It is certainly true that increase of the average life span has caused a greater percentage of elderly people to visit urological departments. From April 1989 to December 1991, patients over 75 years under our observation were 10.8% of all operated patients. Only 5% of patients over 75 who needed to be operated, weren't actually operated. The numerical analysis of our experience has highlighted the fact that in patients who underwent an operation, the incidence and mortality caused by cardiovascular diseases weren't any higher than those found in other people of the same age. We are also convinced that prejudices about age limits should not prevent the elderly afflicted with a benign or malignant urological pathology from undergoing an operation.

The numerical analysis of our experience has highlighted the fact that in operated patients the incidence and mortality caused by cardiovascular diseases were no higher than those found in other people of the same age.

Objective: Populations are aging in many countries, and the proportion of elderly people with severe to profound hearing loss is increasing in parallel with the increasing average life span. The objective of this study was to investigate the outcomes of cochlear implant (CI) surgery in elderly patients compared to those in younger patients. Methods: The outcomes of CI surgery were retrospectively investigated for 81 adults (32 men and 49 women) who underwent CI surgery at our hospital. They were divided according to age at the time of implantation into the younger group (<75 years of age; n = 49) or elderly group (≥75 years of age; n = 32). Results: The mean sentence recognition score on the CI-2004 Japanese open-set test battery (±standard deviation) was 82.9% ± 24.1 in the younger group and 81.9% ± 23.2 in the elderly group, with no significant difference between the groups (Mann–Whitney U test). The incidence of major complications that required surgical treatment was not significantly different between the groups (4.1% vs. 6.2%, respectively). Thus, there were no severe complications that could affect general health status in either group. Three patients in each group died for reasons unrelated to CI surgery during follow-up. The proportion of patients who were alive and continued to use the CI five years after surgery was 92.8% and 91.5%, respectively. Conclusion: Our results show good speech recognition and a low incidence of major complications in elderly patients. This comprehensive report on the outcomes of CI surgery in elderly patients will be helpful to the elderly with severe to profound hearing loss when deciding whether to undergo CI surgery.

An ageing population is mainly due to the reduction of births and the increase in life expectation. In Italy the percentage of people aged at least 65 years increased from 11.3% in 1971 to 15.3% in 1991. This increase mostly involved those aged 75 years and over. In the last four decades the average life span has extended by more than 11 years (from 65.5 to 76.9 years). The average life span in women is currently 80.2 years, almost 7 years longer than that of men. As age increases so does the incidence of various urological diseases requiring surgery, such as prostatic diseases (hyperplasia and carcinoma), urological tumours, urinary infections and incontinence. At the same time the probability of associated pathologies increases and consequently the risk of peri-operative complications, thus negatively influencing the prognosis. Surgery is only indicated in the elderly when it may increase life expectancy and/or improve the quality of life. A careful pre-operative evaluation of all the factors that may influence both life expectancy and quality of life is therefore indispensable, with particular reference to the actual disease and co-morbidity.

With the advent of population aging, aging-related diseases have become a challenge for governments worldwide. Sarcopenia has defined as a clinical syndrome associated with age-related loss such as skeletal muscle mass, strength, function, and physical performance. It is commonly seen in elderly patients with chronic diseases. Changes in lean mass are common critical determinants in the pathophysiology and progression of cardiovascular diseases (CVDs). Sarcopenia may be one of the most important causes of poor physical function and decreased cardiopulmonary function in elderly patients with CVDs. Sarcopenia may induce CVDs through common pathogenic pathways such as malnutrition, physical inactivity, insulin resistance, inflammation; these mechanisms interact. In this study, we aimed to investigate the relationship between sarcopenia and CVDs in the elderly. Further research is urgently needed to understand better the relationship, pathophysiology, clinical presentation, diagnostic criteria, and mechanisms of sarcopenia and CVDs, which may shed light on potential interventions to improve clinical outcomes and provide greater insight into the disorders above.

The objective of our study is to evaluate if patients aged over 75 years show, once operated, a different state compared to younger patients. We have taken two groups of patients from our case histories: patients who underwent major surgery (with the exception of operations which are usually not performed on those over 75 years old) and patients who underwent minor surgery. This second group is made up of patients who underwent treatment for prostate hypertrophy either surgically or endoscopically. In these groups we have tried to see any differences concerning the percentage of operated patients, mortality, early and late complications and the average length of stay in hospital.

The request for surgery for the elderly is increasing from year to year. In fact a longer average age has brought about the necessity to study the physiology of the ageing process. Increasing knowledge has led to a deeper understanding of geriatric physiopathology, so that 65 years is no longer considered the age limit to define a person “elderly”. On the other hand the elderly no longer accept being marginalized due to their infermity. On the contrary they ask for the same therapeutical measures which are used for younger people.

AbstractElderly patients with cardiovascular diseases account for a large proportion of Corona virus Disease 2019(COVID-19)related deaths. COVID-19, as a new coronavirus, mainly targets the patient’s lung triggering a cascade of innate and adaptive immune responses in the host. The principal causes of death among COVID-19 patients, especially elderly subjects with cardiovascular diseases, are acute respiratory distress syndrome(ARDS), multiple organ dysfunction syndrome (MODS), and microvascular thrombosis. All prompted by an excessive uncontrolled systemic inflammatory response. Immunosenescence, characterized by systemic and chronic inflammation as well as innate/adaptive immune imbalance, presents both in the elderly and cardiovascular patients. COVID-19 infection further aggravates the existing inflammatory process and lymphocyte depletion leading to uncontrollable systemic inflammatory responses, which is the primary cause of death. Based on the higher mortality, this study attempts to elucidate the pathophysiological mechanisms of COVID-19 in elderly subjects with cardiovascular diseases as well as the cause of the high mortality result from COVID-19.

The elderly individual is becoming the most common patient in clinical practice due to the increasing average life span, especially in developed countries. The current elderly patients are different from those of some decades ago. They usually have an active social life, want to be informed, and are actively involved in the current society with consequent high expectations for medical and dental treatment. However, not all the elderly patients are like this. Some of them show limited financial resources, reduced mental and manual skills, and poor motivation. The purpose of this communication is to make a brief characterization of the new elderly population in view of prosthetic and dental management.
