In 1987 there were 1,428 psychiatrists in Australia, 8.8 per 100,000 population (Burvill, 1988), 55% identified as in private practice and 45% in public sector practice. Let us be clear about terms. Public sector practice means that each week you receive a salary from the public purse whether you have seen one or a hundred patients. Private practice means that you are paid on a piece-work basis, also largely from the public purse (national health insurance or Medicare), but the income (at about $100 per hour) depends exactly on the number of hours spent with patients. On average, private psychiatrists in Australia gross about $150,000 per year, out of which they must pay practice expenses. The pay for public sector psychiatrists probably averages $70,000 to which, for the purposes of our calculation, we will add the cost of rooms, telephone and secretary provided by the hospital which at $30,000 brings the cost of a public sector psychiatrist to about $100,000 per year. If 45% of psychiatrists are in public practice then the averaged cost of a psychiatrist in Australia can be calculated as $127,500 per annum, and as there are 8.8 psychiatrists per 100,000 the cost, calculated on this simple basis, is $1.12 million per 100,000 population (Andrews, 1989).

The purpose of the article was to reveal the analytical tools for evaluating the effectiveness of the allocated financing funds for the provision of the services provided in the public sector of the economy, the application of which does not have a uniform methodology today. The possibility to solve this issue is shown with regard to the compulsory health insurance system. To achieve the goal, the study reveals the definition of such categories as “effect” and “efficiency” of the medical institutions activities, “efficiency of spending” of the Territorial Fund for Compulsory Health Insurance. There is revealed the analytical significance of the existing methodological approaches to assessing the effectiveness of health care costs; their comparative analysis is carried out. Based on the results, the alternative is proposed, based on the system of the cost-effectiveness indices to achieve the health, social and economic effects of medical institutions; the analytical advantages of the alternative proposed are justified.

The Public sector plays a critical role in the effective delivery of public services that are essential to the functioning of a state economy. The service delivery in the public sector has been noted to be ineffective due to self-interest service from the public sector officers, unlike the private sector, where focus is primarily on shareholder value. The study examined the effect of the balanced scorecard and service delivery at the national health insurance fund in Mombasa County, Kenya. The specific objectives of the study were to determine the effect of the financial perspective of the balanced scorecard, the effect of customer perspective of the balanced scorecard, the internal business perspective of balanced and examine the effect of innovation and learning perspective of balanced scorecard on service delivery at national health insurance fund in Mombasa County. The study is anchored on Balanced score card model and agency theory. The study adopted a descriptive survey research design that depicts the attributes of a specific circumstance, occasion, or case.  The targeted population of the study was 158 and the respondents were senior managers, middle level managers, lower level managers and the support staff working at national health insurance fund in Mombasa County. The study finds that the balanced scorecard improves systems of the cost structure, the organization creates more revenue opportunities, net shareholder value is maintained and the asset is well utilized. Additionally, it can be concluded that most employees are delighted with the financial evaluation process by use of the balanced scorecard. The study found a positive relationship between the balanced scorecard components namely customer focus, financial perspective, customer perspective, internal business perspective, innovation, learning aspect and service delivery. The study concluded that customer focus to enhance the service delivery could be through implementing customer satisfaction measures, implementing customer service charter, maintaining product functionality, maintaining customer relationship management and maintaining customer loyalty. The study recommended that the need for NHIF to achieve the balanced scorecard to be able to track financial results while simultaneously monitoring progress through building the capabilities and as well acquiring the intangible assets they would need future growth. Also, the study recommended the organization to consider the issue cost minimization strategy to enhance the service delivery to the customers. In addition to that, there is a need to have a permanent solution to steady funds to be able to meet its obligations. The study further recommended that the organization to embrace the balanced scorecard and will help improve communication between the management and customers thus improve on the quality service hence satisfy the needs of the customers.

The general objective of the study was to analyze the level of public trust (citizen trust) of e-government based health insurance services, namely the e-mobile National Health Insurance (JKN) BPJS in Makassar City.The design of this research is a quantitative descriptive type. The population in this study were people who used the National Health Insurance (JKN) e-mobile application with 167 respondents. Data collection was carried out using a questionnaire instrument. Data analysis uses multiple linear regression.The results showed that first there was a high level of public trust in JKN e-mobile applications. This means that the application gives satisfaction to the community and is considered beneficial for them. Secondly, the level of public trust is high in the government, where the public considers the government to be serious in providing health insurance services.Tujuan umum penelitian adalah untuk menganalisis tingkat kepercayaan publik (citizen trust) terhadap pelayanan jaminan kesehatan berbasis e-government yaitu e-mobile Jaminan Kesehatan Nasional (JKN) BPJS Kesehatan di Kota Makassar. Desain penelitian ini adalah kuantitatif tipe deskriptif. Populasi dalam penelitian ini adalah masyarakat yang menggunakan aplikasi e-mobile Jaminan Kesehatan Nasional (JKN) dengan jumlah responden sebanyak 383 orang. Pengumpulan data dilakukan dengan menggunakan instrument kuesioner. Analisis data menggunakan regresi linear berganda. Hasil penelitian menunjukkan bahwa, pertama terdapat tingkat kepercayaan tinggi masyarakat terhadap aplikasi e-mobile JKN. Hal ini berarti aplikasi memberi kepuasan kepada masyarakat dan dianggap bermanfaat bagi mereka. Kedua tingkat kepercayaan publik tinggi terhadap pemerintah tinggi, dimana masyarakat menilai pemerintah serius dalam memberikan pelayanan jaminan kesehatan.

This purpose of this article is to explore impediments to effective whistleblowing as a strategy for promoting anti-corruption practices within the South African public sector. Corruption, which violates the public service code of conduct; deters foreign investment, increases the cost of public service delivery, undermines the fight against poverty and unnecessarily burdens the criminal justice system. The article addresses the question on whether legislation on whistleblowing is adequate to encourage whistleblowing in the public sector. A review of literature determines that the effective implementation of whistleblowing legislation is largely dependent on addressing the challenges identified in the article. The quantitative research method was employed in the study to ascertain the views of employees in the public sector on whistleblowing. Empirical findings confirm the hypothesis that the protection of whistleblowers through legislation is inadequate to encourage whistleblowing. The article provides a conceptual framework for the effective achievement of the intended outcomes of whistleblowing in the public sector.

From his personal studies, Glaser analyzes the establishment and summarizes the methods of paying doctors in 16 European countries that have national health insurance programs. Unfortunately, to quote Glaser, "a country's current payment system under its public medical services is usually the arrangement it had before the services were created. . . . Most payment systems in public schemes are simply those inherited from prior practice, since the doctors would be disturbed by changes. Most payment procedures under ‘socialized medicine’ are simply more bureaucratized versions of the methods that were used and often even invented by the doctors under earlier private practice."
