Hemi/Sphere: A Continuing Management Programme for Stroke Patients and Their Families

Hemi/Sphere is a new programme for the continuing care of stroke patients and their families following discharge from an acute hospital or rehabilitation centre. Located at The Wellesley Hospital in Toronto, this is a multi-disciplinary programme which stroke participants and family members may attend indefinitely. Prior to their admission to Hemi/Sphere the majority of participants had reached a plateau in their rehabilitation. Stabilization and maintenance were not being carried out and a resultant backslide and depression had occurred. The programme is described and a detailed case study is given. It is shown that this final stage of rehabilitation deserves further study and should become an essential part of the Health Care System. The article is based on a presentation given at the Seminar and Workshop on the Brain-Damaged Adult on April 22, 23, 1976, at Mount Sinai Hospital in Toronto.

The proportion of older people in the UK is increasing in relation to the population as a whole. This trend is reflected in the age profile of patients in hospital and those requiring continuing care in the community – domicilary, residential and nursing home sector. The HAS Report identified that more than 40% of patients over 65 with disability will require admission to an acute hospital over a two-year period. Ensuring appropriate, equitable and quality care for this age group is therefore a priority.

Introduction: Aphasia is understood as difficulty in understanding or producing language caused by disorders involving the brain hemispheres. Early assessment of aphasia is very important to prevent the emergence of telegraphic speech styles, improve welfare, independence, social participation, quality of life, reduce length of stay and care costs, but there is little literature on this subject, especially in stroke patients. Purpose: This study is to provide an overview of the assessment of aphasia in stroke patients. Methods: The design of a case study involving 6 participants and data collection was carried out by conducting an assessment using the Language Aphasia Screening Test (LAST) instrument which was monitored for 3 days, including monitoring errors for naming images, monitoring mismatches repeating words and sentences, monitoring spontaneous pronunciation, monitoring image comprehension, monitoring comprehension of verbal instructions.Results: A total of 6 ischemic stroke patients were found wrong in repeating words and sentences. Conclusion: In aphasic stroke patients all language modalities are impaired, ranging from spontaneous speech, repetition, naming, language comprehension, reading and writing

Abstract A stroke is an injury to the brain caused by a blockage obstruction of blood flow to the brain. Weakness in the limbs is often found in patients with strokes. Range Of Motion therapy may help to increase the muscle strength that is experencing weakness, avoiding complications from inactivity, such us contractions. This study aims to describe the effect of ROM Therapy in stroke patients with impaired physical mobility. The design of the scientific essay was a case study of two stroke clients who had msucle weakness. A research instrument is an obsevation sheet on the scale of the muscle strength. This case study was conducted for six days of ROM therapy with a frequency of twice a day.The case study result showed that two respondents experienced an increase in muscle strength after ROM therapy. This study concludes that ROM therapy may increase muscle strength among stroke patients. Stroke patient families are expected to assist and support the patient during Range Of Motion exercises.Keywords: Muscle weakness strength, Range Of Motion, Stroke. Abstrak Stroke merupakan keadaan cidera pada otak yang disebabkan sumbatan atau obstruksi aliran darah menuju otak. Kelemahan pada anggota gerak sering dijumpai pada pasien dengan stroke. Pemberian terapi Range Of Motionbermanfaat untuk meningkatkan kekuatan otot yang mengalami kelemahan, menghindari komplikasi akibat kurang gerak, seperti kontraktur. Penelitian ini bertujuan untuk menggambarkan penerapan terapi ROM pada pasien stroke dengan gangguan mobilitas fisik. Desain karya tulis ilmiah berupa studi kasus pada 2 klien stroke yang mengalami masalah kelemahan otot. Instrumen penelitian berupa lembar observasi mengenai skala kekuatan otot. Studi kasus ini dilakukan selama 6 hari dengan frekuensi 2 kali sehari dengan intervensi penerapan terapi Range Of Motion (ROM). Hasil studi kasus menunjukan masalah gangguan mobilitas fisik yang dialami kedua responden mengalami peningkatan kekuatan setelah dilakukan terapi ROM. Kesimpulan terapi Range Of Motion(ROM) dapat untuk mningkatkan kekuatan otot pada pasien stroke. Saran bagi keluarga diharapkan keluarga untuk selalu melakukan pendampingan saat dilakukaSaran bagi keluarga diharapkan keluarga untuk selalu melakukan pendampingan saat dilakukan implementasi agar dapat menjadi supporting sistem bagi pasien dan dapat mengerti sehingga dapat membantu klien dalam melakukan latihan rentang gerak otot.Kata kunci: Kelemahan kekuatan otot, Range Of Motion, Stroke.

Stroke is a leading cause of death and disability in both developed and developing countries. Limited information is available, in South Africa, on the epidemiological profile of stroke survivors requiring rehabilitation. A descriptive study was therefore undertaken to compile, amongst others the demographic and medical profile of stroke patients attending  rehabilitation at a primary health care facility (Bishop Lavis Rehabilitation Centre).  Both medical- and rehabilitation records were reviewed to ensure completeness of information and to minimize missing clinical data bias. Relevant data was  captured on a data capture sheet.  The latter was based on key findings from the literature and developed by the researcher. The mean age of the population of stroke patients studied was 59 years, which is markedly younger than what has been reported globally in the literature. As was expected, hypertension was found to be the most prevalent risk factor. The presence of a combination of risk factors in the  majority of the group (on average, 2.4 risk factors per person) warrants the introduction of aggressive health  education and stroke prevention programmes at this primary health care centre.

The development of the pharmaceutical industry is one of the priority tasks of our state, aimed at providing the population of the Russian Federation with modern safe and effective medicines. The solution to this problem is impossible without the formation of a highly qualified personnel potential that meets the demand and expectations of the pharmaceutical market and society as a whole. In this regard, in the system of training of pharmacists in recent years, quite dynamic and flexible transformations have been taking place, dictated by the urgent needs of domestic health care. It should be noted that in the process of implementing this educational standard, the competency-based approach to organizing the process of training modern pharmacists comes to the fore. One of the effective tools for the formation of professional competencies in various educational fields is the case study method. Case study is a training method based on the analysis of real situations from various areas of professional activity and contributing to the development of specialist competency. The competency-based orientation of the case study method is in line with modern ideas about the organization of the educational process for the training of pharmacists. The case study method is actively used in the process of teaching disciplines of the curriculum of the Federal State Budget Educational Establishment of Higher Education KubGMU of the Ministry of Health of Russia, specialty 33.05.01 Pharmacy. Examples of case study tasks as educational technology are presented in the work programs of the curriculum disciplines of the specialty 33.05.01 Pharmacy developed at the Department of Pharmacy. Depending on the content of the taught discipline, these tasks simulate a particular situation from the professional activities of pharmacists, offered to students for a comprehensive analysis and evaluation. The use of this educational technology contributes to the integration of knowledge, skills acquired in the learning process and their competency-based profiling in accordance with the current level of development of domestic health care.
