Splenic abscess is a rare clinical entity with diagnostic challenges. Though rare, it is potentially a life-threatening clinical condition with high mortality reaching more than 70%. The common signs and symptoms include the triad of fever, left upper quadrant tenderness, and leukocytosis. Early diagnosis, however, can readily be made by the combination of clinical features, abdominal ultrasonography (USG), and computed tomography (CT). The management of splenic abscess includes medical therapy, CT-guided percutaneous aspiration, and splenectomy. We, hereby, present a rare case of splenic abscess with gangrenous gas necrosis, who underwent splenectomy.

<span>Syphilis is often thought to be a disease of the past, largely eradicated in modern health care; however, the rates are still extremely high in certain populations. The diagnosis of syphilis may be overlooked by primary-care clinicians due to the presence of nonspecific signs and symptoms that may be indistinguishable from other diseases. Left undiagnosed and untreated, life-threatening complications, including hepatitis, stroke, and nervous system damage, may occur particularly in immuno-compromised individuals. We present a case of lues maligna an extremely rare presentation of syphilis.</span>

AbstractDental infection has plagued humankind for as long as our civilization has been a fight against microorganisms by man dates back to ancient civilization. The discovery of antibiotics is encouraging trends conquest of the microbial infections. Infection of the deep facial spaces of the head and neck still represents a major challenge in hospitals. Untreated infections may result in abscess formation that can spread through different levels in and between the facial spaces and result in life-threatening situations including mediastinitis, pericarditis, meningitis, septic shock, airway compromise, jugular vein thrombosis, and arterial erosion, Although the complications are rare, they are serious and life threatening. The most critical point in the facial space infections is the understanding of the common and uncommon signs and symptoms, choice of antibiotics, definitive treatment and a thorough knowledge about the sequela if left unmanaged.

Splenic abcess is a rare but potentially life threatening condition which can be less frequently caused by gram negative saprophytes, Burkholderia pseudomallei. Because of the challenges faced in the identification of this organism, it is often under diagnosed or misdiagnosed. Here, we report a case of splenic abcess eventually leading to disseminated melioidosis caused by Burkholderia pseudomallei in a 40 year old diabetic male from a tertiary-care hospital in southern India, who presented with fever and pain over left upper abdomen.

AbstractNonsegmented positive-sense RNA enveloped RNA severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus can result in coronavirus disease 2019 (COVID-19). This virus is from β-coronaviridae family of viruses. The common signs and symptoms of COVID-19 include pyrexia, cough, dyspnea, fatigue, myalgia, cephalgia, diarrhea, and nausea. Physicians and dentists around the world could directly link the COVID-19 and oral diseases such as ageusia and anosmia. After time passes, different aspects of symptoms of the diseases have been discovered. Research suggests that the oral cavity is the most vulnerable region for the virus because of angiotensin-converting enzyme-2 (ACE2) receptor abundance in the mouth. In this case report (no. of patients = 6), we would like to report significant findings in patients who were diagnosed with COVID-19 reported to our clinic during May 2021 complaining about the oral manifestation of it such as xerostomia, gingival inflammation, and cracked teeth. All patients are younger than 40 years with no history of dental complaints and oral diseases. Fortunately, these symptoms are not life threatening and treatable/manageable by current treatment options. To date, there is no clear proof of how and via which pathway, SARS-CoV-2 genomic blueprint causes the oral manifestation of COVID-19 beside ACE2 receptor which is the only known biopathway for such incidents.

Hereditary angioedema (HAE) is a relatively rare clinical entity that can potentially cause life-threatening airway or intestinal oedema, patients with the latter usually presents with symptoms of gastroenteritis like vomiting, diarrhoea and abdominal pain. Here, we present a unique case of a less recognised type of HAE that is type III in a patient who presented with signs and symptoms consistent with infectious colitis. She previously had similar episodes and was managed multiple times with antibiotics, with no satisfactory response. There, she underwent extensive diagnostic evaluation. On the basis of findings of further investigations on the current visit, she was eventually diagnosed with intestinal angioedema. To the best of our knowledge, the present paper represents the third reported case of type III HAE-induced intestinal angioedema. Additionally, we undertake a literature review of HAE.

Dementia is a major public health concern that afflicts an estimated 24.3 million people worldwide. Great strides are being made in order to better diagnose, prevent, and treat these disorders. Dementia is associated with multiple complications, some of which can be life-threatening, such as dysphagia. There is great variability between dementias in terms of when dysphagia and other swallowing disorders occur. In order to prepare the reader for the other articles in this publication discussing swallowing issues in depth, the authors of this article will provide a brief overview of the prevalence, risk factors, pathogenesis, clinical presentation, diagnosis, current treatment options, and implications for eating for the common forms of neurodegenerative dementias.

: The prevalence of primary hypothyroidism is increasing in adults (PHTA), and the reduction in the threshold for treatment and also the requirement to increase drug usage are major problems in approaching this disorder. Persian Medicine (PM) looks from a different view on etiology of diseases. Therefore, we tried to present the etiologies of PHTA according to an important book of PM i.e. “Zakhire Kharazmshahi”. Method: At first, the common symptoms and signs of PHTA were identified by investigating the Medline, Scopus, and Cochrane databases and their Persian equivalents were extracted from PM sources. These synonyms were searched as keywords in the book and the primary causes that were mentioned in association with the signs and symptoms were extracted. Then, we explained the total etiologies that were discussed for the occurrence of the causes, with respect to the principles of health care in PM. Results: "Cold distemperament", an increase in "Phlegmatic Humor ", an increase in "Melancholic Humor" and "Emtela" (repletion) were recognized as four main causes of PHTA. According to the book, the most important etiologies of these conditions are insufficient exercise, overeating, food intake before total stomach emptying and excessive amounts of cold temperament substances (foods, herbs, seeds, spices, etc.) in daily diet. Conclusion: From the viewpoint of PM, lifestyle spatially eating habits and physical activity play important causative roles in occurring and prognosis of PHTA. Hence, it is recommended to assess these results by more observations and clinical studies.

Acute thoracic aortic dissection is an uncommon, although not rare, life-threatening condition. With protean signs and symptoms that often suggest more common cardiac or pulmonary conditions, it can be difficult to diagnose. Ultrasound has proven useful in making the correct diagnosis. This case demonstrates that training gained using standard ultrasound machines can be easily and successfully adapted to newer handheld ultrasound devices. The examination technique using the handheld device is illustrated with photos and a video.

We reported a rare case of spontaneous frank rupture of a small (4 mm) penetrating aortic ulcer in the ascending aorta resulted in catastrophic bleeding. The ulcer only created a pinhole wound in the adventitia without saccular aneurysms, intramural hematomas, or aortic dissections. Notably, the wound could be directly closed because the aortic wall was intact only 5 mm away from the bleeding site. The postoperative course was uneventful, and the patient was discharged on the 11th postoperative day. After 8 months, follow-up computed tomography showed no abnormality of the aortic wall at the repair site.
