Evaluating the effectiveness of the fatigue and shift working risk management strategy for UK home office police forces: a pilot study

PurposeTo evaluate the effectiveness of a fatigue-management training and sleep health promotion intervention in a sample of officers from UK Home Office Police Forces.Design/methodology/approachUsing a pre- and post-design we exposed 50 officers from selected UK police forces to a fatigue-management training intervention. Pre- and post-intervention data collection included wrist actigraphy, a physiological and objective measure of sleep quantity and quality, as well as surveys including the Pittsburg Sleep Quality Index (PSQI), the World Health Organization Quality of Life (WHOQOL) instrument, the Epworth Sleepiness Scale (ESS), the Perceived Stress Scale (PSS) and the PTSD Checklist (PCL-5).FindingsWe found the training significantly increased sleep quantity by 25 min per 24-h period, from 6.9 h to 7.3 h (f = 9.2; df = 519; p = 0.003), and improved sleep quality scores from 84% before the intervention, to 87% after the training (f = 10.6; df = 519; p = 0.001).Research limitations/implicationsContinued research is necessary to guide nationwide implementation of fatigue-management and sleep health promotion programs.Practical implicationsOur findings show that a fatigue-management training resulted in a significant and meaningful increase in sleep among police officers.Originality/valueThis is the first piece of research to emerge from a full population survey (response rate 16.6%) of the UK police service exploring issues of sleep and fatigue.

Purpose Mentally Healthy WA developed and implemented the Mentally Healthy Schools Framework in 2010 in response to demand from schools wanting to promote the community-based Act-Belong-Commit mental health promotion message within a school setting. Schools are an important setting for mental health promotion, therefore, the Framework encourages schools to adopt a whole-of-school approach to mental health promotion based on the World Health Organisation’s Health Promoting Schools framework. The paper aims to discuss these issues. Design/methodology/approach A process evaluation was conducted consisting of six-monthly activity reports from 13 participating Western Australian schools. Semi-structured interviews were also conducted with key school contacts in November 2011 with nine schools who had signed partner agreements prior to July 2011. Findings The schools valued promoting the mentally healthy message and the majority felt the programme was implemented successfully. More intensive implementation was facilitated by a proactive and enthusiastic school “champion” who had influence over other staff, and who did not have too many competing priorities. Factors inhibiting implementation included a lack of effective time management, lack of whole school commitment, and evaluation demands. Originality/value Act-Belong-Commit is a positive, proactive message making it easier for teachers to talk about mental health with their students. For schools reporting implementation success, the Mentally Healthy Schools Framework raised the profile of mental health in the school setting and fostered a sense of belonging among students.

Purpose Sleep deprivation is well known to negatively affect mood, cognition and behaviour. The purpose of this paper is to explore the relationship between sleep quantity, subjective sleep quality and aggression, hostility and well-being levels among adults in a non-clinical population. Design/methodology/approach In total, 201 participants aged 18 and above from Germany, UK and the Netherlands completed an online survey consisting of a Pittsburgh Sleep Quality Index along with measures of psychological well-being, implicit and explicit aggression, and intent attributions. Findings Sleep disturbances were related to decreased levels of psychological well-being. Subjective poor sleep quality predicted increased hostile attributions. The overall sleep experience, however, was not associated with aggression levels. Nevertheless, both a poor sleep experience and low sleep quality were related to increased reactive aggression, but only in British participants. Practical implications The importance of perceived sleep quality rather than sleep quantity in predicting hostile and aggressive behaviours is indicated. The quality of sleep and perception of this quality should be the focus of clinical intervention to limit unwanted behavioural impacts. The importance of accounting for sleep quality perception in intervention that examines attributional biases such as hostility is indicated. Differences across countries should be identified and accommodated for in intervention. Originality/value This is the first study to consider a role for sleep quality (including perception) and sleep quantity in relation to aggression and hostility in a cross-country European sample.

UNSTRUCTURED Billions of people in the world own a smartphone. It is a low-cost, portable computing device with countless features, among which applications stand out, which are programs or software developed to meet a specific goal. A wide range of applications available ranging from entertainment and personal organization to work and education is available currently. It is a vast and profitable market. Health applications have been a means of intervention for different areas, including chronic diseases, epidemics, and health emergencies. A recently published paper in the journal with the highest impact factor in Digital Health (“Journal of Medical Internet Research”) proposes a classification of health applications. This study performs a critical analysis of this organization and presents other sort criteria. This paper also presents and analyzes the “Meu Info Saúde” (“My Health Info”) app – a pioneering government initiative focused on primary care launched by the Oswaldo Cruz Foundation. The application classification proposal that will be presented builds on the intervention strategies in the health-disease process, namely: “Health Promotion”, “Disease Prevention” and “Care, Treatment and Rehabilitation”, as defined by official documents such as the World Health Organization and the Centers for Disease Control and Prevention. Most applications present in the sample are of private and foreign origin, free to download, but with a display of ads or the sale of products and services. The sampled applications were classified as “Health Promotion”, and some applications have also been categorized as “Disease Prevention” or “Care, Treatment or Rehabilitation” because they have multiple functionalities. The applications identified as “Health Promotion” focused only on individuals’ lifestyle and their increased autonomy and self-care management capacity. From this perspective, the apps analyzed in this paper differ from the “Meu Info-Saúde” application developed at Fiocruz.

Aims: Wearable devices are a new strategy for promoting physical activity in a free-living condition that utilizes self-monitoring, self-awareness, and self-determination. The main purpose of this study was to explore health benefits of commercial wearable devices by comparing physical activity, sedentary time, sleep quality, and other health outcomes between individuals who used and those that did not use commercial wearable devices. Methods: The research design was a cross-sectional study using an Internet survey in Taiwan. Self-administered questionnaires included the International Physical Activity Questionnaire–Short Form, Pittsburgh Sleep Quality Index, Health-Promoting Lifestyle Profile, and World Health Organization Quality-of-Life Scale. Results: In total, 781 participants were recruited, including 50% who were users of wearable devices and 50% non-users in the most recent 3 months. Primary outcomes revealed that wearable device users had significantly higher self-reported walking, moderate physical activity, and total physical activity, and significantly lower sedentary time than non-users. Wearable device users had significantly better sleep quality than non-users. Conclusion: Wearable devices inspire users’ motivation, engagement, and interest in physical activity through habit formation. Wearable devices are recommended to increase physical activity and decrease sedentary behavior for promoting good health.

Abstract Introduction Psychological resilience is the ability to withstand setbacks, adapt positively to challenges, and bounce back from the adversities of life. While the construct of resilience is broadly understood, the specific individual factors that contribute to the ability to be resilient and persevere in the face of difficulties remain poorly understood. We recently showed that psychological resilience during the COVID-19 pandemic was associated with a number of factors, including fewer complaints of insomnia, and others have suggested that sleep is an important contributor. We therefore tested the hypothesis that sleep quality and acute sleep quantity would combine to predict measures of psychological resilience and perseverance (i.e. “grit”). Methods We asked 447 adults (18–40 yrs; 72% female) to report the number of hours of sleep obtained the night before their assessment session (SLEEP), and complete several questionnaires, including the Pittsburgh Sleep Quality Index (PSQI), the Connor-Davidson Resilience Scale (CD-RISC), Bartone Dispositional Resilience Scale (Hardiness), and the Grit Scale. Sleep metrics were used to predict resilience, hardiness, and grit using multiple linear regression. Results For resilience, PSQI (β=-.201, p<.00003) and SLEEP (β=.155, p<.001) each contributed uniquely to prediction of CD-RISC (R2=.08, p<.00001). Hardiness was also predicted (R2=.08, p<.00001) by a combination of PSQI (β=-.218, p<.00001) and SLEEP (β=.128, p=.007). Interestingly, worse sleep quality over the past month on the PSQI (β=.13, p=.008) in combination with more SLEEP the night before the assessment (β=.137, p=.005) each contributed uniquely to higher Grit (i.e., perseverance; R2=.03, p=.003). Conclusion Self-reported sleep quality and quantity were both independently associated with greater self-reported resilience, hardiness, and grit. While better sleep quality and more sleep the night before testing each uniquely predicted greater resilience and hardiness, a different pattern emerged for Grit. The combination of lower quality sleep over the past month followed by greater recent sleep duration was associated with increased perseverance. Whereas sleep quality appears to be more important for general resilience/hardiness, recent sleep time appears more important for the subjective perception of perseverance. Because these data are purely self-report and cross sectional, future work will need to determine the longitudinal effects on behavior. Support (if any):

Abstract Background Evidence exists about synergies among universal health coverage, health security and health promotion. Uniting these three global agendas has brought success to the country’s health sector. This study aimed to document the efforts Ethiopia has made to apply nationally synergistic approaches uniting these three global health agendas. Our study is part of the Lancet Commission on synergies between these global agendas. Methods We employed a case study design to describe the synergistic process in the Ethiopian health system based on a review of national strategies and policy documents, and key informant interviews with current and former policymakers, and academics. We analyzed the “hardware” (using the World Health Organization’s building blocks) and the “software” (ideas, interests, and power relations) of the Ethiopian health system according to the aforementioned three global agendas. Results Fragmentation of health system primarily manifested as inequities in access to health services, low health workforce and limited capacity to implementation guidelines. Donor driven vertical programs, multiple modalities of health financing, and inadequate multisectoral collaborations were also found to be key features of fragmentation. Several approaches were found to be instrumental in fostering synergies within the global health agenda. These included strong political and technical leadership within the government, transparent coordination, and engagement of stakeholders in the process of priority setting and annual resource mapping. Furthermore, harmonization and alignment of the national strategic plan with international commitments, joint financial arrangements with stakeholders and standing partnership platforms facilitated efforts for synergy. Conclusions Ethiopia has implemented multiple approaches to overcome fragmentation. Such synergistic efforts of the primary global health agendas have made significant contributions to the improvement of the country’s health indicators and may promote sustained functionality of the health system.

PurposeDesign of bedding textiles that contact the human body affects the sleep quality. Bedding textiles contribute to comfort sense during the sleep duration, in addition to ambient and bed microclimate. The purpose of this study is to evaluate the effects of different layer properties on the compression recovery and thermal characteristics of multilayer bedding textiles.Design/methodology/approachIn this study, woven and knitted multilayer bedding textiles were manufactured from fabric, fiber, sponge and interlining, respectively. Different sponge thickness, fiber and interlining weight were used in the layers of samples. Later, the pilling resistance, compression and recovery, air permeability and thermal conductivity of multilayer bedding textiles were investigated.FindingsThe results indicated that samples with the higher layer weight and thickness provide better compression recovery and lower air permeability properties. It was also found that knitted surfaces show the higher air permeability than the woven surfaces depending on the fabric porosity. Layer properties have insignificant effect on the thermal conductivity values.Originality/valueWhile researchers mostly focus on thermal comfort properties of garments, there are limited studies about comfort properties of bedding textiles in the literature. Furthermore, compression recovery properties of bedding textiles have also a great importance in terms of comfort. Originality of this study is that these properties were analyzed together.
