The effects of age at cochlear implantation on balance in children: A pilot study

Aim: The evaluation of the effects of age at cochlear implantation on balance in children. Research design: Cross-sectional study. Study sample: Twenty children who received their cochlear implants (CI) before the chronologic age of 48 months (Early CI Group), and 20 children who received their CI at 48 months chronologic age or later (Late CI Group). Intervention: All children underwent Tandem Romberg (TR) test, Single-Leg stance (SLS) test, Pediatric Clinical Test of Sensory Interaction for Balance (P-CTSIB), Pediatric Balance scale (PBS) and Timed Up and Go (TUG) test. Data collection and analysis: The scores of TR, SLS and P-CTSIB and TUG tests and PBS were recorded. Mann Whitney U test and Independent-samples t-test were used to compare data between groups. Results: TR (eyes opened-EO), SLS (EO), PBS, and TUG scores and incidence in dizziness symptoms did not show significant differences between the groups ( p > 0.05). Duration of TR and SLS tests (eyes closed-EC), P-CTSIB-positions 5, and 6 were significantly longer in the Early CI Group than the Late implanted group( p < 0.05). Conclusion: Although the age of cochlear implantation did not affect functional balance for children, it has been found to cause impaired balance performance in difficult static conditions and increased frequency of dizziness.

Background and Objectives: Balance problems are the main cause of falls and fall-related complications in the elderly. The objective of this study was to investigate the balance in old people residing in nursing homes using clinical assessments. Methods: A total of 54 people participated in this cross-sectional study. Timed Up and Go (TUG) was used to assess balance regarding mobility impairments. The modified Clinical Test Of Sensory Interaction on Balance (CTSIB) was also used to assess balance regarding sensory problems. This test was used in 4 conditions: standing with eyes open on a firm surface, standing with eyes closed on a firm surface, standing with eyes open on a compliant foam, and standing with eyes closed on a compliant foam. Data analysis was done by SPSS V. 18 software. Results: The mean time for the completion of TUG was significantly higher than standard scores in all age groups (P<0.05). Postural sway during standing with eyes closed on the ground, eyes open and eyes closed on the foam was more than standing with eyes open on the ground (P<0.001). Postural sway in standing with eyes closed on the foam was more in comparison with standing with eyes closed on the ground and eyes open on the foam (P<0.001). Conclusion: Elderly people living in nursing homes had poorer motor skills to maintain balance. They also showed greater dependence on visual and sensory systems to maintain balance.

OBJECTIVES: To characterize balance and mobility among diabetic elderly outpatients and to estimate the extent to which functional balance and mobility abnormalities can be influenced by sociodemographic, clinical and other functional factors in a cross-sectional study. METHODS: Ninety-one elderly (65+ years) outpatients were assessed. Mobility was evaluated by the Timed Up and Go Test (TUGT) and the balance, by the Berg Balance Scale (BS). RESULTS: TUGT mean score was 15.65 ± 5.9 seconds and BS mean score was 49.31 ± 7.3 points. Using linear regression analysis (α < 0.05), significant and independent positive relationships were obtained between TUGT and age, daily activities (ADL/IADL), step strategy, and proprioceptive sensitivity. Factors negatively associated with BS were: ADL/IADL, step strategy, proprioceptive sensitivity, orthostatic hypotension (OH) and conflictive sensory conditions. CONCLUSION: Elderly diabetic outpatients show abnormal balance and mobility related mainly to advanced age, disability, absence of step strategy, absence of proprioceptive sensitivity and presence of OH.

Objective. To evaluate fear of falling, number of falls, and balance performance in women with FM and to examine the relationship between these variables and others, such as balance performance, quality of life, age, pain, and impact of fibromyalgia.Methods. A total of 240 women participated in this cross-sectional study. Of these, 125 had fibromyalgia. Several variables were assessed: age, fear of falling from 0 to 100, number of falls, body composition, balance performance, lower limb strength, health-related quality of life, and impact of fibromyalgia.Results. Women with fibromyalgia reported more falls and more fear of falling. Fear of falling was associated with number of falls in the last year, stiffness, perceived balance problems, impact of FM, and HRQoL whereas the number of falls was related to fear of falling, balance performance with eyes closed, pain, tenderness to touch level, anxiety, self-reported balance problems, impact of FM, and HRQoL.Conclusion. FM has an impact on fear of falling, balance performance, and number of falls. Perceived balance problems seem to be more closely associated with fear of falling than objective balance performance.

Aim: to determine if dynamic balance is related to activities of daily living (ADL) dependence in stroke patients. Methods: a cross-sectional study was carried out. The Barthel index (BI) was used to assess ADL dependence. The Timed Up and Go Test (TUG) and the Four Square Step Test (FSST) were used to evaluate dynamic balance. A Spearman correlation was used assuming a data abnormality in the performance of TUG and FSST and considering a significance of p<0.05. Results: 62 stroke patients were evaluated. The BI showed correlations with the performance of the TUG and the FSST. These correlations were also found between performance of both balance tests and ADLs, such as bathing, moving, and going up and down stairs. In contrast to TUG, correlations were estimated between the ADL related to dressing and the performance of the FSST. Conclusion: dynamic balance evaluated by the TUG and the FSST showed correlations with the ADL dependency in stroke patients.

Background: There are numerous studies assessing the morphological structure of the foot, but there is a notable scarcity of those focused on juxtaposing various longitudinal arch indices with foot loading paradigm. The present study aimed to determine the overall reliability, diagnostic accuracy of respective variables, and their correlation with the foot loading paradigm. Methods: The study group consisted of 336 children, aged 10–15 years (girls 49.1% and boys 50.9%). The morphological structure of the plantar part of the foot in static conditions was assessed with the aid of a 2D podoscan. Individual foot loading paradigm in static conditions was assessed making use of the FreeMed platform. Results: Staheli (SI), Chippaux–Smirak (CSI), and Sztriter–Godunow (KY) indices were strongly correlated with each other (ρ > 0.84, p < 0.001). Own research corroborated an increased pressure of hollow feet, as assessed by the SI, CSI, and KY indices, on the forefoot and the hindfoot, foot zones B, E, F; these correlations being statistically significant. The results yielded by the present study also indicate an increased pressure on the metatarsal, and foot zones C, D of the flat feet. Conclusions: Flatfootedness is not believed to be a common deformity among children and adolescents. The SI, CSI, and KY indices were found to be strongly correlated, as well as proved reliable in assessing the foot’s longitudinal arch.

Introduction: Most studies on postural deviations during single and dual tasks have been extensively studied in neuromuscular and older adult populations. Nevertheless, further research is warranted to identify whether such tasks can impose postural adaptations in young, healthy adults without sensory impairments. Aim: To assess postural stability modifications in young adults during single tasks and dual motor tasks (holding a cup filled with water) while concomitantly challenging the sensory systems. Materials and Methods: This was the cross-sectional study on 82 young adults (18-45 years old) from Texas Woman’s University (TWU) Health Science Center in Dallas, Texas, and surrounding areas. Standing postural control was measured by collecting total sway, direction of sway and velocity in the Anterior-Posterior (AP) and Medial-Lateral (ML) directions during different balance tasks. For single and dual tasks, the tests were performed with a bipedal stance on foam involving challenging the sensory input via Eyes Open (EO), Eyes Closed (EC), and head movements with eyes open (EO HUD) and closed (EC HUD). The dual motor tasks were similar to the single tasks with the addition of holding a cup full of water to split attention. Data were placed into the Statistical Package for Social Sciences (SPSS) Data Analysis 25.0 system and were analysed for repeated measures Analysis of Variance (ANOVA) analysis. Results: Eighty-two healthy young adults participated in this study (mean age of 24.6±2.7 years, 13 males and 69 females). An ANOVA analysis revealed that postural stability was considerably altered during motor tasks. Sway in the Antero-Posterior (AP) direction, and velocity of sway increased as the complexity of the tasks intensified. A substantial difference in total sway during single tasks when eyes were closed compared to eyes open (p-value <0.01) was noted. There was a significant difference in total sway (AP and ML) during eyes open (EOM) to eyes closed (ECM) and during eyes open with head moving up and down (EOM HUD) (p-value 0.001). There were significant differences in mean AP velocity during EO (0.11±0.12) compared to EC HUD (0.19±0.15), and when comparing EOM (0.07±0.04) to ECM HUD (0.13±0.08) (p=0.01) Conclusion: This study identified postural changes when comparing single and dual tasks in healthy young adults, and the outcomes of this study showed definite distinctions in postural responses during single and dual motor tasks.

The study investigated the association of orthostatic hypotension (OH) with functional position change and balance in older adults with hypertension. The presence of OH was assessed with intermittent (OH intermittent) and continuous blood pressure (OH continuous) monitoring. The change in functional position was tested with sitting to standing assessment, balance performance using activity specific balance confidence scale (ABC), and timed up and go test (TUG). Testing unilateral and bilateral standing with and without altered sensory inputs was tested using the Humac balance system. ABC, TUG, and standing up time showed no significant association with OH intermittent and OH continuous. A significant association was found between bilateral standing with eyes closed on foam surface for overall stability index and OH intermittent. Older people with hypertension may be routinely examined for OH and appropriate intervention strategies should be included for comprehensive care.

Abstract Background Cross-sectional studies have shown that balance performance can be challenged by the level of task difficulty (e.g., varying stance conditions, sensory manipulations). However, it remains unclear whether the application of different levels of task difficulty during balance training (BT) leads to altered adaptations in balance performance. Thus, we examined the effects of BT conducted under a high versus a low level of task difficulty on balance performance. Methods Forty male adolescents were randomly assigned to a BT program using a low (BT-low: n = 20; age: 12.4 ± 2.0 yrs) or a high (BT-high: n = 20; age: 12.5 ± 2.5 yrs) level of balance task difficulty. Both groups trained for 7 weeks (2 sessions/week, 30–35 min each). Pre- and post-training assessments included measures of static (one-legged stance [OLS] time), dynamic (10-m gait velocity), and proactive (Y-Balance Test [YBT] reach distance, Functional Reach Test [FRT]; Timed-Up-and-Go Test [TUG]) balance. Results Significant main effects of Test (i.e., pre- to post-test improvements) were observed for all but one balance measure (i.e., 10-m gait velocity). Additionally, a Test x Group interaction was detected for the FRT in favor of the BT-high group (Δ + 8%, p < 0.001, d = 0.35). Further, tendencies toward significant Test x Group interactions were found for the YBT anterior reach (in favor of BT-high: Δ + 9%, p < 0.001, d = 0.60) and for the OLS with eyes opened and on firm surface (in favor of BT-low: Δ + 31%, p = 0.003, d = 0.67). Conclusions Following 7 weeks of BT, enhancements in measures of static, dynamic, and proactive balance were observed in the BT-high and BT-low groups. However, BT-high appears to be more effective for increasing measures of proactive balance, whereas BT-low seems to be more effective for improving proxies of static balance. Abstract Background Malnutrition and poor physical performance are both conditions that increase in prevalence with age; however, their interrelation in a clinically relevant population has not been thoroughly studied. Aims This study aimed to determine the strength of the association between malnutrition and measures of both static and dynamic physical performance in a cohort of geriatric outpatients. Methods This cross-sectional study included 286 older adults (mean age 81.8, SD 7.2 years, and 40.6% male) who were referred to geriatric outpatient mobility clinics. The presence of malnutrition was determined using the Short Nutritional Assessment Questionnaire (SNAQ, cut-off ≥ 2 points). Measures of dynamic physical performance included timed up and go (TUG), 4-m walk test, and chair stand test (CST). Static performance encompassed balance tests and hand grip strength (HGS). Physical performance was standardized into sex-specific Z-scores. The association between malnutrition and each individual measure of physical performance was assessed using linear regression analysis. Results 19.9% of the cohort was identified as malnourished. Malnutrition was most strongly associated with CST and gait speed; less strong but significant associations were found between malnutrition and TUG. There was no significant association between malnutrition and HGS or balance. Discussion Physical performance was associated with malnutrition, specifically, dynamic rather than static measures. This may reflect muscle power being more impacted by nutritional status than muscle strength; however, this needs to be further addressed. Conclusions Malnutrition is associated with dynamic physical performance in geriatric outpatients, which should inform diagnosis and treatment/prevention strategies.

Abstract Introduction It is important to identify older adult fallers in order to implement early prevention management also prevent recurrent falls. This study aimed to explore the profiles of older adult fallers and non-fallers in regards to socio-demographic, physical performance, fear of falls that includes the activities that were feared in regard to falls. Analysis Descriptive and mean comparison test. Methodology Participants for this study were recruited among individuals aged 60 years and above, able to walk 3m, able to stand independently for longer than 1min and able to comprehend and follow instructions. Exclusion criteria include recent vertebral or lower limb fracture (less than 6months), unstable angina, unable to follow command and severe hearing and vision impairment. Design: Cross-sectional study. Results A number of 27 older adults were screened for falls. Based on Timed Up and Go test (cut off 11.18s), 8 (29.6%) of them were identified as fallers. Fallers were majority females (10, 37%), had visual impairment (4,14.8%), older (77.4±2.9years versus 68.4±5.7years) and had slightly higher in fear of falls score (13.2±5.5 versus 12.0±7.0). In view of physical performance, fallers were slower in Timed Up and Go test (13.6±2.4 versus 9.1±1.4), weaker in hand grip strength (14.3±2.5kg versus 21.5±19.3kg) and weaker in sit-to-stand performance (13.3±2.7s versus 10.7±2.7s). Age (p&lt;0.05) and sit-to-stand performance (p&lt;0.05) significantly differed between fallers and non-fallers. Among seven activities assessed using short Falls Efficacy Scale-International, non-fallers were found to have more fear during various activities compared to fallers. Implication Exploration of falls risk profiles in older adults will hopefully allow better understanding and further improvement in developing falls prevention management plans.
