Background: Pediatric cardiology (PC) fellows, tasked with studying a large and dynamic field, may benefit from a quick-access digital resource that reflects contemporary practice. Objective: To design a PC Handbook smartphone app and evaluate its effectiveness in enhancing the accessibility of information such as guidelines and treatment recommendations for PC fellows. Methods: The PC handbook app was designed for iPhone using XCode and Swift programming. A pre-app electronic survey and instructions for download were sent to U.S. PC fellows followed by a post-app survey within 3 months of use. Fellows were asked to rate the ease of rapid access to various types of information, such as guidelines and normal values, with responses given on a 4-point scale (1 = very difficult, 4 = very easy). Responses were compared pre- and post-app using the Wilcoxon signed-rank test. Results: Two-hundred and thirty PC fellows were contacted; 23% (total n = 53) completed both the pre-app and post-app surveys and were included for analysis. Over half (57%) of fellows used the PC handbook app 1-3 times per month while 17% used it at least 1-3 times per week. After using the app, fellows found it easier to quickly access information in 8 out of 9 domains evaluated (Fig 1). All fellows found the app easy to use (100%), most felt that the app is well-organized (98%), contains reliable information (92%), and is useful for finding quick answers to clinical questions (87%). Almost all (91%) would recommend the app to a colleague. Conclusions: The Peds Cardiology Handbook iPhone app is a useful, reliable tool that provides quick access to high yield information, including various guidelines and references, in one location. Overall, PC fellows found it easier to rapidly access clinically relevant information after using the app, though future studies may be necessary to evaluate long-term use and impact on evidenced-based practice.

The type of sport suggested for Diabetes Mellitus patient is Diabetes Gym, but most societies, especially diabetes mellitus patients are lazy to do it. On diabetes mellitus type 2, diabetes gym acts as glycemic control which manages and control blood sugar rate. This research purposes to know diabetes gym influence toward the decrease of blood sugar rate on Diabetes Mellitus patient type 2 in Puskesmas Pakis Surabaya. This research used quasy experiment design. Total of the population of all Diabetes Mellitus patients type 2 is 28 respondents taken by probability sampling - simple random sampling technique; 14 respondents in control group and 14 respondents in treatment group. The independent variable is Diabetes Gym and the dependent variable is the decrease of blood sugar rate. Data are collected by using Glucometer, striptest, needle and observation sheet. Data are analyzed by using Wilcoxon signed rank test with α < 0.05. The result of this research shows that almost all of the respondents in the treatment group have a good category of blood sugar decrease after performing gym, meanwhile almost all of the respondents in control group that do not perform gym have an adequate category of blood sugar decrease. Then, the result is analyzed by Wilcoxon signed rank test and obtained ρ (0,019) < α (0,05) so H0 is refused. It means that there is an influence of diabetes gym to the decrease of blood sugar on the Diabetes Mellitus patients type 2 in Puskesmas Pakis Surabaya. Performing Diabetes Gym influences in decreasing blood sugar rate. Therefore, nurse can take a part in leading diabetes patients to perform diabetes gym so it can be used as the therapy in controlling and decreasing blood sugar rate.

Abstract Objectives: Falls are the leading source of accidental injury and hospitalization among adults over the age of 65. Relative to people with intact cognition, individuals with cognitive impairment are at increased risk for falls; however, few falls prevention programs exist to specifically reduce and prevent falls in this population. To address this issue, we developed a novel, multifactorial, cognitively-based falls prevention program, Stepping Out. Based on the popular and effective evidenced-based program, Stepping On, Stepping Out was modified and tailored to the learning needs of individuals with Mild Cognitive Impairment (MCI). We hypothesized that older adults with MCI would find the program understandable, and that program participants would demonstrate reduced falls. Methods: Sixteen veterans, mean age of 77.5, diagnosed with MCI and at increased risk for falls participated in Stepping Out. Falls were collected for the six months prior to intervention and the six months during and after program participation. All participants completed post-program evaluations. Falls incidence was compared using a Wilcoxon paired signed rank test. Results: Stepping Out was found to be feasible and comprehensible by all participants. Program participants exhibited significantly reduced falls, with median reduction of two falls (p = 0.0020), and a range of zero to 12 falls. Discussion: With appropriate modifications, individuals with MCI were able to benefit from a cognitively-based falls prevention program and to reduce accidental falls incidence. Falls are an important and feasible target to address among individuals with cognitive impairment.

Background Positron emission tomography (PET) findings for gliomatosis and lymphomatosis have been rarely reported. Purpose To compare PET/computed tomography (CT) findings using 11C-methionine (MET) from PET/CT findings using 18F-fluorodeoxy glucose (FDG) for patients with lymphomatosis or gliomatosis of the brain. Material and Methods Participants comprised all 10 patients with lymphomatosis or gliomatosis of the brain treated at our institution in the past 12 years. Underlying pathologies comprised intravascular lymphoma (n = 1), lymphomatosis (n = 3), and gliomatosis (n = 6). All cases were pathologically diagnosed. In seven patients, both MET-PET/CT and FDG-PET/CT were performed simultaneously in a single study. In three patients, only FDG-PET/CT was performed. The degree of tracer accumulation to the lesion was evaluated qualitatively. Quantitatively, the ratio of maximum standard uptake value (SUVmax) in tumor to that in normal tissue (T/N ratio) was measured and compared between FDG and MET. Results Qualitatively, MET accumulated to part of the lesion in six of seven patients and almost all of the lesion in one in seven patients. FDG accumulated to part of the lesion in three of ten patients and almost all of the lesion in one of ten patients. No FDG accumulation was seen in the lesion in six patients. Quantitatively, mean ± SD T/N ratio was significantly higher with MET (2.11 ± 0.63) than with FDG (1.18 ± 0.84; P < 0.05, Wilcoxon signed-rank test). Conclusion In lymphomatosis and gliomatosis, FDG accumulates in only part of the lesion. FDG is thus less suitable than MET for depicting these lesions.
