Safety and efficacy of remote ischemic postconditioning after thrombolysis in patients with stroke

ObjectiveTo determine the effect of remote ischemic postconditioning (RIPC) on patients with acute ischemic stroke (AIS) undergoing IV thrombolysis (IVT).MethodsA single-center randomized controlled trial was performed with patients with AIS receiving IVT. Patients in the RIPC group were administered RIPC treatment (after IVT) during hospitalization. The primary endpoint was a score of 0 or 1 on the modified Rankin scale (mRS) at day 90. The safety, tolerability, and neuroprotection biomarkers associated with RIPC were also evaluated.ResultsWe collected data from both the RIPC group (n = 34) and the control group (n = 34). The average duration of hospitalization was 11.2 days. There was no significant difference between 2 groups at admission for the NIH Stroke Scale score (p = 0.364) or occur-to-treatment time (p = 0.889). Favorable recovery (mRS score 0–1) at 3 months was obtained in 71.9% of patients in the RIPC group vs 50.0% in the control group (adjusted odds ratio 9.85, 95% confidence interval 1.54–63.16; p = 0.016). We further found significantly lower plasma S100-β (p = 0.007) and higher vascular endothelial growth factor (p = 0.003) levels in the RIPC group than in the control group.ConclusionsRepeated RIPC combined with IVT can significantly facilitate recovery of nerve function and improve clinical prognosis of patients with AIS.ClinicalTrials.gov identifierNCT03218293.Classification of evidenceThis study provides Class IV evidence that RIPC after tissue plasminogen activator treatment of AIS significantly increases the proportion of patients with an MRS score of 0 or 1 at 90 days.

The objective of this study was to determine if there was a relationship between serum vascular endothelial growth factor (VEGF) levels and ovarian malignancies by contrasting a population with ovarian malignancies and a population free of gynecological neoplasms. Two hundred forty four serum samples were obtained from the US National Cancer Institute's Prostate, Lung, Colon, and Ovarian Cancer Screening Project. These samples were analyzed by enzyme-linked immunosorbent assays in duplicate, and on completion of the assays, the samples were decoded for age and disease type. Average VEGF values for the nongynecological control group was 4.399 ng/ml; for benign gynecologic cases, 2.515 ng/ml; and for patients with malignancies, 4.287 ng/ml. Specifically, there was no difference between the mean value of VEGF in patients with ovarian malignancies and the patients with benign gynecological tumors (P = 0.8823). Also, there was no difference between the mean value of VEGF in patients with ovarian malignancies and the control patients who did not have gynecological disease (P = 0.3110). Using the Mann–Whitney U-test, no significant differences were found between the three populations of this study. Based on our data, due to the lack of significant difference in mean serum VEGF values between patients with and without ovarian malignancies, we feel that serum VEGF cannot be used as a possible screening tool for ovarian cancer.

Abstract Preeclampsia (PE) is a syndrome related with pregnancy and characterized by hypertension and proteinuria, occurring in approximately 6-8% of pregnancies and accounting for approximately 40% of premature births. This study aimed to investigate the polymorphisms of -634C/G and +936C/T in VEGF gene and their relationship with serum VEGF levels in pregnant women with PE. In this case-control study, peripheral blood samples were collected from 135 women with PE and 135 normal pregnant women as the control group. DNA was extracted using the phenol-chloroform method. Then, the polymorphisms of VEGF gene were detected by PCR-RFLP method using specific primers. Besides, VEGF concentrations were measured by ELISA method on serum samples and control subjects using ELISA kits. In this research, maternal age, gestational week, maternal hemoglobin and BMI were significantly correlated with the likelihood of PE, while the occurrence season variable was not effective in PE among the pregnant women. There was no significant difference in the two polymorphisms of -634C/G and +936C/T in VEGF gene between the two groups. Also, the serum VEGF level in PE patients was significantly higher than the normal group (P<0.001). Despite a significant increase in serum VEGF concentrations in women with PE, it seems that -634C/G and +936C/T polymorphisms of VEGF gene are not related with the onset of PE. Further studies are required to fully understand the risk factors related to preeclampsia syndrome.

Abstract Background: The objective of this study was to investigate whether local regulatory proteins are insufficient in patients with preeclampsia and whether VEGF (vascular endothelial growth factor) can regulate the expression of complement regulatory proteins on trophoblast membranes.Methods: A case-control study was performed. The study group enrolled 20 patients with severe preeclampsia, matched with 16 patients comprising the control group. Western blotting was used to assess CD46 and CD55 protein in the placentas of the patients. Human trophoblast cells (HTR-8/SVneo) were treated with VEGF at different concentrations, and the expression of CD46 and CD55 was assessed by immunofluorescence and western blot.Results: The CD46 protein level was significantly higher both in early-onset and late-onset severe preeclampsia group than the L-control group (P=0.028 and P=0.041, respectively). With the increase of VEGF concentration, there was no significant difference in expression of CD46 and CD55 on the HTR-8/SVneo cells.Conclusions: No deficiency of local complement regulatory proteins was found in the patients with preeclampsia. A maternal-fetal interface feedback mechanism of CD46 may exist in severe preeclampsia. VEGF may have no effect on the expression of CD46 and CD55 on trophoblast membranes.

Objectives: This study aimed to measure the vascular endothelial growth factor (VEGF) expression in menstrual effluent from patients with endometriosis compared to non-endometriosis through immunocytochemical methods. We also measured the degree of pain level, endometrioma cyst size, and infertility status whether it is affected by VEGF expression. Materials and Methods: The present case-control study was conducted in Hasan Sadikin General Hospital, Bandung. Thirty productive-age women diagnosed with endometrioma and 30 productive-age women without endometriosis as the control group were included in this study. Menstrual effluent was taken from the posterior fornix on the second day of menstruation and stained using immunocytochemistry staining for VEGF. Results: The results demonstrated a significant difference between the two study groups in terms of VEGF intensity and histoscore although no difference was found in VEGF distributions between the study groups. The subjects in the endometriosis group had significantly higher VEGF intensity and significantly higher VEGF histoscore compared to the control group. Women with VEGF histoscore more than 6 has 9.33 times risk of developing endometriosis compared to those with lower histoscore. There were no significant correlations between VEGF and pain scale, infertility, and the cyst size. Finally, the cyst size was proportionally related to pain. Conclusions: VEGF distribution and expression in endometriosis women were significantly higher than VEGF levels in non-endometriosis women. Women with menstrual effluent containing higher VEGF levels had more chances of developing endometriosis compared to those with lower VEGF levels. Eventually, larger endometrioma size was proportionally related to higher pain levels in subjects with endometrioma.

Scientific research into the reduction of stigmatization, particularly related to specific problems such as Obsessive-Compulsive Disorder (OCD), is scarce. In the present study, we examine the impact of a video-based antistigma intervention program for OCD in a pretest-posttest control group research. After being randomly assigned to either an intervention (n= 101) or control group (n= 96), the participants reported their attitudes on a hypothetical case vignette before and after OCD vs. Multiple Sclerosis (MS) videos, and again six months later as a follow up assessment. The mixed design analyses for the group comparisons indicated that although there was no significant difference in the measures of the control group, the participants watching the anti-stigma OCD video, in which the focus was psychoeducation and interaction strategies, reported significantly lower scores on social distances and negative beliefs for the case vignettes they read, and this difference was maintained six months later. Then, the present results indicate the effectiveness of our anti-stigma intervention program for OCD. Interventions to reduce stigmatization can also be viewed as effective tools for changing the attitudes of people toward OCD, although further research and applications are needed related to specific disorders if a longlasting impact is to be achieved.

Mother-infant attachment is an intimate, lasting and satisfying relationship that leads to better cognitive, emotional and social growth of the infant. The aim of this study was to determine the effects of breastfeeding training by role-play on mother-infant attachment behaviours. This research was a randomised clinical trial (parallel design). Inclusion criteria were: no history of mental disorders; ability to read and write the Persian language to complete the questionnaire; no history of drug and tobacco intake in primigravida women. The sample comprised 100 pregnant women (in 2 groups), selected through simple random sampling at healthcare centres. The researcher reviewed prenatal care registries of selected healthcare centres and extracted the names of pregnant women in their early third trimester. The data were imported into randomisation software. The control group received routine breastfeeding training, while the intervention group received routine training together with training through role-play. The data collection tool was the Maternal Behaviour Inventory Questionnaire. Consequently 75 samples were analysed in SPSS16. Independent t-tests and chi-square tests were used to examine the difference between the two groups. Results showed that the mean score of mother-infant attachment one week after delivery was significantly higher in the intervention group in comparison to that in the control group (p<0.001). No significant difference was observed between the two groups in maternal age, age of marriage, neonatal gender, maternal employment and education, number of parity, and number of abortions (P>0.05). Since breastfeeding training through role-play could affect mother-infant attachment, it is suggested that this type of training should be provided for pregnant women to promote mother-infant attachment and exclusive breastfeeding.

Retrograde menstruation lead to I Kappa B Kinase (IKK) fosforilation in peritoneum macrophage and cause secretion of proinflammatory cytokine interleukin1β then stimulate endometriosis cell to produce Vascular Endothelial Growth Factor which lead to increasing of endometriosis lession seen as endometriosis implant area. Cytokine secretion was inhibited through prevention of NF-κB activation by dragon red fruit rind extract (Hylocereuspolyrhizus). The aim of this reserach is to know the effect of dragon red fuit rind extract with 0,25; 0,5; and 1 mg/g bodyweight dosage toward IL-1β, VEGF expression and implant area in endometriosis mice model. The design of this experiment was randomized post test only control group design.Endometrios mice model were made in 14 days and split into two group, positive control group and treatment group after two week negative control group and postive control group were given Na-CMC 0,5% solution consequetively, and treatment group were given dragon red fruit extract with different dosage. Signification number for IL-1β is p>0,05, signification number for VEGF is p>0,05, and implant area signification number is p>0,05. Administration of dragon red fruit rind extract can decrease IL-1β, VEGF, and implant area.

Background: Pain control is considered as the key issue in modern midwifery. Along with medical painkillers, reflexology is viewed as a non-medical and noninvasive method. Hence, we aimed to investigate the effect of reflexology on the intensity of pain and length of labor. Materials and Methods: In this clinical trial, participants included 240 Iranian primiparous women with term and singleton pregnancy. Having a 3-4 cm cervical dilatation once they visited the hospital. Through a convenient sampling method, they were selected and then randomly divided into two groups. In the intervention group, reflexology was performed, and the intensity of pain during the active phase of labor along with the length of labor in the active phase was measured by visual analog scale (VAS) and compared with the control group. Data were analyzed using descriptive statistics, t-test, and Mann-Whitney U-test. Result: Data analysis showed a statistically significant difference between the intensity of pain in the 5-7 and 8-10 cm dilatation in the two groups (P=0.01). Moreover, the labor length in the active phase was found to be significantly shorter in the intervention group (P<0.001). Conclusion: It appears that reflexology can lead to a reduction in the pain and length of labor. Therefore, through instructing this technique, a goal of midwifery, which is reducing labor pain and its length can be achieved. Keywords: Reflexology, Labor Pain, Labor Length, Primiparous Women
