IntroductionThis ‘Health and productivity’ project was implemented to identify priority health problems and health related productivity loss in a large automobile industry in India. It hopes to re-emphasize that ‘OH department’ is a partner in productivity.ObjectivesTo estimate prevalence, incidence rates , trends and risk for health problems among employees (year 2010–14)To identify leading causes of hospitalization and out–patient care among employees (year 2010–14).To quantify loss in productive work time attributed to health related absence in year 2014 and forecast to year 2025MethodsA five year records analysis (2010–14) was conducted in a leading automobile industry in year 2015–16. Data was pooled from clinic visit records, annual medical examination , insurance claims and leave records, systematically using employee ID. Trends and incidence rates of leading health risks were computed per 1000 person-months of employment. Leading causes and trends in out-patient and in-patient care of employees were determined. Total work time (man-hours) was computed and health related loss in work-time was derived. Cox-regression was used to assess risk between work department and morbidity. ARIMA method was used to forecast productivity loss till year 2025.ResultsBetween 2010–14, overweight and hypertension were identified as leading health risks. Respiratory, musculoskeletal and digestive disorders were leading causes for clinic visits. Infectious diseases are leading cause for hospitalization. Health related absence accounted for 1.87% of total productive time. Forecasting indicates an increase to 9.31% by year 2025.ConclusionHealth related absence contributes to significant loss in productive work-time. There is scope to reduce these losses by improvement in health situation. Our study provided key information and trends for planning evidence based health promotion programmes in the industry.

Purpose. To investigate the impact of health on job performance using two measures of productivity loss: (1) a self-reported measure of health-related presenteeism and (2) an objective measure of absenteeism. Design. A cross-sectional survey using a Health Risk Appraisal (HRA) to evaluate self-reported presenteeism and the prevalence of 12 health risks and eight medical conditions. Setting and Subjects. Employees (n = 224) of a private insurance provider in Australia. Measures. A Health Risk Appraisal (HRA) questionnaire was used to evaluate self-reported presenteeism on different aspects of job demands and to assess the prevalence of 12 health risks and eight medical conditions. Illness absent hours were obtained from company administrative records. Results. Increased presenteeism was significantly associated with high stress, life dissatisfaction, and back pain, while increased illness absenteeism was significantly associated with overweight, poor perception of health, and diabetes. Excess presenteeism associated with excess health risks (productivity loss among those with medium- or high-risk status compared to those with low-risk status) was independently calculated at 19.0% for presenteeism and 12.8% for illness absenteeism. Conclusions. This study demonstrates an association between health metrics and self-reported work impairment (presenteeism) and measured absenteeism. The study provides a first indication of the potential benefits of health promotion programming to Australian employees in improving health and to the corporation in minimizing health-related productivity loss.

Abstract Backround Chronic diseases and multimorbidity are common in the ageing population and affect the health related quality of life. Health care resources are limited and the continuity of care has to be assured. Therefore it is essential to find demonstrable tools for best treatment practices for patients with chronic diseases. Our aim was to study the influence of a participatory patient care plan on the health-related quality of life and disease specific outcomes related to diabetes, ischemic heart disease and hypertension. Methods The data of the present study were based on the Participatory Patient Care Planning in Primary Care. A total of 605 patients were recruited in the Siilinjärvi Health Center in the years 2017–2018 from those patients who were followed up due to the treatment of hypertension, ischemic heart disease or diabetes. Patients were randomized into usual care and intervention groups. The intervention consisted of a participatory patient care plan, which was formulated in collaboration with the patient and the nurse and the physician during the first health care visit. Health-related quality of life with the 15D instrument and the disease-specific outcomes of body mass index (BMI), low density lipoprotein cholesterol (LDL-C), hemoglobin A1c (HbA1C) and blood pressure were assessed at the baseline and after a one-year follow-up. Results A total of 587 patients with a mean age of 69 years were followed for 12 months. In the intervention group there were 289 patients (54% women) and in the usual care group there were 298 patients (50% women). During the follow-up there were no significant changes between the groups in health-related quality and disease-specific outcomes. Conclusions During the 12-month follow-up, no significant differences between the intervention and the usual care groups were detected, as the intervention and the usual care groups were already in good therapeutic equilibrium at the baseline. Abstract Introduction/objectives To evaluate changes in health-related quality of life (HRQoL) and productivity following treatment with intravenous (IV) golimumab in patients with psoriatic arthritis (PsA). Methods Patients were randomized to IV golimumab 2 mg/kg (n=241) at Weeks 0, 4, then every 8 weeks (q8w) through Week 52 or placebo (n=239) at Weeks 0, 4, then q8w, with crossover to IV golimumab 2 mg/kg at Weeks 24, 28, then q8w through Week 52. Change from baseline in EuroQol-5 dimension-5 level (EQ-5D-5L) index and visual analog scale (EQ-VAS), daily productivity VAS, and the Work Limitations Questionnaire (WLQ) was assessed. Relationships between these outcomes and disease activity and patient functional capability were evaluated post hoc. Results At Week 8, change from baseline in EQ-5D-5L index (0.14 vs 0.04), EQ-VAS (17.16 vs 3.69), daily productivity VAS (−2.91 vs −0.71), and WLQ productivity loss score (−2.92 vs −0.78) was greater in the golimumab group versus the placebo group, respectively. At Week 52, change from baseline was similar in the golimumab and placebo-crossover groups (EQ-5D-5L index: 0.17 and 0.15; EQ-VAS: 21.61 and 20.84; daily productivity VAS: −2.89 and −3.31; WLQ productivity loss: −4.49 and −3.28, respectively). HRQoL and productivity were generally associated with disease activity and functional capability, with continued association from Week 8 through Week 52. Conclusion IV golimumab resulted in early and sustained improvements in HRQoL and productivity from Week 8 through 1 year in patients with PsA. HRQoL and productivity improvements were associated with improvements in disease activity and patient functional capability. Key Points• In patients with active psoriatic arthritis (PsA), intravenous (IV) golimumab improved health-related quality of life (HRQoL) and productivity as early as 8 weeks and maintained improvement through 1 year• Improvements in HRQoL and productivity outcomes in patients with PsA treated with IV golimumab were associated with improvements in disease activity and patient functional capability outcomes• IV golimumab is an effective treatment option for PsA that can mitigate the negative effects of the disease on HRQoL and productivity

BACKGROUND: The health risks that usually affect teachers are already known; however, the predictors of health related absenteeism, presenteeism, and sick leave have not yet been fully described. OBJECTIVE: To analyze the predictors of health-related absenteeism, presenteeism, and sick leave among elementary school teachers. METHODS: This study involved a probabilistic sample of 519 Brazilian elementary school teachers. The outcomes were days of health-related absenteeism, presenteeism, and sick leave in the previous 12 months. Work-place and individual factors were the independent variables analyzed. RESULTS: Inadequate infrastructure of schools, disability, and medical consultations were positively associated with all outcomes. Teachers who reported having a chronic disease, common mental disorders, and voice disorders presented higher absenteeism and presenteeism days. Musculoskeletal pain and low job support were associated with higher presenteeism and sick leave days. Teachers who performed strength and flexibility activities presented less presenteeism, those who reported physical violence at school were more frequently absent, and teachers with depersonalization presented a higher likelihood of sick leave. CONCLUSIONS: The prevention of health-related absenteeism, presenteeism, and sick leave among elementary teachers should ensure adequate work conditions and prevention and monitoring of health risks.

AimThis study was to analyze the sample data collected from divorce marital therapy sessions in the local court system in South Korea as well as diagnose typical symptoms in the divorcing couples.MethodsThe survey forms and marital therapy notes were collected from 200 divorce counseling cases in the local court systems, South Korea. The 200 divorce counseling cases were categorized as twelve major themes. Family therapy notes were utilized as an aid in verifying the divorcing couples’ self-reports. The research method was partially adopted from the article, “Working with Korean-American Families: Multicultural Hermeneutics” (Sang Bok Lee, 2003: The American Journal of Family Therapy, 31, 159 – 178). Multicultural hermeneutics was instrumental for explicating multiple layers of multicultural narratives, psychological dynamics, socio-economic systems, and of family systemic relations when dealing with the divorcing couples.ResultsThe results were summarized as: economic issues (20%); extra-marital relationship (16%); in-law conflict (12%); substance abuse, addiction & marital violence (12%); international marriage & cross-cultural differences (10%); personality difference (8%); re-marriage issues (6%); sexual conflict (6%); age gap (4%); personality disorder & mental disorder (2%); parenting & children issues (2%); pre-mature marriage (2%). The twelve categories were grouped as mainly(1)family systemic or relationship-related issues,(2)socio-economic issues, and(3)mental health-related issues.ConclusionPsychiatrists, family therapists, and mental health professionals who work with the divorcing couples need to be aware of the predominant causes for rapidly increasing divorce rates when conducting therapeutic assessment and implementing intervention for the divorcing couples.

AbstractThe effects of hand immersion on body temperature have been investigated in men wearing impermeable NBC clothing. Six men worked continuously at a rate of approximately 490 J.sec−1 in an environmental temperature of 30°C. Each subject was permitted to rest for a period of 20 minutes when their aural temperature reached 37.5°C, and again on reaching 38°C, and for a third time on reaching 38.5°C (three rest periods in total). Each subject completed three experimental conditions whereby, during the rest periods they either: a.Did not immerse their hands (control).b.Immersed both hands in a water bath set at 25°c.c.Immersed both hands in water at 10°C.Physiological measures of core temperature, skin temperature and heart rate were recorded at intervals throughout the experiment.Measures of mean aural temperature and mean skin temperature were significantly (P<0.05) reduced if hands were immersed during these rest periods, compared to non immersion. As a result, the total work time of subjects was extended when in the immersed conditions by some 10–20 minutes within the confines of the protocol.It is concluded that this technique of simple hand immersion may be effective in reducing heat stress where normal routes to heat loss are compromised.

Background: The use of health-related applications (apps) on smartphones has become widespread. This is especially of value during the ongoing SAR-COV-2 pandemic, where the accessibility for health care services has been greatly limited. Patients with free access to apps can obtain information to improve their understanding and management of health issues. Currently, there are cancer-related apps available on iPhones and androids. However, there are no guidelines to control these apps and ensure their quality. Furthermore, these apps may significantly modify the patients’ perception and knowledge toward drug-related health services. Objective: The aim of this study was to assess the convenience, quality, safety and efficacy of apps for cancer patient care. Methods: The study was conducted by searching all apps related to cancer care on both Google Play Store and Apple iTunes Store. A detailed assessment was then performed using the mobile application rating scale (MARS) and risk assessment tools. Results: The results indicated that on a scale from 1-5, 47% of the apps were rated ≥ 4. The MARS assessment of the apps indicated an overall quality rating of 3.38 ± 0.9 (mean ± SD). The visual appeal of the app was found to have a significant effect on app functionality and user engagement. The potential benefits of these apps come with challenges and limitations. Patents related to smartphone applications targeting patients were also discussed. Conclusion: We recommend a greater emphasis toward producing evidence-based apps. These apps should be rigorously tested, evaluated and updated by experts, particularly clinical pharmacists. Also, these may alter patient attitudes toward services provided by physicians and pharmacists. Finally, these apps should not replace in-person interactive health services.

This study assessed whether community participation in health related activity was a reality or just popular development rhetoric.Using action research methodology, focus group discussions and informal contacts were made with farm workers consisting of twelve families in Umkomaas, south of Durban in the province of Kwa-Zulu Natal. The aim was to establish whether this community could be actively involved in all aspects of community participation. The level of participation was described using Rifkin’s model (Rifkin et al, 1988). Results of this study revealed that the community was able to identify their own health problems, prioritize them and plan appropriate strategies to meet the needs identified.

AimsTo explore the level of supervision between training and non-training posts at LSCFT.Background•Supervision is defined as ‘provision of guidance and feedback on matters of personal, professional and educational development in the context of a trainees' experience of providing safe and appropriate patient care’.•Along with the trainees, doctors working in non-training posts such as staff grade, specialty doctors, trust grade doctors (TJD)and MTI (Medical training initiative) doctors form an integral part of patient care in the NHS.Method•A mixed method approach was adopted with both qualitative and quantitative data collected simultaneously in the form of an online questionnaire.•An anonymous online questionnaire was sent to junior doctors currently in training and non-training posts at LSCFT in 2019 using Meridian software.Result1- Quantitative Data: - Participants included were doctors in training post such as Foundation Doctors (5), Psychiatry Core Trainees (6), GP STs (2) and doctors in non-training post such as TJD (4), Specialty Doctors (2) and MTI doctors (4). Based on the Meridian score, 84% of doctors were satisfied with the supervision. It was found that 72% of doctors received weekly supervisions, 10% monthly (1 TJD, 1 Foundation trainee) and16% bi-monthly (1 MTI, 1 SAS, 2 CTs). The data suggested that there was no difference in the frequency of supervisions between training and non-training posts at LSCFT.2- Qualitative Data: - The feedback was common as there was no major difference between training and non-training doctors. •Positives – WPBAs, discussion on reflections, management of complex cases and medication, personal issues affecting work.•Negatives – Limited discussion on QI, Audit, Research and Psychotherapy.- More specific help, need more support at times.Conclusion1.To prepare a checklist of contents to be discussed during supervision.2.To prepare a timeline chart of supervision.3.Preparing a ‘menu’ of QI projects that junior doctors can sign up to at the start of each post.4.To formulate training packages available to support junior doctors with QI/Audits.
