Clefts are common birth defects. They are accompanied by various malformations, including disturbances in facial look as well as skeletal disorders that include malocclusions, most frequently crossbites and class III anomalies. The aim of the study was to present the commonest malocclusions in patients with total cleft of the lip, alveolar bone and palate (n=154) and compare the results to the healthy on-cleft patients (n=151). Normal occlusion, characteristic for I angle class, was observed in 50% of the control group and 30% of the examined. In the examined patients with clefts, most frequently crossbite and open bite on the cleft side was observed. In patients with clefts, only 2 out of 154 patients presented isolated dental anomalies. In healthy individuals the commonest occlusal disorder was distal occlusion and dental anomalies. The commonest malocclusions among patients with clefts are crossbites and class III malocclusions.

Summary Background/objectives This research aimed to study the malocclusions of children and adolescents with myotonic dystrophy type 1 (DM1), in respect to healthy individuals, and trace the occlusal changes that occurred in these individuals during growth. Materials/methods Thirty-six dental casts, from children and adolescents with DM1 living in western and southern Sweden, were compared with a control group of 50 healthy individuals. To identify potential changes in occlusal traits, 26 casts were assessed and followed-up over a median time of 9 years. Independent samples t-tests were used to compare the two groups and their changes over time. Paired samples t-tests tested changes over time within each group (P < 0.05). Results DM1 patients had a higher prevalence of anterior open bite, posterior crossbite, and Class III malocclusions. When compared to controls, patients presented smaller upper and lower intermolar as well as intercanine widths. In both groups, the individuals revealed longitudinal changes with a decrease in both upper and lower arch lengths and an increase on the palatal vault height. During the follow-up period, the prevalence of malocclusions remained almost the same, only significantly differing regarding the changes that occurred between groups referred to the upper intermolar width, which decreased among DM1 patients. Conclusions/implications In comparison to healthy controls, children and adolescents with DM1 have shown already at an early age a higher prevalence of both anterior open bite and posterior crossbite. These occlusal traits did not change with time apart from the upper narrow intermolar width, which further decreased with time.

Objective. To quantify the severity of malocclusion and dental esthetic problems in untreated Down syndrome (DS) and untreated non-Down syndrome children age 8–14 years old using the PAR and ICON Indices.Materials and Methods. This retrospective study evaluated pretreatment study models, intraoral photographs, and panoramic radiographs of 30 Down syndrome and two groups of 30 non-Down syndrome patients (private practice and university clinic) age 8–14 years. The models were scored via PAR and ICON Indices, and descriptive characteristics such as Angle classification, missing or impacted teeth, crossbites, open bites, and other dental anomalies were recorded.Results. The DS group had significantly greater PAR and ICON scores, as well as 10 times more missing teeth than the non-DS group. The DS group possessed predominantly Class III malocclusions, with the presence of both anterior and posterior crossbites in a majority of the patients. The non-DS group had mostly Class I or II malocclusion with markedly fewer missing teeth and crossbites. The DS group also had more severe malocclusions based upon occlusal traits such as open bite and type of malocclusion.Conclusion. The DS group had very severe malocclusions, while the control group from the university clinic had more severe malocclusions than a control group from a private practice.

Abstract Objective: To investigate the effects of different occlusion types on the mandibular asymmetry in young individuals. Materials and Methods: Mandibular asymmetry measurements were performed on the panoramic radiographs of 189 subjects (104 females and 85 males; age range, 11–15 years), with different occlusion patterns. The subjects were divided into five groups according to the occlusion types, namely, Angle Class I (Cl I), Class II division 1 (Cl II/1), Class II division 2 (Cl II/2), Class III (Cl III), and normal occlusions. The Kruskal-Wallis test was used to determine the possible statistically significant differences between the groups for condyle, ramus, and condyle-plus-ramus asymmetry index measurements. Identified differences between groups were further analyzed using the Mann-Whitney U-test at the 95% confidence interval (P < .05). Results: There were no statistically significant differences between male and female subjects. The Kruskal-Wallis test showed that the occlusion type had a significant effect on the condylar asymmetry. In Cl II/1 cases, condylar asymmetry values were significantly different from the values of Cl II/2 and Cl III malocclusion and normal occlusion types. The normal occlusion control group was significantly different from those of Cl II/1 and the Cl I malocclusion groups. Conclusion: Cl II/1 malocclusion has a significant effect on the condylar asymmetry index when compared to Cl II/2 and Cl III malocclusion and normal occlusion types. However, the mean condylar asymmetry index value in Cl II/1 malocclusion was not different from Cl I malocclusion.

Introduction: Autism spectrum disorder (ASD) is a group of neurodevelopmental disorders diagnosed by impairments in social interaction and communication with repetitive and restrictive stereotyped behavioral patterns. The Prevalence of autism has been reported to be increased in recent years. This study aimed to assess the prevalence of different types of malocclusion among ASD patients in Isfahan in 2018. Materials & Methods: In a descriptive and cross-sectional trial, 92 ASD patients were studied in the age range of 7-18 years at the center for autism patients in Isfahan. Clinical oral examinations of patients are taken to assess the involved malocclusions (Cl I, Cl II and Cl III malocclusions) and malocclusion traits (deep bite, open bite and cross bite) by an educated dental student under the supervision of an orthodontist under natural light. The data are reported using frequency and percentage indices. Results: Class I malocclusion had the highest prevalence 54.3% (50) among ASD patients and the prevalence of class II and class III were found to be 19.6% (18) and 7.6% (7) respectively. The frequency of malocclusions traits of deep bite, cross bite and the open bite were 27.2% (25), 18.5% (17) and 7.6% (7) respectively. Among of the total patients, 65.2% (60) showed normal bite and 18/5% (17) showed Normal occlusion. Conclusion: ASD patients showed class I, class II and class III malocclusions from the most to least frequency and the most frequent malocclusion traits were also deep bite, cross bite and open bite respectively.

Aim of the present study was to evaluate the parameters of computer stabilometry of patients with increased dental attrition. Methods. We have examined 50 patients with increased dental attrition, 45 patients who didn’t have any sign of functional disorders were the control group. The stabilometric examination was carried out using the system for diagnosis and rehabilitation of the musculoskeletal system and the balance function “MBN-Stabilometer” (MBN, Moscow). Inclusion criteria were presence of increased dental attrition 2,3 Bushan’s rate, normal occlusion (Angle’s occlusion classification), age of 35 to 54 years. Exclusion criteria were oral mucosal diseases, periodontitis, temporomandibular joint dysfunction, dental-anomalies and deformities, taking medicine that affect muscle tone; somatic diseases in the acute stage. Results. Analyzing and comparing of the data of the stabilometric study of patients of the main group and the control group showed statistically significant differences in the parameters were obtained. Conclusion. Utilization of computer stabilometry should reveal functional disorders of postural systems, as well as determine the most rational tactics of complex treatment of patients with increased dental attrition.

ABSTRACT Objective: To evaluate the effect of the multiloop edgewise arch wire (MEAW) technique with maxillary mini-implants in the camouflage treatment of skeletal Class III malocclusion. Materials and Methods: Twenty patients were treated with the MEAW technique and modified Class III elastics from the maxillary mini-implants. Twenty-four patients were treated with MEAW and long Class III elastics from the upper second molars as control. Lateral cephalometric radiographs were obtained and analyzed before and after treatment, and 1 year after retention. Results: Satisfactory occlusion was established in both groups. Through principal component analysis, it could be concluded the anterior-posterior dental position, skeletal sagittal and vertical position, and upper molar vertical position changed within groups and between groups; vertical lower teeth position and Wits distance changed in the experimental group and between groups. In the experimental group, the lower incisors tipped lingually 2.7 mm and extruded 2.4 mm. The lingual inclination of the lower incisors increased 3.5°. The mandibular first molars tipped distally 9.1° and intruded 0.4 mm. Their cusps moved 3.4 mm distally. In the control group, the upper incisors proclined 3°, and the upper first molar extruded 2 mm. SN-MP increased 1.6° and S-Go/N-ME decreased 1. Conclusions: The MEAW technique combined with modified Class III elastics by maxillary mini-implants can effectively tip the mandibular molars distally without any extrusion and tip the lower incisors lingually with extrusion to camouflage skeletal Class III malocclusions. Clockwise rotation of the mandible and further proclination of upper incisors can be avoided. The MEAW technique and modified Class III elastics provided an appropriate treatment strategy especially for patients with high angle and open bite tendency.

Background: Transposition of the great arteries (TGA) is a congenital heart defect (CHD) in which the aorta and pulmonary artery are transposed; it requires urgent surgical intervention. The Mustard procedure was an early surgery allowing survival into adulthood. However, this procedure is associated with long-term adverse effects including arrhythmias and heart failure (HF). A key factor impairing HF management in adults with CHD is lack of biomarkers to predict outcome. Soluble ST2 (sST2) is a protein secreted by myocytes in response to mechanical strain and fibrosis. No studies have focused specifically on sST2 in TGA individuals that underwent a Mustard procedure.  In this study, we hypothesized adults with TGA palliated with a Mustard procedure would have higher levels of sST2 than healthy individuals, and this would correlate with functional class status.      Methods: We screened the Pediatric Cardiology database for D-TGA subjects and Mustard operation. Healthy controls were recruited from clinic and the community. Patients were matched by age to a control group of 21 healthy individuals.  Severity of symptoms was assessed by NYHA functional classification. sST2 levels were obtained using Critical Diagnostics Presage ST2 Assay kit. Additionally, cTnI, BNP, lipid panel, insulin, glucose, and EKG and echocardiography (TGA) were obtained at IUSM clinical laboratories.      Results: We identified 45 patients with D-TGA and Mustard operation. 19 patients were included in analysis: 12 male and 7 female subjects aged 18 to 46, mean of 34.9 years. In the Mustard group, 9 subjects were assigned to NYHA class II, and 9 to class III. The control group was asymptomatic. sST2 levels in the Mustard group were elevated in 53%, while only 29% in the controls. Of the Mustard subjects with elevated sST2, 67% had elevated cTnI, 80% arrhythmias, 70% EKG T-wave abnormalities, and 80% took cardiac medications.  90% of these patients had low HDL, 40% high LDL, and 40% low insulin.      Conclusion: This study demonstrates patients with Mustard operation averaged higher sST2 levels than healthy subjects. Importantly, of the patients with elevated levels, there was a significant association of sST2 with biomarker abnormalities and clinical heart failure signs.  This suggests in patients with D-TGA palliated with Mustard procedure, sST2 may add predictive value to cardiac related morbidity and mortality.

ABSTRACT Objective: The aim of this study was to investigate the vertical mandibular asymmetry in a group of adult patients with different types of malocclusions, based on Angle's dental classification. Materials and Methods: A sample of 102 patients (age range 19–28) who went for routine orthodontic treatment in the institution were divided into four groups: Class I, 26 patients; Class II/1, 30 patients; Class III, 23 patients; and control group (CG) with normal occlusion, 23 patients. Condylar asymmetry index (CAI), ramal asymmetry index (RAI), condylar-plus-ramal asymmetry index values were measured for all patients on panoramic radiographs. Data were analyzed using Kruskal–Wallis and Mann–Whitney U-test at the 95% confidence level (P < 0.05). Results: The results of the analysis showed that different occlusal types significantly affected the vertical symmetry of the mandible at the condylar level. Class I and Class II/1 malocclusion groups showed a significant difference in CAI values relative to the CG (P < 0.05, P < 0.001). No statistically significant difference was found between the CG and Class III malocclusion group (P > 0.05). Comparisons between Class II/1 and Class I malocclusions revealed a significant difference in CAI values (P < 0.01). Conclusions: Both Class II/1 and Class I malocclusions patients had significantly higher CAI values compared to CG and Class III group. CAI value was significantly higher in Class II/1 malocclusion compared to Class I malocclusion. Both these malocclusions could act as a predisposing factor for having asymmetric condyles if left untreated.

INTRODUCTION: Anterior open bite (AOB) malocclusion presents as lack of vertical overlap of anterior teeth. It is viewed to be unaesthetic and may affect speech and mastication It develops due to the interaction of hereditary and environmental etiological factors and these usually affect the vertical growth of the face. This study describes the vertical changes of South African black people presenting with AOB. AIMS AND OBJECTIVES: The aim was to determine skeletal morphological features of patients with AOB malocclusion. DESIGN: The design was a retrospective, cross-sectional study. MATERIALS: Archived pre-treatment lateral cephalographs of 181 patients who consulted between 2007 and 2014 were divided into four groups: control group of 62 patients with skeletal Class I pattern without AOB; test groups of patients with AOB (119) divided into 35 Class I, 43 Class II, and 41 Class III malocclusions. Records of each group were divided according to gender. Descriptive statistics, the Pearson correlation coefficient, t-test and. Wilcoxon test were employed to analyze the data, and p values of <0.05 were considered statistically significant. RESULTS AND CONCLUSIONS: Patients with AOB had a larger vertical facial pattern in all classes of malocclusion. Males presented with larger Sn-GoGn angles than females. The PFH/AFH ratio was lower across all classes of malocclusion compared to the control group.
