Abstract Background: Vitamin D deficiency, a prevalent worldwide concern, has been reported to have a pivotal role in many cardiovascular and inflammatory diseases. Percutaneous coronary intervention (PCI) is a therapeutic approach that may have several short-term and long-term cardiovascular complications, supposing to be mediated with high sensitive C-reactive protein (hs-CRP), an inflammatory marker. This study aimed to investigate the association between vitamin D status and hs-CRP level, as a prognostic marker, in patients undergoing elective PCI. Methods: The study population comprised of 150 patients who underwent elective coronary angioplasty. In order to measure vitamin D and hs-CRP levels, venous blood samples were obtained at baseline and 24 hours after PCI. Results: Our results showed a significant inverse relationship between post-PCI hs-CRP and also pre-post hs-CRP difference levels and vitamin D status. Conclusion: Hs-CRP content is higher in vitamin D deficient patients. Therefore, these patients, especially severely-deficient ones (25(OH)D<10ng/ml), may benefit from supplementation with vitamin D prior to PCI.

<p>Proses penuaan mempengaruhi peningkatan distribusi lemak abdominal dengan indikator lingkar pinggang (LP) dan rasio lingkar pinggang panggul (RLPP). Penimbunan lemak abdominal menyebabkan disfungsi jaringan adiposa sehingga mempengaruhi biomarker proinflamasi yaitu kadar serum <em>high-sensitivity C-reactive Protein</em> (hs-CRP). Penelitian bertujuan untuk mengetahui hubungan LP dan RLPP dengan kadar hs-CRP lansia wanita. Rancangan penelitian <em>cross sectional</em> pada 53 subjek dipilih secara <em>consecutive sampling</em>. Antropometri yang diukur adalah LP dan RLPP. Pengukuran kadar serum hs-CRP dianalisis dengan metode <em>enyme-linked immunosorbent assay</em> (ELISA). Wawancara yang dilakukan yaitu data diri, asupan, aktivitas fisik, dan riwayat konsumsi obat. Data asupan diperoleh dengan metode <em>food recall</em> 3x24 jam. Aktivitas fisik diperoleh menggunakan <em>International Physical Activity Questionnaire</em> (IPAQ). Analisis data digunakan uji korelasi <em>Spearman</em>. Hasil penelitian menunjukkan bahwa persentase LP pada lansia wanita yang berisiko sebesar 90,6%, RLPP yang berisiko sebesar 98,1%, dan kadar hs-CRP tinggi sebesar 30,2%. Terdapat hubungan positif antara LP dengan kadar serum hs-CRP (r=0,417 ; p=0,002). Dalam penelitian ini RLPP, aktivitas fisik, asupan energi, karbohidrat, protein, lemak, serat, vitamin A, vitamin C, vitamin D,vitamin E, dan selenium tidak berkorelasi dengan kadar hs-CRP. Simpulan penelitian ini adalah LP berkorelasi positif dengan kadar serum hs-CRP, namun RLPP tidak berkorelasi dengan kadar serum hs-CRP</p>

Abstract Background Competing risk method has not been used in a large-scale prospective study to investigate whether increased levels of high-sensitivity C-reactive protein (hs-CRP) elevate the risk of primary liver cancer (PLC). Our study aims to prospectively investigate the relationship between hs-CRP and new-onset PLC. Methods and results Ninety-five thousand seven hundred fifty-nine participants without the diagnosis of PLC, and who had their demographic characteristics and biochemical parameters recorded, were analyzed from the Kailuan Cohort study. Cox proportional hazards regression models and competing risk regression models were used to evaluate the hazard ratios (HRs) and 95% confidence intervals (95% CIs) of PLC. During a median follow-up of 11.07 years, 357 incidental PLC cases were identified over a total of 1,035,039 person-years. The multivariable HRs (95%CI) for the association of hs-CRP of 1–3 mg/L group and hs-CRP>3 mg/L with PLC were 1.07(0.82 ~ 1.38), 1.51(1.15 ~ 1.98) in a Cox proportional hazard regression analysis adjusted for other potential confounders. In the cause-specific hazard model, the multivariable HRs (95%CI) for the association of hs-CRP of 1–3 mg/L group and hs-CRP>3 mg/L with PLC were 1.06(0.81 ~ 1.40), 1.50(1.14 ~ 1.99). Similar results were also observed in the sub-distribution hazard function model with corresponding multivariate HRs (95%CI) of 1.05(0.80 ~ 1.40), 1.49(1.13 ~ 1.98) in hs-CRP of 1–3 mg/L group and hs-CRP>3 mg/L group, respectively. Conclusions This prospective study found a significant association of higher levels of hs-CRP with new-onset PLC. The main clinical implications would be an increased awareness of hs-CRP and its correlation to the risk of PLC. This study should be a steppingstone to further research on chronic inflammation and PLC. Trial registration Registration number:ChiCTR–TNRC–11001489.

Context: Endocan is a marker showing endothelial dysfunction and inflammation. Significantly increased endocan levels have been observed in serum of patients with sepsis and cancer. Objective: Our aim was to investigate the relationship between vitamin D treatment and serum endocan and high-sensitivity C-reactive protein (hs-CRP) levels as inflammatory markers in transplant patients. Design: Prospective. Setting: Nephrology clinic. Patients: Thirty-eight renal transplant patients with serum 25-hydroxy-vitamin D (25-OH-vitamin D) levels below 20 ng/mL and transplanted at least 12 months. Intervention: One-time oral dose of 300 000 IU vitamin D3. Main Outcome Measures: Before and after vitamin D treatment, serum endocan, hs-CRP, calcium, phosphorus, and parathyroid hormone (PTH) levels were measured. Results: Median serum endocan and PTH values before vitamin D were significantly higher than those of after treatment values ( P = .001 and P < .001, respectively). On the other hand, serum total calcium and phosphorus levels before vitamin D treatment were lower than the values obtained after vitamin D treatment ( P = .0013 and P < .001, respectively). Serum hs-CRP was lower after vitamin D therapy than before, but the difference was not statistically significant ( P = .06). A moderate negative correlation was determined between endocan and 25-OH-vitamin D levels after treatment with vitamin D ( r = −.36, P = .02). Conclusion: This study has revealed that vitamin D treatment reduced markers of endothelial dysfunction in patients with renal transplantation and vitamin D deficiency.

Aim: This study aimed to investigate the mediating effect of C-reactive protein (CRP) on obesity and hyperglycemia. Materials & methods: Fasting blood glucose, high-sensitivity CRP (hs-CRP) levels and waist circumference (WC) were measured on 353 participants. Multilevel regression modeling and mediation analyses were used to investigate the link between abdominal obesity, hs-CRP and hyperglycemia. Results: Elevation in hs-CRP was predictive of hyperglycemia in nonobese individuals (OR = 1.3, p = 0.03). With every 1-mg/l increase in hs-CRP, there was a 1-cm increase in WC (B = 0.87, p = 0.001). hs-CRP was a full mediator in the relationship between WC and hyperglycemia. Conclusion: hs-CRP predicts hyperglycemia development in nonobese individuals and the effect of increased WC on hyperglycemia was fully mediated by hs-CRP.

The aim of the study was to compare the effect of the administration of a mixture of fibres on body weight-loss, satiety, lipid profile and glucose metabolism. We included 200 overweight or obese patients in a parallel, double-blind, placebo-controlled clinical trial, who were randomised to receive, in the context of an energy-restricted diet for a period of 16 weeks, a mixed fibre dose (3 gPlantago ovatahusk and 1 g glucomannan) twice (b.i.d. group) or three times daily (t.i.d. group) or placebo. Weight change was the primary efficacy endpoint. Satiety, dietary compliance, lipid profile, glucose tolerance, insulin resistance and high-sensitivity C-reactive protein were secondary endpoints. Weight loss tended to be higher after both doses of fibre ( − 4·52 (sd0·56) and − 4·60 (sd0·55) kg) than placebo ( − 0·79 (sd0·58) kg); the differences in changes between groups were not statistically significant. Postprandial satiety increased in both fibre groups compared to the placebo. The differences between groups in LDL-cholesterol levels were significant (P = 0·03), with greater reductions in the two fibre-supplemented groups ( − 0·38 (sd0·10) and − 0·24 (sd0·09) mmol/l in the b.i.d. and t.i.d. groupsv.− 0·06 (sd0·09) mmol/l in placebo group). A similar pattern was observed for changes in total cholesterol:HDL-cholesterol and HDL-cholesterol:LDL-cholesterol ratios. Interventions were well tolerated and had no effects on HDL-cholesterol, glucose and insulin concentrations, glucose tolerance or high-sensitivity C-reactive protein. In conclusion, a 16-week dietary supplement of soluble fibre in overweight or obese patients was well tolerated, induced satiety and had beneficial effects on some CVD risk factors, the most important of which was a significant decrease in plasma LDL-cholesterol concentrations.
