Trends in Endovascular Surgery Training

Purpose: To gather vascular surgery fellows' opinions on various issues related to endovascular surgery (EVS) over a 2-year period and analyze the responses to identify trends in EVS training. Methods: Vascular surgery fellows in 2 consecutive years were given a 2-page questionnaire inquiring about training protocols and local practice habits. Respondents included 64 vascular fellows from the academic year 1998–1999 (F98) and 52 vascular fellows from the academic year 1999–2000 (F99) (78% men in the entire population; mean age 34 years), representing a significant fraction of trainees in North America. Data from F98 and F99 were compared and analyzed. Results: The majority (66%) of vascular surgery fellows were trained at university hospitals and performed EVS at the time of the survey: 83% in the F98 class and 92% in the F99 group (p=0.17). Utilization rates among the 9 interventions surveyed ranged from angiography (83%) and angioplasty (77%) to intravascular ultrasound (33%) and atherectomy (15%). Performance of endovascular grafting significantly increased among trainees (50% versus 81%, p<0.005), while atherectomy and angioscopy decreased. EVS performed in the operating room with portable imaging equipment decreased (67% versus 42%, p=0.02) as access to the radiology and cardiology suites increased. In most communities (63%), radiology specialists performed most of the EVS procedures, but the portion of communities where vascular surgery performed the majority of EVS procedures increased from 20% to 35% (p=0.10) from F98 to F99. Responders (90%) believed that EVS would become a major component of vascular surgery and comprise 30% of their future practice. The proportion of fellows who believed they were sufficiently trained in endovascular techniques increased from 30% to 50% (p=0.04), with the remainder willing to devote a short period (<3 months) for further training. Conclusions: The vast majority of vascular trainees perform EVS and believe that it will have an increasing role in their practice. Trends include increased endovascular grafting and performance of EVS by vascular surgeons in interventional suites.

Abstract Background Youth are at high risk for HIV, but are often left out of designing interventions, including those focused on adolescents. We organized a designathon for Nigerian youth to develop HIV self-testing (HIVST) strategies for potential implementation in their local communities. A designathon is a problem-focused event where participants work together over a short period to create and present solutions to a judging panel. Methods We organized a 72-h designathon for youth (14–24 years old) in Nigeria to design strategies to increase youth HIVST uptake. Proposals included details about HIVST kit service delivery, method of distribution, promotional strategy, and youth audience. Teams pitched their proposals to a diverse seven-member judging panel who scored proposals based on desirability, feasibility, potential impact and teamwork. We examined participants’ socio-demographic characteristics and summarized themes from their HIVST proposals. Results Forty-two youth on 13 teams participated in the designathon. The median team size was 3 participants (IQR: 2–4). The median age was 22.5 years (IQR: 21–24), 66.7% were male, 47.4% completed tertiary education, and 50% lived in Lagos State. Themes from proposals included HIVST integration with other health services, digital marketing and distribution approaches, and engaging students. Judges identified seven teams with exceptional HIVST proposals and five teams were supported for further training. Conclusions The designathon provided a structured method for incorporating youth ideas into HIV service delivery. This approach could differentiate HIV services to be more youth-friendly in Nigeria and other settings.

The study aimed to identify the reality of practicing accountability among school leaders in public education schools in Sharourah Governorate / Najran and the degree of accountability in the administrative and technical domains. To achieve this goal, the researcher used the analytical survey approach. The study was applied to a sample of )177 ( teachers, including (101) male teachers and (76) female teachers, in the first semester of the academic year 2018- 2019. The results of the study found that, the practice of accountability among the leaders of public education schools in Sharourah governorate / Najran from teachers point of view has got a general average (4.02), high rating, and according to the domains level, practicing accountability in administrative domain has got an average; (3.93), with a high rating, also, practicing accountability in the technical domain has got an average of (3.90) with a high degree. There were no statistically significant differences at the level of (0.05 = α) between the arithmetical averages of the total score for the degree of accountability in administrative and technical domains due to the differences in the variables of the study (years of experience and the scientific qualification). There were statistically significant differences in the degree of accountability in administrative and technical domains due to gender in favor of males; and in the variable of a school level in favor of high school. The study recommended the need to spread the culture of accountability, And to clarify the nature, methods, objectives and mechanisms for teachers, and the need to provide standards and performance indicators used in accountability process.

Tujuan Penelitian ini adalah untuk mengetahui sejauh mana peningkatan keterampilan siswa dalam menulis letter of opinion di Kelas XI. Penelitian ini dilakukan pada kelas XI-IPS-2 SMA Negeri 15 Semarang semester 1 Tahun Pelajaran 2017-2018 sebagai hasil penggunaan media fantastic card. Penelitian ini juga digunakan untuk mengetahui sejauh mana peningkatan pengetahuan dan sikap belajar siswa selama pembelajaran. Metode yang digunakan dalam penelitian ini adalah metode Penelitian Tindakan Kelas dengan teknik siklus yang terdiri dari dua tindakan siklus yaitu siklus 1 dan siklus 2. Alat pengumpulan data yang digunakan untuk data kuantitatif berupa tes tertulis ulangan harian dan data kualitatif digunakan lembar observasi atau pengamatan, lembar refleksi diri, lembar penilaian antar teman, dan rubrik penilaian kinerja. Hasil penelitian menunjukkan media fantastic card dapat meningkatkan keterampilan menulis letter of opinion. Ketuntasan Belajar mengalami peningkatan dari kondisi awal 53,13% menjadi 65,63% pada siklus I dan meningkat menjadi 93,75% pada siklus II. Peningkatan keterampilan siswa dalam menulis letter of opinion  pada siklus I mencapai 2,53% dari rata-rata hasil belajar pada pra siklus dan hasil belajar pada siklus II mengalami peningkatan sebesar 6,81% dari rata-rata hasil belajar  pada siklus I atau mengalami peningkatan sebesar 9,34% jika dibandingkan dengan rata-rata hasil belajar pada pra siklus.Kata kunci: menulis, letter of opinion, media fantastic cardABSTRACTThis research was aimed to find out the extent of students’ skill at writing letter of opinion in 11th grade. The research was conducted in the 11th Grade of Social Science 2at Public Senior High School 15 Semarang in Academic Year 2017-2018 after using fantastic card. It was to know the improvement of knowledge and attitude of the students during learning. Method used was class action research method with cycle technique consisting of two cycles, i.e. cycle 1 and cycle 2. Instruments of data taking were written daily tests and as for qualitative data, observation sheet, self-reflection sheet, peer evaluation sheet, and performance evaluation rubric were used. Results of the research showed that fantastic card could improve the skill of writing letter of opinion. The learning completeness improved from 53.13% to 65.63% in cycle I, and 93.75% in cycle II. The students’ skill in writing letter of opinion in cycle I improved up to 2.53% from the average learning result in pre-cycle. Meanwhile, th elearning result in cycle II improved as much as 6.81% from the average learning result in cycle I, or 9.34%, compared to the average learning result in pre-cycle.

Abstract Purpose: To study the usefulness of virtual reality (VR) based training for diagnosing strabismusMethods: Fourteen ophthalmology residents performed at least 30 VR training sessions to diagnose esotropia and exotropia. Examinations of real patients with esotropia or exotropia before and after the VR training were video-recorded and presented to a strabismus expert to assess accuracy and performance scores for measuring the deviation angle and diagnosing strabismus with anonymization. A feedback survey regarding the usefulness and ease of use of the VR application was conducted for participants.Results: The mean age of 14 ophthalmology residents, including 10 men and 4 women, was 29.7 years. Before VR training, participants showed a mean accuracy score of 14.50 ± 5.45 and performance score of 9.64 ± 4.67 for measuring the deviation angle and diagnosing strabismus in real patients with strabismus. After VR training, they showed a significantly improved accuracy score of 22.14 ± 4.37 (p = 0.012) and performance score of 15.50 ± 1.99 (p = 0.011). According to the survey, most participants agreed on the usefulness of VR application.Conclusions: This study suggests that VR based training improves ophthalmology residents’ clinical diagnostic skills for strabismus in a short period.

In this review, the fundamentals of endovascular procedures that are necessary to take advantage of this advancing technology are outlined and explained. Preoperative patient assessment, appropriate access site selection, and vascular access technique are explained. The selection and use of appropriate guide wires, catheters, and sheaths are also reviewed and outlined. The basic techniques for angioplasty and stenting are described as well. Finally, appropriate closure techniques and postprocedure care are described. As with any operation or procedure, mastering the fundamentals is necessary to proceed to advanced intervention. In this work, a systematic study is conducted on the behavior of three-dimensional offshore oil/natural gas platforms under the action of seismic sequences. Such repeated earthquakes result in a noteworthy accumulation of damage in a platform since, in many cases, any rehabilitation process between any two or more successive ground motions cannot be essentially materialized because of lack of time. Conversely, in the past, the seismic response of offshore platforms has been exclusively investigated for the case of single earthquakes. In this study, two three-dimensional platforms are examined, where the first one is assumed to be completely constrained at its base (fixed boundary conditions), while the second one is founded in deformable soil with the aid of long piles. These structures are subjected to real seismic sequences which have been recorded by the same station in a short period of time. Additionally, the platforms under consideration are also subjected to artificial seismic sequences. In this study, we found that sequential earthquakes have a significant effect on the response of these special structures, and this finding should be taken into account in their design.

As millennial workers enter and dominate the global workforce by 2025, understanding their leadership styles is vital to bring about higher level of performance and productivity in the workplace. The Millennials, the next generation of leaders, are entering the workforce and assuming leadership positions in a relatively short period. More often, they found themselves leading employees that are older than themselves and yet their leadership traits and styles are not fully understood. The study explores factors affecting leadership styles of millennial managers and how they are manifested and applied in managing and resolving conflict involving older subordinates. Using a qualitative approach, it employs interviews, participant observation, and prolonged engagement with four millennial managers from the National Food Authority in the Philippines as they lead, manage, and interact with their older staff. Data collected were triangulated by document analysis and interview of the millennial managers’ subordinates themselves. The results of the study show that the participants’ leadership competencies are in the early stages of development. The participants consider building good impression and rapport highlighting the importance of soft skills in leadership, strong sense of respect, reverse mentoring, delegation of tasks, and the ability to handle expectations and performance as factors in managing older employees. This study draws the attention to examining the leadership attributes and potentials of Filipino millennial managers in the context of Philippine workplace culture using insights from Public Administration, Social Sciences, and Psychology.
