The Dynamics of Dementia Communication

Despite a plethora of good advice, it can be hard to sustain effective communicative behaviours when someone is living with a dementia. This book asks why that is. Part 1 explores how various dementia-causing diseases affect the linguistic, pragmatic (reasoning), and memory systems; how social perceptions and practices exacerbate the underlying biological problems; how people living with a dementia describe their experiences; and how dementia care currently addresses the challenges of communication. Part 2 asks why people communicate and what shapes how they communicate. The Communicative Impact model of communication is introduced and theoretically justified. It is argued that all communication is driven by people’s desire to make beneficial changes to their experiential world by getting the hearer to do, say, think, or feel something. Part 3 applies the model from part 2 to the range of considerations explored in part 1, helping readers see how and why communication is undermined and reshaped by the various biological, social, and emotional factors underlying the dementia experience. The model is used to shed light on how people living with a dementia are perceived and, as a result, treated, with particular attention to the acceptability of (well-intentioned) deception. The final chapter asks what needs to change if communication and well-being are to be optimized for people living with a dementia. In pursuit of truly person- and relationship-centred care, proposals for advanced skills in communication with a person living with a dementia are presented and explained, helping anchor the ubiquitous dos and don’ts in a deeper understanding of why interaction is difficult.

This final chapter draws together the ideas from the book to consider what the priorities should be for improving communication by and with people living with a dementia. The main ideas from the book are summarized. Desirable features for effective and humane communication in the dementia context are suggested, including: opportunities for ‘real’ communication; practical support, flexibility, and empowerment; and respect, dignity, and kindness. The importance of building and sustaining social and emotional reserve is reiterated. These ideas are linked to existing practices to help shed light on what is most likely to work and why. Kindness is identified as the lynchpin for intuitively embracing these approaches.

Among the most vulnerable immigrants and refugees are young children, whose lack of maturity and autonomy leave them dependent on adult caregivers. With a focus on the transition from home to school, this chapter integrates developmental and school transition theories in order to shed light on risks among young immigrant and refugee children as well as opportunities to support their development. Guided by this combined framework, the authors examine linkages from parents' experiences with acculturation stress and microaggressions to children's social and emotional development. The chapter also discusses sources of resilience, such as parents' own positive childhood experiences. The authors then address the need for future research to utilize mixed methods designs to help advance knowledge on the adaptation of preschoolers from immigrant and refugee backgrounds. Lastly, the chapter discusses using such methods as part of research-community partnerships focused on the early education of immigrant and refugee children.

Lastly, on the basis of this definition, the author shows how affects shed light on the body-mind relationship and provide an opportunity to produce a mixed discourse that focuses, by turns, on the mental, physical, or psychophysical aspect of affect. The final chapter has two parts: – An analysis of the three categories of affects: mental, physical, and psychophysical – An examination of the variations of Spinoza’s discourse Some affects, such as satisfaction of the mind, are presented as mental, even though they are correlated with the body. Others, such as pain or pleasure, cheerfulness (hilaritas) or melancholy are mainly rooted in the body, even though the mind forms an idea of them. Still others are psychophysical, such as humility or pride, which are expressed at once as bodily postures and states of mind. These affects thus show us how the mind and body are united, all the while expressing themselves differently and specifically, according to their own modalities.

The final chapter concludes with five contemporary ‘uses’ of social investment, in full recognition of limits underscored by critics. The first ‘use’ of social investment therefore concerns its ‘paradigmatic’ bearings. To what extent does social investment represent a distinct policy paradigm for twenty-first-century welfare capitalism? A second ‘use’ relates to paradigm change, in the sense of theoretical progress inspiring interdisciplinary methodological innovation, in particular with respect to the empirical assessment of well-being ‘returns’ on social investment. The third more practical ‘use’ covers the identification of virtuous social investment policy mixes of ‘stocks’, ‘flows’, and ‘buffers’. The fourth ‘use’ is geographically confined to the European conundrum of overcoming the fiscal austerity to make way for social investment reform, as means to reignite socioeconomic convergence, at least for the Eurozone. The more general final use of social investment bears on the ‘politics of social investment’ in the aftermath of the financial crisis.

Deaf and hard-of-hearing (DHH) children who use spoken language face unique challenges when communicating with others who have typical hearing, particularly their peers. In such contexts, the social use of language has been recognized as an area of vulnerability among individuals in this population and has become a focus for research and intervention. The development of pragmatic skills intersects with many aspects of child development, including emotional intelligence and executive function, as well as social and emotional development. While all these areas are important, they are beyond the scope of this chapter, which highlights the impact of pragmatics on the specific area of cognition. Cognitive pragmatics is broadly defined as the study of the mental processes involved in the understanding of meaning in the context of a cooperative interaction. This chapter explores how DHH children and young people construe meaning in the context of conversations and expository interactions with their peers. The chapter aims to examine the role played by the cognitive processes of making inferences and comprehending implicature, within the overall display of pragmatic skills. Further, the authors use this lens in the analysis of interactions between DHH children and their peers in order to shed light on the development of pragmatic skills in children who are DHH.

Chronic rhinosinusitis is a common condition affecting 5–12% of the general population worldwide. In a limited number of cases, the disease is recalcitrant to medical and surgical interventions, causing a major impact on physical, social and emotional well-being and increasing pressure on healthcare systems. Biofilm formation and dysbiosis caused by Staphylococcus aureus and Pseudomonas aeruginosa play a role in the pathogenesis of recalcitrant chronic rhinosinusitis. In these cases, a promising treatment alternative is the application of bacteriophages, which are viruses that infect and lyse bacteria. In this review, we appraise the evidence for the use of bacteriophages in the treatment of recalcitrant chronic rhinosinusitis. Additionally, (dis)advantages of bacteriophages and considerations for implementation of phage therapy in otorhinolaryngology practice will be discussed.

Abstract Background Lithuanian has experienced rapid and relatively successful transition to capitalism and transatlantic-Western values and practices in recent decades. Suicide and legal alcohol consumption rates remain the highest in the EU and OECD, and Lithuania's Positive emotions index score has been consistently amongst the 10 lowest in the world in recent years. Emigration rate is the highest in the EU and OECD, social inequality is amongst the highest in the same groups. Despite a relatively strong economy these changes are not automatically transferred to better social and emotional well-being. Methods This unconventional situation calls for strong and unconventional policy responses in the area of mental health, amongst others. Health impact assessment using theory-based approach and desk research. Results Recent attempts include strategic reorientation towards social and emotional well-being, stronger focus on prevention and promotion and primary care, intensifying efforts to reduce suicide rates, and implementation of strong regulatory measures to reduce alcohol use. Suicide rates have been declining since 2013 but remain high. Effort is made to implement best practices in media reporting, recognition and providing appropriate help for those in suicide crisis, expanding prevention efforts. Conclusions Finally, WHOs “best buys” in alcohol control were implemented since 2017, including steep increases in excise taxes, ban on alcohol advertisement, increase of allowed age, reducing alcohol trading hours and others. This resulted in the steepest decline of legal alcohol consumption in decades. The chosen policy route may be called iterative policy-making, whereby improvements are made along the way, which allows for quicker policy decisions and results. Key messages Mental health policies need to be based on the socio-political context of the specific country. Smaller countries such as Lithuania might benefit form flexible approaches to mental health policies.

Abstract Dementia poses a societal challenge that is life-changing not only for persons with dementia (PWD) but also for family members and friends (informal carers) directly involved in the care arrangement. Informal carers (IC) have typically poorer outcomes in terms of well-being, quality of life (QoL), health status, and use of health care resources. Dyads of PWD and IC living with dementia are characterized by strong reciprocal relationships and complex living contexts. Therefore, research should investigate home based dementia caregiving from a dyadic perspective to yield interventions that support the PWD, the IC, and the unit as a whole. However, it is an ongoing challenge to investigate dyadic needs and preferences in daily practice and develop effective interventions. Challenges are related to incomplete understanding of dyadic characteristics, attitudes and beliefs within the dyad, as well as how to adapt research approach to engage and retain the dyad in research. This international symposium will therefore address these issues. The first presentation will describe a typology of dementia care dyad characteristics and needs in Germany. The second presentation will examine the challenges and opportunities associated with recruiting and retaining dementia dyads. The third presentation will explore ethical challenges posed in communication with dyads and possible solutions for the researcher. The final presentation reports on the Meeting Centre Support Program as an example of an effective psychosocial intervention employing research strategies that transcend cultural barriers. Our discussant, Katie Maslow, will synthesize the presentations and lead a discussion of future directions for policy and practice.

Although several studies have documented a distinct marriage advantage in well-being, it is still unclear what it is about marriage that renders this benefit. We hypothesize that it is due to factors theorized to accrue to matrimony, such as elevated financial status and specific social psychological supports. We examine the trajectory of subjective well-being for 1135 respondents from the three-wave 2010 GSS panel survey utilizing linear mixed-effects modeling. We find that about two-fifths of the marriage advantage in subjective well-being is accounted for by a mixture of control variables, finances, and emotional factors, with most of this due to elements that are associated with the marital context. Higher annual income, enhanced interpersonal trust, greater sociability, and less of a sense of loneliness and isolation appear to be responsible for a substantial component of the marital advantage. We further find that the marriage advantage is invariant to both race and gender.

Abstract Background Increasing academic demands, including larger amounts of assigned homework, is correlated with various challenges for children. While homework stress in middle and high school has been studied, research evidence is scant concerning the effects of homework on elementary-aged children. Objective The objective of this study was to understand rater perception of the purpose of homework, the existence of homework policy, and the relationship, if any, between homework and the emotional health, sleep habits, and parent–child relationships for children in grades 3–6. Method Survey research was conducted in the schools examining student (n = 397), parent (n = 442), and teacher (n = 28) perception of homework, including purpose, existing policy, and the childrens’ social and emotional well-being. Results Preliminary findings from teacher, parent, and student surveys suggest the presence of modest impact of homework in the area of emotional health (namely, student report of boredom and frustration), parent–child relationships (with over 25% of the parent and child samples reporting homework always or often interferes with family time and creates a power struggle), and sleep (36.8% of the children surveyed reported they sometimes get less sleep) in grades 3–6. Additionally, findings suggest misperceptions surrounding the existence of homework policies among parents and teachers, the reasons teachers cite assigning homework, and a disconnect between child-reported and teacher reported emotional impact of homework. Conclusions Preliminary findings suggest homework modestly impacts child well-being in various domains in grades 3–6, including sleep, emotional health, and parent/child relationships. School districts, educators, and parents must continue to advocate for evidence-based homework policies that support children’s overall well-being.
