised MRI scorecard for assessing response in leptomeningeal metastasis. METHODS Coded paired cerebrospinal MRI of 20 patients with leptomeningeal metastases from solid cancers at baseline and follow-up after treatment and instructions for assessment were provided via the EORTC imaging platform. The kappa coefficient (K) was used to evaluate inter-observer pairwise agreement. Statistical analyses were performed using SAS V9.4 software (Cary, NC). The sponsor of the study was the University Hospital Zurich (2018-00192). RESULTS Thirty-five raters participated, including 9 neuroradiologists, 17 neurologists, 4 radiation oncologists, 3 neurosurgeons and 2 medical oncologists. Among leptomeningeal metastases-related items at baseline, the best median concordance was noted for hydrocephalus (K=0.63), and the worst median for spinal linear enhancing disease (K=0.46). The median concordance for overall response was moderate (K=0.44). Notably, the interobserver agreement for the presence of parenchymal brain metastases at baseline was minimal (K=0.29). Significant differences were observed when considering the specialty of the raters. Only 394 of 700 ratings (56%) were fully completed. Among 394 fully completed ratings, perfect concordance was noted in 293 ratings (74%) when comparing the overall response according to the guidelines provided in the scorecard and the overall response provided by the raters. The main discordances were noted for partial response according to the rater versus stable disease according to the guidelines (n=44), followed by progression according to the raters versus stable disease according to the guidelines (n=23). CONCLUSION Electronic case report forms with "blocking solutions" are probably required to enforce completeness and quality of scoring. These results confirm the necessity of central review and the need for training of MRI assessment in clinical trials.

Objective: To report the experience lived during the Integrated Multiprofessional Residency in Family Health at the Faculty of Medical Sciences of the University of Pernambuco – FCM/UPE – at the interface with the Residency in Hospital Dentistry with a focus on Oncology at the University of Pernambuco developed at the Oncology Center at the Oswaldo Cruz University Hospital of the University of Pernambuco – CEON/HUOC/UPE. Methods: This is an experience report of dental practice in the context of professional training, in SUS, both in primary, secondary and tertiary care. Results: It is noticeable the need for greater professional qualification of the graduate, their integration in the Services of the institutionalized network and their performance with efficiency in multiprofissional teams, emphasizing the importance of professional training through Residencies, allowing the dental surgeon to improve in the diferente fields of Dentistry activities and, in particular, rescuing the inter-trans-multidisciplinary performance in both primary and hospital care and in the area of oncology. It was noticed that the integrated performance of the dental surgeon in a multidisciplinar team in low and medium complexity with resolvability in oral care is essential for the quality of care in the hospital context. Conclusion: Dental care directly impacts the quality of care and patient’s lives. The training of the dental surgeon throug the Residencies can provide skills and contribute with the other professions to the patient’s quality of life. In addition to its assistance character, the Dentistry Service – CEON/HUOC/UPE assumes an important education function, serving as a teaching research and extension field.

Trata-se de um relato de experiência com o objetivo de descrever a construção de manual educativo com informações e orientações sobre cateterismo vesical intermitente limpo. O estudo foi realizado entre os meses de junho e agosto de 2018, no setor de traumatologia e neurocirurgia do Hospital Universitário da Universidade Federal do Maranhão, São Luís, Maranhão, Brasil. O manual discorre sobre a técnica do procedimento cateterismo vesical intermitente limpo, com orientações e figuras ilustrativas voltadas aos pacientes com disfunção neurogênica do trato urinário inferior como sequela de trauma raquimedular. Identificou-se a importância da elaboração deste manual, como um material de apoio para o fornecimento de instruções e ensinamentos para a realização do cateterismo vesical intermitente limpo, visando a prevenção e redução de infecções urinárias e insuficiência renal, e ainda reinserção do paciente lesado medular ao convívio social proporcionando melhoria em sua qualidade de vida.Descritores: Cateterismo Uretral Intermitente, Educação em Saúde, Enfermagem. Construction of educational manual on clear intermitent bladder cathederism: experience reportAbstract: This is an experience report with the objective of to describe the construction of an educational manual with information and guidance on clean intermittent bladder catheterization. The study was carried out between June and August 2018, in the trauma and neurosurgery sector of the University Hospital of the Federal University of Maranhão, São Luís, Maranhão, Brazil. The manual discusses the technique of the clean intermittent bladder catheterization procedure, with guidelines and illustrative figures aimed at patients with neurogenic lower urinary tract dysfunction as a sequel of spinal cord trauma. In order to assess hospital emergency rooms, a comprehensive national epidemiological investigation was conducted in all 155 public emergency structures in Tunisia. Here we present the main results of the levels of availability and use of emergency services. Coverage of the population by services is adequate [one emergency service per 60, 000 people]. Emergency wards admit about 2, 500, 000 patients every year [a quarter of the population of the country]. The university hospital emergency services are the most heavily used [150 patients per day on average]. Emergency services are sought for medical [60%], surgical [18%], paediatric [14%] and gynaecological reasons [5%]. It would be useful to assess the quality of care delivered and the satisfaction of citizens and health workers.

he article analyzes the teaching methods at the Department of Orthopedic Dentistry of KNMU, taking into account the modern needs of the development of teaching and reforming the healthcare system of Ukraine. At present, when the process of Ukraine’s integration into the European space is on going, the European integration of medical education is the introduction of the European credit transfer system (ECTS) into the educational process, which helps to achieve the quality of medical education and is by introducing standards, recommendations and basic tools for the adaptation of our students and teachers to the system of education and teaching in European universities. At the Department of Orthopedic Dentistry, various teaching methods are used, namely: practical exercises that use modern pedagogical systems; various forms of presentation of lecture material, taking into account modern methods; phantom-simulation system for work-out of practical skills; consolidation of the gained practical and theoretical knowledge in the process of industrial practice on the basis of the University Hospital. Teaching students the skills of finding and using information in Internet resources. Teaching in English and writing methodological instructions on the subject allows you to attract students from different countries to study and to be involved in communication at the same level with the international dental community.

Background. The correlation between epidurography contrast patterns and the clinical outcomes of percutaneous epidural neuroplasty (PEN) remains unclear. Objective. To analyze the correlation between postadhesiolysis epidurography contrast patterns and the clinical outcomes of patients who undergo lumbar PEN. Design. This study is a retrospective analysis of 78 consecutive patients who underwent lumbar PEN between April 2012 and March 2013. Setting. The analysis was done in the university hospital center. Method. The clinical outcomes of all patients were assessed before and 1, 3, 6, and 12 months after undergoing lumbar PEN. Specifically, the intensity of back and leg pain, quality of life, and procedural outcomes were evaluated using a visual analog scale (VAS), the Oswestry Disability Index (ODI), and the 12-Item Short-Form Health Survey (SF-12). Results. The VAS scores for back and leg pain, ODI score, and SF-12 score exhibited a significant improvement during the follow-up period (P<0.01 versus preprocedural scores). At most follow-up time points, patients exhibiting extraforaminal contrast distribution n=22 on postadhesiolysis epidurograms exhibited a similar improvement in VAS scores and a significantly better improvement in ODI and SF-12 scores compared with patients exhibiting intracanal contrast distribution n=56. Conclusion. Extraforaminal contrast distribution during lumbar PEN may be associated with better functional outcomes.
