summary Tele-ophthalmology has been employed mainly for patients in under-served rural areas in need of specialty care, but other applications such as telementoring have also been used. In certain populations, cost containment is a significant issue and telemedicine is a solution. Tele-ophthalmology can be performed in realtime, by store-and-forward mode, or by hybrid techniques. After appropriate modification, a range of peripherals may be used for tele-ophthalmology, including the direct ophthalmoscope, indirect ophthalmoscope, slit lamp or retinal camera. Tele-ophthalmology applications include: detecting, screening and diagnosing diabetic retinopathy; anterior segment imaging; glaucoma screening; low vision consultation; telementoring. Tele-ophthalmology shows great promise for improving patient care and increasing access to specialty care not available in under-served areas. In developing countries tele-ophthalmology may be a cost-effective method by which richer countries can assist them.

In prerecorded telemedicine (also known as asynchronous or store-and-forward), the person sending the information and the person receiving it do not need to do so simultaneously; thus, viewing the information can be done at some later time. Prerecorded telemedicine is therefore not appropriate for emergency consultations. In prerecorded telemedicine systems, the following steps can be distinguished: (1) the acquisition of diagnostic information at the remote site; (2) its storage, which can be at either site, or at both; (3) its delivery to the expert site through an appropriate connection; and (4) its display at the expert site. The types of information transferred include audio, data and text, still images and moving images (i.e. video). An increasingly common way of doing prerecorded telemedicine is by email sent via the Internet. Although there are some problems associated with the Internet, its wide availability and low cost have encouraged its use. Examples where email has been used successfully include teleradiology, telecardiology, teledermatology and telepathology. In some situations prerecorded telemedicine is the only way to provide remote medical services, or the most cost-effective method. Clearly, there are also situations when prerecorded telemedicine is not an appropriate way to deliver health services, for example whenever the sender of the information is not qualified to sample the information acquired or the specialist receiving the information must manipulate it, during acquisition, in some way.

Excessive fluoride in potable water is a serious health problem in rural areas of many developing countries. Hence, there is a need to find a simple and cost-effective method for water defluoridation in such areas. In the northern part of Cameroon, clay pots are used for cooking food and water storage. The firing of these pots consists of intensive burning using fire wood. They were tested as a potential adsorbent for removing excess fluoride from water. Experiments were carried out in a jar test at room temperature (25 ± 2°C). Effects of contact time (0–90 min), pH (4, 5, 7, 8, and 9), stirring speed (60, 90, 120, and 200 rpm), and ionic strength (0–1000 mg/L) were investigated. Results showed that equilibrium was attained in 10 min whatever the pH. Pseudo-second-order and pore diffusion models described well the adsorption process. The highest amount of fluoride adsorbed (1.6 mg/g) was obtained at pH 4-5 and the optimum stirring speed is 120 rpm. Ionic strength has a significant effect on fluoride adsorption.

The Community Trial of Breast Cancer Screening Promotion assessed the effectiveness and cost-effectiveness of mammography promotion by community volunteer groups in rural areas using three different intervention approaches: individual counseling, community activities, and a combined intervention including both. Societal costs of the interventions were calculated and used in conjunction with measures of effectiveness to calculate cost-effectiveness in terms of cost per additional mammogram and cost per year of life saved. Methods of collecting and using cost information to assess the cost-effectiveness of community interventions are described. The Community Activities intervention was found to be the most cost-effective, at approximately $2,000 for each additional regular mammography user in the community. The cost per year of life saved associated with mammography promotion was approximately $56,000 per year of life saved. Exploratory analyses suggest that the most cost-effective method of promoting mammography use may vary with the target population.

A convenient, environment-friendly, and cost-effective method to keep anti-icing for a long time was highly desirable. Slippery lubricant layers were regarded to be effective and promising for anti-icing on different surfaces, but the drought-out of lubricants and the possible detriments to the environment were inevitable. By combining super-high molecular weight sodium polyacrylate (H-PAAS) with polyolefin through a one-pot method, a self-sustainable lubricating layer with extremely low ice adhesion of un-freezable water hydrogel was achieved at subzero conditions. The lubricant hydrogel layer could auto-spread and cover the surface of polyolefin after encountering supercooled water, frost, or ice. Due to the reduction of storage modulus in the interface, the ice adhesion of the specimen surfaces was far below 20 kPa, varying from 5.13 kPa to 18.95 kPa. Furthermore, the surfaces could preserve the fairly low adhesion after icing/de-icing cycles for over 15 times and thus exhibited sustainable durability. More importantly, this method could be introducing to various polymers and is of great promise for practical applications.

SummaryProblems with unfractionated heparin as an antithrombotic have led to the development of new therapeutic agents. Of these, low molecular weight heparin shows great promise and has led to out-patient therapy of DVT/PE in selected patients. Oral anticoagulants remain the choice for long-term therapy. More cost-effective ways to give oral anticoagulants are needed.


The choice of cost-effective method of anticorrosive protection of steel structures is an urgent and time consuming task, considering the significant number of protection ways, differing from each other in the complex of technological, physical, chemical and economic characteristics. To reduce the complexity of solving this problem, the author proposes a computational tool that can be considered as a subsystem of computer-aided design and used at the stage of variant and detailed design of steel structures. As a criterion of the effectiveness of the anti-corrosion protection method, the cost of the protective coating during the service life is accepted. The analysis of existing methods of steel protection against corrosion is performed, the possibility of their use for the protection of the most common steel structures is established, as well as the estimated period of effective operation of the coating. The developed computational tool makes it possible to choose the best method of protection of steel structures against corrosion, taking into account the operating conditions of the protected structure and the possibility of using a protective coating.

About one hundred and fifty wastewater reservoirs store effluents for irrigation in Israel. Effluent qualities differ according to the inflowing wastewater quality, the degree of pretreatment and the operational parameters. Certain aspects of water quality like concentration of organic matter, suspended solids and chlorophyll are significantly correlated with the water column transparency and colour. Accordingly optical images of the reservoirs obtained from the SPOT satellite demonstrate pronounced differences correlated with the water quality. The analysis of satellite multispectral images is based on a theoretical model. The model calculates, using the radiation transfer equation, the volume reflectance of the water body. Satellite images of 99 reservoirs were analyzed in the chromacity space in order to classify them according to water quality. Principal Component Analysis backed by the theoretical model increases the method sensitivity. Further elaboration of this approach will lead to the establishment of a time and cost effective method for the routine monitoring of these hypertrophic wastewater reservoirs.

This article systematizes the legal framework, forms, and volumes of support for agriculture from the federal budget of Russia and the regional budget (on the example of the Tyumen Region). The authors have performed a detailed analysis of the directions of such support for 2016-2019. The historical, economic and social features of the relationship between the state and agriculture are shown. State support for agriculture is objectively necessary, and competent budget planning makes it possible to develop. However, the dominance of direct forms of spending support does not stimulate cost-effective and innovative activities, which in the future can bring the industry to a competitive level. There is very little and irregular support for social forms, the development of farming and other forms of private farming in rural areas. The authors conclude that the policy of state support is sufficiently provided by legislative acts, resolutions and state programs. Many programs are updated, and new versions are adopted even before the previous ones expire. Such strong volatility hurts strategic projects and agricultural initiatives. Less expensive forms of activity-mediation and trade turn profitable. Living conditions in the villages significantly stay behind in quality and opportunities, which leads to the human capital leaving rural areas. The authors propose to pay more attention to the development of indirect forms of support, to stimulate economic activity and small businesses.
