Study of Insertion Force and Deformation for Suturing With Precurved NiTi Guidewire

This research presents an experimental study evaluating stomach suturing using a precurved nickel–titanium (NiTi) guidewire for an endoscopic minimally invasive obesity treatment. Precise path planning is critical for accurate and effective suturing. A position measurement system utilizing a hand-held magnetic sensor was used to measure the shape of a precurved guidewire and to determine the radius of curvature before and after suturing. Ex vivo stomach suturing experiments using four different guidewire tip designs varying the radius of curvature and bevel angles were conducted. The changes in radius of curvature and suturing force during suturing were measured. A model was developed to predict the guidewire radius of curvature based on the measured suturing force. Results show that a small bevel angle and a large radius of curvature reduce the suturing force and the combination of small bevel angle and small radius of curvature can maintain the shape of guidewire for accurate suturing.

Spermatozoa of several mammalian species were studied by means of high-speed cinematography and electron microscopy. Three types of motile patterns were observed in mouse spermatozoa. The first type involved an asymmetrical beat which seemed to propel the sperm in circular paths. The second type involved rotation of the sperm and appeared to allow them to maintain straight paths. In the third type of pattern, the sperm appeared to move by crawling on surfaces in a snakelike manner. Spermatozoa of rabbit and Chinese hamster also had an asymmetrical beat which sometimes caused them to swim in circles. In spite of the asymmetry of the beat, these spermatozoa were also able to swim in straight paths by rotating around a central axis as they swam. Spermatozoa of some species appeared very flexible; their flagella formed arcs with a very small radius of curvature as they beat. Spermatozoa of other species appeared very stiff, and their flagella formed arcs with a very large radius of curvature. The stiffness of the spermatozoan appeared to correlate positively with the cross-sectional area of the dense fibers. This suggests that the dense fibers may be stiff elastic elements. Opossum sperm become paired as they pass through the epididymis. Pairs of opossum spermatozoa beat in a coordinated, alternating manner.

<p>To determine the frequency of apical and cervical curvatures in human molars using the radius method and cone-beam computed tomography (CBCT) images. Four hundred images of mandibular and maxillary first and second molars were selected from a database of CBCT exams. The radius of curvature of curved root canals was measured using a circumcenter based on three mathematical points. Radii were classified according to the following scores: 0 - straight line; 1 - large radius (r>8 mm, mild curvature); 2 - intermediate radius (r>4 and r<8 mm, moderate curvature); and 3 - small radius (r≤4 mm, severe curvature). The frequency of curved root canals was analyzed according to root canal, root thirds, and coronal and sagittal planes, and assessed using the chi-square test (significance at α=0.05). Of the 1,200 evaluated root canals, 92.75% presented curved root canals in the apical third and 73.25% in the cervical third on coronal plane images; sagittal plane analysis yielded 89.75% of curved canals in the apical third and 77% in the cervical third. Root canals with a large radius were significantly more frequent when compared with the other categories, regardless of root third or plane. Most root canals of maxillary and mandibular first and second molars showed some degree of curvature in the apical and cervical thirds, regardless of the analyzed plane (coronal or sagittal).</p>

We performed comparative analysis of curvature characteristics of bird eggs and used ovoid profiles from various authors, our own geometric profiles, and archive bird egg profiles from our database. We suggested that the possible ovoids arise by changing the curvature radii of the polar and lateral zones. We compared the constructed curves and curves presented in oological literature with the pictures of the real bird egg profiles. The volume of actual material includes 16,490 eggs from 800 species of 20 bird orders. Specially designed computer software had calculated the radius of curvature of real bird eggs from photos and drew out the half-profiles presented in our article. We supposed that the asymmetrical ovoid is the most widespread geometric egg-shaped figure, which can easily be obtained by combination of circles. We also calculated that if the ovoid diameter were taken as a unit, then the radius obtuse (infundibular) pole would be equal to its half-length, lateral (side arcs) is equal two lengths, and the cloacal arch is 1–√2/2. From this suggestion, we concluded that the length of the ovoid is 2–√2/2 and its cloacal radius is equal to the difference between the length of ovoid and its diameter (rc = L–D). We analyzed the geometry of this asymmetrical oval and came to the conclusion that this is the basic form able to generate the derivative forms. We discovered that the centres of the arcs which form it lie on one basic/supporting circle, the diameter of which is equal to the ovoid diameter. All other ovoids, which have radius equal to radius of the infundibular zone we called the real ovoids. The changes in the radii of the lateral arcs are caused by the movement of their centres along a horizontal line passing through the centre of the base circle. This causes the prolongation or shortening of the ovoids. Sizes of cloacal arcs depended on the lateral, and their centres are placed along the vertical axis. Variability of the sizes of the abovementioned arcs caused the variability of form of the bird eggs. For their classification, we have proposed to divide them into five groups: sphere-like, circle, obtuse, droplet, and cone-shaped. Further, we separated some more groups: short, shortened, normal, lengthened, and long; according to the sizes of cloacal arcs – large radius, middle radius, and small radius ovoids. As a result, we have 80 forms of real ovoids – standards which describe the varieties of bird eggs. Each of them has its own number, name, geometrical figure, polynomic equation, and correlations of ovoid parameters. This set of ovoids is sufficient to describe the specifics of any form of bird eggs.

SummaryWe investigated the comparative antithrombotic properties of clopidogrel, an analogue of ticlopidine, and aspirin, using the Folts' model on femoral arteries in 22 pigs. On each animal, clopidogrel or aspirin were used to treat the thrombotic process on the left femoral artery and to prevent this process on the right femoral artery. Sequentially: an injury and stenosis were carried out on the left femoral artery; the thrombotic process was monitored with a Doppler during a 30-min observation period for cyclic flow reductions or permanent cessation of flow; after the first cyclic flow reduction occurred, clopidogrel (5 mg kg-1) or aspirin (2.5, 5, 100 mg kg-1) were injected intravenously; if cyclic flow reductions were abolished, epinephrine (0.4 µg kg-1 min-1) was injected to try to restore cyclic flow reductions and/or permanent cessation of flow; then injury and stenosis were applied on the right femoral artery. Before and after injection of clopidogrel or aspirin, ear immersion bleeding times and ex-vivo platelet aggregation were performed. Clopidogrel (n = 7) abolished cyclic flow reductions in all animals and epinephrine did not restore any cyclic flow reduction. On the right femoral artery, cyclic flow reductions were efficiently prevented, even for two injuries. Basal bleeding time (5 min 28) was lengthened (>15 min, 30 min after clopidogrel and remained prolonged even after 24 h). ADP-induced platelet aggregation was inhibited (more than 78%). Comparatively, aspirin had a moderate and no dose-dependent effect. Aspirin 2.5 mg kg-1 (n = 6) abolished cyclic flow reductions in 2 animals, CFR reoccurred spontaneously in one animal and epinephrine restored it in a second animal. Aspirin 5 mg kg-1 (n = 6) abolished cyclic flow reductions in only 3 animals and epinephrine always restored it. Aspirin 100 mg kg-1 (n = 3) was unable to abolish cyclic flow reductions. On the right femoral artery, aspirin did not significantly prevent cyclic flow reductions which occurred in all animals after one (n = 14) or two injuries (n = 1), except for one animal. Basal bleeding time was lengthened but it shortened rapidly, reaching its basal value after 24 h. ADP-induced aggregation was not significantly inhibited, whereas arachidonic acid induced aggregation was always inhibited. Clopidogrel appears as a more potent antithrombotic drug than aspirin in this model, in treating and preventing spontaneous or epinephrine-induced cyclic flow reductions and lengthening bleeding time.

SummaryTiclopidine (T) and aspirin (ASA) are two antiplatelet drugs both capable of prolonging bleeding time (BT), with a different mechanism of action. A synergism in BT prolongation has been reported and is currently considered an argument for not recommending their combination. However, a profound suppression of platelet function might be a desirable counterpart of a marked prolongation of BT, with a possible use in selected clinical situations. We therefore studied ex vivo platelet function (aggregation by ADP 0.5-1-2.5 μM; adrenaline 0.75-2.5 μM; collagen 1.5-150 μg/ml; arachidonic acid 1 mM; PAF 1 μM; adrenaline 0.17 μM + ADP 0.62 μM; serum thromboxane ([TX]B2 generation) and BT (Mielke) in 6 patients with stable coronary artery disease receiving such combination. Patients underwent sequential laboratory evaluations at baseline, after 7 days of T 250 mg b.i.d., before and after the intravenous administration of ASA 500 mg, respectively, and, finally, after a minimum of 7 days of sole ASA oral administration (50 mg/day). The experimental design, therefore, allowed a comparison of T and ASA effects (2nd and 4th evaluation), and an assessment of the combination effect (3rd evaluation). Platelet aggregation in response to all doses of ADP was depressed more by T than by ASA. Conversely, responses to adrenaline, and arachidonate were affected more by ASA than by T. For all other agents, differences were not significant. T + ASA combination was more effective (p <0.05) than either treatment alone in depressing responses to high-dose collagen (% over control, mean ± SEM: T: 95 ± 3; ASA: 96 ± 5; T + ASA: 89 ± 4). Serum TXB2 (basal, ng/ml: 380 ± 54) did not change with T (372 ± 36), dropped to <1 ng/ml on ASA injection and slightly re-increased to 9.1 ± 3.1 ng/ml on oral low-dose ASA. BT (basal 7.4 ± 0.6 min) was affected similarly by T (9.2 ± 0.8) or ASA (9.7 ± 0.9) alone, but increased to 15.0 ± 0.7 min on combination treatment (106% increase over control). Thus, the strong synergism in BT prolongation by ASA-T combination has a counterpart in the inhibition of platelet function in response to strong stimuli such as high-dose collagen, not otherwise affected significantly by single-drug treatment. This effect is a possible rationale for the clinical evaluation of T + ASA combination.

OBJECTIVEAlthough the advent of magnetic growing rod technology for scoliosis has provided a means to bypass multiple hardware lengthening operations, it is important to be aware that many of these same patients have a codiagnosis of hydrocephalus with magnet-sensitive programmable ventricular shunts. As the magnetic distraction of scoliosis rods has not previously been described to affect the shunt valve setting, the authors conducted an investigation to characterize the interaction between the two devices.METHODSIn this ex vivo study, the authors carried out 360 encounters between four different shunt valve types at varying distances from the magnetic external remote control (ERC) used to distract the growing rods. Valve settings were examined before and after every interaction with the remote control to determine if there was a change in the setting.RESULTSThe Medtronic Strata and Codman Hakim valves were found to have setting changes at distances of 3 and 6 inches but not at 12 inches. The Aesculap proGAV and Codman Certas valves, typically described as MRI-resistant, did not have any setting changes due to the magnetic ERC regardless of distance.CONCLUSIONSAlthough it is not necessary to check a shunt valve after every magnetic distraction of scoliosis growing rods, if there is concern that the magnetic ERC may have been within 12 inches (30 cm) of a programmable ventricular shunt valve, the valve should be checked at the bedside with a programmer or with a skull radiograph along with postdistraction scoliosis radiographs.

Cosmetic facial surgery procedures and therapies have continued to evolve with an emphasis on minimally invasive techniques with a shorter recovery time. We present a how-to guide for nonsurgical rhinoplasty including the use of different hyaluronic acid fillers. We provide examples of before and after patient photos as well as photos demonstrating our technique. This technical note highlights the popular concept of minimally invasive nasal contouring using dermal fillers. As such, we provide a brief overview of different dermal fillers that can be used for this application, potential problems and complications, as well as remedies. Dermal fillers have become an entry point into cosmetic surgery for many patients. Our technique of nasal contouring with hyaluronic acid fillers in particular yields safe, effective, and repeatable results.

In the field of cold bending, it is necessary to use ball mandrels, especially to bend thin-walled tubes with a small radius. However, the bending process with a ball mandrel is complex and expensive, and it is easy to jam the core ball inside the tube. To solve these issues, we designed two kinds of hollow non-ball mandrel schemes with low stiffness that were suitable for the small radius bending of thin-walled tubes. We evaluated the forming quality of cold bending numerically and the influence of the hollow section length and thickness on the forming indices. Our results showed that the thickness of the hollow section has a greater influence on forming quality than the length. As the hollow section’s thickness increased, the wrinkling rate first declined by approximately 40% and then increased by above 50%. When the thickness was 11 mm in scheme 1 and 13 mm in scheme 2, the wrinkling rate reached minimum values of 1.32% and 1.50%, respectively. As the hollow section’s thickness increased, the flattening rate decreased by more than 60% and the thinning rate increased by about 40%. A multi-objective optimization of forming indices was carried out by ideal point method and grey wolf optimizer. By comparing the forming results before and after optimization, the feasibility of using the proposed hollow mandrel was proved, and the hollow mandrel scheme of standard cylinder is therefore recommended.
