Anticholinesterase Poisoning in Turkey - Clinical, Laboratory and Radiologic Evaluation of 269 cases

A retrospective study was conducted on 269 patients with anticholinesterase poisoning who had been admitted to the Internal Medicine and Pediatric Clinics over the past 10 years. Their ages ranged from 17 d to 62 years. In adults, suicide was the most common means of intoxication. Miosis was the most frequent symptom and was seen in 80.66% of the patients. Hyperglycaemia, hypoglycaemia, hypokalaemia, hyperkalaemia, hyperchloraemia, hypochloraemia and hyponatraemia were among the electrolytic and haematologic disorders (hyperglycaemia 48.00%, hypoglycaemia 6.40%). There were also electrocardiographic abnormalities. Asthma, although usually a late complication, was also noted at the beginning of treatment. Mortality rates were 8.98% for children and 9.6% for adults. Since hypoglycaemia was found in all the patients who died, it was accepted as an ominous prognostic sign. We think that anticholinesterase poisoning is an important health problem in our country. It is much less common in developed than semi-developed countries, and a complete understanding of it is essential for the emergency physician.

ABSTRACT Background Toxic epidermal necrolysis (TEN) is a rare idiosyncratic mucocutaneous reaction associated with high mortality. Drugs are most commonly implicated in TEN. The treatment constitutes stopping the offending drug, along with symptomatic management. In this study, we searched for case reports/series of TEN and analyzed data to find the most commonly implicated drugs in TEN, effects of use of corticosteroids on mortality in TEN patients, changing trends in mortality over the past 3 decades and difference in mortality rates in both developing and developed countries. Materials and methods We searched for case reports/series of TEN to evaluate most commonly implicated drugs in TEN, effects of use of corticosteroids on mortality in TEN patients, changing trends in mortality over the past three decades and difference in mortality rates in both developing and developed countries. Results Antibiotics (28.6%) followed by antiepileptics (17.4%) and nonsteroidal anti-inflammatory drugs (9.6%) are most commonly implicated. There was nonsignificant decrease in mortality among steroids users as compared to nonusers (OR = 2.0, CI 0.96-4.24). During the period between 1980 and 1989 the reported mortality in TEN cases was approximately 33.4%, which decreased to 27% in the next two decades. There was a nonsignificant difference in mortality in develo- ping countries as compared to developed countries (OR: 0.70, CI 0.32-1.53). Conclusion Corticosteroids have been associated with non significant reduction in the mortality. Apart from this, mortality did not differ over years together in both developing and developed countries. In the past decade rickets has re-emerged in developed countries due to changes in lifestyles and dietary habits. We describe a case of a 28-month-old black infant with failure to thrive. He was exclusively breastfed until nine months of age, without vitamin supplementation, and never ingested milk products due to alleged cow’s milk intolerance. His examination revealed bowlegs, rachitic rosary and wide wrists. Alkaline phosphatase and intact parathyroid hormone levels were elevated, and calcidiol was decreased. Radiographic images showed bone demineralization, fraying and cupping of the distal radius and ulna. Nutritional rickets was considered and treatment with colecalciferol and calcium carbonate was initiated, with clinical, laboratory and radiologic improvement. In this case, a group of factors contributed to severe nutritional rickets, alerting to the re-emergence of this disease.

Abstract: The rapid socioeconomic growth has led to a more sedentary lifestyle and change in diet over the past several decades. As a result of prosperity,obesity has become an important health problem. And then problem healthnutrition world, both in developed countries and in developing countries. Waist circumference more than normal is one component in the metabolic syndrome. An increased incidence of metabolic syndrome in line with the increase in obesity. Hs-CRP is a protein in the synthesis in the liver in response to inflammatory stimuli during the acute phase. This research was conducted to find out the waist circumference and hs-CRP levels in students faculty of medicine University of Sam Ratulangi Manado. And the type of research used is the observational approach cross sectional analytic. Results anasis statistics by using test mann whitney u showed there are different meaning ( p = 0.00 ( 0,00 ). Based on the test spearmen statistics then obtained value a correlation coefficient ( r ) by 0.472 and value ρ = 0.000 < α= 0.05.. This shows there is a relationship between waist circumference with hs-CRP levels on students obese and not obese in medical schools university sam ratulangi manado. Lingkar Pinggang yang besar dari normal adalah salah satu komponen dalam sindroma metabolik. Peningkatan kejadian sindroma metabolik sejalan dengan peningkatan obesitas.Hs-CRP merupakan protein yang di sintesis di hati sebagai respon terhadap rangsangan pada saat inflamasi fase akut.Penelitian ini dilakukan untuk mengetahui Lingkar Pinggang dan kadar hs-CRP pada Mahasiswa Fakultas Kedokteran Universitas Sam Ratulangi Manado. Dan jenis penelitian yang dipakai adalah observasional analitik dengan pendekatan cross sectional. Hasil anasis statistik dengan menggunakan uji Mann Whitney U menunjukan terdapat perbedan bermakna (p=0.00(0,00). Berdasarkan hasil uji spearmen statistik maka didapatkan nilai koefisien korelasi (r) sebesar 0.472 dan nilai ρ = 0.000 < α= 0.05. Hal ini menunjukan terdapat hubungan antara lingkar pinggang dengan kadar  hs-CRP pada mahasiswa obes dan tidak obes di Fakultas Kedokteran Universitas Sam Ratulangi Manado. Vitamin A (retinol) is one of the key elements for regulating the immune response and controls the division and differentiation of epithelial cells of the mucous membranes of the bronchopulmonary system, gastrointestinal tract, urinary tract, eyes, etc. Its significance in the context of the COVID‑19 pandemic is difficult to overestimate. However, a number of studies conducted in the past have associated the additional intake of vitamin A with an increased risk of developing cancer, as a result of which vitamin A was practically excluded from therapeutic practice in developed countries. Our review highlights the role of vitamin A in maintaining human health and the latest data on its effect on the development mechanisms of somatic pathology.

This article presents information about modern principles of diagnosis and treatment of HPV-associated diseases. Behind cervical cancer morbidity and mortality rates over the past 10 years increase significantly. Examination and observation of patients with human papillomavirus persistence of highly oncogenic types is important a link in cancer prevention. The article presents diagnostic methods, treatment of cervical diseases, as well as the possibility of pharmacotherapy in HPV-associated diseases.

Beach nourishment has been widely used for beach protection around the world. However, there is limited information about beach nourishment in China. This study offers an overview of beach nourishment practices, status and technological advances in China, based on the literature, reports, and personal communications. The results demonstrate that beach nourishment has been recognized as an effective and environmentally friendly measure to combat coastal erosion and has been increasingly adopted in China, especially in the past decade. The unique characteristics of coastal China resulted in a difference in beach nourishment between China and Western developed countries in terms of the types, objectives, and shapes of beach nourishment. For the types of nourishments in China, there were approximately the same number of restored beaches and newly constructed beaches. For fill sediment, homogeneous fill and heterogeneous fill comprised 51.1% and 48.9% of projects, respectively. The objective of beach nourishment was mainly to promote coastal tourism, and the shape of nourished beaches was dominated by headland bays. This study also indicated that China has achieved a number of technological advances in beach nourishment, including methods of beach nourishment on severely eroded coasts and muddy coasts, an optimized design of drain pipes involved in urban beaches, and ecological design considerations. From the past decade of practices, four aspects were proposed as considerations for future nourishment: sand sources, technique advances, ecological effects, and management of beach nourishments.

IntroductionIn the USA, Food and Drug Administration regulations prohibit the sale of flavoured cigarettes, with menthol being the exception. However, the manufacture, advertisement and sale of flavoured cigar products are permitted. Such flavourings influence positive perceptions of tobacco products and are linked to increased use. Flavourings may mask the taste of tobacco and enhance smoke inhalation, influencing toxicant exposure and abuse liability among novice tobacco users. Using clinical laboratory methods, this study investigates how flavour availability affects measures of abuse liability in young adult cigarette smokers. The specific aims are to evaluate the effect of cigar flavours on nicotine exposure, and behavioural and subjective measures of abuse liability.Methods and analysesParticipants (projected n=25) are healthy smokers of five or more cigarettes per day over the past 3 months, 18–25 years old, naive to cigar use (lifetime use of 50 or fewer cigar products and no more than 10 cigars smoked in the past 30 days) and without a desire to quit cigarette smoking in the next 30 days. Participants complete five laboratory sessions in a Latin square design with either their own brand cigarette or a session-specific Black & Mild cigar differing in flavour (apple, cream, original and wine). Participants are single-blinded to cigar flavours. Each session consists of two 10-puff smoking bouts (30 s interpuff interval) separated by 1 hour. Primary outcomes include saliva nicotine concentration, behavioural economic task performance and response to various questionnaire items assessing subjective effects predictive of abuse liability. Differences in outcomes across own brand cigarette and flavoured cigar conditions will be tested using linear mixed models.Ethics and disseminationThe Virginia Commonwealth University Institutional Review Board approved the study (VCU IRB: HM20007848). Dissemination channels for study findings include scientific journals, scientific meetings, and policy briefs.Trial registration numberNCT02937051.

Airport security system is mandatory in all developing and developed countries. The major threat to any country’s economy, health and development is targeted and transported via airports which are evident from the past historical examples. The developments in Science and Technology have improved the methods of surveillance but failed to address few key health issues including privacy. The main objective of this paper is to develop a smart IOT based surveillance systems that is implemented in airport in smart screening without causing any unpleasant disturbances to the passengers. This paper makes use of the recently developed smart sensors, Tiva processor from Texas instruments connected via IOT and cloud and a remote station to effectively secure the airport area and overcoming all the real time challenges.

Staphylococci have various strategies for resisting therapy that extend beyond classic mechanisms. Clinical experience with device-associated infections as well as with infections due to small-colony variants (SCVs) clearly shows that both antibacterial chemotherapy and host defense mechanisms are often unable to eliminate the pathogens and cure these infections. Of particular interest is the fact that in the past few years an increasing number of various foreign body-related infections due to staphylococcal SCVs have been reported. In this overview, the characteristics of SCVs recovered from clinical specimens and of defined mutants displaying the SCV phenotype are described. Their slow growth and changing biochemical and physiological features represent a challenge to clinical laboratory personnel, because recovery, identification, as well as susceptibility testing of these variants need particular efforts. In addition, the reduced susceptibility to aminoglycosides and the ability of SCVs to persist intracellularly require specific attention for the treatment of these infections. Thus, special efforts to search for these variants formed by Staphylococcus aureus or by coagulase-negative staphylococci should be considered when an infection is particularly resistant to therapy, persists for a long period or fails to respond to apparently adequate therapy with antimicrobial compounds.

The term ‘ecodisaster’ may be defined as ‘a global catastrophe of the human species’. Any ecodisasters occurring in the near future will, almost certainly, be caused, directly or indirectly, by the present overpopulation of the world, accompanied by unwise and irresponsible disregard of environmental deterioration.The suggestion is made here that Man's first and, it is to be hoped, last, ecodisaster may already have begun. Although not dramatic, it is taking the form of a steady decline in the standard of living nearly everywhere, coupled with massive pollution, and widespread malnutrition in the under-developed countries of the world. It will persist until world population eventually becomes adjusted to environmental resources.It is ironical that control of the pests and diseases which have inflicted so much misery on mankind in the past, should have helped to engender the present population explosion with all the hunger and privation that accompany it in the under-developed regions of the world.
