In recent decades, the role played by the immune response to bacteria in the pathogenesis of chronic periodontal disease (PD) has long been studied. Although from the clinical point of view, adequate oral hygiene is essential to ensure a satisfactory response of the host to infections, modulation of the reaction of immune system could be influenced by genetic factors. The aim of the present study was to investigate the distribution of alleles of polymorphisms relevant for chronic periodontitis in a sample of adult subjects affected by chronic PD. The present study was conducted with sample collected in Italian private practice offices from January 2013 to December 2017. The sample included 744 adult patients diagnosed with chronic periodontitis. The inclusion criteria were as follows: age > 18 years, diagnosis of chronic PD. The diagnosis of chronic periodontitis was based on the criteria established by the American Academy of Periodontology. No significant difference in allele distribution among patients from different Italian regions was found. Results, supporting absence of population heterogeneity for the investigated polymorphisms in Italy, suggest similar effect in periodontitis etiology.

Introduction: Various chemical agents such as nonsteroidal, anti-inflammatory drugs and antimicrobial agents has gained popularity in treatment of periodontal disease but simultaneously lead to condition such as drug resistance and drug allergy. Hence , the topical application of herbal agents such as propolis, aloevera, green tea extracts, Neem reduces the potency and effectiveness to prevent progression of periodontal disease. NanoBioFusion(NBF)gel contains the natural antioxidant power of propolis,vit C,vit E which allows the ultrafine antioxidant to surpase the moist intraoral environment to enter the cells and rejuvenate,revitalize,support,protect and optimize gum and soft oral tissue.Hence the present study is aimed to evaluate the clinical effect of locally delivered NBF gel as an adjunctive therapy to scaling and polishing in the treatment of Periodontitis. Materials and Methods: Chronic Periodontitis patients with 40 sites and probing pocket depth (PD) between 5 and 7 mm were selected in a randomized controlled clinical trial. SRP was performed in both control and test group followed by NBF gel application in 40 sites. The plaque index, gingival index and probing Pocket depth,were recorded at baseline, 6 weeks, and 3 months.The statistical analysis with paired t‑test was used to compare the test and control sites. Results: From baseline to a period of 3 months, a statistically significant difference was seen between both groups for Pocket probing depth and from baseline to 6 weeks the mean GI and PI score have a statistically significant result was obtained(P=0.01& 0.00). Conclusions: Locally delivered NBF gel exhibited a significant improvement compared with SRP alone in chronic periodontitis.

ABSTRACT Objective: Oxidative stress (OS) refers to the disequilibrium between free radicals and antioxidant defense mechanisms and is significantly implicated in the pathogenesis of chronic degenerative and inflammatory diseases such as chronic periodontal disease (CP) and diabetes mellitus (DM). This study aimed to evaluate the total antioxidants capacity (TAOC) and total oxidants status (TOS) in the gingival crevicular fluid (GCF) in CP participants with type II DM. Materials and Methods: A total of 80 participants were allotted into four groups as follows: Group 1: Generalized CP (GCP) without type II DM (n = 20); Group 2: GCP with type II DM (n = 20); Group 3: Type II DM without CP (n = 20); and Group 4: Systemically and periodontally healthy (PH) (n = 20). Clinical parameters such as plaque index, gingival index, probing pocket depth, and clinical attachment level were recorded. Pooled GCF was collected followed by the estimation of TAOC, TOS, and OS index (OSI) using Erel O Colorimetric analysis. Results: The clinical parameters recorded showed the statistically significant difference (P < 0.001) between the groups. The mean TAOC value was the highest in PH group. The mean TOS and OSI were higher in Group 1, 2, and 3 participants when compared to the PH participants. All the biochemical parameters evaluated showed a statistically significant difference (P < 0.001) between groups. Conclusions: The study further validates the use of OSI as a marker for periodontal disease activity and emphasizes the role of OS in the pathogenesis of Type II diabetic patients with the chronic periodontal disease.

Objectives: Folic acid is a vitamin which belongs to the B-complex group. It is critical to cellular division and new cell production because it is an essential co-factor in DNA synthesis. Repair and maintenance of periodontal tissue generates a high turnover rate of squamous epithelium. Without folic acid, epithelial cells do not divide properly. Cigarette smoking is one of the factors that affect the folic acid levels. The aim of this study was to compare the serum folic acid levels in patients with chronic periodontal disease in relation to the patients’ smoking habits. Materials and Methods: A total of 60 subjects were included in the study with 30 subjects in each of the following groups, I - patients who have chronic periodontitis and are smokers and II - patients who have chronic periodontitis and are non-smokers. Clinical parameters like gingival index (GI), plaque index (PI), bleeding on probing (BOP), probing depth (PD) and clinical gingival attachment levels (CAL) were recorded for all the patients. Blood was collected and tested in the laboratory for folic acid levels using a fully automated serum analyser. The results were statistically analysed. Results: The results suggested that serum folic acid levels of smokers were significantly lower than that of non-smokers (p < 0.05). PI, PD and CAL means were significantly higher in Group I (chronic periodontitis and smokers) than Group II (chronic periodontitis and non-smokers). GI and BOP was lower in smokers. Conclusion: Among patients with periodontal disease the serum folic acid level is lower in smokers compared with non-smokers. Key Words: Chronic periodontitis; folic acid; smoking. There is lot of literature suggesting a gender based heterogeneity in many human chronic diseases including periodontal disease. To analyze and compare the clinical periodontal disease parameters & serum biochemical markers of periodontal inflammation in systemically healthy age stratified adult male and female patients suffering from chronic destructive periodontal disease. Cross sectional observational study in a hospital setting A total of 300 subjects, both genders were enrolled based on predefined criteria and were categorized in 6 groups of 50 subjects each. Complete medical and dental history was taken to screen before enrollment. All subjects underwent complete periodontal examination, including evaluation of Plaque index (PI), Probing pocket depth (PPD) and Clinical attachment level (CAL), Bleeding on Probing (BOP). Blood samples were taken for analysis of inflammatory biomarkers viz interleukin (IL)-1β, osteoprotegrin (OPG), matrix metalloproteinase (MMP)- 8 & interleukin (IL)- 6. IBM SPSS STATISTICS (version 22.0) Clinical parameters of periodontal status were higher in females as compared to males except BOP. However, only PPD and CAL showed significant difference. Higher serum levels of IL-1β, OPG and Il-6 were observed in females (2.10 + 26.82, 168.18 + 49.84 , 29.17 + 99.20 pg/ml) than males (1.90 + 7.27 , 145.00 + 39.60 , (25.83 + 189.09pg/ml) respectively, but significant difference was observed only for OPG. A statistically significant higher level of MMP-8 was observed in males (3003.33 + 772.33 pg/ml) as compared to females (1398.33 + 1218.10 pg/ml). The findings of current investigation has identified significant differences in the clinical and specific biochemical mediators(IL-1β, IL-6, OPG, MMP-8) across groups and subgroups of the population To ascertain the impact of gender and age in the causation and pathogenesis of inflammatory periodontal disease, further well designed prospective investigations are needed . The study findings point towards the identification of specific biomarkers in individual subgroup/group based on age and gender. These shall pave path to develop predictive models, screening tools and early diagnostic strategies for chronic periodontal disease for Indian population.

Background. Periodontal disease leading to clinical findings such as increased periodontal probing depth involves a complex interaction between invading pathogenic microorganisms and the patient's immune system. Lymphotoxin alpha (LT-α) is a potent multifunctional immune modulator that contributes toward susceptibility to immune regulation disorders, including periodontal disease. Objective. In this study, we tested the hypothesis that chronic periodontitis (CP) is associated with polymorphisms of the LT-α gene. Materials and Methods. A total of 126 subjects, 44 healthy subjects, and 82 subjects with CP, were evaluated for periodontal disease by measuring clinical attachment loss and separation. Samples of epithelial cells were obtained for DNA analysis by scraping of the buccal mucosa. The LT-α gene was analyzed by polymerase chain reaction followed by endonuclease digestion with NcoI to analyze restriction fragment length polymorphisms. Results. The LT-α gene (+252A/G) polymorphism was associated with CP. LT-α allele frequencies were significantly different (P=0.0019) between patients with CP and healthy individuals, with an odds ratio of 2.67 for patients with CP with the G allele. Conclusions. These findings suggest the LT-α gene genotype is a risk indicator for susceptibility to chronic periodontal disease in the Brazilian population studied.

Background/Aim. Healthy periodontium comprises the dento-gingival junction. Periodontal disease starts to appear when the integrity of the junctional epithelium is disturbed. Assessment of the supracrestal tissue attachment (SCTA) is essential because there is a frequent need for restoration or prosthesis after periodontal surgical and non-surgical therapy. The aim of the present study was to evaluate the SCTA variations in a patients with chronic periodontitis before and after treatment. Methods. Thirty systemically healthy patients with periodontitis were enrolled in the study. Fifteen patients were subjected to scaling and root planing and 15 to open flap debridement. Radiographic and clinical findings of the SCTA were assessed before and after treatment at 3-month and 6-month intervals. Results. Comparison between clinical and radiographic findings of the SCTA showed a significant difference in patients with periodontitis (p < 0.05). This difference was not significant after treatment of patients with shallow pockets with scaling and root planing (p > 0.05), but showed a significant difference in patients with moderate pockets treated by open flap debridement (p < 0.05). Conclusion. Progression in periodontal disease causes a reduction in the SCTA dimension, which regains its original dimensions after periodontal therapy. It takes around 3 months for the shallow pockets to regain the supracrestal tissue attachment to the original dimension when treated by scaling and root planing, whereas moderate pockets regain it after 6 months when treated with open flap debridement.

Introduction: Explorations into the periodontal medicine relationship have discovered interleukin-17 (IL-17) cytokine as one of the pro-inflammatory mediators that play a crucial role in the initiation and progression of the chronic periodontal disease. This case and control study aimed to compare the serum IL-17 concentration between chronic periodontitis and healthy subjects and to assess the relationship between the IL-17 serum and the clinical periodontal parameters in chronic periodontitis patients. Methods: This study was a case-control study. The subjects were selected using purposive sampling method. The periodontal screening assessment in the Family Treatment Center and Postgraduate Dental Clinic, Universiti Sains Malaysia Hospital. A total of 55 samples who fit with the inclusion and exclusion study criteria of full-mouth clinical periodontal records together with blood serums were obtained from 28 chronic periodontitis patients and from 27 healthy subjects. The concentration of IL-17 cytokine in serum was measured using ELISA test. Results: Subjects with periodontal disease presented significantly worse clinical parameters (p<0.001) compared to control. The level of serum IL-17 concentration was significantly higher (p=0.026) in chronic periodontitis subjects 3.6 (1.03) pg/mL compared to the control 3.1 (0.70) pg/mL. Conclusions: There was no correlation between the level of IL-17 concentration in the serum and clinical periodontal parameters.

ABSTRACT Objective: To evaluate the association of glycemic control and cytokine production in type 2 diabetic subjects with chronic periodontitis Methods: Gingival biopsies were performed in 40 patients, divided into four groups: systemically healthy subjects without periodontal disease (S); systemically healthy patients with chronic periodontitis (P); patients with well-controlled type 2 diabetes mellitus (DM) with chronic periodontitis (C); poorly controlled type 2 diabetes mellitus with chronic periodontitis (D). The production of interleukin (IL) -4, -6, -10, -17 and interferon (IFN) -γ was quantified by ELISA. Results: The production of IL-4, IL-10, IL-17 and INF-γ was higher on group D when compared to other groups (p <0.05), which in turn were similar (p ≥0.05). In addition, there was no difference in the production of IL-6 in any of the evaluated groups (p≥0.05). Conclusion: Were observed significantly elevated levels of pro-inflammatory and anti-inflammatory cytokines in patients with poorly controlled type 2 diabetes and chronic periodontitis, demonstrating that glycemic control may be associated to the immune inflammatory response of sites with chronic periodontitis.

The association between diabetes mellitus and chronic periodontal disease has long been established. Most of the researches linking these two very common chronic diseases were based on type 2 diabetes mellitus and chronic periodontal disease. However, this study was conducted to investigate the association between type 1 diabetes and chronic periodontal disease in Malaysian subjects. Forty-one Malaysian subjects, of which 20 subjects were type 1 diabetics and with chronic periodontal disease (test group) and 21 subjects with only chronic periodontal disease (control group), were included in the study. Periodontal parameters and plaque samples for microbiological evaluation were done at baseline, 2 and 3 months after nonsurgical periodontal therapy. Blood samples were taken from only the test group and evaluated for HbA1c at baseline and 3 months after periodontal therapy. There were no statistically significant difference in periodontal parameters between groups (P>0.05) and no significant improvement in the level of HbA1c in the test group. Microbiological studies indicated that there were significant reductions in the levels of the tested pathogens in both groups. The results of our study were similar to the findings of several other studies that had been done previously.

Objective: Reactive oxygen species have been identified as potential factors causing periodontal tissue destruction. Elevated levels of these in patients with chronic periodontitis and diabetes may aggravate the oxidative stress burden thereby accelerating the tissue damage associated with diabetes. The present study aimed to assess the effect of diabetes and periodontal disease on the oxidative stress markers and the effect of non-surgical therapy on these markers.Methods: A total of 50 participants were divided into four groups based on the selection criteria. In addition to clinical parameters and biochemical parameters (salivary nitric oxide [NO] and malondialdehyde [MDA] levels) were assessed using spectrophotometric method at baseline and 3 months after non-surgical periodontal therapy.Result: There was a statistically significant difference in the clinical parameters as well as NO and MDA levels in patients with type 2 diabetes and chronic periodontitis than other three groups at baseline and at 3 months after non- surgical periodontal therapy.Conclusion: Diabetes mellitus and periodontitis have a compounding effect on the oxidative stress. Periodontal therapy is essential for diabetic patients as it can lower the levels of oxidative damage.
