Few reliable data exist on the prevalence of secretory otitis media (SOM) in the Third World. A large epidemiologic cross-sectional study was undertaken in two communes in southern Vietnam to study an urban and a rural community during two different climatic conditions: the dry and rainy seasons. The participants included 3,300 children (6,598 ears) ages 6 months to 10 years. Otolaryngological and medical histories were obtained, and an otolaryngological examination was carried out on 1,669 children in April 1995 (the dry season) and on 1,631 children in December 1995 (the rainy season). Tympanograms were obtained (n = 6,055), 429 of which were type B curves. The overall prevalence of SOM was 7.1%, the highest incidence was at the age of 2 years (with a prevalence of 22%), and there was a significantly higher prevalence of SOM during the rainy season than during the dry season. No significant difference in incidence was found in the urban district as compared to the rural district.

AbstractA study on the epidemiology of bovine trypanosomosis was conducted from September 2006 to April 2007 in six villages of the Gimbi district in west Wollega zone of Ethiopia. The prevalence of the disease, the apparent densities and distribution of tsetse and other biting flies in two seasons, the dry and rainy, were determined. The results of a questionnaire survey from 80 farmers revealed that trypanosomosis was a major health problem affecting animals and impeding agricultural activities.A total of 568 blood samples were collected from randomly selected animals (280 animals in rainy and 288 in dry season) and revealed the presence of Trypanosoma congolense Broden, 1904 and T. vivax Zieman, 1905 in the area. Trypanosoma congolense was the dominant species that accounted for 66.2% of the infections. The mean packed cell volume (PCV) concentrations were 22.77% (95% CI =19.99-21.55) in parasitaemic and 25.25% (95% CI=24.88-25.61) in aparasitaemic animals with a significant difference (P<0.005). There was a significant (P<0.012) difference in trypanosome infection between age groups of cattle, being higher in adults. The overall prevalence of trypanosomosis was 12.5%, while the disease prevalence was higher during the rainy season (15 %) than the dry season (10.1%). In three villages of lowland areas (below 1600 meter above sea level), a higher prevalence was recorded 20.9% and 7.9% as compared to three villages of midland areas (≥ 1600 meter above sea level) 11.8% and 8.3% in late rainy and dry season, respectively.A fly-survey was conducted by using 80 monoconical pyramidal traps and revealed that two tsetse species, namely Glossina morsitans submorsitans Newstead and Glossina tachinoides Westwood were found along with other biting flies (Tabanus, Haematopota and Stomoxys species). Higher numerical catches of Glossina were recorded in late rainy season and the apparent density was positively correlated (r=0.5171) with the prevalence of infection.

The relationship between otitis media with effusion (OME) and chronic rhinosinusitis with nasal polyposis (CRSwNP) remains unclear. We conducted a cross-sectional study of 80 consecutively presenting patients—42 males and 38 females, aged 15 to 76 years (median: 48)—who were diagnosed with CRSwNP. Our aim was to ascertain the prevalence of OME in CRSwNP patients, to determine whether the severity of CRSwNP affected OME, and to identify risk factors for OME in CRSwNP patients. The severity of CRSwNP was assessed on the basis of nasal symptoms, endoscopic nasal examinations, and Lund-Mackay staging scores. In addition to demographic data, we obtained information on each patient's history of otitis, otoscopic findings, and the results of pure-tone audiometry and tympanometry. We then compared the data between CRSwNP patients with OME (n = 20) and those without (n = 60). In the OME group, a conductive hearing loss was present in 16 patients (80.0%); all patients in the control group had normal hearing. With regard to symptoms, only rhinorrhea appeared to be more common in patients with OME than in those without, although the difference was not statistically significant. We found no significant difference in nasal polyposis grades between the two groups. Also, we found no correlations between the risk of OME and previous surgical treatment, asthma, allergy, aspirin intolerance, aspirin and/or NSAID intolerance, aspirin and/or sulfite intolerance, and aspirin-exacerbated respiratory disease. Based on our findings, we conclude that OME occurs frequently during the evolution of CRSwNP, even when the nasal disease is well controlled. This finding suggests the possible presence, in OME and/or CRSwNP, of a global inflammatory process that involves the epithelium in both the middle ear and upper airway.

Aim: To determine the improvement of hearing after grommet insertion in patients with secretory otitis media. Study Design:A Quasi-Experimental study. Place and Duration: ENT, Head & Neck Surgery department of Lady Reading Hospital MTI, Peshawar and Azra Nahid Medical College, Lahore for duration of two years from February 2019 to February 2021. Methods: The study was performed on 48 ears with secretory otitis media. Before the operation,evaluation of both ears along with tympanmontometry and Pure Tone Audiometry were performed. Documentation ofhearing loss was done pre-operatively. During the operation accomplished under GA, a grommetwas placed in the anterior inferior quadrant of the tympanic membrane. After the surgery, PTA was repeated in the postoperative period before the patient was discharged from the hospital. Results:Our study included 28 patients with secretory otitis media and a total of 48 ears. Of the ears, 27 (56.25%) were male and 21 (43.75%) were female. Both ears were affected in 21 patients. Two patients had unilateral ear involvement. The right ear was affected in 26 cases (51.1%) and the left ear in 22 cases (48.9%). 7-55 years was the age range of the patients and 14.10 ± 9.11 years was the mean age. The degree of preoperative hearing loss was mild (20-40 dB) in 5 (10.41%) ears, moderate (40-60 dB) in 37 (77.1%) ears and severe (60-80 dB) in 6 (12.5%) ears. The degree of postoperative hearing loss was mild (20-40 dB) in 36 ears (75%), moderate (40-60 dB) in 11 ears (22.9%) and severe (60-80 dB) in one ear (2.1%). Hearing improvement was not seen in 8 (16.6%) ears, an improvement of 5-10 dB in 34 (70.8%) ears, and an improvement of 10-20 dB in 6 (12.5%) ears. There was a statistically significant difference between preoperative and postoperative hearing loss in the ears, the hearing loss was significantly less after grommet insertion; p = 0.017. Conclusion:The insertion of Grommet provides a significant improvement in hearing in patients with secretory otitis media. <p class="abstract"><strong>Background:</strong> Both otitis media with effusion (OME) and copper or selenium have been associated to the immune system, hence it could be proposed that cooper and/or selenium deficiency predisposes children to OME through immune cells dysfunction. The aim of this study was to determine the serum levels of Copper and Selenium in an immundeficient-linked disease - OME – in childhood.</p><p class="abstract"><strong>Methods:</strong> This cross-sectional study was conducted between July 2015 and December 2016 including 80 patients age 1 to 8 years old who were divided into two groups: otitis media with effusion (OME) consisted of 40 patients and the control (n=40). Patients who did not recover after three months conservative treatments were included. Serum levels of Copper and Selenium were measured using graphite furnace atomic absorption spectrometry.  </p><p class="abstract"><strong>Results:</strong> Statistical analysis showed no significant differences regarding age and sex were noted between groups. Patients with OME had significantly lower serum copper levels than the control group (p=0.007). However, there was no statistically significant difference in serum selenium levels between groups. The analysis also showed a significant positive correlation between serum copper level and age (correlation coefficient=0.348, p=0.028), whereas, there was no correlation between the measured selenium level and age.</p><p class="abstract"><strong>Conclusions:</strong> The results of the present study showed that serum level of copper might play a role on development of otitis media with effusion in childhood.</p>

The presence of cyathostomin larvae is directly associated to climatic conditions of each region. This study aimed to evaluate the ecology of infective larvae on Brachiaria humidicola during the dry and rainy seasons from October 2007 to September 2008 in a tropical region, Rio de Janeiro state, southeastern Brazil. Stools were collected from the rectum of horses naturally infected with cyathostomins at the beginning of the rainy season (October to March) and dry season (April to September). They were divided into four samples of 500 g and deposited on a grass patch of B. humidicola. Seven days later and every 15 days thereafter samples of feces and grass were collected and processed by the Baermann technique. The mean number of larvae recovered from the grass varied according to the season, with greater recovery of larvae during the peak of the dry season (14,700 L3.kg-¹ DM). There was a statistically significant difference between L3 recovered from feces and grass, but not between L3 recovered from the grass base and apex. These results show that the region’s climate favors the development and survival of infective cyathostomin larvae throughout the year, with a greater number of larvae during the dry season.

This study presents measurements of the cross-sectional luminal area of the eustachian tube. Comparisons are made between the lumens of eustachian tubes obtained from temporal bones presenting acute or secretory otitis media and those from noninflamed temporal bones. The material consisted of 71 temporal bones obtained postmortem from individuals up to 2 years of age. Forty-six of these showed no middle ear inflammation, while 25 presented either acute or secretory otitis media. In both groups the lumens of all the eustachian tubes were patent, presenting no obstruction. The mean cross-sectional area of the lumens of inflamed temporal bones was smaller than that of the noninflamed ones. This difference was not found to be statistically significant in the cartilaginous regions and was found to be statistically significant or borderline significant in the bony parts of the eustachian tube.

Abstract BACKGROUND: Dengue, yellow fever, chikungunya and Zika are among the most important emerging infectious vector-borne diseases worldwide. In the Democratic Republic of Congo (DRC), increases in cases of dengue and outbreaks of yellow fever and chikungunya have been reported since 2010. The main vectors of these arboviruses, Aedes aegypti and Aedes albopictus, have been reported in DRC, but there is a lack of detailed information on their presence and spread to guide disease control efforts.METHODS: In 2018, two cross-sectional surveys were conducted in Kinshasa province (DRC), one in the rainy (January/February) and one in the dry season (July). Four hundred houses were visited in each of the four selected communes (N’Djili, Mont Ngafula, Lingwala and Kalamu). Within the peri-domestic area of each household, searches were conducted for larval habitats which were then surveyed for the presence of Aedes larvae and pupae. A subset of the immature specimens were reared to adults for morphological identification followed by DNA-barcoding of the specimens to validate identifications.RESULTS: The most rural commune (Mont Ngafula) had the highest pupal index of 246 (20) pupae/100 houses and a Breteau Index of 82.2 (19.5)/100 houses, while this latter was 21.5 (4.7), 36.7 (9.8) and 41.7 (7.5) in Kalamu, Lingwala and N’Djili in the rainy (and dry) season, respectively. The House Index was on average across all communes 27.5% (7.6%); and the Container Index 15.0% (10.0%) in rainy (and dry) season, respectively. The vast majority of Aedes positive containers were found outside the houses (adjusted OR 27.4 (95%CI 14.9-50.1)). During the dry season, the most productive containers were the ones used for water storage, whereas in the rainy season rubbish and tires constituted key habitats. Both Ae. aegypti and Ae. albopictus were found. Anopheles larvae were found in different types of Aedes larval habitats, especially during the rainy season. CONCLUSIONS: In both surveys and in all communes, the larval indices (BI) were higher than the arbovirus transmission threshold values established by the World Health Organization. Management strategies for controlling Aedes in Kinshasa need to target the key containers, mainly located in outdoor spaces, for larval habitats destruction or reduction.

Objective: This study set out to assess sex differences and seasonal variations in Pediatric Infectious Diseases (PID) admissions. Methods: One year retrospective study of PID admissions was conducted among children aged one month to 15 years. Relevant information retrieved from patients’ hospital records were dates of admission and discharge, age, sex, final PID diagnoses and outcome. Results: Of 1,035 patients’ records assessed, 603 (58.3%) were males and 432 (41.7%) females (p<0.001). Males in ages 1-12 and 13-59 months contributed largely to the gender difference. Over the 12 months period, PID admissions mean was 86.25±21.92, with rainy and dry seasons means of 92.57±20.7 and 77.4±20.9, respectively (p<0.001). Ages 13-19, 60-119 and ≥120 months had significantly higher admissions mean during the rainy season (p<0.001 in each group), while higher admissions occurred among ages 1-12months in dry season (p<0.001). Top six PID managed were malaria, gastroenteritis, pneumonia, septicemia, meningitis and typhoid, and males had higher incidence in all except for typhoid, with only malaria having significant difference (p<0.001). Malaria, septicemia and typhoid incidence were higher significantly during the rainy season, while gastroenteritis occurred more in the dry season. Overall fatality rate was 4.6%; females and males rates were 6.3% and 3.5%, respectively (p=0.661). Rainy and dry seasons mortality means were 5±1.77 and 2.6±0.8, respectively (p<0.001). Conclusion: Male children were more vulnerable to PID, with significant higher malaria incidence among them than females. PID incidence and mortality were significantly higher during the rainy season than dry. Malaria and gastroenteritis occurred significantly in rainy and dry seasons, respectively.

Aims: Malariometric indices are essential for the assessment of both new therapies and control strategies. As part of the characterization of a new malaria clinical trial site, this study was carried out to assess malariometric indices during the two seasons of a Sudanese area of Burkina Faso, in children aged under five years. Study Design: Two community-based cross-sectional surveys were conducted as follow: the first during the rainy season of 2009 and the second during the following dry season. Socio-demographic and clinical data were recorded. A finger prick blood sample was collected to perform malaria blood films and to measure the hemoglobin level. Results: Malaria parasitemia prevalence was 55.2% (N = 677) in the rainy season with a geometric mean of parasite density (GMPD) of 3439 trophozoites/µl against 23.3% (N = 720) in the dry season with a GMPD of 1368 trophozoites/µl. Gametocytemia prevalence was 21.7% and 6.5% respectively in rainy and dry season while splenomegaly prevalence was 11.2% (N = 689) in rainy season against 4.2% (N = 752) in dry season. The prevalence of anemia (hemoglobin < 11.0 g/dl) was 90.0% in rainy season and 70.6% in dry season. All indices in rainy season were statistically higher than those in dry season (p-value < 0.0001). The odds of parasite carriage were 3 to 5 times higher in rainy season compared to dry season (95% CI for OR = [3.1, 5.0]). Conclusion: The site is located in a seasonal hyper-endemic malaria area and seems appropriate for the conduct of malaria drugs or vaccines studies. Though the gap between seasons is considerable, the residual level of parasite carriage during low transmission period is not negligible and may command the development of strategies targeting this specific period, to break the chain of transmission of the disease.

Background Malaria prevalence in the highlands of Northern Tanzania is currently below 1% making this an elimination prone setting. As climate changes may facilitate increasing distribution of Anopheles mosquitoes in such settings, there is a need to monitor changes in risks of exposure to ensure that established control tools meet the required needs. This study explored the use of human antibodies against gambiae salivary gland protein 6 peptide 1 (gSG6-P1) as a biomarker of Anopheles exposure and assessed temporal exposure to mosquito bites in populations living in Lower Moshi, Northern Tanzania. Methods Three cross-sectional surveys were conducted in 2019: during the dry season in March, at the end of the rainy season in June and during the dry season in September. Blood samples were collected from enrolled participants and analysed for the presence of anti-gSG6-P1 IgG. Mosquitoes were sampled from 10% of the participants’ households, quantified and identified to species level. Possible associations between gSG6-P1 seroprevalence and participants’ characteristics were determined. Results The total number of Anopheles mosquitoes collected was highest during the rainy season (n = 1364) when compared to the two dry seasons (n = 360 and n = 1075, respectively). The gSG6-P1 seroprevalence increased from 18.8% during the dry season to 25.0% during the rainy season (χ2 = 2.66; p = 0.103) followed by a significant decline to 11.0% during the next dry season (χ2 = 12.56; p = 0.001). The largest number of mosquitoes were collected in one village (Oria), but the seroprevalence was significantly lower among the residents as compared to the rest of the villages (p = 0.039), explained by Oria having the highest number of participants owning and using bed nets. Both individual and household gSG6-P1 IgG levels had no correlation with numbers of Anopheles mosquitoes collected. Conclusion Anti-gSG6-P1 IgG is a potential tool in detecting and distinguishing temporal and spatial variations in exposure to Anopheles mosquito bites in settings of extremely low malaria transmission where entomological tools may be obsolete. However studies with larger sample size and extensive mosquito sampling are warranted to further explore the association between this serological marker and abundance of Anopheles mosquito.
