Abstract Background AarF domain-containing kinase 4 (ADCK4)-associated glomerulopathy is a mitochondrial nephropathy caused by mutations in the ADCK4 gene, which disrupt coenzyme Q10 biosynthesis. Case presentation We report the case of a 25-year-old female patient with ADCK4-associated glomerulopathy presenting with proteinuria (and with no additional systemic symptoms). A known missense substitution c.737G > A (p.S246N) and a novel frameshift c.577-600del (p.193-200del) mutation were found. We followed the patient for 24 months during supplementation with coenzyme Q10 (20 mg/kg/d – 30 mg/kg/d) and describe the clinical course. In addition, we measured serum and urine coenzyme Q10 levels before and after coenzyme Q10 supplementation and compared them with those of healthy control subjects. The patient’s urinary coenzyme Q10 to creatinine ratio was higher than that of healthy controls before coenzyme Q10 supplementation, but decreased consistently with proteinuria after coenzyme Q10 supplementation. Conclusions Although the use of urinary coenzyme Q10 as a diagnostic biomarker and predictor of clinical remission in patients with ADCK4-associated glomerulopathy should be confirmed by larger studies, we recommend measuring urinary coenzyme Q10 in patients with isolated proteinuria of unknown cause, since it may provide a diagnostic clue to mitochondrial nephropathy.

The effects of dietary supplementation with Coenzyme Q10 (CoQlO), a reputed performance enhancer and antioxidant, on physiological and biochemical parameters were examined. Ten male bicycle racers performed graded cycle ergometry both before and after being given 100 mg per day CoQlO or placebo for 8 weeks. Analysis of variance showed a significant difference between groups for postsupplementation serum CoQ10. Although both groups demonstrated training related improvements in all physiological parameters over the course of the study, there were no significant differences between the two groups (p>.05). Both groups showed a 21 % increase in serum MDA (an index of lipid peroxidation) after the presupplementation exercise test. After 8 weeks this increase was only 5 % , and again was identical for both groups. Supplementation with CoQlO has no measurable effect on cycling performance,, submaximal physiological parameters, or lipid peroxidation. However, chronic intense training seems to result in marked attenuation of exercise-induced lipid peroxidation.

Background: Occurrence of cellular damage in various tissues of the body and inflammatory response is evident during intense sporting competitions, such as half-endurance running racing. Therefore, the aim of this study was to determining the effect of acute and 14-day Coenzyme Q10 supplementation on inflammatory, anti-inflammatory, muscle and hepatic damage indicators response in elite male runners. Methods: In quasi-experimental study, eighteen elite male middle-distance runners (aged 20.45±2.48 years, and body fat 7.4 ±1.63 % and VO2max 60.85 ± 3.96) in a randomized and double-blind design were allocated in two equal groups: supplement group (n=9, Coenzyme Q10: 5 mg/kg/day) and placebo group (n=9, Dextrose: 5 mg/kg/day). Before and after supplementation acute and 14-day period, all subjects were participated in a training like running a competitive 3000 meters. Leukocyte count, serum Alanine Aminotransferase (ALT) and Aspartate Aminotransferase (AST) and interleukin-10 (IL-10) concentration were analyzed before and after two like competitive running 3000 meters and supplementation protocols. Data were analyzed by repeated measure ANOVA, Bonferroni and independent t test, with a <0.05 significance level. Results: The short-term coenzyme Q10 supplementation attenuated the exercise-induced increase in response of ALT and AST and Leukocyte count (p<0.05). However, acute supplementation had no significant effect on any of the parameters. Also, short-term supplementation had no significant effect on baseline and exercise-induced response of IL-10. Conclusion: Based on the results, Short-term supplementation of coenzyme Q10 may reduce markers of liver damage and muscle response to competitive and exhaustive exercise.

Abstract Background Cytomegalovirus (CMV) is an important pathogen among immunocompromised hosts. Typically, CMV in human immunodeficiency virus (HIV) infection causes diseases of the retina, digestive tract, lungs and liver, but there are few cases of CMV infection of the pharynx and larynx. Case presentation A 57-year-old man with HIV infection was admitted because of pharyngeal pain. Before and after admission, pharyngeal biopsies guided by laryngeal endoscopy were performed four times, but pathological examination showed nonspecific inflammation, and the cause of pharyngeal ulceration was unclear. Additionally, the ulceration deteriorated after initiation of retroviral therapy. Laryngomicrosurgery was conducted under general anesthesia to remove tissue, and pathological diagnosis confirmed CMV infection. Pathological features included enlargement of the cytoplasm and nucleus in infected cells, and intranuclear bodies called owl’s eye inclusions. Ganciclovir dramatically improved the symptoms and laryngoscopic findings. Conclusions This case was diagnosed as pharyngitis and pharyngeal ulceration caused by CMV infection, related to immune reconstitution inflammatory syndrome. In previous reports of CMV-induced pharyngeal or laryngeal ulceration in HIV infection, we found six cases similar to our present case. All cases were diagnosed by biopsy. The present case indicates the importance of biopsy for definitive diagnosis. CMV infection should be considered as a differential diagnosis of pharyngeal ulceration in patients with HIV infection.

Galectin-3 (gal-3) is a multifunctional regulator of various biological processes and diseases, which are common comorbidities in psoriasis. Data regarding potential diagnostic role of gal-3 in psoriasis are insufficient. Serum gal-3 levels were evaluated before and after twelve weeks of treatment with acitretin or methotrexate in 31 patients with plaque-type psoriasis and compared to 11 healthy control group. The mean serum galectin-3 level in patients with psoriasis was significantly higher compared to the control group (p < 0.01). In patients with obesity and long-lasting psoriasis (>20 years) positive relations of gal-3 and PASI were noted. In psoriatics with low gal-3 levels, positive correlations between the gal-3 and BMI, glucose level, and with the latter in short-lasting psoriasis (<20 years) were noted. In the long history of psoriasis, gal-3 was negatively correlated with lipids levels. The Gal-3 level might be a multifaceted modulator of the course of psoriasis and predictive factor of cardiometabolic comorbidities’ development, especially in patients with a long history of the disease or obesity. Patients with low serum gal-3 and short history of psoriasis are presumably at greater risk of diabetes. In patients with long-lasting psoriasis and concomitant obesity, gal-3 may exert a protective role against dyslipidemia or perhaps further CMD development.
