Fifteen-minute consultation: a structured approach to the child with primary amenorrhea

Puberty is a life-changing time in the life of a young person, with physical, psychological and social considerations. Amenorrhea is derived from Latin: a—‘not’, men—‘month’ and rhein—‘flow’, meaning absence of monthly flow. In medical terms, it is a symptom describing absence of menstruation. It can be classified as either primary or secondary. This article will focus solely on primary amenorrhea. Primary amenorrhea can induce great anxiety in both the patient and the family and often presents to the general paediatrician. A thorough history and examination and judicious use of investigations is crucial to ensure timely diagnosis and management.

The identification of an abdominal mass in a child, either coincidental or symptomatic, may be due to a tumour. An abdominal tumour may present with life-threatening symptoms, requiring prompt assessment and management. Although the discovery of such a finding usually warrants inpatient transfer or outpatient referral to the tertiary oncology centre, the initial evaluation, management and communication with the family by the general paediatrician is crucial. A thorough history and examination, which includes an organised, structured approach to abdominal masses, is paramount. The anatomical location of the mass, age of the patient and the presence of any associated symptoms or signs must be considered together in order to formulate a list of potential differential diagnoses and guide the next appropriate investigations. This article aims to guide general paediatricians through the assessment and initial management of a child presenting with an abdominal mass suspected to be a tumour.

The resilience of parents can play a decisive role as a resource that favors the inclusion and development of students with disabilities, representing a decisive contribution in school-family co-responsibility. This work showed a conceptual analysis related to resilience from a family dimension and especially the role played by parents. The research was carried out in the context of the Technical University of Manabí, a representative sample of students with disabilities and their families was selected, two instruments were applied to obtain the data: Family Functioning Scale [1] and the Mother Resilience Scale [2]. The attention to the young person with a disability was analyzed, and it is particularized in the related to the family of these. The results are shown in tables that allow the final results to be identified.

PurposeThe authors identify the theoretical constructions measuring the intentions to pursue succession as well as the socioemotional wealth theoretical framework, and the authors propose an objective of testing the relationships existing between them so as their importance giving evidence of their relevance.Design/methodology/approachIt is a research with a positivist philosophical position measuring in a quantitative way with a deductive and structured approach applied to 98 CEO owners of Mexican companies, using nonparametric methodologies the authors simulated subsamples with structural equation modeling in SmartPLS 3.3.2, the metrics on the model are described as a functionalist paradigm.FindingsDirectors' attitudes paired up with the intentions of succession are significantly related to the socioemotional aspect of the family business; although the theory proposes three aspects to measure these intentions, the social norm in this research has not been strong enough to be a predictor as an influence on the company's socioemotional wealth.Originality/valueThe authors found this a valuable paper for the complement of theory focused on purely manifesting aspects in family companies, because they identified theoretical and empirical relationships opening up guidelines for new research in socioemotional aspects in accordance with the entrepreneurs attitudes to achieve succession, the differentiation lies in measuring psychological aspects of the director's behavior toward succession and not to the succession per se as done in most research; also, the methodology of data analysis facilitates the reader to easily recognize the relationships between the proposed theoretical constructions, showing the detailed metrics development by researchers in the family business field.

AbstractThe purpose of this review is to provide an update of the research regarding the etiology, diagnosis and management of psychogenic non-epileptic seizures (PNES). A literature search using Pubmed, Ovid MEDLINE and EMBASE database was performed from 2000 up to August 2017. We have evaluated the different factors leading to PNES as well as the diagnostic approach and management of this disorder which continue to be very difficult. The coexistence of epilepsy and PNES poses special challenges and requires the coordinated efforts of the family physicians, psychiatrists, psychologists and neurologists. Although this condition has an overall poor prognosis, a multidisciplinary approach in the diagnosis and management of this disorder would likely improve the outcomes. We have proposed a diagnostic and treatment algorithm for PNES and suggested a national registry of patients suffering from this condition. The registry would contain data regarding treatment and outcomes to aid in the understanding of this entity.

The psychiatric anamnesis is a lengthy enquiry based on the Meyerian principle that most aspects of a patient's past and present history are relevant to the diagnosis and management of psychiatric disorder. Despite widespread use by both clinicians and research workers, the anamnesis has been the subject of few investigations regarding its reliability and validity. The studies reported have not encouraged confidence in traditional history taking (Haggard et al., 1960; Wenar and Coulter, 1962; Beckett et al., 1967). The present study was carried out to investigate the reliability of the family and personal history.

Abstract Introduction. Low percentage of women who attend prophylactic examinations and also react early to the first symptoms of a disease is a common problem. Aim. The aim of the study was to determine demographic and environmental characteristics which had positive effects on having prophylactic gynecological examinations by women as well as to investigate what significance women's practicing of hormonal contraception had in that question. Material and methods. The study covered 430 women. The conditions of including women into the study group were as follows: age <45 years, commencement of sexual intercourses, maintaining of sexual activity, consent to participate in the study. The method of the study was a diagnostics survey using the questionnaire. Results. The majority of the surveyed women said that they attended prophylactic gynecological examinations regularly. Having prophylactic gynecological examinations was significantly affected (p<0.05) by mean monthly income per capita in the family, practicing and the type of contraception. The joint effect of the surveyed characteristics proved to be significant (p<0.05). Conclusions. The model woman who willingly attends prophylactic gynecological examinations is a young person who lives in a big city, has a university education and a very good financial position confirmed by high family income per capita. Practicing of hormonal contraception by women is a factor that has a beneficial effect on women's care of their health understood as having regular prophylactic gynecological examinations.
