Non-traumatic ligamentum teres tears: association with MRI morphometry of the hip

Background The ligamentum teres has been recognized as an important stabilizer of the hip. Purpose We aimed to examine the relationship between non-traumatic ligamentum teres (LT) tear and hip morphometry on magnetic resonance imaging (MRI). Material and Methods Fifty patients who had undergone hip MRI were included (27 men, 23 women; average age = 54.0 years). The status of the LT and the morphometric hip parameters were assessed, including acetabular anteversion angle (AAA), acetabular depth (AD), acetabular index (AI), lateral center edge angle (LCEA), and extrusion index (EI). The morphometric hip parameters were compared between groups with one-way ANOVA, Student’s t-test, and Mann–Whitney U test. Results A decreased acetabular coverage was noted in the severe tear group compared to the normal group, indicated by a significantly larger AD ( P = 0.001) and smaller LCEA ( P = 0.016). There was a statistically significant difference in the AAA, AD, and LCEA between the normal group and the complete tear group; the AAA was significantly larger ( P = 0.031), the AD was significantly larger ( P = 0.01), and the LCEA was significantly smaller ( P = 0.043) in the complete tear group compared to the normal group. Conclusion There is an association between LT tears and acetabular bony morphology; an insufficient acetabular coverage is associated with complete tear of the LT. As the insufficient acetabular coverage may predispose to ligamentum teres tear, the ligamentum teres should be thoroughly evaluated in those with insufficient acetabular coverage, as a potential cause of hip pain.

Objectives: (1) To report minimum five-year patient reported outcomes (PROs) in borderline dysplastic patients with ligamentum teres (LT) tears who underwent hip arthroscopy and (2) to compare these PROs to a pair-matched control group of borderline dysplastic hips without LT tears. Methods: Data was prospectively collected for patients who underwent hip arthroscopy during September 2008 and August 2013. Patients were included if had a preoperative diagnosis of borderline dysplasia [lateral center-edge angle (LCEA) of 18°- 25°] and had preoperative and minimum five-year postoperative modified Harris Hip Scores (mHHS), Non-Arthritic Hip Scores (NAHS), and Visual Analogue Scale for pain (VAS) scores. Exclusion criteria were Tönnis grade osteoarthritis >1, previous hip conditions, any prior ipsilateral hip surgery, or workers’ compensation status. Two borderline dysplastic groups were created. An LT tear group was matched 1:1 to a control group (no LT tear) with similar age, sex, body mass index (BMI), and laterality via propensity score matching. Significance was set at P < 0.05. Results: 24 LT tear patients (24 hips) were matched 24 patients (24 hips) without an LT tear. There was no significant difference in age, sex, BMI, or laterality between groups. Mean age was 36.2 ± 17.2 and 34.9 ± 15.9 years for the control and LT tear group, respectively (P = 0.783). There were 17 (70.8%) and 16 (66.7%) females in the control and LT tear group, respectively and the mean preoperative LCEA was 23.3° and 22.2° in the control and LT tear group, respectively. No differences were observed between groups in baseline PROs, intraoperative findings, or surgical procedures, except for LT treatment. Five-year postoperative PROs were comparable in both groups, with the control group exhibiting superior Veterans RAND 12-Item Health Survey Mental Component (VR-12M) (P = 0.041) and Short Format 12 Mental Health (SF-12M) (P = 0.042). The control group exhibited less pain and higher satisfaction with surgery; however, this trend was not significant. Finally, the LT tear group was significantly less likely to achieve the patient acceptable symptomatic state (PASS) for mHHS (P = 0.022). Conclusion: Following hip arthroscopy, patients with borderline dysplasia and LT tears demonstrated favorable PROs at minimum five-year follow-up. Outcomes were similar to a pair-matched control group without LT tears, with the control group showing higher VR-12M and SF-12M scores. Furthermore, borderline dysplastic patients with LT tears were significantly less likely to achieve the PASS for mHHS.

This study was conducted in the Khulna metropolitan city to determine the status of noise pollution at major traffic intersections and assess the possible effect of noise on regular noise afflicted people. For this research, five busiest roadways and traffic intersections; two mixed zone and three commercial zones were selected purposively and 'ST-8850 Sound Level Meter' was used to record the noise pressure level. Average noise pressure level and some noise parameters (Leq, L10, L50, L90, and Lnp) were measured at different shifts in each location both on working days and holidays. The result evidently specified that noise level in all major traffic circles of Khulna metropolitan city was much higher than the recommended threshold of acceptable noise level set by the Department of Environment (DOE) of Bangladesh. It was observed that the mixed area (Sonadanga, Gollamari) tends to have a higher average noise level than those of the commercial area (Shibbari, Notunrasta, Dakbangla). The noise pollution level (Lnp) in mixed areas varied from 92.77 dB (A) to 104.74 dB(A) on working days and 86.9 dB(A) to 105.5 dB(A) on holidays. On working days, the highest Leq was observed 92.65 ± 4.15 dB(A) in Sonadanga (mixed area), whereas the lowest was 85.13 ± 2.83 dB(A) in Dakbangla (commercial area). However, on holidays the highest level of Leq was found 90.41 ± 2.25 dB(A) in Sonadanga, while the lowest levels were 80.43 ± 3.20 dB(A) in Dakbangla. One-way ANOVA with LSD post-hoc test results illustrated that on working days there was a significant difference between the noise pollution levels (NPL) in different shifts of day time (F = 8.412, p < .05). The respondents of the study area addressed that they were the most affected by annoyance (70%), headache (62%), and hearing loss (46%) due to regular exposure to the traffic noise. Therefore, it is recommended to enhance awareness regarding the cause and effect of noise pollution among the noise producers and oppressed people. Subsequently, administrative measures have to take to attenuate noise pollution and indemnify the quality health of the citizens.

Abstract Purpose To compare the relative IPSS (International Prostate Symptom Score) improvement in storage and voiding symptoms between prostatic artery embolization (PAE) and transurethral resection of the prostate (TURP). Method Retrospective analysis of the UK-ROPE (UK Register of Prostate Embolization) multicentre database was conducted with inclusion of all patients with full IPSS questionnaire score data. The voiding and storage subscore improvement was compared between interventions. Student’s t-test (paired and unpaired) and ANOVA (Analysis of variance) were used to identify significant differences between the groups. Results 146 patients (121 PAE, 25 TURP) were included in the analysis. Storage symptoms were more frequently the most severe symptom (‘storage’ in 75 patients vs ‘voiding’ in 17 patients). Between groups, no significant difference was seen in raw storage subscore improvement (TURP 4.9 vs PAE 4.2; p = 0.34) or voiding subscore improvement (TURP 8.4 vs PAE 6.7; p = 0.1). ANOVA demonstrated a greater proportionate reduction (relative to total IPSS) towards voiding symptoms in the TURP group (27.3% TURP vs 9.9% PAE, p = 0.001). Conclusion Although both TURP and PAE improve voiding symptoms more than storage, a significantly larger proportion of total symptom reduction is due to voiding in the TURP cohort, with PAE providing a more balanced improvement between voiding and storage.

Introduction: The basic determinant of healthy behaviour—among other human behaviours—is the fact that it consistently affects health. Nowadays, health behaviour studies are considered to be an important method of measuring the health of a population. Objective: To assess the health behaviours and value-based health analysis of people aged 50+ who were hospitalized due to cardiovascular disease, depending on the selected descriptive variables. Materials and methods: The study was conducted between April 2018 and December 2018 among 411 subjects aged 50+ who were hospitalized due to cardiovascular disease at the Independent Public Health Care Unit in Sanok (Podkarpackie voivodship in Poland). The method used in the study was a diagnostic survey. The study used the authors’ survey questionnaire and two standardized tests: Inventory of Health-Related Behaviour (IHB) and List of Health Criteria (LHC). A statistical analysis was carried out in the R program, version 3.5.1. The obtained results were subjected to thorough statistical analysis using the following tests: Student’s t, Mann–Whitney U, ANOVA, Kruskal–Wallis, Fisher’s Least Significant Difference (LSD), Pearson, and Spearman. Results: The strongest correlation between health status and health behaviours (according to the IHB questionnaire) was in the area of ‘health practices’, while the lowest correlation was found in the areas of ‘correct eating habits’ and ‘preventive behaviours’. Based on the LHC questionnaire, the most important health criteria according to the subjects were ‘not feeling any physical ailments’; ‘having all body parts functional’; ‘feeling well’; ‘eating properly’; and ‘infrequent need of going to the doctor’. A positive correlation was found in the group of respondents where the ‘preventive health behaviours’ were more intense; herein, the more important criterion for the respondents was ‘eating properly’. Conclusions: Respondents aged 50+ and hospitalized for cardiovascular diseases indicated (based on the IHB questionnaire) that health behaviours in the area of ‘health practices’ had the strongest correlation with their health, while the lowest correlation was found in the areas of ‘correct eating habits’ and ‘preventive behaviours’. According to the respondents, the most important criteria determining health (according to the LHC questionnaire) included ’not feeling any physical ailments’; ‘having all body parts functional’; ‘feeling well’; ‘eating properly’; and ‘infrequent need of going to the doctor’. Based on the information collected from the respondents, it was found that the most important criteria determining health depended on selected descriptive variables, such as age, gender, place of residence, education, and marital status.

The effects of human disturbance represent one of the major threats for wildlife conservation. Many studies have shown that wildlife avoids or reduces direct contact with human activities through changes in activity patterns, and by minimizing spatiotemporal overlap. In this study, we investigated the possible effects of human presence on the temporal activity of medium-to-large mammals using two areas in Myanmar that differ in the intensity of human disturbance. We monitored temporal segregation mechanisms using camera trapping data and with two statistical approaches: daily activity overlaps between humans and wildlife and circular statistics. We did not find a significant difference in overlapping activity between areas but, thanks to circular statistics, we found that some species show changes in activity patterns, suggesting temporal avoidance. We observed that the daily activity of five species differed between areas of Myanmar, likely adopting mechanisms to reduce overlap in areas highly frequented by humans. Interestingly, these species are all threatened by hunting or poaching activities, four of which have been described in literature as “cathemeral”, or species that are active through day and night. This study suggests that some species adapt their behavior, at least partially, to avoid human presence in habitats with higher anthropic occurrence and increase our knowledge on the status of medium–large mammals in a poorly studied country as Myanmar.

Objective: To analyze 68-Ga prostate-specific membrane antigen (PSMA) uptake pattern of the prostate and its correlation with prostate-specific antigen (PSA), digital rectal examination (DRE), and Gleason’s score in the diagnosis of carcinoma of the prostate (CaP). Methods: This was a retrospective study conducted between June 2015 and August 2017. Patients who had undergone whole body 68-Ga PSMA HBED-CC simultaneous positron emission tomography (PET) or magnetic resonance imaging (MRI) for the diagnosis or staging of CaP were eligible. Patients who presented with persistently raised serum PSA (>4 ng/mL) and normal urine routine and negative culture were included in the study. Results: A total of 74 patients were included in the study. Significant positive correlation was observed between PSMA delayed uptake with the Prostate Imaging Reporting and Data System (PI-RADS) score ( p<0.001, ρ=0.750), PSA level ( p<0.001, ρ=0.414), DRE ( p<0.002, ρ=0.400), and Gleason’s score ( p<0.300, ρ=0.02). There was a significant difference between early and delayed phase of PSMA uptake in malignant prostatic lesions ( p<0.001). Delayed phase of PSMA uptake was able to characterize prostate lesions with an area under curve (AUC) of 0.91. Combined receiver operating characteristic analysis of PI-RADS score derived from multiparametric MRI and differential PSMA uptake to characterize prostatic lesions improved AUC to 0.94. Conclusion: Results demonstrated that the correlation with clinicopathological features (PSA, DRE, and Gleason’s score) could be used in prognostication of prostatic lesion along with PSMA PET/MRI.

Introduction: Borderline acetabular dysplasia is classically defined as a lateral center edge angle (LCEA) of 20-25 degrees. The optimal treatment strategy in this patient group remains controversial, with some patients having primarily hip instability-based symptoms, while others have primarily impingement-based symptoms (non-instability). The purpose of the current study was to define the 3D characteristics on low-dose CT that differentiate patients with instability symptoms from those without instability in the setting of borderline acetabular dysplasia. Methods: Seventy consecutive hips with borderline acetabular dysplasia undergoing surgical treatment were included in the current study. All patients underwent low-dose pelvic CT with femoral version assessment for preoperative planning. CT measurements included alpha angle and radial acetabular coverage (RAC) at standardized clockface positions (9:00-posterior to 3:00-anterior), central and cranial acetabular version. RAC was assessed in three sectors (anterior, superior, and posterior) and defined (relative to published normative data) as normal (-1 SD, +1 SD), undercoverage (<-1 SD), or overcoverage (>+1 SD). Statistical analysis was performed to compare the CT characteristics of the symptomatic instability and non-instability groups. Results: Of the 70 hips, 62.9% had the diagnosis of symptomatic instability, while 37.1% had no instability symptoms. Hips with instability (compared to non-instability) had significantly lower alpha angle (maximal difference at 1:00 - 47.0° vs. 59.4°), increased femoral version (22.3° vs. 15.3°), and decreased radial acetabular coverage (maximal difference at 1:00 – 59.9% vs. 62.2%) (all p<0.001). Multivariate analysis identified femoral version (OR 1.1, p=0.02), alpha angle at 1:00 (OR 0.91, p=0.02), and RAC at 1:00 (OR 0.46, p=0.003) as independent predictors of the presence of instability. The model combining these three factors had excellent predictive probability with a c-statistic 0.92. Conclusion: We found significant differences in the 3D hip morphology of the symptomatic instability and non-instability subgroups within the borderline dysplasia cohort. In the setting of borderline dysplasia, three-dimensional deformity characterization with low-dose CT allowed for differentiation of patients diagnosed with underlying instability vs. non-instability. Femoral version, alpha angle at 1:00, and radial acetabular coverage at 1:00 were identified as independent predictors of diagnosis in borderline acetabular dysplasia. Summary: This study attempts to define 3D CT characteristics to help distinguish between patients with impingement-based vs instability-based symptoms of borderline acetabular dysplasia.

Objective: Metabolic syndrome is a cluster of disease condition characterized by truncal obesity, hypertriglyceridemia, elevated blood pressure, and insulin resistance. An excessive circulating uric acid (UA) level even within normal range is always comorbid with metabolic syndrome and its components. The aim of the current study was to investigate the association between metabolic syndrome and serum UA level.Methods: A total of 60 subjects were divided into two groups of healthy (30 individuals) and metabolic syndrome patients (30 individuals) from dental outpatient department of Saveetha Dental College and Hospitals. 5 ml of fasting venous blood was collected in the plain collection tubes and centrifuged, and then serum was separated. Then, the serum was used to analyze the fasting blood glucose, serum triglycerides (TGLs), and serum UA by GOD-POD, enzymatic colorimetric, and uricase method, respectively. A statistical analysis was performed using Student’s t-test. p<0.05 was considered to be statistically significant.Result: Mean body mass index (BMI), fasting blood sugar (FBS), TGL, and UA level of control group were 23.36±1.81, 84.45±13.1, 110.9±22.6, and 3.48±1.21 respectively. Mean BMI, FBS, TGL, and UA level of study group were 35.24±3.04, 122.85±23.3, 212.1±39.6 and 9.08±2.63 respectively. There is a significant difference between these two groups with p<0.0001.Conclusion: This study showed that those individuals with metabolic syndrome have higher UA level that indicates hyperuricemia which is a significant predictor of metabolic syndrome.

Introduction Atrial fibrillation (AF) recurrence after ablation is associated with left atrial (LA) fibrosis on late gadolinium enhanced (LGE) magnetic resonance imaging (MRI). We sought to determine pre-ablation, clinical characteristics that associate with the extent of LA fibrosis in patients undergoing catheter ablation for AF. Methods and Results Consecutive patients presenting for catheter ablation of AF were enrolled and underwent LGE-MRI prior to initial AF ablation. The extent of fibrosis as a percentage of total LA myocardium was calculated in all patients prior to ablation. The cohort was divided into quartiles based on the percentage of fibrosis. Of 60 patients enrolled in the cohort, 13 had <5% fibrosis (Group 1), 15 had 5-7% fibrosis (Group 2), 17 had 8-13% fibrosis (Group 3), and 15 had 14-36% fibrosis (Group 4). The extent of LA fibrosis was positively associated with time in continuous AF, and the presence of persistent or longstanding persistent AF. However, no statistically significant difference was observed in the presence of comorbid conditions, age, BMI, LA volume, or family history of AF among the four groups. After adjusting for diabetes and hypertension in a multivariable linear regression model, paroxysmal AF remained independently and negatively associated with the extent of fibrosis (-4.0 ± 1.8, P = 0.034). Conclusion The extent of LA fibrosis in patients undergoing AF ablation is associated with AF type and time in continuous AF. Our results suggest that the presence and duration of AF are primary determinants of increased atrial LGE.

ABSTRACT The objective of this study was to investigate the effect of the ketoprofen on the preemptive analgesia in female cats submitted to ovariohysterectomy. Sixteen healthy cats were distributed into two groups, with eight animals each, by means of a draw, in a preemptive group (PREG), in which the animal received ketoprofen (1 mg/kg) subcutaneously two hours before the surgery; and postsurgery group (POSG), in which ketoprofen (1 mg/kg) was administered subcutaneously immediately after surgery. In both groups, ketoprofen was given 24, 48 and 72 hours after the first adminstration. Heart and respiratory frequencies and glycemia were measured in all animals during the days first ten postsurgery. Analgesia was measured by assigning scores at zero, two, four, six, 12, 24, 48, 72, 96, 120, 144, 168, 192, 216 and 240 hours after the surgical procedure. Statistical analysis was performed using two-way analysis of variance followed by Tukey's test to compare the moments and Student's t-test for comparison between groups. Pain scores were evaluated using test of U-Mann-Withney or Kruskall-Wallis, all at the 5% level of significance. Pain scores were significantly lower in PREG in M0, M6, M72, M96 and M120. No significant difference was found in the levels of glycemia in comparison to baseline values. Ketoprofen promotes postsurgery analgesia in female cats submitted to ovariohysterectomy and preemptive and postsurgery administration provides an earlier reduction of pain scores when compared to postoperative administration, only.
