Abstract Objective Investigate the association between physical activity and pain severity in individuals with knee osteoarthritis. Design Cross-sectional; systematic review with meta-analyses. Methods Thirty-one participants with knee osteoarthritis underwent assessment of symptoms via self-report questionnaires and quantitative sensory testing. Following testing, physical activity and symptoms were monitored for seven days using accelerometers and logbooks. Cross-correlation analyses were performed on fluctuations in symptoms and physical activity across the week to detect the relative timing of the strongest association between pain and activity. These data were complemented by meta-analyses of studies that examined correlations between pain from knee osteoarthritis and physical activity or fitness. Results Pain severity at baseline correlated with moderate to vigorous physical activity (r2 = 0.161–0.212, P < 0.05), whereby participants who were more physically active had less pain. Conversely, the peak of the cross-correlation analyses was most often positive and lagging, which indicated that pain was increased subsequent to periods of increased activity. These superficially discrepant findings were supported by the results of a meta-analysis of 13 studies and 9,363 participants, which identified significant heterogeneity for associations between physical activity and pain (I2 = 91%). Stronger inverse associations were found between fitness and pain. Conclusions Associations between physical activity and pain in people with knee osteoarthritis are variable and dynamic. These results reflect the beneficial impact of an active lifestyle and accompanying higher fitness. Yet, the side effect of acute periods of physical activity to transiently exacerbate pain may influence the behavior of some people to avoid activity because of pain.

Abstract Background Fatigue is believed to include both physical and mental components, suggesting there may be fatigue dimensions. Evidence suggests that physical activity (PA) and mental activity (MA) play a role on fatigue in those with knee osteoarthritis (OA); however, it is unclear how the patterns of physical and mental activity relate to fatigue and its dimensions, particularly in knee OA population. Therefore, the study aims were (1) to estimate the within day contribution of physical fatigue (PF) and mental fatigue (MF) to general fatigue (GF) and (2) to explore within day dynamic and temporal associations between PA categories, MA intensity and fatigue dimensions (PF, MF, & GF) using an ecological momentary assessment (EMA) and time series approach in individuals with doctor-diagnosed knee OA. Methods Twenty-three participants with self-reported doctor-diagnosed knee OA were included in this EMA study. Participants self-reported GF, PF, MF and MA intensity three times daily at designated periods for seven days using a 0-10 visual analogue scale. Concurrently, PA (step counts, standing time, light intensity physical activity [LIPA] time, LIPA+standing time and moderate-to-vigorous physical activity [MVPA] time) was measured using an activPAL accelerometer for one week. Compositional data analysis was used to estimate the contribution of within day PF and MF to GF and cross-correlation analyses to estimate within day association of PA categories, MA intensity and fatigue dimensions (PF, MF, & GF). The absolute value of cross correlation was considered significant if it fell within the 95% confidence interval of the mean. Results Compositional analysis revealed that PF contributed more than MF to GF within day. However, the contribution of MF to GF rose within day while PF’s contribution stayed stable. Cross-correlation analyses showed that previous GF negatively predicted future step count (r= -0.62; CI[-0.99, -0.25]), LIPA time (r= -0.62; CI[-0.99, -0.25]), LIPA + standing time (r= -0.43; CI[-0.85, -0.01]) within day. MA and GF had a bidirectional association, with MA positively predicting GF and GF negatively predicting MA intensity. A concurrent positive association was noted for MA and MF (r = 0.34; CI[0.01, 0.67]) and MF negatively predicted MA (r= -0.43; CI[-0.85, -0.01]). Conclusion These findings suggest higher contribution of PF to GF within day while noting the potential persistence of MF within day. Further, the within day cross-correlation support a potential reduction in GF may increase within day PA; but did not confirm a possible beneficial effect of within day increase in PA on fatigue types. GF and MF may be important components that influence MA intensity within day. Decreasing or treating MF may have positive impact on increasing MA intensity within day. Background: To improve compliance and adherence to exercise, the concept of temporal consistency has been proposed. Before- and after-work are periods when most working adults may reasonably incorporate exercise into their schedule. However, it is unknown if there is an association between the time-of-day that exercise is performed and overall physical activity levels. Methods: Activity was assessed over 1 week in a sample of 69 active adults (n = 41 females; mean age = 34.9 [12.3] y). At the end of the study, participants completed an interviewer-assisted questionnaire detailing their motivation to exercise and their exercise time-of-day preferences. Results: Participants were classified as “temporally consistent” (n = 37) or “temporally inconsistent” (n = 32) exercisers based on their accelerometry data. The “temporally consistent” group was further analyzed to compare exercise volume between “morning-exercisers” (n = 16) and “evening-exercisers” (n = 21). “Morning-exercisers” performed a greater volume of exercise than “evening-exercisers” (419 [178] vs 330 [233] min by self-report; 368 [224] vs 325 [156] min actigraph-derived moderate to vigorous physical activity, respectively). Conclusions: Our findings suggest that active individuals use a mixture of temporal patterns to meet PA guidelines. Time-of-day of exercise should be reported in intervention studies so the relationship between exercise time-of-day, exercise behavior, and associated outcomes can be better understood.

Maintenance of recommended physical activity levels is associated with favorable cardiovascular outcomes. Consequently, the American Heart Association (AHA) included physical activity in the new Ideal Cardiovascular Health concept. Although physical activity is known to decline with age, the age-specific probability of transitioning between AHA defined levels (ideal, intermediate, poor) is unclear. This knowledge could inform interventions seeking to increase or maintain physical activity levels across the life-course. We estimated age-specific prevalence of ideal (≥150 min/wk moderate or ≥75 min/wk vigorous or equivalent combination), intermediate (1-149 min/wk moderate or 1-74 min/wk vigorous or equivalent combination) and poor (0 min of moderate and vigorous) physical activity among adult (16+) European Americans (EA) and African Americans (AA) from the 2007-2010 NHANES (n=13,418) and Hispanic/Latino participants (H/L) from the 2008-2011 HCHS/SOL (n=14,291). Prevalence estimation incorporated data from identical NHANES and HCHS/SOL self-report questionnaires. We then used the prevalence estimates to calculate age, race, and sex-specific probabilities of transitioning between levels using Markov-type state-transition models. These models are designed to estimate transition probabilities from cross-sectional data assuming time-constant transitions, and have been updated to accommodate complex sampling procedures. The estimated 5-year probability of maintaining ideal physical activity levels remained high through the fourth decade of life, after which race and sex differences were observed. For example, the estimated 5-year probabilities of transitioning from ideal to intermediate physical activity levels were similar at age 40 for EA and AA men (1.2% [95%CI: 0.3, 2.3] vs. 2.2% [95%CI: 1.0, 3.6]), but by age 65 diverged to 5.3% (95%CI: 3.9, 6.9) vs. 11.6% (95%CI: 8.5, 15.2), respectively. During middle age, the estimated probability of transitioning from intermediate to poor physical activity levels was accelerated, although variation by race was noted. For example, the estimated probability of transitioning from intermediate to poor physical activity levels was <25% for EA, AA and H/L men through age 40, but by age 65 this increased to 32.6% (95%CI: 22.0, 44.1) for EA men and 41.1% (95%CI: 26.1, 58.0) for AA men while remaining low (11.1% (95%CI: 0.3, 24.3) for H/L men. Across all race-sex groups, individuals with poor physical activity levels had a 0% estimated probability of transitioning to ideal or intermediate physical activity levels, regardless of age. These results indicate need for interventions aimed at maintaining ideal physical activity levels beginning in young adulthood, particularly in AAs who transition from ideal levels at earlier ages than other race/ethnic groups. Innovative efforts may be needed for those with poor physical activity.

Background:Perceived barriers to moderate-to-vigorous physical activity (PA) may contribute to the low rates of moderate-to-vigorous PA in adolescents. We examined the psychometric properties of scores from the perceived barriers to moderate-to-vigorous PA scale (PB-MVPA) by examining composite reliability and validity evidence based on the internal structure of the PB-MVPA and relations with other variables.Methods:This study was a cross-sectional analysis of data collected in 2013 from adolescents (N = 507; Mage = 12.40, SD = .62) via self-report scales.Results:Using exploratory and confirmatory factor analyses, we found that perceived barriers were best represented as two factors representing internal (e.g., “I am not interested in physical activity”) and external (e.g., “I need equipment I don’t have”) dimensions. Composite reliability was over .80. Using multiple regression to examine the relationship between perceived barriers and moderate-to-vigorous PA, we found that perceived internal barriers were inversely related to moderate-to-vigorous PA (β = -.32, p < .05). Based on results of the analysis of variances, there were no known-group sex differences for perceived internal and external barriers (p < .26).Conclusions:The PB-MVPA scale demonstrated evidence of score reliability and validity. To improve the understanding of the impact of perceived barriers on moderate-to-vigorous PA in adolescents, researchers should examine internal and external barriers separately.

Abstract Background Monitoring population-level physical activity is crucial for examining adherence to global guidelines and addressing obesity. This study validated self-reported moderate-to-vigorous physical activity (MVPA) against an accurate device-based method in Namibia. Methods Adolescent girls (n = 52, mean age 16.2 years [SD 1.6]) and adult women (n = 51, mean age 31.3 years [SD 4.7]) completed the PACE+/GPAQ self-report questionnaires and were asked to wear an Actigraph accelerometer for 7 days. Validity of self-reported MVPA was assessed using rank-order correlations between self-report and accelerometry, and classification ability of the questionnaires with Mann–Whitney tests, kappa’s, sensitivity and specificity. Results In the adolescents, Spearman’s rank coefficients between self-reported MVPA (days/week) and accelerometry measured MVPA were positive but not significant (r = 0.240; P = 0.104). In the adults, self-reported MVPA (minutes/day) was moderately and significantly correlated with accelerometer-measured MVPA (r = 0.396; P = 0.008). In both groups, there was fair agreement between accelerometry and questionnaire-defined tertiles of MVPA (adolescents κ = 0.267; P = 0.010; adults κ = 0.284; P = 0.008), and measured MVPA was significantly higher in the individuals self-reporting higher MVPA than those reporting lower MVPA. Conclusions The PACE+ and GPAQ questionnaires have a degree of validity in adolescent girls and adult females in Namibia, though more suitable for population than individual level measurement.

Background: The self-distancing measures imposed major changes in daily life routine. This study aimed to (i) evaluate the changes (pre-versus during pandemic) in time spent in moderate to vigorous physical activity (MVPA) and sedentary behavior (SB) in selfisolating Brazilians during the COVID-19 pandemic, and (ii) to explore correlates of MVPA and SB. Methods: A cross-sectional self-report online survey, evaluating the time spent in MVPA and SB pre and during the COVID-19 pandemic. Sociodemographic, behavioral, clinical, variables, and time in self-isolation were also obtained. Changes in MVPA and SB and their correlates were explored using generalized estimating equations (GEE). Results: A total of 877 participants (72.7% women, 53.7% young adults [18-34 years]) were included. Overall, participants reported a 59.7% reduction (95%CI:35.6 to 82.2) in time spent on MVPA during the pandemic. Time spent in SB increased 42.0% (95%CI:31.7 to 52.5). Greater reductions in MVPA and/or increases in SB were seen in younger adults, those not married, those employed and those with a self-reported previous diagnosis of a mental disorder. Conclusions: People in self-isolation significantly reduced MVPA levels and increased SB. Public health strategies should be implemented during epidemic times to mitigate the impact of self-isolation on MVPA and SB, particularly in vulnerable groups.

AbstractIt remains unclear if participation in moderate-to-vigorous physical activity (MVPA) differs between children with ASD and typically developing children (TDC). We compared daily MVPA, time spent in MVPA during physical education (PE) and recess, and odds of not meeting MVPA recommendation (60 min/day) between children with ASD and TDC. Nine studies reporting accelerometer-measured MVPA were included in the meta-analyses. MVPA was 30 min lower/day, 12% and 8% lower during PE and recess, respectively, in children with ASD, and they had 4 times higher odds of not meeting MVPA recommendation when compared to TDC. Children with ASD engage in daily MVPA less than TDC and below the guidelines. Tailored interventions to increase MVPA in children with ASD are warranted.

Background:The purpose of this study was to evaluate cross-sectional and longitudinal associations between physical activity and triglyceride and HDLc levels in young male adults.Methods:We used information about males belonging 1982 Pelotas Birth Cohort. Physical activity in 4 domains (leisure time, transportation, household, and occupation) was assessed by self-report in participants of the cohort at ages of 18 and 23 years. Subjects were active if reached the recommendation of 150 min/week of moderate to vigorous physical activity. At 23 years of age, blood sample was collected, and triglycerides and HDLc levels estimated. Multivariate linear and Poisson regression were used to adjust the estimates for confounders.Results:Males who were inactive at 18 and active at 23 years had 41% lower risk (β = 0.59; 95% confidence interval: 0.40; 0.89) for borderline-high triglycerides (≥ 150 mg/dL) as compared with those who were inactive at both follow-ups. No association was found between changes of physical activity and HDLc level. In cross-sectional analyses, greater HDLc levels were found in active subjects in 4 domains, whereas there was no difference in HDL levels according physical activity during leisure time.Conclusion:Becoming active from adolescence to early adulthood reduced the risk for high triglycerides. Current physical activity was associated with greater HDLc levels.

Purpose:To assess correlates of physical activity, and to examine the relationship between physical activity and physical functioning, in 160 older (66 ± 6 years old), overweight/obese (mean body mass index = 33.5 ± 3.8 kg/m2), sedentary (less than 30 mins of activity, 3 days a week) individuals with knee osteoarthritis.Methods:Physical activity was measured with accelerometers and by self-report. Physical function was assessed by 6-min walk distance, knee strength, and the Short Physical Performance Battery. Pain and perceived function were measured by questionnaires. Pearson correlations and general linear models were used to analyze the relationships.Results:The mean number of steps taken per day was 6209 and the average PAEE was 237 ± 124 kcal/day. Participants engaged in 131 ± 39 minutes of light physical activity (LPA) and 10.6 ± 8.9 minutes of moderate-vigorous physical activity (MPA/VPA). Total steps/day, PAEE, and minutes of MPA/VPA were all negatively correlated with age. The 6-min walk distance and lower extremity function were better in those who had higher total steps/day, higher PAEE, higher minutes of MPA/VPA, and a higher PASE score.Conclusions:This study demonstrates that a population who has higher levels of spontaneous activity have better overall physical function than those who engage in less activity.

Purpose: Moderate-to-vigorous physical activity (MVPA) positively influences bone mineral content (BMC) in prepubertal children, but it is unknown whether this relationship is partially mediated by free leptin index. The aim of this study was to examine whether the relationship between MVPA and total body less head (TBLH) BMC is mediated or moderated by free leptin index in prepubertal children. Methods: We performed a cross-sectional analysis on 401 children (194 girls) from baseline examinations of the Physical Activity and Nutrition in Childhood Study. We applied the four-way decomposition mediation analysis method to assess whether free leptin index, measured from fasted blood samples, mediated the relationship between accelerometer-measured MVPA and TBLH BMC measured by dual-energy X-ray absorptiometry. Results: MVPA had a positive controlled direct effect on TBLH BMC in girls and boys (β = 0.010 to 0.011, p < 0.05). There was no mediation or interaction between MVPA, free leptin index and TBLH BMC in girls or boys (β = −0.000 to 0.001, p > 0.05). Conclusion: Our study indicates that MVPA positively influences TBLH BMC through pathways not related to free leptin index in predominantly normal-weight prepubertal children, likely primarily through mechanical loading. The relationships between MVPA, free leptin index and TBLH BMC may be influenced by other factors such as pubertal status and adiposity, so it is unknown whether these observations extend to overweight and obese children at different stages of puberty.
