Abstract Objectives MicroRNAs (miRNAs) are short single-stranded RNAs that play a role in the post-transcriptional regulation of gene expression. Their deregulation can be associated with various diseases, such as cancer, neurodegenerative, and immune-related diseases. The aim of our study was to compare miRNA levels in plasma that could potentially influence the progression of hyperuricemia to gout, since the mechanism of progression is still unclear. Methods Total RNA, including miRNA, was isolated from the plasma of 45 patients with asymptomatic hyperuricemia, 131 patients with primary gout (including 16 patients having a gout attack), and 130 normouricemic controls. The expression of 18 selected miRNAs (cel-miR-39 and cel-miR-54 as spike-in controls, hsa-miR-16-5p and hsa-miR-25-3p as endogenous controls, hsa-miR-17-5p, hsa-miR-18a-5p, hsa-miR-30a-3p, hsa-miR-30c-5p, hsa-miR-126-3p, hsa-miR-133a-3p, hsa-miR-142-3p, hsa-miR-143-3p, hsa-miR-146a-5p, hsa-miR-155-5p, hsa-miR-222-3p, hsa-miR-223-3p, hsa-miR-488-3p and hsa-miR-920) was measured using qPCR. Results We found that hsa-miR-17-5p, hsa-miR-18a-5p, hsa-miR-30c-5p, hsa-miR-142-3p, and hsa-miR-223-3p were significantly upregulated (p < 0.001) in the plasma of hyperuricemia and gout patients compared to normouricemic individuals. As part of the follow-up of our previous study, we found a negative correlation between hsa-miR-17-5p, hsa-miR-30c-5p, hsa-miR-126-3p, hsa-miR-142-3p, and hsa-miR-223-3p with plasma levels of chemokine MCP-1. Additionally, we found a positive correlation between CRP and plasma levels of hsa-miR-17-5p, hsa-miR-18a-5p, hsa-miR-30c-5p, hsa-miR-126-3p, hsa-miR-142-3p, hsa-miR-146a-5p, hsa-miR-155-5p, hsa-miR-222-3p, and hsa-miR-223-3p. Five of those miRNAs (hsa-miR-126-3p, hsa-miR-142-3p, hsa-miR-146a-5p, hsa-miR-155-5p, and hsa-miR-222-3p) also had a positive correlation with serum creatinine and therefore a negative correlation with eGFR. Conclusion Five miRNAs were significantly upregulated in the plasma of patients with hyperuricemia and gout (and those during a gout attack) compared to normouricemic controls. We also found a correlation between the plasma levels of several miRNA and plasma levels of MCP-1, CRP, serum creatinine, and eGFR.

SynopsisFasting plasma levels of tryptophan, kynurenine and the pteridines, neopterin and tetrahydrobiopterin were measured in seven patients with Gilles de la Tourette syndrome (GTS) and 10 healthy controls. Plasma kynurenine was significantly elevated in the GTS patients. The lowest patient value was higher than the highest control value. Values for tryptophan, neopterin and tetrahydrobiopterin were similar in TS patients and controls. However, in TS patients only, there was a significant negative correlation between tryptophan and neopterin and a significant positive correlation between kynurenine and neopterin when controlling for tryptophan. This finding indicates that activation of cellular immune processes is a possible explanation for the rise in plasma kynurenine.

Introduction: Posterior urethral valve (PUV) is the most frequent cause of urethral obstruction in male child. These lesions usually result in lifelong disabilities with incontinence and decreased renal function despite optimal medical management. Primary fulguration without upper tract diversion is the preferred modality of treatment in most cases of PUV. Regular follow-up is needed to check completion of valve fulguration, renal function, status of hydronephrosis, vesicoureteric reflux (VUR), urinary tract infection (UTI), and bladder function.Materials and methods: We conducted interventional study among 30 purposively selected patients of PUV in the Department of Pediatric surgery, Dhaka Medical College Hospital (DMCH), Dhaka, over a period of 20 months from December 2009 to July 2011. Age of study subjects varied from 2 days to 14 years. Among the 30 patients, 16 were children in the age group between 1year to14 years (53.3%), 11(36.7%) were infants and the rest 03(10%) were neonates. Most of the patients presented with weak urinary stream, dribbling of urine, straining at micturition, UTI and palpable bladder. All children were subjected to ultrasonography (USG), blood urea, serum creatinine, routine urine examination and culture studies. Structured questionnaire was used to collect information regarding improvement or disappearance of VUR and renal functional status before & after primary fulguration of PUV.Results: Average serum creatinine level was found gradually decreased in subsequent follow up in comparison with the previous one. This difference of creatinine level was found statistically significant in t-test (p<0.01). Average blood urea nitrogen (BUN) was also decreased which was found statistically significant (p<0.05). VUR was present in 63.3% cases. Non- VUR was found in 60% cases on right side and 50% cases on left side. On the third follow-up after 3 months it became 73.3% on right side and 63.3% on left side. Positive correlation found in Pearson correlation test about the changes of reflux grades before and after fulguration was significant at the level of 0.01(p<0.001). It was significant on both left and right kidneys. Positive correlation found in Pearson correlation test about the changes of GFR before and after fulguration was also significant at the level of 0.01(p <0.001). Collected data was cleaned, edited and analyzed with the help of software SPSS window version 15.0.Conclusion: In this study, VUR disappeared in some cases and decreased in majority of the cases by 3 months after adequate restoration of urethral patency. Background: MiRNAs are a class of small non-coding RNAs that are involved in the post-transcriptional regulation of gene expression. MiRNAs are considered a class of epigenetic biomarkers. These biomarkers can investigate a disease at different stages: diagnosis, therapy or clinical follow up. Objective: The aim of this paper is to highlight the innovative use of miRNAs in several children diseases. Methods: We conducted a literature review to search the usage of miRNAs in pediatric clinical routine or experimental trials. Results: We found a possible key role of miRNAs in different pediatric illness (metabolic alterations, coagulation defects, cancer). Conclusion: The modest literature production denote that further investigation are needed to assess and validate the promising role of miRNAs as non-invasive biomarkers in pediatric disorders.

Abstract Background: Sarcoidosis is a systemic granulomatous disease of unknown etiology with a significant heterogeneity in organ manifestations, severity, and clinical course. Subjects with sarcoidosis share several features such as, non-necrotizing granuloma, hypergammaglobulinemia, increased local and circulating inflammatory cytokines, such as interleukin (IL)-6, IL-18, and interferon gamma (IFN-γ). Macrophage migration inhibitory factor (MIF) is a pluripotent chemokine produced by various cell types. The expression of MIF at sites of inflammation suggests a regulatory role in the function of macrophages. The objective of this study was to investigate the role of MIF in the serum and bronchoalveolar lavage (BAL) fluid of sarcoidosis patients in association with clinical features and other cytokines. Methods: Sera and BALs of sarcoidosis patients (n=55) were collected at the time of diagnosis and patients were followed longitudinally for 3 years. Additionally, fifteen healthy controls participated in the study. The medical records of all patients including, demographics, radiography stages, pulmonary function tests, and organ involvements were recorded. The levels of MIF, IL-18, IL-10, IL-6, IFN-γ and lysozyme in serum and BAL samples were measured by ELISA. Statistical analyses were performed using SPSS software. Results: Serum MIF had a remarkable positive correlation with IL-10 and IFN-γ but had a negative correlation with serum IgG levels. Importantly, longitudinal follow-up showed a positive correlation between MIF and % predicted diffusion capacity (%DLCO) at 3-year. Serum IL-18 had a significant positive correlation with serum lysozyme, but a negative correlation with % predicted total lung capacity at 3-year follow up. We identified two groups of sarcoidosis subjects with distinct clinical and cytokine features. A group with prominent extrapulmonary involvement, and low serum MIF, IL-10 and IFN-γ levels and a group with elevated serum MIF, IL-10 and IFN-γ levels. Moreover, we found a negative correlation between BAL IL-18 and BAL MIF in sarcoidosis subjects.Conclusions: Patients with low serum MIF, IL-10 and IFN-γ levels has severe and mostly extrapulmonary sarcoidosis with elevated lysozyme and IL-18 levels. Our work provides understanding of phenotypic diversity in association with heterogeneity in cytokine landscape in sarcoidosis.

SummaryThe use of plasma thrombospondin (TSP) concentration was investigated as an indicator of intravascular platelet activation. Patients (n = 20) with diseases that have known vasculitis were included in the study. The range and the mean of plasma TSP concentrations of patients with vasculitis were 117 ng/ml to 6500 ng/ml and 791±1412 ng/ml (mean ± SD); the range and the mean of plasma TSP concentrations of control individuals (n = 33) were 13 ng/ml to 137 ng/ml and 59±29 ng/ml. When plasma TSP concentrations were correlated with plasma concentrations of another platelet activation marker, β-thromboglobulin (P-TG), it was found that the TSP concentration inei eased exponentially as the plasma β-TG level rose. A positive correlation between plasma levels of plasma TSP and serum fibrin degradation products was also observed. The results suggest that platelets are the primary source of plasma TSP in patients with various vasculitis and that plasma TSP can be a better indicator than β-TG to assess intravascular platelet activation due to its longer circulation half life.

Background: Data about efficacy of Tenofovir in patients of HBV –related cirrhosis in Vietnam was still limited. This study is aimed at: - evaluating the clinical, biochemical, virological and Child-Pugh score responses 3,6,9 months after Tenofovir therapy; - assessing possible side effects of tenofovir. Patients and methods: 40 patients with HBV-related cirrhosis were enrolled. All has received Tenofovir disoproxil fumarate 300mg/day. Follow-up after 3,6 and 9 months. Results: Anorexia, oedema and ascites were significantly improved after treatment. HBV DNA became undetectable in 92.5%, 94.55 and 100% after 3,6 and 9 months, respectively. Child- Pugh score was improved after treatment (5.94±0.22 after treatment vs 7.47±0.28 before treatment). Side effects were minors (nausea, vomiting). No case of increase in serum creatinine was found. Conclusion: Tenofovir showed effective and safe in patients of HBV-related cirrhosis. Key words: Cirrhosis, tenofovir, HBV. The article deals with the interconnection and interdependence of phraseological semantic fields of Russian study of aphorisms in synchronic and in diachronic approaches. The correlation of phraseological semantic fields of Russian study of aphorisms is considered as their interdependence due to various factors (linguistic and extra-linguistic ones). The correlation of the phraseological semantic fields of Russian study of aphorisms is manifested in the existence of many antinomies. The natural linguistic antinomies of life / death / immortality or war / peace, or good / evil, or friend / enemy, or villainy / nobility are reflected in Russian aphorisms and have entered the corresponding phraseological semantic fields. The corpus of Russian study of aphorisms containsnot only antinomic aphorisms, but also antinomic relations that extend to the level of language and speech. Therefore, in Russian study of aphorisms there are phraseological semantic fields that implement these antinomies. In addition to the antinomic phraseological semantic fields in the corpus of classical Russian study of aphorisms there are other types of correlations. The keywords (concepts) of many phraseological semantic fields are closely thematically connected. When the number of units from one field is changed, the number of units in another phraseological semantic field also changes. Most phraseological semantic fields of Russian study of aphorisms do not show a zero correlation in either synchronic or diachronic approaches. This is due to, first of all, the universality of the aphoristic theme, with all the ideological and thematic uniqueness of the sayings used by Russian writers. However, a few phraseological and semantic fields of aphorisms by Russian writers may show a negative correlation, which is due to the diversity of the thematic groups that comprise them. A positive correlation of phraseological semantic fields, the most significant in the number of their constituent components, shows deep internal linguistic systemic connections in Russian classical study of aphorisms.

MicroRNAs (miRNAs) are non-coding RNAs with 19 to 24 nucleotides which are evolutionally conserved. MicroRNAs play a regulatory role in many cellular functions such as immune mechanisms, apoptosis, and tumorigenesis. The main function of miRNAs is the post-transcriptional regulation of gene expression via mRNA degradation or inhibition of translation. In fact, many of them act as an oncogene or tumor suppressor. These molecular structures participate in many physiological and pathological processes of the cell. The virus can also produce them for developing its pathogenic processes. It was initially thought that viruses without nuclear replication cycle such as Poxviridae and RNA viruses can not code miRNA, but recently, it has been proven that RNA viruses can also produce miRNA. The aim of this articles is to describe viral miRNAs biogenesis and their effects on cellular and viral genes.
