The care of the self as a moral foundation of physiotherapy

The aim of this paper is to offer theoretical insights into the care of the self, which often initiates therapist-patient relationships in clinical practice. The reason is that when patients care about their health status, they are inclined to establish a therapeutic relationship with physical therapists. Hence, the care for self may bridge the world of the patient's private experiences and the world of the healthcare system together with its interventions, which is represented by the physical therapist In this framework, care means not only the patient's choice to undergo therapy but also his/her commitment to sustaining its effects in private life. This involves educational interventions by the physiotherapist who inscribes him/herself in his/her patients’ care of the self in order to alter their habits of physical exercise, diet, personal hygiene, body posture, etc. The argument in this paper is informed by the concepts of ethics developed by Michel Foucault and Richard Shusterman.

ABSTRACT: This paper tries to give a philosophical foundation to the rights to freedom of religion/belief. So far, the rights to freedom of religion/belief have been considered legitimated because resulted in General Assembly of the United Nations as mutual consensus of all nations around the world. Although, the normativity of the rights to freedom of religion/belief based on that mutual consensus is not ethical-philosophical, but political. By the justice argument of John Rawls and the self-ownership argument of Robert Nozick, this paper tries to give a moral foundation to the guarantee of respect and protection of the freedom of religion/belief. KEYWORDS: freedom of religion/belief, human rights instruments, forum internum, forum eksternum, justice, slef-ownership.ABSTRAK: Tujuan makalah ini adalah memberikan pendasaran filosofis bagi hak atas kebebasan beragama/berkeyakinan. Sejauh ini, hak atas kebebasan beragama/berkeyakinan (KBB) dipandang sebagai sesuatu yang bersifat normatif, karena dihasilkan dalam sidang-sidang Majelis Umum Perserikatan Bangsa-bangsa (PBB) sehingga telah menjadi konsensus bersama bangsa-bangsa di seluruh dunia. Namun demikian, normativitas hak atas KBB yang didasarkan pada konsensus bersama itu masih bersifat politis, belum memiliki dasar etis-filosofis. Melalui argumen keadilan yang didasarkan pada pemikiran John Rawls dan argumen kepemilikan-diri yang didasarkan pada pemikiran Robert Nozick, makalah ini mencoba memberikan landasan moral bagi jaminan penghormatan dan perlindungan bagi kebebasan beragama/berkeyakinan. AbstractThe theory of technological mediation aims to take technological artifacts seriously, recognizing the constitutive role they play in how we experience the world, act in it, and how we are constituted as (moral) subjects. Its quest for a compatible ethics has led it to Foucault’s “care of the self,” i.e., a transformation of the self by oneself through self-discipline. In this regard, technologies have been interpreted as power structures to which one can relate through Foucaultian “technologies of the self” or ascetic practices. However, this leaves unexplored how concrete technologies can actually support the process of self-care. This paper explores this possibility by examining one such technology: a gamified To-Do list app. Doing so, it first shows that despite the apparent straightforwardness of gamification, confrontation and shame play an important role in how the app motivates me to do better. Second, inspired by Ihde’s schema of human-technology relations, it presents different ways in which the app may confront me with myself. Subsequently, it accounts for the motivation and shame that this technologically mediated confrontation with myself invokes through a Levinasian account of ethical subjectivity. In so doing, it also shows how Levinas’ phenomenology implies a responsibility for self-care and how nonhuman, technological others may still call me to responsibility. It concludes with a reflection on the role of gamification in technologically mediated subjectivation and some implications for design.

Today, machines observe, record, and sense the world—not just for us but also often instead of us and indifferently to our meaning. The intertwined problems of technological knowledge and (our) knowledge of technology manifest in the growing industry of smart machines, the Internet of Things, and other means for self-tracking. The automation of the care of the self is buoyed by a popular fantasy of data’s intimacy, of machines that know you better than yourself. Yet as the technology becomes normalized, the hacker ethic gives way to a market-driven shift in which more and more of “my” personal truth is colonized by machines (and the people behind the machines) that I cannot question.

This article deals with the dilemma related to the purpose of upbringing. M. Foucault's concept of care of the self and J. Patočka's works allow us to ask questions about the goal of education. Is it to become an intellectual or a spiritual person? The possibility of such a distinction is embedded in the ethics of care of the self, an ancient tradition, which even nowadays, among others, thanks to P. Sloterdijk or Foucault himself, has become an important category. An ethical attitude towards the self opens the way to spirituality through a set of appropriate practices. It becomes the basis for relations with others and with the world.

AbstractAlthough the physical therapist profession is the leading established, largely nonpharmacological health profession in the world and is committed to health promotion and noncommunicable disease (NCD) prevention, these have yet to be designated as core physical therapist competencies. Based on findings of 3 Physical Therapy Summits on Global Health, addressing NCDs (heart disease, cancer, hypertension, stroke, diabetes, obesity, and chronic lung disease) has been declared an urgent professional priority. The Third Summit established the status of health competencies in physical therapist practice across the 5 World Confederation for Physical Therapy (WCPT) regions with a view to establish health competency standards, this article's focus. Three general principles related to health-focused practice emerged, along with 3 recommendations for its inclusion. Participants acknowledged that specific competencies are needed to ensure that health promotion and NCD prevention are practiced consistently by physical therapists within and across WCPT regions (ie, effective counseling for smoking cessation, basic nutrition, weight control, and reduced sitting and increased activity/exercise in patients and clients, irrespective of their presenting complaints/diagnoses). Minimum accreditable health competency standards within the profession, including use of the WCPT-supported Health Improvement Card, were recommended for inclusion into practice, entry-to-practice education, and research. Such standards are highly consistent with the mission of the WCPT and the World Health Organization. The physical therapist profession needs to assume a leadership role vis-à-vis eliminating the gap between what we know unequivocally about the causes of and contributors to NCDs and the long-term benefits of effective, sustained, nonpharmacological lifestyle behavior change, which no drug nor many surgical procedures have been reported to match.

Narrative reasoning is an inductive cognitive strategy used to understand patients’ experiences with illness within the biosocial context of their lives. The purpose of this study is to examine the meaning of the patient’s illness experience to physical therapist students and propose a theory of narrative reasoning development in these novice clinicians.The data collected from twenty-one first year physical therapy students consisted of narratives written during their clinical internships describing a patient’s experience with illness and post-internship interviews telling the patient’s story. Data were analyzed in a process of constant comparison, evolving codes, and researcher memoing using both grounded theory and narrative analyses. Themes that emerged in open coding and core categories were validated in selective coding. In the final stage of theoretical coding, relationships between the saturated core categories and relevance to current theory were established. The final result was a model of the development of narrative reasoning, illustrating contributions to current theory. The main themes that emerged from the data related to the novice therapist’s development of narrative reasoning were in four areas: perspective, clinical narrative focus, reflective skills, and therapeutic alliance. The theoretical model advanced by this study is a recursive, spiraling developmental process of narrative reasoning through phases that are influenced by drivers and/or barriers. The process includes co-construction of narrative reasoning through encounters with patients, families, clinical instructors and other team members. Establishing a therapeutic alliance is a pivotal point in the development of narrative reasoning and is based on an empathic perspective and strengthened through a reflective approach. In conclusion, this study contributes to the understanding of the development of narrative reasoning. This model of narrative reasoning in student physical therapists has implications for educational interventions aimed at providing learning experiences that parallel the development of narrative reasoning.

Spirituality is more or less measured with religion and morality, where both these words are emotional and deal with the public and private life. If education as widely accepted, is learning to see with new eyes then agreeably attending to spirituality is consciousness of the self and learning. Spirituality is a way of life where a person acquires greater understanding of himself and the outer world. People with spiritual education and awakening, possess a completely different view of the self and the world and further possess greater virtues and good behavioral traits. The chapter further throws light on the need and importance of spiritual education and learning in the lives of women.

We seek to answer a deceptively simple question: why do private citizens in liberal democracies offer hospitality to undocumented migrants? Through qualitative interviews with French citizens, we propose three reasons. The first is widely recognized in the scholarly literature: hospitality is offered out of a care and concern for vulnerable and precarious migrants. However, we uncover two additional reasons that are not acknowledged in studies on hospitality. One is the desire by citizens to uphold the basic principles and ideals of their own society (what we call “care for the world”). Another is the desire on the part of citizens themselves to become a different and better kind of person by practicing hospitality (what we call “care of the self”). We provide a multifaceted account of what motivates citizens to offer hospitality even in situations where it is outlawed by their own governments.

Abstract Foucault is critical of the tendency to reduce all social and political problems according to predetermined ends and verifiable procedures. For Foucault, philosophical activity is a condition of possibility for the articulation of the question of the self. Inspired by his work on the desiring subject, Foucault begins to explore the ethical and political implications of self-care for modern day concerns. He presents an account of self-care that centres on developing an attitude that questions the personal relationship to truth, and puts to test those ideas and truths held most dearly. Processes of self-care evaluate the consistency between those truths a person regards as necessary and a person’s actions in the world. Interested in the ways in which people see themselves as subjects, Foucault directs his attention to the connection between systems of knowledge, power, and practices of the self. Crucial to Foucault’s process is the recognition that the self-subject is not given and does not have ontological precedence, and that subjectivity is transformable. By finding the lines and fractures in external and internal modes of objectification Foucault hopes to open up the space of freedom to bring about transformative events. The care of the self serves as a form of critique and resistance where it is both a way of living and acting in the world, and a critical response to a particular time and place.
