CANCER OF THE LARYNX IN THE AMUR REGION OF FAR EASTERN FEDERAL DISTRICT

The article analyzes the main indicators of morbidity and mortality from cancer of the larynx in the period from 1998 to 2014 on the territory of the Amur region of Far Eastern Federal District. In the studied period of time, the incidence of malignant tumors of the larynx substantially decreased with the growth/loss in men of “rough indicators” by -11.89%, of standardized ones by -7.75%. In women, they were +98.31 and +77.77%, respectively. Mortality in contrast to the incidence throughout the years had a negative growth (loss) in absolute terms, “rude” and standardized indicators. The age and sex features of morbidity with larynx cancer were revealed; they show that the maximum number of cancer cases is in the age group of 50 years old and older. The multistage character of the process based on its prevalence was determined. The early stages (I-II) of laryngeal cancer were registered in 13.7% of patients. The quality of cancer services activity was studied by reliability index of accounting for this category of patients, which is usually lower than in other regions of the country (-0.45). The analysis of dynamics of mortality in the first year after diagnosis shows its reduction in cancer of the larynx by 11.0% over the period of 1998-2014. The number of patients who were registered five or more years ago decreased significantly. The estimation of primary care, which is not yet fully in line with current medicine, in the provision of specialized aid to cancer patients in the region was done.

The article presents an overview of the data of statistical observation of the Khabarovsk Krai, the Russian Federation and the Far Eastern Federal District (Okrug) on tuberculosis; their significance for assessing the epidemic situation and the quality of anti-tuberculosis care in these territories in 2002–2019 is studied. The dynamics of statistical indicators for 18 years is given using the methodology for calculating rank values. The dependence of the incidence of tuberculosis on the population density in the constituent entities of the country, the Far Eastern Federal District and its regions has been determined

Abstract Background : To support our goal of providing optimal HIV care to our patients, we started applying the value-based health care principle to the HIV care that we offer in our HIV center, measuring relevant health outcomes and costs to allow continuous implementation of improvements (Value-Based HIV Care; VBHiC). Methods : In line with the principles of Michael Porter, our approach consisted of the following steps: 1) Organizing into integrated practice units / describing the HIV care path; 2) Defining an HIV outcome indicator set; 3) Building an enabling information technology platform; 4) Integrating care delivery across separate facilities; 5) Moving to bundled payments for care cycles and 6) Expanding excellent services and interventions for improvement across geographic boundaries. Results : The following set of 9 outcome indicators was developed: undetectable HIV load within the first year of care; quality of life within the first year of care; mortality within the first year of care; retention in care; therapy effectiveness; therapy tolerance; cardiovascular risk; quality of life for every subsequent year and overall annual mortality. These indicators, which were evaluated retrospectively, are shown in figures 1-5. Collection of the underlying data started in January 2016. The HIV care path was also integrated into the electronic file system. Creation of the ability to monitor outcome indicators at patient level, population level and process level allowed us to implement a quality cycle (plan-do-study-act). Conclusion : Our Value-Based HIV Care approach facilitated structured evaluation of parameters that are of value to the patient. It also boosted the quality of the HIV care that we provide and allowed us to increase the number of patients to whom we can offer high quality HIV care.

The Primorskiy Kray takes 1st place in the Far Eastern Federal District by number of population (the proportion of the total number in the Far Eastern Federal District is 31.1%, in the total population of Russia is 1.4%). The population decline was due to the influence of several factors: a decline in fertility, increased mortality and intense migratory movement of population of the region mainly into the central regions, which are more prosperous in social and economic relations. Issues on demographics, inherently, reflect the quality of life, the moral and ethical climate in society, the status of the most important sectors. The implementation of the priority activities of the demographic development in the field of health promotion and increased life expectancy; stimulate fertility and strengthen the family; migration development should affect the stabilization of the population of the Primorye Territory.

The article examines the current specificity of foreign trade ties of the Far Eastern Federal District. It presents a brief analysis of dynamics of the volumes and commodity structure of the export-import operations of the Far Eastern Federal District. At present, of specific significance for the regions of the Far East and Siberia is search for tolls of modernizing the economy with purpose of transition to innovative development. Foreign trade can be a perspective direction of socio-economic development of Russia’s Far East. Far Eastern Federal District is export-oriented, but in the export commodity structure the mineral raw groups of commodities prevail, which decreases its positive effect. Expansion of trade-economic and investment cooperation with the countries of the Asian-Pacific region in the field of developing processing industries and machine building oriented at increasing the depth of mineral resources processing and growth of the ready-made products export can become the basis of sustainable development of the Far East.

The first two decades of the XXI century in the Far Eastern Federal District continue to be characterized by a constantly decreasing level of the population living on its territory with an annual increase in the number of patients with cancer pathology, which makes a negative contribution to the already negative demographic situation in the region. The purpose of this study was to make a scientific assessment of the main morbidity and mortality rates of the population from malignant tumors of the trachea, bronchi and lung in the Far Eastern Federal District over the past ten years (2008-2017). In the course of the work, morbidity and mortality indicators, reporting forms of statistical records of cancer patients and regulatory documents of higher organizations, information on ten-year observations of the dynamics of changes in the numerical values of indicators of the studied pathology were used. The main indicators of medical care for oncology patients in the Far Eastern Federal District are shown, and it was found out that 3180 new cases of tracheal, bronchial and lung malignant neoplasms were registered in 2017, which is 30.9% more than ten years ago (in 2008 there were 2429 cases). The maximum number of cases was in the age group of 50-69 years. The number of patients with stages I-II (29.4%) of the cancer process increased slightly whereas the number of patients with advanced forms of the disease increased significantly (41.5%). The percentage of patients identified during routine check-ups was 28.0% (in 2008 it was 15.8%). Mortality in the first year from the time of diagnosis was reduced to 49.2% (in 2008 – 55.0%) remaining, at the same time, at very high numbers, which makes it impossible to characterize medical care for oncology patients as adequate for the time required. In the structure of total mortality, trachea, bronchus and lung tumors (20.7%) take the first place significantly exceeding the Russian level (17.3%). There was designated the quality of the service’s activity according to the confidence index of accounting for this category of patients, which in 2017 did not fall below 0.6 in any of the territories forming the Far Eastern Federal District. A great deal of work of primary health care institutions responsible for identifying malignant neoplasms in the initial stages of the process, unfortunately, has not led to the desired reduction in morbidity and mortality in the Far Eastern Federal District in the last decade. On the contrary, the level of intensive indicators of medical care for cancer patients in the region remains high, with an annual increase in mortality and the number of cases, especially among the female population

On the second part of the article the main attention is paid to the analysis of survival of patients with malignant tumors calculated on the base of the database of the population cancer register of the northwestern Federal district. The main criterion for assessing the state of oncological service is 1 and 5-years observed and relative survivals of patients with malignant tumors. Unfortunately, the number of cancer registers calculating survival rates of patients with malignant tumors according to international standards is limited in Russia. We present calculations of the dynamic series of the survival median. Defects of its objective assessment are revealed. Positive dynamics of 5-year observed survival of patients with malignant tumors in the Russian Federation is shown. Dynamics of 1 and 5 year survival of cancer patients is considered in pairs taking into account sex, age and stage of disease. There has been some improvement, especially among the female population. As can be seen from the data presented at the first year of observation, about 35 percent of patients die in the region, as in the most European countries.

Background. COVID-19 pandemic caused by novel coronavirus has destabilized the world's health systems, revealing flaws in the organization of work in extreme conditions. Currently, there is limited empirical data on the impact of COVID-19 on tuberculosis outcomes. Materials and methods. Non-interventional comparative retrospective cohort study of the structure of the incidence of extrapulmonary tuberculosis (EPTB) in the Siberian and Far Eastern Federal Districts for the period 1999–2020 was carried out. Data of 13 852 patients were analyzed. Results. Of all 13 852 EPTB patients who were first diagnosed during the study period, the majority of cases (973) were registered in 1999, and the smallest number (550) – in 2017. Then, for 2 years, an increase in EPTB incidence was observed again (582 cases in 2018, and 563 cases in 2019) with a natural reduction by 18.5% in 2020 (459 cases). Over the first year of the COVID-19 epidemic, the incidence of central nervous system tuberculosis decreased by almost half (by 45.9%), the incidence of peripheral lymph nodes tuberculosis remained virtually unchanged, and the number of cases of "other" localizations, mainly abdominal tuberculosis, increased by 55.3%. Overall, in the Siberian and Far Eastern Federal Districts, the detection of isolated forms of EPTB over the first year of the pandemic reduced by 18.5% (from 563 to 459 patients). A large variability of data was found across regions. Thus, in the Novosibirsk, Tomsk and Omsk regions there was a significant decrease in the number of patients diagnosed with isolated EPTB (by 30.2; 47.1 and 64.5%, respectively) while in the Kemerovo and Irkutsk regions, the number of patients with isolated EPTB increased (by 80.9 and 32.3%, respectively). Conclusion. COVID-19 has had a significant negative impact on all services of national and global healthcare, the ultimate losses we have yet to calculate. Diagnostics of any diseases, and particularly EPTB, in the first year of the pandemic was difficult due to the restriction of patient visits to medical and preventive institutions, the closure or re-profiling of some of them, as well as a decrease in the number of specialists due to their selfisolation, re-profiling and COVID-19 disease. Nevertheless, the role of EPTB is still important.

The aim of this study was to perform a comprehensive hygienic assessment of environmental conditions in the port cities of the Sakhalin region to identify priority risk factors affecting on population health and management decisions for the optimization of living conditions. As a result of the assessment of risk and damages for public health from the effects of air pollution on the dose-response, effects were found to excess of impact on the target organs by 10 times. The main ecotoxicant was determined to be manganese oxide, which is associated with a priority manganese content in soil samples ofport cities. The positive dynamics of the gain in the accumulation of soil heavy metals according to the total index indicates to the existence of problems for soil contamination. Analysis of demographic variables shows that the population of the Sakhalin region in general and the port cities in particular relates to a regressive type. The main causes of the population decline are mortality and migration outflow of able-bodied population in other regions of Russia. However, in the port cities there is an increase in the number of work places, contributing to an increase in the labor force. The primary and general morbidity of the population ofport cities is characterized by higher levels compared with the average for the Sakhalin Region and the Far Eastern Federal District. Among all the classes of diseases as priority ones there are marked “neoplasm”, “diseases of the nervous system”, “respiratory diseases”, “diseases of the skin and subcutaneous tissue”. Port cities occupy the top ranking places on the incidence of malignant tumors among the cities of the Sakhalin region.

Modern cancer therapy programs allow an increasing number of patients to be cured, and therefore the quality of their subsequent life is an important task of clinical oncology. The ability to realize reproductive potential is one of the main criteria for the quality of life. Nevertheless, chemoradiation therapy used in the treatment of malignant tumors can cause temporary or permanent infertility. Currently, there are options to protect the reproductive system from chemoradiation treatment (administration of ethinylestradiol drugs, gonadotropin-releasing hormone agonists) or harvesting seminal fluid/oocytes, and cryopreservation of ovarian tissue. However, the low awareness of doctors about modern reproductive technologies in the treatment of persons cured from cancer leads to the fact that these methods are not used actively enough. The article presents the main chemotherapy drugs used in the treatment of malignant tumors that can cause an impairment of reproductive function; describes their pathological effects on oocytes and spermatozoa; provides methods of protecting the reproductive function in persons who take chemoradiation therapy or have completed antitumor therapy.
