In this paper, we examine the interaction of convection, rotation and mean flows in a thermal annulus. In this system, mean flows are driven by correlations induced by rotation leading to non-trivial Reynolds stresses. The mean flows act back on the convective turbulence acting as a barrier to transport. For this system, we demonstrate that the generalized quasilinear approximation (Marston et al . 2016 Phys. Rev. Lett. 116 , 214501. ( doi:10.1103/PhysRevLett.116.214501 )) may provide a much better approximation to the complicated full nonlinear dynamics than the widely used quasilinear approximation. This result will enable the construction of more accurate statistical theories for the description of geophysical and astrophysical flows.

Abstract Background Metabolic status can be impacted by general anesthesia and surgery. However, the exact effects of general anesthesia and surgery on systemic metabolome remain unclear, which might contribute to postoperative outcomes. Methods Five hundred patients who underwent abdominal surgery were included. General anesthesia was mainly maintained with sevoflurane. The end-tidal sevoflurane concentration (ETsevo) was adjusted to maintain BIS (Bispectral index) value between 40 and 60. The mean ETsevo from 20 min after endotracheal intubation to 2 h after the beginning of surgery was calculated for each patient. The patients were further divided into low ETsevo group (mean − SD) and high ETsevo group (mean + SD) to investigate the possible metabolic changes relevant to the amount of sevoflurane exposure. Results The mean ETsevo of the 500 patients was 1.60% ± 0.34%. Patients with low ETsevo (n = 55) and high ETsevo (n = 59) were selected for metabolomic analysis (1.06% ± 0.13% vs. 2.17% ± 0.16%, P < 0.001). Sevoflurane and abdominal surgery disturbed the tricarboxylic acid cycle as identified by increased citrate and cis-aconitate levels and impacted glycometabolism as identified by increased sucrose and D-glucose levels in these 114 patients. Glutamate metabolism was also impacted by sevoflurane and abdominal surgery in all the patients. In the patients with high ETsevo, levels of L-glutamine, pyroglutamic acid, sphinganine and L-selenocysteine after sevoflurane anesthesia and abdominal surgery were significantly higher than those of the patients with low ETsevo, suggesting that these metabolic changes might be relevant to the amount of sevoflurane exposure. Conclusions Sevoflurane anesthesia and abdominal surgery can impact principal metabolic pathways in clinical patients including tricarboxylic acid cycle, glycometabolism and glutamate metabolism. This study may provide a resource data for future studies about metabolism relevant to general anaesthesia and surgeries. Trial registration www.chictr.org.cn. identifier: ChiCTR1800014327.

Background:Symptom checkers (SC) promise to reduce diagnostic delay, misdiagnosis and effectively guide patients through healthcare systems. They are increasingly used, however little evidence exists about their real-life effectiveness.Objectives:The aim of this study was to evaluate the diagnostic accuracy, usage time, usability and perceived usefulness of two promising SC, ADA (www.ada.com) and Rheport (www.rheport.de). Furthermore, symptom duration and previous symptom checking was recorded.Methods:Cross-sectional interim clinical data from the first of three recruiting centers from the prospective, real-world, multicenter bETTeR-study (DKRS DRKS00017642) was used. Patients newly presenting to a secondary rheumatology outpatient clinic between September and December 2019 completed the ADA and Rheport SC. The time and answers were recorded and compared to the patient’s actual diagnosis. ADA provides up to 5 disease suggestions, Rheport calculates a risk score for rheumatic musculoskeletal diseases (RMDs) (≥1=RMD). For both SC the sensitivity, specificity was calculated regarding RMDs. Furthermore, patients completed a survey evaluating the SC usability using the system usability scale (SUS), perceived usefulness, previous symptom checking and symptom duration.Results:Of the 129 consecutive patients approached, 97 agreed to participate. 38% (37/97) of the presenting patients presented with an RMD (Figure 1). Mean symptom duration was 146 weeks and a mean number of 10 physician contacts occurred previously, to evaluate current symptoms. 56% (54/96) had previously checked their symptoms on the internet using search engines, spending a mean of 6 hours. Rheport showed a sensitivity of 49% (18/37) and specificity of 58% (35/60) concerning RMDs. ADA’s top 1 and top 5 disease suggestions concerning RMD showed a sensitivity of 43% (16/37) and 54% (20/37) and a specificity of 58% (35/60) and 52% (31/60), respectively. ADA listed the correct diagnosis of the patients with RMDs first or within the first 5 disease suggestions in 19% (7/37) and 30% (11/37), respectively. The average perceived usefulness for checking symptoms using ADA, internet search engines and Rheport was 3.0, 3.5 and 3.1 on a visual analog scale from 1-5 (5=very useful). 61% (59/96) and 64% (61/96) would recommend using ADA and Rheport, respectively. The mean SUS score of ADA and Rheport was 72/100 and 73/100. The mean usage time for ADA and Rheport was 8 and 9 minutes, respectively.Conclusion:This is the first prospective, real-world, multicenter study evaluating the diagnostic accuracy and other features of two currently used SC in rheumatology. Abstract Isothermal turbulent mixing of an axisymmetric primary air jet with a low velocity annular secondary air stream inside a constant diameter cylindrical enclosure is predicted. The flow domain from the inlet to the fully developed downstream locations is considered. The predicted flow field properties include the mean velocity and pressure and the Reynolds stresses. Different velocity and diameter ratios between the primary and the secondary jets have been investigated to characterize the flow in terms of these parameters. A bounded stream-wise differencing scheme is used to minimize numerical diffusion and oscillation errors. Predictions are compared with available experimental data to back up numerical findings.

Hot wire measurements at the exit of a small centrifugal compressor impeller are reported. Three different hot wire readings were obtained and stored on a magnetic tape for each point by gating the analogue hot wire signal with a pulse which indicated circumferential position. The combination of the three readings yielded the mean velocity and some Reynolds stresses at each point. The measurements show a ‘jet-wake’ profile towards the shroud and ‘isentropic’ flow near the hub.

Objective. Patients are increasingly using the Internet to inform themselves of health-related topics and procedures, including EGD. We analyzed the quality of information and readability of websites after a search on 3 different search engines. Methods. We used an assessment tool for website quality analysis that we developed in addition to using validated instruments for website quality, Global Quality Score (GQS) and Health on Net (HON) certification. The readability was assessed using Flesch-Kincaid Reading Ease (FRE) and Flesch-Kincaid Grade level (FKG). 30 results of each search terms ‘EGD’ and ‘Upper Endoscopy’ from Google and 15 each from Bing and Yahoo were analyzed. A total of 45 websites were included from 100 URLs after removing duplicates, video links, and journal articles. Results. Only 3 websites were found to have good quality and comprehensive and authentic information. These websites were https://www.healthline.com, https://www.uptodate.com, and https://www.emedicine.medscape.com. There were additional 13 sites with moderate quality of information. The mean Flesch-Kincaid Reading Ease (FRE) score was 46.92 (range 81.6-6.5). The mean Flesch-Kincaid Grade level (FKG) was 11th grade, with a range of 6th grade to 12th grade and above making them difficult to read. Conclusions. Our study shows that there are quite a few websites with moderate quality content. We recommend 3 comprehensive and authentic websites out of 45 URLs analyzed for information on Internet for EGD. In addition, the readability of the websites was consistently at a higher level than recommended by AMA at 11th grade level. In addition, we identified 3 websites with moderate quality content written at 8th grade and below readability level. We feel that gastroenterologists can help their patients better understand this procedure by directing them to these comprehensive websites.

We investigate the saturation of harmonically forced disturbances in the turbulent flow over a backward-facing step subjected to a finite amplitude forcing. The analysis relies on a triple decomposition of the unsteady flow into mean, coherent and incoherent components. The coherent–incoherent interaction is lumped into a Reynolds averaged Navier–Stokes (RANS) eddy viscosity model, and the mean–coherent interaction is analysed via a semi-linear resolvent analysis building on the laminar approach by Mantič-Lugo & Gallaire (2016 J. Fluid Mech. 793 , 777–797. ( doi:10.1017/jfm.2016.109 )). This provides a self-consistent modelling of the interaction between all three components, in the sense that the coherent perturbation structures selected by the resolvent analysis are those whose Reynolds stresses force the mean flow in such a way that the mean flow generates exactly the aforementioned perturbations, while also accounting for the effect of the incoherent scale. The model does not require any input from numerical or experimental data, and accurately predicts the saturation of the forced coherent disturbances, as established from comparison to time-averages of unsteady RANS simulation data.

Abstract Turbulent flow in an elbow has been numerically investigated. The flow was modeled using two approaches; Reynolds Averaged Navier-Stokes (RANS) and Direct Numerical Simulation (DNS) methods. The DNS allows for all the scales of turbulence to be evaluated, providing a detailed depiction of the flow. The RANS simulation, which is typically used in industry, evaluates time-averaged components of the flow. The numerical results are accompanied by experimental data, which was used to validate the two methods. Profiles of the mean and root-mean-square (RMS) fluctuating components were compared at various points along the midplane of the elbow. Upstream of the elbow, the predicted mean and RMS velocities from the RANS and DNS simulations compared well with the experiment, differing slightly near the walls. However, downstream of the elbow, the RANS deviated from the experiment and DNS, showing a longer region of flow re-circulation. This caused the mean and RMS velocities to significantly differ. Examining the cross-section flow field, secondary motion was clearly present. Upstream secondary motion of the first kind was observed which is caused by anisotropy of the reynolds stresses in the turbulent flow. Downstream of the bend, the flow transitions to secondary motion of the second kind which is caused by streamline curvature. Qualitatively, the RANS and DNS showed similar results upstream of the bend, however downstream, the magnitude of the secondary motion differed significantly.

Abstract Separating and reattaching turbulent flows induced by a forward-facing step submerged in thick oncoming turbulent boundary layers developed over smooth and rough walls were investigated using time-resolved particle image velocimetry. Both smooth and fully rough upstream bottom wall conditions were examined and the resultant oncoming boundary layer thickness were 4.3 and 6.7 times the step height, respectively. The Reynolds number based on the step height and free-stream velocity was 7800. The mean velocities, Reynolds stresses analyzed in both Cartesian and curvilinear coordinate systems, eddy viscosity, correlation coefficient and third order moments are discussed. The results indicate that, due to the enhanced turbulence intensity and shear rate in the fully rough case, distinct elevated regions of vertical and shear Reynolds stresses are consistent upstream of the leading edge of the step while the magnitude of the Reynolds stresses are consistently higher than observed in the smooth case. The correlation coefficient, eddy viscosity and third order moments also show distinct elevated regions upstream of the leading edge of the step in the fully rough case. Above the step, distinct elevated regions of the Reynolds stresses, eddy viscosity and correlation coefficient are observed in both cases with the peak values at a vertical location corresponding to the maximum elevation of the mean separating streamline.

We considered the topic of explosions from spherical high-explosive (HE) charges. We studied how the turbulent combustion fields scale. On the basis of theories of dimensional analysis by Bridgman and similarity theories of Sedov and Barenblatt, we found that all fields scaled with the explosion length scale r0. This included the blast wave, the mean and root mean squared (RMS) profiles of thermodynamic variables, combustion variables, velocities, vorticity, and turbulent Reynolds stresses. This was a consequence of the formulation of the problem and our numerical method, which both satisfied the similarity conditions of Sedov. We performed numerical simulations of 1 g charges and 1 kg charges; the solutions were identical (within roundoff error) when plotted in scaled variables. We also explored scaling laws related to three-phase pyrotechnic explosions. We show that although the scaling formally broke down, the fireball still essentially scaled with the explosion length scale r0. However, the discrete Lagrange particles (DLP) (phase 2) and the heterogeneous continuum model (HCM) of the DLP wakes (phase 3) did not scale with r0, and mean and RMS profiles could differ by a factor of 10 in some regions. This was because the DLP particles and wakes introduced an additional scale that broke the similarity conditions.
