Assessment of Telemedicine and Phone Consultation Effects on Rate of Hand Transfers to Trauma Centers and Specialty Services Referrals

Background Telemedicine consultation can increase patients’ access to subspecialty care and decrease the number of unnecessary hospital transfers. In 2014, the Arkansas Hand Trauma Telemedicine Program (AHTTP) was established to extend specialized hand care throughout Arkansas. The purpose of this study is to assess whether transfers are affected when consultation with a hand specialist is performed by phone compared with using a live audiovisual consultation. Methods We reviewed data from the first year of the AHTTP. Data collection included type of consultation (telephone only or live audiovisual), need for transfer, and type of transfer (general orthopedic or hand specialist). Results In 2014, the first year of AHTTP there were 331 hand injuries identified; of those, 298 used the AHTTP with 195 (65%) using telemedicine and 103 (35%) using phone consultation only. The use of video when compared with phone consultation did not significantly affect the decision to transfer ( P = .42) or alter the rate of transfer for general orthopedic or hand specialist care ( P = .25). Conclusions The assessment of both phone and telemedicine modalities showed that there was no significant difference in transfer rates for either consultation, highlighting that communication with a hand surgeon was the key to accurate assessment of the need for transfer.

Students who receive architectural education are expected to be individuals who can think critically. For this reason, it becomes a necessity to cultivate creative individuals who are curious, inquiring and multi-faceted thinkers. The aim of this research was to determine the levels of tendency for critical thinking of first-year and fourth-year architecture students. The research sample consisted of 88 (54 females, 34 males) first-year and 83 (47 females, 36 males) fourth-year students who are studying at Karadeniz Technical University, Faculty of Architecture, Department of Architecture. The "California Critical Thinking Disposition Inventory (CCTDI)" was used as data collection tool in the research. In addition, information about the demographic characteristics of students were collected through the Personal Information Form. Findings of the research showed that there was no statistically significant difference between the GPAs and critical thinking tendency levels of the first-year and fourth-year students. It can be argued that all students need some guidance to attach importance on critical thinking starting from the first year of the program. This study aims to prove whether transformational leadership training is effective for building anti-corruption attitudes of villages in Kebonharjo village, subdistrict Samigaluh Kulonprogo. This research is an experimental research with one group pre and posttest design.Subject design is 17 people from village of 21 candidates registered. Measuring tool used in this research is the scale of anti-corruption perception made by the researcher referring to the 9 anti-corruption values with the value of reliability coefficient of 0.871. The module used as an intervention made by the researcher refers to the transformational leadership dimension (Bass, 1990). The data collected is analyzed by statistical analysis of different test Paired Sample Test. Initial data collection results obtained sign value of 0.770 which means> 0.05 or no significant difference between anti-corruption perception score between before and after training. After a period of less than 1 (one) month then conducted again the measurement of follow-up of the study subjects in the measurement again using the scale of anti-corruption perception. The results of the second data collection were analysed with Paired Samples Test and obtained the value of 0.623 sign meaning p> 0.05 or no significant difference between post test data with follow-up data so that the hypothesis of this study was rejected.

The introductory programming course, commonly known as CS1 and offered as a core course in the first year in all engineering programs in India, is unique because it can address higher cognitive levels, metacognition and some aspects of the affective domain. It can provide much needed transformative experiences to students coming from a system of school education that is dominantly performance-driven. Unfortunately, the CS1 course, as practiced in almost all engineering programs, is also performance-driven because of a variety of compulsions. This paper suggests that the inclusion of a course CS0 can bring about transformative learning that can potentially make a significant difference in the quality of learning in all subsequent engineering courses. The suggested instruction design of this course takes the advantage of the unique features of a course in programming. The proposed CS0 course uses “extreme apprenticeship” and “guided discovery” methods of instruction. The effectiveness of these instruction methods was established through the use of the thematic analysis, a well-known qualitative research method, and the associated coding of transformative learning experiences and instruction components.

Abstract Introduction The Queen Victoria Hospital (QVH) was designated a cancer and trauma hub during the COVID-19 pandemic. With this, a new virtual hand trauma clinic was set up. We assess accuracy of assessment within this virtual set-up with comparison to pre COVID-19 face-to-face assessment. Method Two weeks of clinic sessions during and pre lockdown were analysed. Initial assessment was compared with the patient's operation note. Results In the pre COVID-19 two-week period 129 face-to-face appointments were analysed. Of 99 patients that required surgery 77 (78%) had an accurate assessment. 6 were overestimated, 12 were underestimated. 189 patients were seen over two weeks during lockdown via telephone or video call. Accuracy of assessment increased with seniority of the clinician. Of 126 patients that required an operation 109 (87%) had an accurate assessment; all structures injured were correctly predicted. 12 were overestimated, 5 had their injury underestimated. Conclusions The new virtual clinic allowed patients to be remotely assessed during lockdown, reducing footfall and unnecessary journeys. We found that virtual clinic assessments are accurate, and no patient underwent an unnecessary procedure. Using a telephone call plus photo gave similar accuracy as a video call. Virtual assessment was more accurate than face-to-face assessment.

AbstractThe aim of this study was to examine the effect of scenario-based learning (SBL) compared to traditional demonstration method on the development of patient safety behavior in first year nursing students. During the 2016–2017 academic year, the Fundamentals of Nursing course curriculum contained the teaching of demonstration method (n=168). In the academic year 2017–2018 was performed with SBL method in the same context (n=183). Objective Structured Clinical Examination (OSCE) that assesses the same three skills was implemented in both academic terms to provide standardization so that students could evaluated in terms of patient safety competency. It was found that students’ performance of some of the steps assessed were not consistently between the demonstration and SBL methods across the three skills. There was a statistically significant difference between demonstration method and SBL method for students’ performing the skill steps related to patient safety in intramuscular injection (p<0.05) Our results suggest that the integration of SBL into the nursing skills training may be used as a method of teaching in order to the development of patient safety skills.

Abstract Background Appropriate management of inflammatory bowel disease (IBD) often requires multiple specialist appointments per year. Living in rural locations may pose a barrier to regular specialist care. Saskatchewan (SK) has a large rural population. Prior to COVID-19, telehealth (TH) in SK was not routinely used for either patient assessment or follow up. Furthermore, TH was exclusively between hospitals and specific TH sites without direct contact using patient’s personal phones. Aims The objective of this study was to assess the differences in demographics, disease characteristics, outcomes, and health care utilization between patients from rural SK with IBD who used TH and those who did not. Methods A retrospective chart review was completed on all rural patients (postal code S0*) with IBD in SK who were followed at the Multidisciplinary IBD Clinic in Saskatoon between January 2018 and February 2020. Patients were classified as using TH if they had ever used it. Information on demographics, disease characteristics, and access to IBD-related health care in the year prior to their last IBD clinic visit or endoscopy was collected. Data was not collected for clinic visits after March 1, 2020 as all outpatient care became remote secondary to the COVID-19 pandemic. Mean, standard deviations, median and interquartile ranges (IQR) were reported. Mann-Witney U and Chi-Square tests were used to determine differences between the groups. Results In total, 288 rural SK IBD patients were included, 30 (10.4%) used TH and 258 (89.6%) did not. Patient demographics were not significantly different between the two groups; although, there was a statistically significant difference in the proportion of ulcerative colitis patients (17% TH vs. 38% non-TH, p=0.02). The percentage of patients with clinical remission was 87% for TH patients and 74% for non-TH patients (p=0.13). There were no significant differences in health care utilization patterns and biochemical markers of disease, including c-reactive protein (CRP) and fecal calprotectin (FCP) (p&gt;0.05). Conclusions Prior to the pandemic, a small percentage of patients with IBD in rural SK ever used TH. A small proportion of UC patients used TH. No significant differences in disease characteristics, outcomes, or health care utilization were identified. Further study is warranted to identify barriers to use of this technology to tailor care to this patient group and improve access to care, especially now as the COVID-19 pandemic has drastically changed the use of virtual care. Biochar holds promise as a soil amendment with potential to sequester carbon, improve soil fertility, adsorb organic pollutants, stimulate soil microbial activities, and improve crop yield. We used a hardwood biochar to assess its impact on corn (Zea mays) grain, biomass yields and greenhouse gas emission in central Kentucky, USA. Six treatments included as follows: control (C) with no amendment applied; poultry litter (PL); biochar (B); biochar + poultry litter (B + PL); fertilizers N-P-K (F); and biochar + fertilizers (B + F). Biochar was applied only once to plots in 2010 followed by rototilling all plots. Only PL and fertilizer were applied annually. When applied alone, biochar did not significantly increase dry matter, grain yield, and N-P-K uptake. There was also no significant difference between the combined treatments when compared with PL or F applications alone. We observed a slight increasing trend in corn grain yield in the following 2 years compared to the first year from biochar treatment. Poultry litter treatment produced significantly greater N2O and CO2 emissions, but emissions were lower from the B+PL treatment. We conclude that this biochar did not improve corn productivity in the short term but has potential to increase yield in the long term and may have some benefit when combined with PL or F in reducing N2O and CO2 emissions.

Abstract The mobile app Schnëssen establishes a digital and participatory research platform to collect data on present-day spoken Luxembourgish through crowdsourcing and to present the results of data analysis to the general public. Users can participate in different kinds of audio recording tasks (translation, picture naming, reading, question) as well as in sociolinguistic surveys. All audio recordings are accessible to the public via an interactive map, which allows the participants to explore variation in Luxembourgish themselves. In the first year of data collection, roughly 210.000 recordings have be collected covering numerous variation phenomena on all linguistic levels. Additionally, over 2800 sociolinguistic questionnaires have been filled out. Compiling such amounts of data, the Schnëssen app represents the largest research corpus of spoken Luxembourgish.

This study aims to investigate the prospective teachers' epistemological beliefs and creativity fostering behaviour and changes with the same according to various variables. The study was designed based on the relational survey model. The sample of the research consisted of 420 prospective teachers. For the purpose of the study, the epistemological beliefs scale and creativity fostering teacher index were used as data collection tools. The difference between the epistemological beliefs and creativity fostering teacher index sub-dimensions according to gender, programme and university variables was examined by "Multivariate Variance Analysis MANOVA". As a result of the research, the prospective teachers' epistemological belief and creativity fostering behaviour were found to be average. However, when the changes in the sub-dimensions of epistemological belief scale according to gender and university variables were examined, the effect of both variables was significant. The changes in epistemological beliefs according to the class and education programme were not significant. There was also no significant difference in sub-dimensions of creativity fostering teacher index scales according to gender and programme and class and programme.
