Objective. To provide a comprehensive overview of published evidence on the impact of erythritol, a noncaloric polyol bulk sweetener, on oral health.Methods. A literature review was conducted regarding the potential effects of erythritol on dental plaque (biofilm), dental caries, and periodontal therapy. The efficacy of erythritol on oral health was compared with xylitol and sorbitol.Results. Erythritol effectively decreased weight of dental plaque and adherence of common streptococcal oral bacteria to tooth surfaces, inhibited growth and activity of associated bacteria likeS. mutans, decreased expression of bacterial genes involved in sucrose metabolism, reduced the overall number of dental caries, and served as a suitable matrix for subgingival air-polishing to replace traditional root scaling.Conclusions. Important differences were reported in the effect of individual polyols on oral health. The current review provides evidence demonstrating better efficacy of erythritol compared to sorbitol and xylitol to maintain and improve oral health.

ABSTRACT Bacteriocins are ribosomally synthesized proteinaceous antibacterial peptides. They selectively interfere with the growth of other bacteria. The production and secretion of bacteriocins confer a distinct ecological advantage to the producer in competing against other bacteria that are present in the same ecological niche. Streptococcus mutans, a significant contributor to the development of dental caries, is one of the most prolific producers of bacteriocins, known as mutacins in S. mutans. In this study, we characterized the locus encoding mutacin B-Ny266, a lantibiotic with a broad spectrum of activity. The chromosomal locus is composed of six predicted operon structures encoding proteins involved in regulation, antimicrobial activity, biosynthesis, modification, transport, and immunity. Mutacin B-Ny266 was purified from semisolid cultures, and two inhibitory peptides, LanA and LanA′, were detected. Both peptides were highly modified. Such modifications include dehydration of serine and threonine and the formation of a C-terminal aminovinyl-cysteine (AviCys) ring. While LanA peptide alone is absolutely required for antimicrobial activity, the presence of LanA′ enhanced the activity of LanA, suggesting that B-Ny266 may function as a two-peptide lantibiotic. The activation of lanAA′ expression is most likely controlled by the conserved two-component system NsrRS, which is activated by LanA peptide but not by LanA′. The chromosomal locus encoding mutacin B-Ny266 was not universally conserved in all sequenced S. mutans genomes. Intriguingly, the genes encoding LanAA′ peptides were restricted to the most invasive serotypes of S. mutans. IMPORTANCE Although dental caries is largely preventable, it remains the most common and costly infectious disease worldwide. Caries is initiated by the presence of dental plaque biofilm that contains Streptococcus mutans, a species extensively characterized by its role in caries development and formation. S. mutans deploys an arsenal of strategies to establish itself within the oral cavity. One of them is the production of bacteriocins that confer a competitive advantage by targeting and killing closely related competitors. In this work, we found that mutacin B-Ny266 is a potent lantibiotic that is effective at killing a wide array of oral streptococci, including nearly all S. mutans strains tested. Lantibiotics produced by oral bacteria could represent a promising strategy to target caries pathogens embedded in dental plaque biofilm.

OBJECTIVE : To analyze the effectiveness of motivational interviewing (MI) at improving oral health behaviors (oral hygiene habits, sugar consumption, dental services utilization or use of fluoride) and dental clinical outcomes (dental plaque, dental caries and periodontal status). METHODS : A systematic search of PubMed, LILACS, SciELO, PsyINFO, Cochrane and Google Scholar bibliographic databases was conducted looking for intervention studies that investigated MI as the main approach to improving the oral health outcomes investigated. RESULTS : Of the 78 articles found, ten met the inclusion criteria, all based on randomized controlled trials. Most studies (n = 8) assessed multiple outcomes. Five interventions assessed the impact of MI on oral health behaviors and nine on clinical outcomes (three on dental caries, six on dental plaque, four on gingivitis and three on periodontal pockets). Better quality of evidence was provided by studies that investigated dental caries, which also had the largest population samples. The evidence of the effect of MI on improving oral health outcomes is conflicting. Four studies reported positive effects of MI on oral health outcomes whereas another four showed null effect. In two interventions, the actual difference between groups was not reported or able to be recalculated. CONCLUSIONS : We found inconclusive effectiveness for most oral health outcomes. We need more and better designed and reported interventions to fully assess the impact of MI on oral health and understand the appropriate dosage for the counseling interventions.

Abstract Background Childhood dental caries can affect the children’s and their parents’ oral health-related quality of life. The aim of the present study was to evaluate the impact of oral and dental health conditions on the oral health-related quality of life in preschool children and their parents. Methods In this descriptive-analytical cross-sectional study, samples were selected from children 3 to 6 years old enrolled in licensed kindergartens using "proportional allocation" sampling. Then, the parents of the children were asked to complete the Early Childhood Oral Health Impact Scale (ECOHIS). Results In this study, 350 children aged 3 to 6 years were evaluated with a mean age of 4.73 years. The mean dmft index (decayed, missed, and filled teeth) was 3.94 ± 4.17. The mean score of oral health-related quality of life was 11.88 ± 6.9, which 9.36 ± 5.02 belongs to the impact on children and 2.52 ± 3.20 to parents' impact. Conclusions The mean score of ECOHIS increased with the dmft index increase in children, indicating a significant relationship between the dmft and ECOHIS score. These outcomes can be used as proper resources to develop preventive policies and promote oral health in young children.

Sugary carbohydrate foods have long been associated with increased risk of dental caries formation, but the dental health impact of starchy carbohydrates, particularly those with a high glycemic index (GI), has not been well examined. Aim: To investigate the effect of different starchy foods varying in their GI, on acute changes in dental plaque pH. Methods: In a series of sub-studies in healthy adults, common starchy carbohydrate foods, including white bread, instant mashed potatoes, canned chickpeas, pasta, breakfast cereals, white rice, and an oral glucose solution were consumed in fixed 25 g available carbohydrate portions. The change in dental plaque pH was assessed postprandially over 1 h and capillary plasma glucose was measured at regular intervals over 2 h. Results: Higher GI starchy foods produced greater acute plaque pH decreases and larger overall postprandial glucose responses compared to lower GI starchy foods (white bread compared with canned chickpeas: −1.5 vs. −0.7 pH units, p = 0.001, and 99 ± 8 mmol/L min vs. 47 ± 7 mmol/L min, p = 0.026). Controlling for other food factors (food form and nutritional composition), lower GI versions of matched food pairs produced smaller plaque pH excursions compared to higher GI versions of the same food. Using linear regression analysis, the GI value of starchy carbohydrate foods explained 60% of the variation in maximum plaque pH nadir and 64% of the variation in overall acute dental plaque pH excursion (p < 0.01). Conclusion: The findings imply that starchy foods, in particular those with a higher GI, may play a role in increasing the risk of dental caries.

Objective: To identify the prevalence of caries, plaque score, and periodontal disease and explore the correlations between oral health literacy scores and oral health status among undergraduate students. Materials and Methods: A cross-sectional study was conducted with a group of undergraduate students using the Malay version of the Oral Health Literacy (OHL) Index. A stratified random sampling technique was used to select the samples. Clinical oral examinations were conducted by a single trained examiner using a Community Periodontal Index (CPI) probe and a mouth mirror. Data analysis was conducted using statistical tests contained in SPSS version 26, and statistical significance was set at p<0.05. Results: A total of 280 students were recruited into the study. The prevalence of caries and periodontal disease was 58.9 and 13.2%, respectively. There was a significant association between OHL and field of study (p<0.001). There was also a significant negative but weak correlation between OHL and dental plaque scores (r= -0.147; p=0.014). A significant positive but weak correlation was found between dental plaque scores and dental caries (r= 0.135; p=0.024) and periodontal pocket depth (r= 0.168; p=0.005). Conclusion: The prevalence of dental caries was high, while the prevalence of periodontal diseases was low in this study. OHL correlated significantly with dental plaque scores and field of the study. Thus, OHL assessment is essential to understand a person’s OHL levels and can be considered a screening tool for early detection of poor oral hygiene.

Dental caries is a disease that usually begins very early in life and from which few people remain unaffected. However, because caries is associated with local etiologic vectors, it can be controlled. Altering the diet to render it less cariogenic and making tooth surfaces more resistant to acid attack through the use of fluoride are two potent mechanisms of prevention. Pediatricians are in a unique position to contribute to the dental health of their young patients because of the early age at which children are brought to their offices and because mothers are accustomed to accept their recommendations. The pediatrician's role in caries control includes diagnosis, referral, and preventive therapy. Early referral of children who appear dentally healthy as well as those in which there are obvious cavities in the teeth has been stressed. As part of a caries preventive program, a system of systemic fluoride supplementation has been outlined for those children who are not drinking optimally fluoridated water. In addition, food-caries relationships have been discussed and practical suggestions for reducing the cariogenicity of a child's diet have been made. Although toothbrushing is considered a traditional method of caries prevention, it has not been discussed in this article for two reasons. First, published evidence does not conclusively support the premise that toothbrushing per se significantly reduces dental caries. Second, the results of the general efforts of the dental profession to motivate patients to adopt either meticulous or frequent brushing habits have been poor. It would be unrealistic to expect the medical profession to initiate procedures that would significantly alter the behavior of their patients' toothbrushing habits. Toothbrushing in the young child, especially the preschooler, should be regarded as another method for providing topical fluoride to the teeth. Parents should be encouraged to brush their young children's teeth as soon as they erupt. A soft, child-size toothbrush should be employed with a scrubbing motion. An anticariogenic toothpaste§ that has proven effective in controlled clinical trials should be recommended by the pediatrician or his nurse. It is hoped that the mechanisms for influencing children's dental health that were discussed in this article can be incorporated with minimum effort into the normal routine of pediatricians' practices.

Background: Dextran is a branched polysaccharide and one of the polymers, present in the biofilm matrix. The dextran plays a perilous role in dental plaque formation, which is involved in the development of some common oral diseases like dental caries. The dextran-hydrolyzing enzymes are under investigation to treat and manage the dental plaques. Aims and Objective: The present study reporting the preliminary observations on the effect of the use of dextranase-containing mouthwash (DMW) on dental plaque and oral health. Materials and Methods: DMW was prepared with food-grade dextranase, preservatives, gellingagents, and water as detailed. Four weeks of experimental design was employed in fourteen healthy volunteers. The selected volunteers were recommended to use DMW for at least twice a day. The plaque index (PI), probing depth (PD), gingival index (GI) and bleeding on probing (BOP) of the volunteer's teeth have been assessed before and after four weeks of DMW use. Results:The volunteers were insisted to use a DMW solution twice a day for four weeks. The PI, PD, GI, and BOP was measured before and after the treatment. The plaque index of the subject at baseline and after treatment was 2.22 ± 0.48, and 1.88 ± 0.50, respectively. PI was significantly reduced after the use of DMW solution for four weeks. The value of PD was 2.00 and 2.00 at baseline and after the use of DMW, respectively. The value of PD was not changed when compared to the baseline values. The sensory evaluation of DMW was performed using questionnaires. Conclusion: The preliminary study results suggested that the use of DMW solution for four weeks (twice a day) notably reduced the PI without any change in PD. However, GI and BOP values were not affected after the use of DMW. The participants, based on the sensory evaluation, accepted the prepared DMW solution. Additional detailed research on the impact of DMW on oral hygiene is needed to confirm the beneficial effects of DMW.

The dental plaque is an oral microbiome hardly associated to be the etiological agent of dental caries and periodontal disease which are still considered serious health public problems. Silver nanoparticles (AgNPs) have demonstrated to have good antimicrobial properties affecting a wide variety of microorganisms, including oral bacteria; however, there is no scientific information that has evaluated the antimicrobial effect of AgNPs against clinical oral biofilms associated with dental caries and periodontal disease. The aim of this study was to determine the antimicrobial and substantivity effects of AgNPs in oral biofilms isolated clinically from patients with dental caries and periodontal disease. Sixty-seven young and young-adult subjects with dental caries and periodontal disease were clinically sampled through the collection of subgingival dental plaque. The inhibitory effect of AgNPs was performed with standard microbiological assays by triplicate using two sizes of particle. Polymerase chain reaction (PCR) assay was used to identify the presence of specific bacterial species. All AgNPs showed an inhibitory effect for all oral biofilms for any age and, generally, any gender (p>0.05); however, the effectiveness of the antimicrobial and substantivity effects was related to particle size, time, and gender (p<0.05). The identified microorganisms were S. mutans, S. sobrinus, S. sanguinis, S. gordonii, S. oralis, P. gingivalis, T. forsythia, and P. intermedia. The AgNPs could be considered as a potential antimicrobial agent for the control and prevention of dental caries and periodontal disease.

ABSTRACT Objective The aim was to evaluate the prevalence of dental caries, periodontal disease and edentulism in morbidly obese and bariatric patients, and to verify the oral health impact on quality of life. Methods We evaluated 33 morbidly obese patients (G1) and 50 patients submitted to bariatric surgery (G2). The DMFT, CPI, prosthetic status and prosthetic need for edentulism were used to analyze oral conditions. The impact of oral health on quality of life was assessed using the OIDP questionnaire. Results CPOD were G1: 9.0±7.0 and G2: 13.3±8.0 (p= 0.020). The dental calculus was the worst periodontal condition G1: 72.7% and G2: 80.0% (p= 0.752). The majority of G1 and G2 patients did not use (p = 0.878) and did not need (p= 0.132) dental prosthesis. The mean of OIDP was G1: 11.2±23.8 and G2: 14.16±23.8 (p= 0.089). Conclusion It concluded that bariatric patients have a higher prevalence of dental caries and the need for dental prostheses is related to the impact of oral health on the quality of life of obese patients.
