Abstract Over the past 20 years, the rheumatoid arthritis (RA) treatment landscape has been continuously evolving. A range of novel biologic agents, different from the conventional therapies, became available. However, some understandable concerns, such as long-term safety, accompanied their development. Over the years in rheumatology research, I aimed to broaden the knowledge of the new treatments of RA through real-word research, which proved to be valuable in providing important evidence to clinicians and enabling them to make informed treatment decisions. Nevertheless, many unanswered questions remain—it will be interesting to see how the research progresses over the next 20 years.

Back Ground: Depression is a natural disturbance of feeling that is characterized by feelings of sadness, loss of interest and easily tired. Steroids are drugs that can reduce swelling, pain and heat due to inflammation through reducing the immune response. Steroids are often used in cases of systemic Lupus Erythematosus in the long term. Therefore management of the disease must be done well because steroids can cause depression. Case Report:Women, 37 years old, depressed mood, disappointment in life, loss of enthusiasm, fatigue, decreased appetite and difficulty sleeping for 1 year. 4 years ago I took steroids for 2 years at a dose of 20mg / day, because rheumatoid arthritis was stopped by os, and for the past 1 year, steroid consumption was due to Systemic Lupus Erythematosus around 60 mg / day. . Before the os consumes steroids, the OS has never experienced depression.BP: 110/70 mmHg , HR 88x/menit, RR 20x/menit, Temp 37°C. Skor BDI 21. Laboratorium Hb 11,8 g/dl, Leukosit 7500/mm3, trombosit 201.000/mm3,,Kalium 2,8, KGDad 99 mg/dlSGOT 29 ,SGPT32, Ureum 15,78 mg/dl, Creatinin 0,65 mg/dl.  Tot Colesterol 198mg/dl,  LDL 128 mg/dl, HDL 35 mg/dl Fototoraks : Jantung dan Parudalambatas normal, FotoLumbosakral : SpondilosisLumbalis. The patient is diagnosed with depression. Given psychotherapy, sandepril 2 x 25 mg for 3 months. Patients show clinical improvement marked by reduced sadness and can understand the disease. Conclusion :Steroid-induced depression has been reported after psychotherapy and sandepril 2 x 25 mg of the patient's condition showed improvement.

Autoimmune diseases occur when the body begins to attack normal cells instead of fighting off diseases and infections. There are ways of treating autoimmune diseases, but these provide only short-term relief, and there is no cure. Therefore, relapse tends to be an inevitable part of autoimmune diseases. Dr Yuya Yoshida is a specialist in immunology based in the Department of Pathological Biochemistry, Faculty of Pharmaceutical Sciences, Setsunan University, Japan, who is investigating the possibility of inducing immune tolerance and, in doing so, eliminating the need for long-term treatment. He and his team are working to devise a treatment strategy that enables complete short-term treatment and can then maintain long-term remission without the need for drug treatment. The ultimate goal for the team is a breakthrough in the treatment of autoimmune diseases, which would have far-reaching benefits for patients and the field of medicine. A key focus for Yoshida and his team is how an immunomodulating medication called fingolimod (FTY720) could be used to induce immune tolerance and, in doing so, stop the cycle of remission and relapse. There is potential for FTY720 to be used to develop new treatments and eliminate the need for patients to rely on long-term treatment. In particular, the researchers are focusing on multiple sclerosis and rheumatoid arthritis. One investigation involves the use of animal models to explore the construction of effective induction of immune tolerance to rheumatoid arthritis using FTY720.

Currently, developing the high-quality human resources to meet the socio-economic development requirements of Vietnam in general and Quang Ngai province in particular is a very urgent need, is a very urgent need. Due to the specific requirements of industries with modern technology, Quang Ngai is required to have highly qualified and skilled human resources. Being well aware of this, Quang Ngai province has paid great attention to the development of high-quality human resources and initially achieved certain achievements over the past time.

The objective of this study was to analyze the trends in seven annual extreme indices of precipitation for Idaho, USA. The analyses were conducted for 35 meteorological stations, during the period from 1970 to 2006, a high quality and a fairly long-term dataset. The analyses of precipitation indices presented large spatial variability and few statistically significant trends. Thus it is not possible to conclude that significant changes in precipitation have occurred in this region during the past few decades.Keywords:Climate change, global warming, environmental impact Tendências nos Índices Extremos de Precipitação Diária sobre Idaho - USA  RESUMO O objetivo deste estudo foi analisar as tendências em sete índices anuais extremos de precipitação para Idaho, EUA. As análises foram conduzidas para 35 estações meteorológicas, durante o período de 1970 a 2006, com dados de alta qualidade e longo prazo. As análises dos índices de precipitação apresentaram alta variabilidade espacial e poucas tendências estatisticamente significativas. Logo, não é possível concluir que ocorreram mudanças significativas na precipitação nesta região durante as últimas décadas. Rheumatoid arthritis is a chronic progressive disease causing substantial morbidity and mortality for which current treatments are largely unsatisfactory. Over the past 20 years we have developed a novel therapeutic approach based on the intra-articular administration of rifamycin. The published studies on rifamycin therapy of rheumatoid arthritis and other chronic arthritic disorders, mainly from our group, are reviewed. Our results indicate that intra-articular rifamycin is effective against active synovitis and can profitably be combined with any basic therapy with slow-acting antirheumatic drugs. There is good evidence that the development of new erosions can be prevented or delayed by this treatment and that the balance between side-effects (mainly local pain) and antiarthritic activity is very favourable in the long term. Our observations have led us to hypothesize a possible systemic effect of rifamycin injected multilocally in peripheral joints; we believe that the available data deserve further investigation by independent researchers.
