IntroductionLarge granular lymphocyte (LGL) leukaemia is considered a mature T-cell or natural killer (NK) cell neoplasm, characterised by a clonal proliferation of LGL.AimsTo analyse the characteristics and to establish (if possible) the prognostic parameters of these patients diagnosed in a single centre: University Hospital of Donostia.MethodsWe retrospectively studied data about 308 patients with LGL leukaemia diagnosed in our centre.ResultsThe frequency of T-LGL leukaemia and chronic lymphoproliferative disorder of NK cells was 89% and 6.8% respectively, and no aggressive NK-LGL leukaemia was seen in our population. The median age at diagnosis was 65.7 years and male-to-female ratio was 1.08. 59% of our patients were asymptomatic at the time of diagnosis. Most patients presented lymphocytosis and 63.6% more than 20% LGLs in the peripheral blood count, but it has to be taken into account that these results may be influenced by the selection bias of our study, as we recognised these patients as ‘alarms of the laboratory analysers’. Neutropenia was the most common cytopenia, and autoimmune disorders were described in 16.5% of the patients. Only 12 patients (3.9%) required treatment, a much lower percentage that the one reported in the literature, and this is consistent with the fact that patients were less symptomatic than in other series, as we expected. The 5-year and 15-year overall survival was 92% and 87%, respectively.ConclusionsOur patients may represent the even more benign end of the spectrum of clonal T LGL and NK proliferations.

The purpose of our study was to comprehensively assess the effect of the COVID-19 pandemic on knee and shoulder arthroscopy performed in an orthopedic department of a university hospital in Poland. This study compared the data on all shoulder and knee arthroscopy procedures performed in two different periods: The period of the COVID-19 pandemic in Poland (from March 4, 2020, to October 15, 2020) and the corresponding period prior to the pandemic (March 4, 2019, to October 15, 2019). The study evaluated epidemiological data, demographic data, and hospital stay duration. The total number of arthroscopy procedures conducted in the evaluated period in 2020 was approximately 8.6% higher than that in the corresponding 2019 period. The mean duration of hospital stay for orthopedic patients after their knee or shoulder arthroscopy was 3.1 days in 2020 and 2.8 days in 2019. Our study revealed the mean age of arthroscopy patients during the pandemic to be lower at 48.4 years than the 51.2 years recorded in 2019. The male-to-female ratio was shown to be lower at .85 during the pandemic, having decreased from 1.5 in 2019. The COVID-19 pandemic did not reduce the number of arthroscopy performed at our center, and the mean age of the patients did not change. However, the pandemic had a marked effect on the mean duration of hospital stay and male-to-female ratio.

Objective: To evaluate the aetiological factors of hoarseness. Methods: This is a prospective, non- randomized and longitudinal study conducted from 1st august 2011 to 1st august 2012 in department of otorhinolaryngology of Kathmandu university Hospital, Dhulikhel, Nepal. All the patients with history of hoarseness underwent clinical examination, routine as well as special investigation to find the diagnosis. The final results were analyzed by simple manual analysis with frequency and percentage using Microsoft Excel software 2007. Results: There were total 280 patients included in the study. Among them the age groups of 21 – 30 years and 31 – 40 years were mainly suffer from hoarseness. Similarly, among 280 patients 200 (71.45%) were males whereas 80 (28.6%) were females with male to female ratio of 2.5:1.. The most common cause as per the distribution was acid peptic laryngitis with frequency of 37.8% whereas tuberculosis of larynx, papillary carcinoma of thyroid and papilloma of vocal cord accounts for only 0.4% each. Conclusion: There was etiological variation in hoarseness ranging from simple laryngitis to malignancies. So it is important not to ignore the hoarseness and precise history, examination and investigations should be done. The case records of 53 consecutive Nigerian inpatients with stroke in the University of Maiduguri Teaching Hospital, Maiduguri were retrospectively reviewed. The mean age at presentation was 55 years and the male to female ratio was 2.5: 1. The mean duration of symptoms before presentation was 11.1 weeks and the average duration of stay in hospital was 3 weeks. Thirty-three (63%) of the lesions were infarctive and 19 (37%) were haemorrhagic. Only 3 (6%) patients gave a history of prior transient ischaemic attacks (TIAs). Forty-two (79%) patients were hypertensive at presentation out of which 27 (64%) had the hypertension diagnosed for the first time. Four (8%) patients were non-insulin dependent diabetics. There were 11 hospital deaths (21%). Thus hypertension, more than half undiagnosed at admission, was the most common risk factor for stroke in the hospital population studied.

Introduction : Obstructive anuria due to pelvic cancers is the most common obstructive anuria in our context, its diagnosis is easy, but it causes a lot of morbidity with often heavy and long management.                                      Materials and methods : This is a retrospective study containing 40 cases of confirmed neoplastic anuria treated in the urology department at the Ibn Rochd University Hospital in Casablanca Morocco between January 2016 and October 2018. Purpose : The aim of our study was to establish the epidemiological, clinical, therapeutic and natural historyin patients with neoplastic obstructive anuria. Results : The mean age was 56 years old with a notable female predominance. The male to female ratio was 0.6. Sixty percent (60%) of our patients were consulted within24 hours after the onset of anuria, the main symptoms preceding or accompanying anuria were lower back pain (75%) and hematuria (37.5%). Criteria for a positive diagnosis included urinary retention with complete absence of diuresis, the anuria was confirmed by probing or ultrasound, dilation of the urinary excretory system on ultrasound and acute impairment of renal function. To prevent patient from going into metabolic disorders, hemodialysis was performed in 27 patients (67.5%). Internal diversion of urine by mounting a JJ catheter was initially attempted in 40% of the cases. In case of attempt failure, or in case of locally advanced pelvic tumors, percutaneous nephrostomy was performed with a success rate of 100%. Upon follow up for 30 months, mortality rate was 5% (n=2), 95% of the patients benefited from a specialized follow-up for their etiological neoplasm.  Conclusion : The management of neoplastic obstructive anuria must be rapid, the type of urinary diversion is chosen according to the clinical presentation and the technical platform available.

Vernal keratoconjunctivitis was the most common conjunctival disease seen over a 2 year period (January 1997–December 1998) at the University of Benin Teaching Hospital, Benin City, Nigeria. One hundred and nine consecutive patients were seen with vernal keratoconjunctivitis. There was a male to female ratio of 1:1.3. The age range of the patients was 5 months to 38 years with a mean age of 15.5 ± 8.3 years (SD). Of the patients 54.1% were children. Itching was the most common symptom, followed by redness, tearing, aching eye and photophobia. Ninety patients (82.6%) had predominantly tarsal papillae, while the others had mixed and limbal papillae. A history of atopic diseases such as asthma and rhinitis was present in only five patients (4.5%). There was no patient with corneal ulcer or scarring. Although the complications were few, health education of the patients about the dangers of self medication with steroids should be emphasized.

Background: In the past six months, Croatia faced a short lockdown and a slow return to most hospitals’ everyday activities. During the lockdown, our center, as a part of the University Hospital Centre specialized for solid cancer, was enabled to maintain most of the routine practices with the one-month colonoscopy exception. Aim: To determine the oscillation in the number of endoscopies and colorectal surgery for 13 months (six months pre and post COVID-19 lockdown). Materials and methods: From August 1st, 2019, until August 31st 2020, the hospital analytics determine the number of colonoscopies, screening colonoscopies, and surgeries. Results: During the given period number of detected and operated colorectal cancers was stable, except for April, when we mostly did not perform colonoscopies. Conclusion: We maintained a pre-COVID-19 pace in colorectal cancer treatment, colonoscopies, and colorectal surgery after epidemiological guidelines for colonoscopies and colorectal surgery were applied, owing to the relatively stable overall epidemiological situation.
