Home-based balance pilot intervention for adults with visual impairments

Older adults with visual impairments face many barriers to being physically active in their communities, which include their risk of suffering serious falls due to poor balance. Thus, the purpose of this study was to examine the effectiveness of a home-based intervention upon gait, balance, and well-being with older adults with visual impairments. A total of 17 adults with visual impairments above the age of 55 participated and were placed into either the experimental or the control group. All participants were assessed on well-being, balance, and gait. Experimental group participants attended balance workshops and received exercise equipment and a balance intervention manual. Following eight weeks, both groups were tested, and the experimental group was reassessed five months following the intervention. Four separate 2 × 2 mixed model analyses of variance (ANOVAs) were used to test for significant main effects and interactions related to their baseline and posttest scores for the four tests completed. Both groups improved their gait ( p = .006). Although not significant, the experimental group improved across the tests, and maintained their balance abilities scores 5 months after the intervention was delivered. A home-based exercise program can be an effective means for improving balance and gait in older adults with visual impairments.

6524 Background: In 2018, an estimated 266,120 women faced the challenge of living with breast cancer and approximately 40,920 died from their disease. Nearly 100% of these women experienced significant treatment related side effects that negatively impact quality of life (QOL). Exercise has repeatedly demonstrated to alleviate many of the side effects, improve QOL, and decrease cancer recurrence and mortality. In spite of this evidence, women do not maintain exercise during treatment for breast cancer. The purpose of this randomized controlled trial was to determine the effectiveness of exercising the day chemotherapy is administered on 1) the persistence with an exercise program, 2) side effects, and 3) QOL. Methods: Eligible women were randomly assigned to a control or experimental group. As part of their treatment plan, all participants were referred to one of 14 community-based exercise centers to exercise. Experimental group also exercised at one of two cancer centers each day chemotherapy was administered. Outcome measures include exercise retention and chemotherapy, completion; cancer-related fatigue, nausea/vomiting, peripheral neuropathy, weight gain, and QOL. Outcome measures were assessed prior to every second course of chemotherapy and 3- and 6-months post-chemotherapy. Results: 273 women with Stage I-III breast cancer receiving chemotherapy were enrolled in the study. The number of participants who withdrew from exercise was higher among those in the control group (n= 16; 12.4%) compared with those in the experimental group (n = 10; 6.9%). At cycle 5, those in the experimental group reported less motor peripheral neuropathy than those in the control group (p = .018) and higher physical well-being scores than those in the control group (p = .047). Conclusions: The highest impact of the intervention was on attrition from exercise. Since participants in both the control and experimental groups exercised throughout chemotherapy, it is not surprising that the groups performed comparably with regard to side effects and QOL. Given the positive effects exercise has demonstrated on persons receiving cancer treatment in numerous studies, having patients exercise in the cancer center on the day of chemotherapy is a significant step toward engaging persons receiving chemotherapy in an exercise program.

SummaryThe aim of the study was to determine the effect of regular participation in home-based exercise programme on cognitive functioning changes in institutionalised older adults. Two groups of participants were recruited for the study: experimental (n = 17) in mean age 76 ± 5.6 years, who participated in home-based exercise program and control (n = 14) in mean age 80 ± 4.2 years. The standardised Stroop Color-Word Test-Victoria version (VST) was used to measure the level of cognitive functions. Group differences were analyzed with Mann-Whitney U-test for independent samples and for differences between pre-measurements and post-measurements on experimental and control group we used non-parametric Wilcoxon Signed - Rank Test. The level of significance was α < 0.05. Application of 3-months home-based exercise program significantly improved the cognitive functions only in one (Word condition; p<0.01) from three VST conditions in institutionalised older adults. That’s why we recommend longer participation in home-based exercise program, at least 6- months, with combination of various types of cognitive interventions, like concepts of cognitive training, cognitive rehabilitation, and cognitive stimulation to improve cognitive functioning in older adults living in old peoples’ homes.

To assess the effects of a group resistance exercise program on prepubescent children, an experimental group of boys (n = 26) and girls (n = 24), with a mean age of 8.4 ± 0.5 years, participated in 12 weeks of school based training. The program consisted of upper body exercise using hand-held weights, stretch tubing, balls, and self-supported movements. A control group of boys (n = 30) and girls (n = 16), mean age 8.6 ± 0.5 years, had a free-play period. Boys were significantly stronger than girls on all initial strength evaluations and were taller and had lesser skinfold sums. ANCOVA was used to evaluate pre/post changes in cable tensiometer elbow flexion and extension, right and left handgrip strength, pull-ups, flexed arm hang, sit-ups, sit-and-reach flexibility, and body composition parameters. Following the training period, significantly greater gains were made by the experimental group for right handgrip, flexed arm hang, pull-ups, and flexibility. Greater decreases in sum of skinfolds were also found. Training responses of boys and girls were similar. It was concluded that a group strength training program can be an effective means of increasing fitness levels and improving body composition in both boys and girls of this age.

Leprosy, also known as Hansen’s disease is caused by Mycobacterium Leprae. Despite being curable, it continues to be a significant health problem in many parts of the world. The prevalence of psychiatric disorders is higher in the leprosy affected population than in the general population. Physical activity has been associated with reduced symptoms of depression and anxiety. It is also associated with improved life satisfaction and psychological well-being. Mental Imagery holds belief as an intervention in the treatment of psychological disorders. This is attributable to its harmless, time and cost effective nature. The study aimed to assess the effect of mental imagery on Depression, Anxiety and Stress in institutionalised Leprosy patients using the DASS-21 (Depression, Anxiety and Stress Scale -21). It was carried out in 34 subjects divided equally in the control and experimental groups. The control group received an aerobic exercise program whereas the experimental group received an aerobic exercise program with mental imagery. It was thus concluded that a statistically significant difference was obtained in the scores of Depression in the experimental group. However, only clinically significant differences could be obtained in the scores of Anxiety and Stress. Thus, mental imagery can be used as an effective adjunct with conventional aerobic exercises for reducing Depression, Anxiety and Stress in institutionalised Leprosy patients.

The purpose of this study was to define if Outcomes Expectations for Exercise (OEE) and Decisional Balance (DB) scales predict adherence to guided exercise programs and associate with the improvement in physical performance in the dialysis population. Participants (n = 40; age 63.6 ± 12.5 years) completed OEE and DB questionnaires before randomization to the experimental group (n = 20) and control group (n = 20) of a two-phased exercise program—the experimental group received eight weeks of supervised functional exercise and exercise counseling (1st phase) before commencing eight weeks of home-based exercise on non-dialysis days (2nd phase). Both groups performed intradialytic cycling on dialysis days during both study phases. Patients with above-median OEE and DB scores (>3.15 and >1.3, respectively) expressed significantly better adherence to intradialytic cycling (89% vs. 76%, 89% vs. 77%, respectively, p < 0.05). Experimental group patients with an above-median OEE (but not DB) score had significantly better adherence to supervised and home-based functional exercise (93% vs. 81% and 85% vs. 60%, respectively, p < 0.05). Baseline DB score predicted the final result in the hand-grip test and 6-min walk test. Low OEE and, to a lesser degree, low DB questionnaire scores associate with inferior adherence to dialysis bundled and home-based exercise programs and may help define patient subsets in need of intensified motivational input by exercise caregivers.

The efficacy of exercise to reverse frailty in the aging population has not been extensively investigated. This study aimed to investigate the effectiveness of a multicomponent exercise program (MCEP) on frailty, physical performance (handgrip strength, Berg Balance Scale (BBS), Timed Up and Go test (TUG), and VO2Max), blood biomarkers (Interleukin-6 (IL-6) and C-reactive protein (CRP)) in frail older adults. A randomized controlled trial using an allocation concealment method, included 64 older adults (77.78 ± 7.24 years), were divided into two parallel groups using block randomization: an MCEP group (n = 32) and a control group (n = 32). The combined center- and home-based MCEP training consisted of chair aerobic, resistance, and balance, which was carried out 3 days per week for 24 weeks. A mixed model repeated measure ANOVA demonstrated significant interaction effects of group x time for BBS, TUG and frailty scores (p < 0.001). Additionally, the post-hoc analysis revealed that the MCEP group showed significantly improved BBS, TUG, and frailty scores (p < 0.01), at both 12- and 24-weeks. When compared with controls at 12-weeks, the MCEP group decreased IL-6 and CRP levels (p < 0.05). The combined center- and home-based MCEP were effective in reversing frailty to pre-frailty and improving physical performance especially balance in the older population.

Abstract This session will present the findings of a randomized control trial evaluating the impact Age-Tastic has on behavioral change. Age-Tastic is an eight-week intervention that uses game play, group facilitated discussion, and at home activities to promote positive behavioral change. There are five behavioral change domains: emotional well-being, nutrition, financial exploitation, falls prevention, and health literacy. A randomized control trial was conducted in New York City senior centers and include a diverse group of older adults. There were 98 older adults assigned to an experimental or control group. Participants in both groups completed a baseline survey, which was repeated at the end of the intervention and at week sixteen. The experimental (n = 64) and control (n = 34) groups did not significantly differ at baseline in regard to behaviors; however, upon completion of the intervention, the experimental group had significantly changed health behaviors (p &lt; .05) compared to the control group. The behavior change reported by the experimental group was maintained at week sixteen (p &lt; .001) with a medium effect size (ε2 = .17). In addition the experimental group also reported significant improvements at weeks eight (p &lt; .001) and sixteen (p &lt; .001) with large effect sizes (r2 = .62 and .52, respectively). This study found that Age-Tastic is an effective intervention for engaging older adults holistically about their health and wellness and for promoting positive behavioral change among diverse racial and ethnic populations.

The purpose of this study was to examine the feasibility and acceptability of remotely delivered, home-based exercise programs on physical function and well-being in self-isolating older adults during the COVID-19 pandemic. In a four-arm randomized controlled trial, 63 participants (aged 65 years and older) were allocated to one of three home-based daily (2 × 10-min) exercise interventions (exercise snacking, tai chi snacking, and combination) or control (UK National Health Service Web pages). Functional assessments were conducted via video call at baseline and 4-week follow-up. A web-based survey assessed the acceptability of each exercise program and secondary psychological/well-being outcomes. Ecological momentary assessment data, collected in Weeks 1 and 4, explored feeling states as antecedents and consequences of exercise. All intervention groups saw increased physical function at follow-up and displayed good adherence with exercise snacking considered the most acceptable program. Multilevel models revealed reciprocal associations between feelings of energy and exercise engagement. Further studies are needed with larger, more diverse demographic samples.

A recently completed randomized controlled trial documented the effects of a 1-year group-based exercise program in osteopenic women. The present study concerns the comparison of these effects to those produced by a home-based exercise program tested on the same population. All 63 women who had been randomly assigned to the control group in the previous study were invited to enroll in the home program. Comparison of pre- and posttest scores of home exercisers revealed improvements in agility and well-being. In comparison, women who had participated 1 year earlier in the group-based exercise program had improved on four of the five fitness tests, well-being, and pain intensity. More women in the group-based exercise program showed improvement in self-rated health in comparison to those enrolled in the home program. Results suggest that for osteopenic women, a group-based exercise program is much more effective than a home-based exercise program.

Background : The geriatric population is defined as population aged 60 years and above. (1)During this age many of the systems undergo deterioration. Balance problems in elderly are most commonly due to multi factorial condition which may include age related or disease-related declines in the balance system. Research shows that altered balance is the greatest collaborator towards falls in the elderly with a high correlation between balance deficit and the incidence of falls. Iyengar yoga, one of the active, or Hatha, yoga techniques, is a system for developing physical and mental well-being through stretching of all muscle groups for strength, flexibility, and physical balance. Yoga as a complementary therapy is thought to be more therapeutic than traditional exercise because it involves active engagement between mind and body. Its practice has been associated with increased muscle strength, endurance, flexibility, range of motion and cardiopulmonary endurance. It mainly works on increasing body awareness and proprioception, which will lead to improvement of balance in older adults. Objectives: To compare pre and post score of Modified Clinical Test of Sensory Interaction in Balance (CTSIB-M). To compare pre and post score of Time up and go test. Materials and Methodology: This was an experimental study that included 40 participants, healthy older adults between the age group 60-75 years, both male and female. The subjects were randomly divided in to 2 groups; Group A was the experimental group whereas group B was the control group. The experimental group was made to perform yogasanas for the duration of four weeks for 5 times a week whereas the control group was not given any intervention. Result and Conclusion: The study conducted concludes that yogasanas are effective in improving balance in elderly individuals at the end of four weeks compared to control group. Thus, it can be used clinically to improve balance in geriatric population.
