Abstract Objectives: Many older adults experience memory changes that can have a meaningful impact on their everyday lives, such as restrictions to lifestyle activities and negative emotions. Older adults also report a variety of positive coping responses that help them manage these changes. The purpose of this study was to determine how objective cognitive performance and self-reported memory are related to the everyday impact of memory change. Methods: We examined these associations in a sample of 94 older adults (age 60–89, 52% female) along a cognitive ability continuum from normal cognition to mild cognitive impairment. Results: Correlational analyses revealed that greater restrictions to lifestyle activities (|rs| = .36–.66), more negative emotion associated with memory change (|rs| = .27–.76), and an overall greater burden of memory change on everyday living (|rs| = .28–.61) were associated with poorer objective memory performance and lower self-reported memory ability and satisfaction. Performance on objective measures of executive attention was unrelated to the impact of memory change. Self-reported strategy use was positively related to positive coping with memory change (|r| = .26), but self-reported strategy use was associated with more negative emotions regarding memory change (|r| = .23). Conclusions: Given the prevalence of memory complaints among older adults, it is important to understand the experience of memory change and its impact on everyday functioning in order to develop services that target the specific needs of this population.

Aim. To examine the prevalence of low back pain, to identify self-perceived triggers of low back pain, and to investigate the impact of perceived pain on the daily activities and mood among medical students. Methods. This cross-sectional study enrolled 459 fourth year students at the Faculty of Medicine in Belgrade during December 2014. The anonymous questionnaire was used for data collection. In data analysis, the chi-square test and t-test were used. Results. The lifetime prevalence of low back pain was 75.8%, 12-month prevalence 59.5%, and point prevalence 17.2%. Chronic low back pain was experienced by 12.4% of the students. Both the lifetime (p=0.001) and the 12-month (p=0.001) low back pain prevalence rates were significantly higher among female medical students. Mental stress during an exam period (p=0.001), sitting at the university (p=0.002), fatigue (p=0.043), improper body posture (p=0.005), and lack of exercise (p=0.001) as self-perceived triggers of low back pain were significantly more often reported by female students, compared to males. Regarding daily functioning, the experience of low back pain mostly affects students sleeping (14.6%) and walking (12.0%). Conclusions. The prevalence of LBP is high among Belgrade medical students and significantly affects their everyday functioning.

Оценка качества жизни с точки зрения ребенка, больного сахарным диабетом 1 типа, и его родителей имеет важность для планирования терапии, но может существенно различаться. Целью работы было оценить качество жизни детей дошкольного возраста и их матерей для оптимизации пациент-ориентированных схем лечения и улучшения метаболического контроля диабета. Обследованы 35 детей в возрасте 4-6 лет (19 мальчиков и 16 девочек), больных сахарным диабетом 1 типа, находящихся на лечении в клинике Центра. Для оценки качества жизни использовали русифицированную версию опросника KINDL для детей дошкольного возраста и их матерей. Оценка проводилась по шкалам физического и эмоционального благополучия, самооценки, семейных отношений, взаимоотношения с друзьями, ежедневного функционирования, влияния заболевания, а также суммарных показателей качества жизни по 100-балльной шкале. Статистическая обработка данных проведена с использованием прикладной программы JAMOVI 1.6. Статистически значимые различия были выявлены по шкале суммарной оценки качества жизни, а также по шкалам влияния заболевания и эмоционального благополучия на качество жизни. Установлены более низкие оценки суммарного качества жизни, а также влияния на него заболевания, по мнению матерей по сравнению с детьми с сахарным диабетом 1 типа. Напротив, оценка влияния эмоционального благополучия на качество жизни при сахарном диабете 1 типа была более высокой со стороны матерей по сравнению с их детьми. Наиболее значимое модифицирующее влияние на исследуемые показатели было установлено по факторам образования матери, семейного положения, способа введения инсулина и улучшения контроля диабета. Оценка качества жизни может различаться по мнению детей с сахарным диабетом 1 типа и их матерей. Выявленные особенности рекомендуется использовать в планировании пациент-ориентированных программ лечения диабета и в работе школ сахарного диабета. Assessment of Health Related Quality of Life (HRQoL) features frequently assesses both the child and their mothers perspectives in preschool children with diabetes mellitus type 1 (T1DM), but parent-proxy and child self-reports may differ, and knowledge of these differences are not enough. The aim was to investigate HRQoL assessed by preschool children with T1DM and by their mothers, potential differences in the children-maternal estimates and the factors influencing these discrepancies. 35 preschool children (16 girls from 4-to-6-year-old ) with T1DM underwent the self-report KINDL questionnaire for children aged 4-6 years (Kiddy-KINDL for children). Their mothers underwent the parental version (Kiddy-KINDL for parents). Both versions enable measuring child HRQoL in physical, emotional wellbeing, self-esteem, family, friends, everyday functioning, and the disease dimensions, as well as KINDL total on a 0-100 scale. Statistically significant differences were found on the scale of the total assessment of the quality of life, as well as on the scales of the impact of disease and emotional well-being on the quality of life. Lower estimates of the total quality of life, as well as the effect of the disease on it, in the opinion of mothers, compared with children with type 1 diabetes, were established. In contrast, the assessment of the impact of emotional well-being on the quality of life in type 1 diabetes was higher on the part of mothers compared to their children. The most significant modifying effect on the studied parameters was found for the factors of mother's education, marital status, method of insulin administration, and improved diabetes control. The assessment of the quality of life may differ according to the opinion of children with type 1 diabetes and their mothers. The identified features are recommended for use in planning patient-oriented diabetes treatment programs and in the work of «diabetes schools».

AbstractIn theory, semantic memory may trigger and support the execution of everyday activities. This study explored this question by comparing three patients with semantic dementia to 40 normal controls performing different everyday activities. Participants were tested in their home using the Instrumental Activities of Daily Living Profile, an ecological measure of everyday functioning. Participants were informed that they had unknowingly invited two guests for lunch and should prepare accordingly. With these instructions, they dress to go outdoors, go to the grocery store, shop for food, prepare a hot meal, have the meal with the guests, and clean up after the meal. Performance was analyzed on the basis of four operations related to problem solving: formulate a goal, plan, execute, and verify attainment of the goal. Results indicate that compared to normal controls, two patients had significant difficulties and needed assistance with all operations of problem-solving, particularly while preparing a meal and cleaning up after the meal. One patient showed no difficulties despite severe semantic deficits. These results suggest that semantic deficits alone cannot explain the difficulties observed, but may contribute to some aspects of everyday actions such as those involved in everyday problem-solving. Structural and functional brain alterations as well as cognitive deficits are well-documented findings in schizophrenia patients. Cognitive impairments affect the long-term outcome of schizophrenia and are the main contributors to disability. Despite their clinical impact, however, no effective options are available to treat them sufficiently. Aerobic endurance training has been shown to have effects on brain plasticity, gray and white matter volume as well as functional connectivity measures and on cognitive functioning in animal models and healthy humans. However, effects of physical exercise in combination in combination with cognitive remediation are unknown in Schizophrenia. 21 chronic schizophrenia patients and 21 age and gender–matched healthy controls underwent 3 months of aerobic exercise (endurance training, 30 min, 3 times per week). 21 additionally recruited schizophrenia patients played table soccer (known as “foosball” in the USA) over the same period. After 6 weeks of endurance training or table soccer, all participants commenced standardized cognitive training with a computer-assisted training program. We could show that a 3-month endurance-training program combined with CR therapy had positive effects on everyday functioning in multi-episode Schizophrenia patients. Deficits improved from medium to mild as assessed with the GAF. Negative symptoms, short and long-term verbal memory and cognitive flexibility also improved with training. We could demonstrate grey matter volume increase in the left temporal lobe in schizophrenia patients undergoing endurance training. A non-endurance and coordinative training stimulus like playing table soccer led to a clearly distinct pattern of grey matter alterations in Schizophrenia patients.Disclosure of interestThe author declares that he has no competing interest.

Cognitive systems that track, update, and utilize information about reward (consequences) and risk (uncertainty) are critical for adaptive decision-making as well as everyday functioning and wellbeing. However, little is known about what shapes individual differences in reward and risk sensitivity, independent of each other, during decision-making. Here, we investigate the impact of early life experience—a potent sculptor of development—on behavioral sensitivity to reward and risk. We administered a widely used decision-making paradigm to 62 adolescents and young adults exposed to early life adversity in the form of institutional orphanage care and 81 comparison individuals. Leveraging random coefficient regression and computational modeling, we observed that previously institutionalized individuals displayed general reward hyposensitivity, contributing to a decreased propensity for adaptive decision-making relative to comparison individuals (e.g., when prospective rewards are high). By contrast, group differences in risk sensitivity were selectively observed on loss, but not gain, trials. These results are the first to independently and explicitly link early experiences to reward and risk sensitivity during decision-making. As such, they lay the groundwork for therapeutic efforts to identify and treat adversity-exposed individuals at risk for psychiatric disorders characterized by aberrant decision-making processes.

BACKGROUND Sleep quality has been associated with cognitive and mood outcomes in otherwise healthy older adults. However, most studies have evaluated sleep quality as aggregate and mean measures, rather than addressing the impact of previous night’s sleep on next-day functioning. OBJECTIVE This study aims to evaluate the ability of previous night’s sleep parameters on self-reported mood, cognition, and fatigue to understand short-term impacts of sleep quality on next-day functioning. METHODS In total, 73 cognitively healthy older adults (19 males, 54 females) completed 7 days of phone-based self-report questions, along with 24-hour actigraph data collection. We evaluated a model of previous night’s sleep parameters as predictors of mood, fatigue, and perceived thinking abilities the following day. RESULTS Previous night’s sleep predicted fatigue in the morning and midday, as well as sleepiness or drowsiness in the morning; however, sleep measures did not predict subjective report of mood or perceived thinking abilities the following day. CONCLUSIONS This study suggests that objectively measured sleep quality from the previous night may not have a direct or substantial relationship with subjective reporting of cognition or mood the following day, despite frequent patient reports. Continued efforts to examine the relationship among cognition, sleep, and everyday functioning are encouraged.
